[Spasmophilia or panic attack?].

For many years, the symptoms grouped under the label "spasmophilia" have been differently evaluated in France by psychiatrists, who ascribe them to hysteria or anxiety, and by endocrinologists and general practitioners for whom they are all due to neuromuscular hyperexcitability, the cause of which must be sought in the biochemistry of calcium. Recent studies on anxiety should reduce the gap between these discordant views. The concept of anxiety itself has changed owing to the discriminant effects of psychotropic drugs and to a classification based on precise diagnostic criteria without theore

[Treatment of panic attacks].

Acute anxiety is a common symptom in psychiatric and medical illnesses. Panic attacks i.e. discrete episodes of severe acute anxiety with clear "onset" and "offset" are frequently associated with agoraphobia and respond to tricyclic and MAO-inhibitor antidepressants. These agents appear to have specific antipanic efficacy independent of their antidepressant properties. Despite the frequent association between panic disorder and major depressive illness the nature and the extent of this relationship remain unclear.

[Panic attacks during sleep].

Panic attack is a separate episode of fear or anxiety with the simultaneous development of typical autonomic and mental symptoms. Attack is a widespread phenomenon that makes up the central core of panic disorder and affects about 5% of the population. Up to 71% of patients suffering from attacks report panic episodes not only in wakefulness, but also during sleep. This review focuses on the distinctions of night panic attacks, the particular qualities of sleep disturbances associated with them, and methods of treating these conditions.

Treating Panic Disorder Hypnotically.

A hypnosis protocol for treating panic disorder is provided. The implementation of this protocol is demonstrated through a case example involving the successful treatment of a 28-year-old firefighter presenting with a 4-month history of near-daily panic attacks. Core principles associated with this protocol include: (1) Elementary education about the physiology of panic; (2) A review of primary factors contributing to the evolution and manifestation of panic; (3) Encouragement of physical activity; (4) Utilization of hypnosis applications; and (5) Monitoring and measuring progress evidenced by

Sertraline-induced panic attack.

Selective Serotonin Reuptake Inhibitors are the treatment of choice for depression and anxiety disorders due to its tolerability, safety and side effects profile. However, with the current and frequent use of these medications in patients with these disorders, new side effects have been reported. This case report describes the development of panic attacks in a 24-year-old-female patient with depressive and anxiety symptoms, and no previous predisposing history for panic attacks.

[Non-fearful panic disorder].

Panic disorder is among the most prevalent mental health disorders. Among other reasons, it is an important disorder due to its influence on all other medical professionals that these patients consult repeatedly. One of the major reasons for sub-optimal diagnosis is the different and less familiar presentations of the disorder. This article will focus on the "non-fearful" variant. Four case reports are described with prevalent presentations of the disorder and a summary of recommendations for better diagnosis of the disorder.

Extreme anxiety/panic disorder.

A 45 year old man is referred to a specialist psychiatric centre. For 3 years he has been experiencing worsening impending fear of death and anxiety which he describes as 'panic attacks'. During these episodes he often experiences palpitations and becomes sweaty. These episodes have not been relieved despite several courses of electroconvulsive therapy. On examination, the man is alert and orientated. He has a normal blood pressure of 125/75 mmHg and electrocardiogram reveals a pulse rate of 102 beats per minute. A psychiatrist orders a number of routine tests to rule out organic disease.

Differentiating ictal panic with low-grade temporal lobe tumors from psychogenic panic attacks.

OBJECTIVE: Indolent low-grade temporal lobe tumors may present with ictal panic that may be difficult to differentiate from psychogenic panic attacks. The current study aims to demonstrate the differences between the two disorders and help physicians generate a diagnostic paradigm. METHOD: This was a retrospective study of 43 patients who underwent a temporal lobectomy between 1981 and 2008 for the treatment of intractable temporal lobe epilepsy secondary to low-grade neoplasms at Rush University Medical Center. A total of 10 patients in this group presented with ictal panic who were previous

Do patterns of change during treatment for panic disorder predict future panic symptoms?

BACKGROUND AND OBJECTIVES: Cognitive-behavioral therapies are currently the gold standard for panic disorder treatment, with well-documented treatment response. However, following interventions, some individuals continue to improve, while others experience a return of symptoms. The field lacks reliable ways to predict follow-up symptomatology. In the current study, a cluster analysis with a repeated measures design was conducted to examine change patterns over 12 weeks of cognitive behavioral group therapy for panic disorder. The central aim of the study was to evaluate if change patterns pred

Pregnancy outcomes among women with panic disorder - do panic attacks during pregnancy matter?

