Support services for pediatric trainees. A survey of training program directors.

We conducted a survey of pediatric training program directors (75% response rate) regarding program support services for house staff, directors' attitudes about stress in training, and program plans to ameliorate such stress. Support services included explicit measures to alleviate stress, policies that may minimize stress, evaluation of house staff performance, feedback regarding career concerns, and benefits (eg, medical insurance and child care). Most programs offered services to reduce stress from training, but few offered preventive services. Support policies were reported to be inadequat

Stress and Burden Experienced by Parents of Children with Type 1 Diabetes-A Qualitative Content Analysis Interview Study.

Background: Parents of children with type 1 diabetes play a key role in managing their child's self-management, which can be stressful and burdensome. High involvement can lead to reactions such as emotional, cognitive, and physical exhaustion in parents. Understanding parents' psychosocial impact due to their child's disease is crucial for the family's overall well-being. The purpose of this study was to describe stress and burden experienced by parents in families with children living with type 1 diabetes. Methods: This study utilized a qualitative approach, analyzing interviews with 16 pare

Taking Leave During Breast Surgical Oncology Fellowship: A National Evaluation of Policy and Practice.

INTRODUCTION: The American Board of Medical Specialties and the Fellowship Council provide general leave recommendations for many surgical fellowships, yet no standardized policy exists for Breast Surgical Oncology (BSO) programs. Leave is granted on a case-by-case basis by institutions and governing bodies including the Society of Surgical Oncology (SSO) and the American Society of Breast Surgeons (ASBrS). This study characterized the frequency, duration, and types of leave taken by BSO fellows, evaluated institutional leave policies, and barriers to leave implementation. METHODS: An anonymo

A survey of students' experiences of returning to midwifery studies after maternity leave: A pilot survey.

AIM: To identify the barriers and facilitators having an impact on the progression and completion of studies for students who give birth during their midwifery program. BACKGROUND: Balancing the demands of pregnancy and new motherhood with the rigorous academic requirements of a tertiary-level midwifery program is challenging for students wishing to progress and complete their studies. Understanding the barriers and facilitators students face when resuming midwifery studies following birth can assist universities in providing educational environments that support students in achieving their m

The pregnant urologist.

Female urologists represent an ever-increasing percentage of the work force; more and more of our colleagues will be working through pregnancy. There is a lack of clear and concise advice for pregnant urologists about occupational risks during pregnancy. Urology exposes expectant mothers to potential risks from radiation, teratogenic and cytotoxic drugs, iodine hand scrub, infections, and long working hours. We aim to provide a review of the current evidence and guidance to aid expectant mothers in their decision making. Relevant research articles and up-to-date guidance were reviewed. The mil

Contexts of reproductive loss in lesbian couples.

Lesbian couples seek to become parents in a heteronormative world and in the context of complex biological, social, and legal challenges that may constrain same-sex parenting. Because of these constraints and challenges, lesbian couples experiencing a reproductive loss may encounter issues that heterosexual couples typically will not. Prior to pregnancy, lesbians may experience loss and grief because they cannot conceive a child together without the assistance of a third party. Same-sex families are marginalized; simply deciding to become parents leaves them open to criticism and negative judg

Occupational exposure to whole body vibrations and birth outcomes - A nationwide cohort study of Swedish women.

BACKGROUND: More women in reproductive ages are entering occupations where exposure to whole body vibrations (WBV) is common (e.g. in transportation and construction). Previous studies based on self-assessed exposure suggest increased risks of adverse birth outcomes, but it is unclear at what exposure levels and if the current exposure guidelines are appropriate during pregnancy. OBJECTIVES: To investigate whether occupational WBV-exposure increases the risk of preterm birth, low birth weight, and/or small-for-gestational age, in a large, nationwide, prospective, cohort study. DESIGN: The Fe

Structural and Organizational Dimensions of Compassion Fatigue in Pediatric Oncology Nursing: A Qualitative Study.

Pediatric oncology nursing is among the highest-intensity care specialties, exposing nurses to repeated child death and sustained emotional investment in family relationships. This occupational burden is a recognized psychosocial risk factor and a driver of compassion fatigue, burnout, and reported intention to leave, yet the organizational and educational determinants of this risk remain poorly captured by existing assessment approaches. This study investigated the lived experiences of nurses caring for terminally ill children in a pediatric onco-hematology unit and hospice ward in central It

Day-care versus inpatient pediatric surgery: a comparison of costs incurred by parents.

The cost-effectiveness for parents of day-care pediatric surgery was assessed by comparing time and financial costs associated with two surgical procedures, one (squint repair) performed exclusively as a day-care procedure, the other (adenoidectomy) performed exclusively as an inpatient procedure. All but 1 of 165 eligible families participated. The children underwent surgery between February and July 1981. The day-care surgery group (59 families) incurred average total time costs of 16.1 hours, compared with 37.1 hours for the inpatient surgery group (105 families), as parents in the latter g

Anticipation in myotonic dystrophy: fact or fiction?

