Paramedic-performed Fascia Iliaca Compartment Block for Femoral Fractures: A Controlled Trial.

BACKGROUND: Femoral (thigh) fractures are an important clinical problem commonly encountered by paramedics. These injuries are painful, and the need for extrication and transport adds complexity to the management of this condition. Whereas traditional analgesia involves parenteral opioids, regional nerve blockade for femoral fractures have been demonstrated to be effective when performed by physicians. Regional peripheral nerve blockade performed by paramedics may be suitable in the prehospital setting. STUDY OBJECTIVES: To examine the efficacy and feasibility of paramedic-performed fascia il

Neurological symptoms occurring in the context of ruptured abdominal aortic aneurysm: a paramedic's perspective.

Ruptured abdominal aortic aneurysm (RAAA) classically presents with sudden onset, severe ripping or tearing abdominal pain radiating through to the back. This case report describes features of an atypical presentation of a patient with RAAA and highlights the difficulties and uncertainties surrounding the prehospital assessment, appropriate treatment and management of these patients. All prehospital care educators and practitioners should be made aware of the wide spectrum of clinical manifestations for this condition.

Drugs on a medical campus. III. Drug use among nursing and paramedical personnel.

Fifty nurses and 50 paramedical staff working in the Rajendra Hospital and Medical College, Patiala, were studied by means of a structured, self-report questionnaire. The life-time prevalence of drug use among nurses was comparatively low--55%, compared to 81% among the paramedical staff. Current use of drugs as shown by the 30-day prevalence rate was also very low among the nurses, a majority of whom restricted themselves to using tranquillizers and sedatives for the specific purpose of relaxation or inducing sleep; only a few had experimented with alcohol, cannabis, and tobacco. On the other

A European paramedic curriculum for geriatric emergency medicine developed via a modified Delphi technique.

BACKGROUND: Older emergency patients currently account for most European emergency medical service dispatches. Due to demographic changes and increasing comorbidities in advanced age, this number is expected to rise substantially in the coming years. Prehospital professionals require specialised training to provide high-quality care for complex, multimorbid patients. The aim of this study is to define minimum competencies for paramedic education in Europe on the management of emergencies in older adults. METHODS: A modified electronic Delphi study was performed from January 2023 to November 2

Is paramedic palliative care different in rural compared to urban settings: A mixed methods study.

BACKGROUND: Nova Scotia (NS) Paramedics have been provided with palliative care training, allowing for care at home without the requirement of transport to the emergency department. It is largely unknown if rural populations have different experiences with paramedic palliative support services (transport vs no transport assessed/treated). AIM: The aim of this research was to describe differences between Paramedics Providing Palliative Care in rural versus urban settings, specifically paramedic call characteristics (reason, level of responder) and outcome (transport, non-transport), challenges

Thoracic ultrasound may improve paramedic diagnostic and management accuracy in undifferentiated respiratory distress.

OBJECTIVES: Patients with chronic obstructive pulmonary disease (COPD) and congestive heart failure (CHF) exacerbations present with similar history and physical examination findings. This complicates both the diagnostic process and the creation of appropriate treatment plans for patients presenting in respiratory distress, particularly in the prehospital setting. Thoracic point-of-care-ultrasound (POCUS) may increase diagnostic accuracy; however, its potential to improve patient management by emergency medical services clinicians is unknown. We aimed to determine whether a brief thoracic POCU

Development of examination objectives for the Korean paramedic and emergency medical technician examination: a survey study.

PURPOSE: The duties of paramedics and emergency medical technicians (P&EMTs) are continuously changing due to developments in medical systems. This study presents evaluation goals for P&EMTs by analyzing their work, especially the tasks that new P&EMTs (with less than 3 years’ experience) find difficult, to foster the training of P&EMTs who could adapt to emergency situations after graduation. METHODS: A questionnaire was created based on prior job analyses of P&EMTs. The survey questions were reviewed through focus group interviews, from which 253 task elements were derived. A survey was con

Paramedic-performed Prehospital Point-of-care Ultrasound for Patients with Undifferentiated Dyspnea: A Pilot Study.

INTRODUCTION: Thoracic ultrasound is frequently used in the emergency department (ED) to determine the etiology of dyspnea, yet its use is not widespread in the prehospital setting. We sought to investigate the feasibility and diagnostic performance of paramedic acquisition and assessment of thoracic ultrasound images in the prehospital environment, specifically for the detection of B-lines in congestive heart failure (CHF). METHODS: This was a prospective observational study of a convenience sample of adult patients with a chief complaint of dyspnea. Paramedics participated in a didactic and

Paramedic and Emergency Medical Technician Reflections on the Ongoing Impact of the 9/11 Terrorist Attacks.

