The Effect of DynaCleft® on Cleft Width in Unilateral Cleft Lip and Palate Patients.

OBJECTIVE: The specific aim of this retrospective cross-sectional study was to assess the efficacy of DynaCleft® as a method of presurgical orthopedics with infants with a unilateral cleft lip and cleft palate who used an oral obturator. STUDY DESIGN: Data was collected from 25 infants all of comparable age diagnosed with a unilateral complete cleft lip and palate. Eight patients used DynaCleft ® and an obturator (Group Alpha) and seventeen patients only had an obturator (Group Beta). Maxillary impression casts were obtained from each patient at the initial clinic visit and at the time of cle

Restoration of Trumpet-Playing Ability using a Maxillofacial Prosthesis and External Pressure Support in a Patient with a Maxillary Defect: A Case Report.

PURPOSE: We present what is the first reported case of restoring playing function under high intraoral pressure using external compression applied to a maxillofacial prosthesis in a trumpet player with a maxillary defect, to our best knowledge. MATERIALS AND METHODS: A 51-year-old man with left maxillary sinus sarcoma underwent total maxillectomy with rectus abdominis myocutaneous flap reconstruction. A definitive obturator was fabricated; obturator and palatal contours were refined at chairside under simulated blowing with a trumpet mouthpiece and nasal endoscopic visualization. After adjuva

Management and timing of cleft palate fistula repair.

This study reviewed 199 cleft palate repairs resulting in 22 percent fistula formation. Of these, 49 percent were judged to be symptomatic. Of 44 fistulas, 21 required treatment, of which 14 had conventional type surgical closure with an overall success rate of 35 percent. Good surgical technique and good surgical judgment were felt to be important factors both in preventing postoperative fistula and in the success of their repair. Conventional methods of surgical repair of hard palate fistulas were seen to result in a very poor success rate. Orthodontic movement of maxillary segments was seen

Prosthetic treatment of congenital hard and soft palate defects.

Obturator prostheses are used to improve mastication, speech, and swallowing by reestablishing oronasal separation and aesthetics in maxillary defect patients. A sectional and magnetically retained functional removable speech bulb prosthesis was planned to treat the congenitally cleft hard and soft palates of this patient. The obturator part, localized into lateral nasal undercuts covered with sound mucosa, was used to retain the complete denture. Two pieces of the prosthesis were joined together by a magnet in the mouth. A special hinge mechanism was added to join the complete denture and fun

Prosthetic rehabilitation after hemimaxillectomy: a case report.

Maxillofacial defects created by the surgical resection of a tumor negatively impact the mastication, phonation, swallowing, and psychological function of patients. A 68-year-old woman received a diagnosis of oral squamous cell carcinoma involving the hard palate. The hemimaxillectomy procedure for tumor resection created a defect on the left side of the palate. This case report describes the patient's cancer treatment from presurgical planning to final rehabilitation with a conventional obturator and subsequent follow-up care for 1 year. The rehabilitation strategy was satisfactory for the pa

Prosthetic rehabilitation of palatal perforation in a patient with 'syphilis: the great imitator'.

Syphilis is a sexually transmissible disease caused by treponema palladium, a microaerophilic spirochete. Syphilis may progress from primary to tertiary stage if left unnoticed and untreated. Dentists should be vigilant and suspect sexually transmitted infections such as syphilis in the differential diagnosis of oral inflammatory or ulcerative lesions with palatal perforation. Moreover, it is imperative that dentists should have knowledge about its stages, characteristic features, oral presentation and prosthetic rehabilitation. This case report describes a case of tertiary syphilis with palat

Rehabilitation of a mid-facial defect using maxillary obturator with a maxillary expansion device and orbital prosthesis.

Midfacial defects are defined as congenital or acquired defects in the horizontal plane at the middle third of the face and communicate with intraoral maxillary defects. These defects lead to speaking difficulty, difficulty in saliva control and deglutition, mastication, and esthetics. Prosthetic rehabilitation of such defects with maxillofacial prosthesis is a challenging task. Maxillary defects with bilateral undercuts present are common. This case report explains to achieve retention by engaging the bilateral undercuts with the desired path of insertion and obtaining adequate retention of t

Fabricating an obturator using rapid prototyping to design the framework: a case report.