In 1918 Fleischer reported that after transmission from one generation to the next, myotonic dystrophy has an earlier onset and is more severe. The hypothesis put forward by Penrose in 1948 that 'anticipation' is caused by bias of index case selection was based on theoretical arguments only and has not been supported by clinical observations. This hypothesis was tested in a clinical and genetic study of 14 families with myotonic dystrophy. Excluding index patients, an earlier onset in the child was found in 98% of 61 parent-child pairs. A greater mean difference in age of onset was found with

Against nature: The experiences of parents caring for an adult child with young-onset dementia.

BackgroundYoung-onset dementia (YOD), diagnosed before age 65, most often appears as Alzheimer's disease (AD) or frontotemporal dementia (FTD). Compared to late-onset dementia (LOD), YOD poses distinct caregiving, emotional, and social challenges. While research has expanded, it has largely centered on spouses and children, leaving the experiences of parents caring for an adult child with YOD underexplored and in need of greater scholarly attention.ObjectiveThis study aims to investigate the unique experiences of parents caring for an adult child diagnosed with YOD.MethodsQualitative semi-stru

Challenges of optimizing glycaemic control in children with Type 1 diabetes: a qualitative study of parents' experiences and views.

AIMS: To explore the difficulties parents encounter in trying to achieve clinically recommended blood glucose levels and how they could be better supported to optimize their child's glycaemic control. METHODS: In-depth interviews were conducted with 54 parents of children with Type 1 diabetes (≤ 12 years). Data were analysed thematically. RESULTS: Parents described being reluctant and finding it difficult to keep their child's blood glucose levels consistently within clinically recommended ranges. As well as worrying about their child's ability to detect/report hypoglycaemia, parents highlig

[Humanizing care in medically assisted reproduction].

Medically assisted reproduction is forty years old. Since the summer of 2021, care is no longer reserved for heterosexual couples only, but is extended to female couples and single women. This societal evolution, which has been debated at length, has highlighted the need to rethink the organization of care. Long stigmatized and trying, they have a strong impact on the life trajectories of patients, leaving traces that can have an impact on their parentality.

Dimensions of Parenting and Children's Conduct Problems: The Importance of Considering Children's Callous-Unemotional Traits.

Research has clearly indicated that the development of serious behavioral problems in children and adolescents is influenced by parenting. However, recent research has refined the role of parenting by showing the importance of distinguishing between different types of parenting and in considering the role of callous-unemotional traits (CU traits) and conduct problems (CP) of the children. In the current study, we advance this research by distinguishing between emotional (e.g., parental warmth; parental hostility) and behavioral (e.g., use of positive reinforcement; inconsistent discipline/hars

Analysis of herbal teas made from the leaves of comfrey (Symphytum officinale): reduction of N-oxides results in order of magnitude increases in the measurable concentration of pyrrolizidine alkaloids.

OBJECTIVES: To determine the relative quantities of two hepatotoxic pyrrolizidine alkaloids, symphytine and echimidine, in teas prepared from comfrey leaves (Symphytum officinale), and to determine the potential contribution of the N-oxide forms of these alkaloids to levels of the parent alkaloids. DESIGN: Comfrey leaves were purchased from three commercial sources and used to prepare tea in a manner consistent with the methods used by consumers. An extraction scheme was devised for extraction of the alkaloids, and a gas chromatographic method was developed to quantify the two major alkaloids

Parent-to-Parent Coaching for Parents of Children Requiring Invasive Mechanical Ventilation with Incomplete Home Health Nursing Teams: A Pilot Support and Empowerment Model.

Parents of children requiring invasive home mechanical ventilation (IMV) rely on, and are expected to manage, complex home health nursing care teams. However, ubiquitous shortages in the pediatric home healthcare workforce leave most families with incomplete nursing support, with negative impacts on child and family well-being. Parents report feeling stressed, overwhelmed, and unsupported by community healthcare services. Parent-to-parent support programs in other populations of children with disabilities have been shown to embolden parents with practical solutions, emotional support, and soci

'Once you open that Pandora's box, you cannot close it': a qualitative study on family relationships following insemination fraud.

RESEARCH QUESTION: Insemination fraud occurs when the spermatozoa intended for insemination have been intentionally swapped for another person's without the knowledge of the intended family. In what ways is this experienced by recipient parents and their offspring? DESIGN: This was a qualitative study involving semi-structured interviews with 15 participants (seven parents and eight donor-conceived individuals) affected by insemination fraud involving the same doctor in Canada. RESULTS: This study documents how insemination fraud is experienced by recipient parents and (their) offspring at t