PURPOSE: To assess the risks that maternal panic disorder (PD) during pregnancy contribute to adverse pregnancy outcomes, with the effects further specifically differentiated into mothers who experienced a panic attack during pregnancy and those who did not. METHOD: This study linked two nationwide population-based datasets: the birth certificate registry and the Taiwan National Health Insurance Research Dataset. We identified a total of 371 women who gave birth from 2001 to 2003, who had been diagnosed with PD within 2 years prior to the index delivery, together with 1585 matched women witho

Thresholds for health and thresholds for illness: panic disorder versus subthreshold panic disorder.

BACKGROUND: There is increasing evidence that subthreshold forms of psychopathology are both common and clinically relevant. To enable classification of these subthreshold forms of psychopathology, it may be useful to distinguish not only a threshold for illness but also for health. Our aim was to investigate this with regard to panic. METHOD: Data were derived from the Netherlands Mental Health Survey and Incidence Study (NEMESIS), which is based on a large representative sample of the adult general population (18-65 years) of The Netherlands (n=7076). The Composite International Diagnostic

Panic attacks, panic disorder, and agoraphobia: associations with substance use, abuse, and dependence.

Anxiety and substance use disorders frequently co-occur. Despite the clinical importance of this co-occurrence, theory and research addressing the relations between anxiety-substance use disorder comorbidity remain limited. The present commentary is intended to briefly review and summarize key aspects of this literature, with a specific focus on panic-spectrum psychopathology (panic attacks, panic disorder, and agoraphobia) and its associations with tobacco, alcohol, and marijuana use, abuse, and dependence. A heuristic theoretical model for better understanding the panic-substance use relatio

Pentagastrin-induced sleep panic attacks: panic in the absence of elevated baseline arousal.

BACKGROUND: It has been suggested that pharmacological challenges that induce panic attacks are confounded by effects of environmental stress, elevated baseline arousal, and expectancy bias. METHODS: To control for effects of arousal and cognition on the panicogenic effects of pentagastrin, pharmacological challenges were conducted during sleep in seven patients with panic disorder or social phobia. All patients had previously experienced pentagastrin-induced panic while awake. Infusions of normal saline and pentagastrin (0.6 microg/kg) were administered in fixed order and timed so that penta

Effects of tryptophan depletion on carbon dioxide provoked panic in panic disorder patients.

Results of an earlier study in healthy volunteers suggest that the serotonergic system is involved in anxiety-related mechanisms. We studied the influence of tryptophan depletion on the response to a 35% carbon dioxide challenge. Twenty-four panic disorder patients received a mixture of amino acids, either with or without tryptophan, under double-blind conditions. There was a significant increase in anxiety as well as in neurovegetative symptoms in the depletion group, compared to the placebo condition. Furthermore, when we compare the results of the placebo group with earlier panic provocatio

Thyroid hormones in panic disorder, panic disorder with agoraphobia, and generalized anxiety disorder.

Fifty-two patients with generalized anxiety disorder who had symptoms persisting for at least 6 months, 41 patients suffering from either panic disorder (32 patients) or panic disorder with agoraphobia (9 patients), and 14 control subjects were screened for thyroid disease. Total serum thyroxine (TT4), serum-free thyroxine index (FT4I), and triiodothyronine resin uptake (T3RU), were examined for the entire sample, using a one-way analysis of variance (ANOVA). No significant differences were found in TT4 (p = .24), FT4I (p = .24), and T3RU (p = .19). Thyroid-stimulating hormone (TSH) was examin

When to panic about a panic attack: A challenging case of hypersensitivity pneumonitis.

The COVID-19 pandemic has created diagnostic difficulties with the increase in mental health illnesses that often present with nonspecific symptoms, like hypersensitivity pneumonitis. Hypersensitivity pneumonitis is a complex syndrome of varying triggers, onset, severity, and clinical manifestations that can be challenging to diagnose in many cases. Typical symptoms are nonspecific and can be attributed to other entities. There are no pediatric guidelines, which contributes to diagnostic difficulties and delays in treatment. It is particularly important to avoid diagnostic biases, have an inde

Don't panic: interpretation bias is predictive of new onsets of panic disorder.

Psychological models of panic disorder postulate that interpretation of ambiguous material as threatening is an important maintaining factor for the disorder. However, demonstrations of whether such a bias predicts onset of panic disorder are missing. In the present study, we used data from the Dresden Prediction Study, in which a epidemiologic sample of young German women was tested at two time points approximately 17 months apart, allowing the study of biased interpretation as a potential risk factor. At time point one, participants completed an Interpretation Questionnaire including two typ