Congenital or acquired maxillary defects cause various esthetic and functional problems. Maxillary deficiency can be prosthetically restored with an obturator. Computer-aided design/computer-assisted manufacture technology, which had been used exclusively in fixed prosthodontics, is now being more widely used in other fields, including removable prosthodontics, after the introduction of software and availability of a comprehensive solution for designing and manufacturing a removable partial denture (RPD). The rapid prototyping (RP) technique enables precise fabrication of the RPD framework in

A new technique for feeding dogs with a congenital cleft palate for surgical research.

In humans, cleft palate (CP) is one of the most common malformations. Although surgeons use palatoplasty to close CP defects in children, its consequences for subsequent facial growth have prompted investigations into other novel surgical alternatives. The animal models of CP used to evaluate new surgical treatments are frequently obtained by creating surgically induced clefts in adult dogs. This procedure has been ethically criticized due to its severity and questionable value as an animal model for human CP. Dogs born with a congenital CP would be much better for this purpose, provided they

Rehabilitation of patients with congenital unrepaired cleft palate defects using free iliac crest bone grafts and dental implants.

PURPOSE: To rehabilitate the mastication and speech of edentulous congenital cleft lip and palate patients with the use of endosseous implants in conjunction with bone augmentation. MATERIALS AND METHODS: Between 1992 and 1999, 6 partially and 4 completely edentulous adult patients with complex cleft palate defects were treated. Six patients had large, unrepaired defects of the hard and soft palate, whereas the other 4 had residual oronasal fistulas after failed palatoplasty and bone grafting. In 8 patients, free inlay-antral and simultaneous lateral-onlay bone grafts (3 patients) were obtain

Zygoma implant-supported prosthetic rehabilitation of a patient after subtotal bilateral maxillectomy.

This clinical report describes the successful implant-supported prosthetic rehabilitation of a patient who underwent subtotal bilateral maxillectomy for an oral squamous cell carcinoma with a consequent wide defect interesting the whole hard palate and most of the soft palate, causing a large opening that directly connects the oral cavity to the nasal fossa bilaterally. The innovative aspect of this case is represented by the realization of an obturator prosthesis supported by just 3 zygoma implants. The maxillary bone had been largely excised by radical surgery. Despite the resection had a com

Application of a nasal speaking valve in a patient with a digitally fabricated obturator provided after a maxillectomy: A clinical report on hypernasality management.

A patient with a maxillectomy and persistent hypernasality despite wearing a digitally fabricated obturator and receiving speech therapy was treated with a nasal speaking valve (NSV) adjunct. Using speech intelligibility tests, the Kumakura Speech Handicap Index-7, nasalance scores, and the blowing test, speech outcomes were assessed under 3 conditions: without a prosthesis, with digitally fabricated obturator alone, and with the combined use of an obturator and NSV. The combined use of the obturator and NSV improved speech intelligibility and reduced hypernasality, suggesting clinical benefit

Management of the traumatic oronasal fistula in the cat with a conical silastic prosthetic device.

Management of a traumatic oronasal fistula with a prosthetic device in an 8-year-old male neutered domestic shorthair cat is described. The animal had fallen from a height of three stories at the age of 1 year. The fall had resulted in a fracture of the hard palate and a split in the palatal mucosa. Two surgical attempts to close the defect had been unsuccessful and an autopolymerizable acrylic resin prosthesis had produced unsatisfactory results. The management of the chronic traumatic fistula with a conical prosthetic device was fast, easy and efficient. The device was removed and a second o

The Role of Biofilms and Material Surface Characteristics in Microbial Adhesion to Maxillary Obturator Materials: A Literature Review.

BACKGROUND: Maxillofacial prosthetics includes restoration of maxillary defects resulting from resection of palate and nasosinus neoplasms with obturator prostheses which may be colonized by microorganisms and function as a reservoir of infection. Patients with neoplasms commonly also require radiotherapy that can result in changes in saliva quality and quantity and changes in the oral microbial flora. The altered flora, in individuals immunocompromised from cancer therapy, increases their risk of prosthesis-related infections. OBJECTIVES: In this review article, we explore microbial biofilms

Palatal insufficiency: a surgical technique for reconstruction.

Nasal regurgitation and hypernasality are the major complications after extirpation of the soft palate, retromolar trigone, and tonsillar areas. The surgical procedure described herein provides satisfactory correction of the defect. The combined approach of advancement-rotation palatoplasty and an advanced lateral pharyngeal wall flap appears to be an alternative solution to the use of an obturator or inferiorly based pharyngeal flap.