IntroductionIn the years following the September 11, 2001 terrorist attacks in New York City (New York USA), otherwise known as 9/11, first responders began experiencing a range of health and psychosocial impacts. Publications documenting these largely focus on firefighters. This research explores paramedic and emergency medical technician (EMT) reflections on the long-term impact of responding to the 9/11 terrorist attacks. METHODS: Qualitative methods were used to conduct interviews with 54 paramedics and EMTs on the 15-year anniversary of 9/11. RESULTS: Research participants reported a rang

Association between Dry Eye Disease and Psychological Stress among Paramedical Workers in Korea.

This study was performed to evaluate the prevalence and risk factors of dry eye disease (DED) among paramedical workers at a university hospital in Korea. This cross-sectional study included 566 paramedical workers at a university hospital in Korea. Dry eye symptoms were assessed using a 9-item questionnaire, and DED was defined as having 1 or more dry eye symptoms often or all the time. A survey including demographic data and potential risk factors of DED was also performed. Psychological stress was measured using stress VAS and perceived stress scale 4 (PSS-4) questionnaires. Of the 566 para

Comparison of the McGrath® Series 5 and GlideScope® Ranger with the Macintosh laryngoscope by paramedics.

BACKGROUND: Out-of-hospital endotracheal intubation performed by paramedics using the Macintosh blade for direct laryngoscopy is associated with a high incidence of complications. The novel technique of video laryngoscopy has been shown to improve glottic view and intubation success in the operating room. The aim of this study was to compare glottic view, time of intubation and success rate of the McGrath® Series 5 and GlideScope® Ranger video laryngoscopes with the Macintosh laryngoscope by paramedics. METHODS: Thirty paramedics performed six intubations in a randomised order with all three

Risk of hepatitis B infection among medical and paramedical workers in a general hospital in Zimbabwe.

To evaluate the addition of hepatitis vaccine to health schemes for hospital workers in Zimbabwe we undertook a cross sectional study of viral markers in 226 hospital workers and compared the results with 97 volunteer blood donor controls. One hundred and thirty one (58%) hospital workers had hepatitis markers compared with 45 (46%) of the donor group. Racial group was the strongest risk factor. Blacks were 70% more likely to have markers than whites. This racial difference was not explained by job status or patient contact. Our data suggest that work in a district general hospital does not co

A randomised study on the comparative performance of German paramedics after technical versus non-technical skills training.

BACKGROUND: The working environment of pre-hospital emergency medicine represents the framework conditions of high-risk organisations. Non-technical skills are an important foundation for successful performance. In Germany, a new job profile - advanced paramedic - was introduced in 2014, which for the first time also included non-technical skills in the curriculum. Another special feature is that for the first time it is regulated how paramedics proceed with selected extended procedures, such as the application of selected medication. The advanced paramedic has a mandatory annual training requ

Developing a clinical decision tool to support paramedics when assessing and managing children with minor head injury.

BACKGROUND: Head-injured children are commonly transported to the Emergency Department (ED) by ambulance. However, most of those conveyed are deemed non-serious and are discharged at triage. Hospital clinicians use clinical decision tools to support their assessment of head-injured children; however, this is generally to determine whether a computed tomography (CT) scan is indicated. Currently, there is no pre-hospital clinical decision tool designed to support paramedics when assessing and managing head-injured children at scene. The aim of this study was to determine consensus amongst expert

Soiled airway tracheal intubation and the effectiveness of decontamination by paramedics (SATIATED): a randomised controlled manikin study.

INTRODUCTION: Vomiting and regurgitation are commonly encountered in out-of-hospital cardiac arrest (OHCA), with a reported incidence of 20-30%. This is of concern since patients who have suffered an OHCA are already in extremis. If standard suctioning techniques are not sufficient to maintain a clear airway and provide ventilation, then these patients will die, irrespective of the quality of chest compressions and the timeliness of defibrillation. This study aimed to determine whether a short teaching session of the suction assisted laryngoscopy and airway decontamination (SALAD) technique im

Public social trust and acceptance of task shifting to hospital paramedics as a response to physician shortages.

BACKGROUND: Physician shortage is a crucial global issue. Some countries have introduced task shifting to expand paramedics' roles. This expansion requires citizens' social trust in paramedics in general. During medical emergencies, citizens must allow treatment by paramedics, with whom they often have no prior relationship. Although social trust is vital in medical emergencies, its level among hospital paramedics is unclear. This study aimed to evaluate citizens' social trust in hospital paramedics and their acceptance of the paramedics' medical practices, as well as the impact of educational

Ultrasound on the Frontlines: Empowering Paramedics with Lung Ultrasound for Dyspnea Diagnosis in Adults-A Pilot Study.

Lung transthoracic ultrasound (LUS) is an accessible and widely applicable method of rapidly imaging certain pathologies in the thorax. LUS proves to be an optimal tool in respiratory emergency medicine, applicable in various clinical settings. However, despite the rapid development of bedside ultrasonography, or point-of-care (POCUS) ultrasound, there remains a scarcity of knowledge about the use of LUS in pre-hospital settings. Therefore, our aim was to assess the usefulness of LUS as an additional tool in diagnosing dyspnea when performed by experienced paramedics in real-life, pre-hospital
