Syphilitic Panuveitis Presenting as Bilateral Granulomatous Uveitis in an Immunocompetent Patient: A Case Report.

Ocular syphilis is a re-emerging, sight-threatening manifestation of Treponema pallidum infection that can mimic a wide range of inflammatory eye diseases. We report the case of a 31-year-old immunocompetent woman who presented with a two-month history of progressive bilateral blurred vision, ocular pain, redness, photophobia, and floaters. Ophthalmic examination revealed bilateral granulomatous panuveitis with mutton-fat keratic precipitates, anterior chamber inflammation, vitritis, optic disc hyperemia, and multifocal chorioretinal lesions. Dermatologic examination demonstrated non-pruritic

Coexistent Ankylosing Spondylitis and Ocular Toxocariasis in a Pediatric Patient Manifesting As Bilateral Panuveitis.

The coexistence of ankylosing spondylitis and ocular toxocariasis in the literature is rare and limited to a few case reports. Typically, such cases present as acute nongranulomatous anterior uveitis with Toxocara IgG seropositivity. A patient manifesting with findings of both ankylosing spondylitis and toxocariasis bilaterally has not been reported previously in the literature. We present a case of coexistent juvenile spondyloarthritis and ocular toxocariasis in a 16-year-old male presenting with generalized pustules, back pain, peripheral polyarthritis, and bilateral panuveitis. Both eyes di

Non-biologic, steroid-sparing therapies for non-infectious intermediate, posterior, and panuveitis in adults.

BACKGROUND: Non-infectious intermediate, posterior, and panuveitis (NIIPPU) represent a heterogenous collection of autoimmune and inflammatory disorders isolated to or concentrated in the posterior structures of the eye. Because NIIPPU is typically a chronic condition, people with NIIPPU frequently require treatment with steroid-sparing immunosuppressive therapy. Methotrexate, mycophenolate, cyclosporine, azathioprine, and tacrolimus are non-biologic, disease-modifying antirheumatic drugs (DMARDs) which have been used to treat people with NIIPPU. OBJECTIVES: To compare the effectiveness and s

Spectral optical coherence tomography findings in an elderly patient with syphilitic bilateral chronic panuveitis.

PURPOSE: To report the spectral domain optical coherence tomography (SD-OCT) features of a focal retinitis in an elderly male patient with bilateral syphilitic panuveitis. OBSERVATIONS: In the left eye (LE), spectral domain SD-OCT images during the active period revealed hyperreflectivity extending through the full thickness of the retina with no individualization of the layers, except for the retinal pigment epithelium. Once the lesion healed, SD-OCT imaging revealed an inner retinal atrophy and a mild disruption of the retinal pigment epithelium. CONCLUSIONS AND IMPORTANCE: In our patient,

Management of bilateral uveitis in a Toxoplasma gondii-seropositive cat with histopathologic evidence of fungal panuveitis.

A 5-year-old, neutered male Domestic Short-haired cat was referred with a 5-month history of anterior uveitis and cataract in the right eye. Clinical examination confirmed anterior uveitis and immature cataract in the right eye and chorioretinitis in the left eye. Ocular ultrasound showed a retinal detachment in the right eye. Diagnostic testing revealed elevated serum titers for Toxoplasma gondii. Anterior uveitis in the right eye and chorioretinitis in the left eye progressed, resulting in blindness despite a 21-day course of clindamycin and aggressive topical medical management of uveitis.
