Homogenization of the management of acute paronychia with abscess formation within the context of an evaluation of professional practices.

INTRODUCTION: There is no consensus in the literature, or even within the same team, on the most appropriate treatment option for acute paronychia with abscess formation. The performance of an evaluation of professional practices (EPP) using a clinical audit measures the quality of our practices with the aim of standardizing them. Therefore, the primary objective of this study was to develop a clinical pathway for the management of acute paronychia with abscess formation. The secondary objectives were to evaluate our professional practices using a clinical audit before and after the disseminat

Purulent Pericarditis Due to Paronychia in a 16-Month-Old Child: A Nail-Biting Story.

Purulent pericarditis is a rare infectious disease with significant mortality, even in the modern antibiotic era. The presenting signs can often be subtle and patients can deteriorate rapidly with cardiac tamponade. We report a previously healthy 16-month-old female who developed purulent pericarditis associated with paronychia and sepsis caused by methicillin-sensitive Staphylococcus aureus. In addition to antibiotic treatment, she required emergent pericardiocentesis for cardiac tamponade, followed by two surgical interventions including full median sternotomy incision and partial pericardie

The effects of clonidine added to mepivacaine for paronychia surgery under axillary brachial plexus block.

We hypothesized that onset of sensory block is delayed in infected versus healthy tissues within the same nerve distribution after axillary brachial plexus block (ABPB) and that clonidine added to mepivacaine would enhance anesthesia and postoperative analgesia. Forty-one outpatients undergoing thumb/index paronychia surgery under ABPB were randomly assigned to receive in a double-blind fashion 400 mg mepivacaine plus either 100 microg clonidine (clonidine group, n = 21) or 2 mL saline (placebo group, n = 20). Onset of sensory block in the infected area was delayed compared with healthy areas

Acute felon and paronychia: Antibiotics not necessary after surgical treatment. Prospective study of 46 patients.

Paronychia and felon are the most common infections of the hand. Surgical treatment is required once an abscess develops, but systematic use of postoperative antibiotic therapy remains open for discussion. Antibiotics both favor the selection of resistant bacteria and increase the cost of treatment. To our knowledge, no study has demonstrated their benefit following excision, yet many practitioners prescribe them systematically and empirically. In our current practices, we do not use antibiotic coverage following excision of uncomplicated paronychia or felon (no signs of arthritis, osteitis, f

Tips to treat the 5 most common nail disorders: brittle nails, onycholysis, paronychia, psoriasis, onychomycosis.

Nail disorders are difficult to treat and often frustrating both for patients and clinicians. Because of the slow growth rate of the nail plate and the difficulty of getting the drug actives to penetrate the nail tissues, it is usually necessary to wait several months before seeing the results of treatments. This delay often leads to discontinuation of therapy by the patients. This article therefore helps clinicians to find the right treatment of the 5 most common nail disorders (brittle nails, onycholysis, paronychia, psoriasis, and onychomycosis) and provides practical tips that might improv

Disseminated Fusarium infection originating from paronychia in a neutropenic patient: a case report and review of the literature.

Fusarium is a saprophytic organism that is widely found distributed in soil, subterranean and aerial plants, plant debris, and other organic substrates. It can cause local tissue infections in immunocompetent patients, such as onychomycosis, bone and joint infections, or sinusitis. The incidence of disseminated disease has notably increased since the initial cases of disseminated Fusarium were described, particularly affecting immunocompromised patients with hematologic malignancies. We report a 39-year-old man hospitalized with newly diagnosed acute myelocytic leukemia who developed dissemina

Paronychia with pyogenic granuloma in a child treated with indinavir: the retinoid-mediated side effect theory revisited.

BACKGROUND: The introduction of HIV-1 protease inhibitors into the treatment of patients infected with HIV-1 has had a major influence on clinical practice. However, the use of protease inhibitors is frequently associated with the development of resistance and several side effects and interactions with other drugs have been reported. OBSERVATIONS: We present the first pediatric patient with paronychia with pyogenic granuloma associated with the administration of the protease inhibitor indinavir. Clinical findings are discussed in view of a possible interference of indinavir with endogenous re

Relationship between recurrent colorectal cancer and EGFR inhibitor-induced dermatological side effects: a focus on paronychia.

This case describes a woman in her mid-40s with recurrent colorectal cancer who developed severe dermatologic toxicity during panitumumab-based therapy. Three months after treatment initiation, she developed a diffuse acneiform eruption, pronounced xerosis and painful periungual inflammation with exuberant granulation tissue consistent with epidermal growth factor receptor (EGFR)-inhibitor-induced paronychia. Multidisciplinary, guideline-concordant supportive care (including prompt topical measures and short-course systemic therapy) achieved symptom control and allowed continuation of anticanc

Cosmetic-Associated Acute Paronychia With Green Nail-Bed Pigmentation in a Child Following Nail Polish Application.

Acute paronychia in children is typically associated with trauma or habitual behaviors, whereas cosmetic-related cases are rarely reported. We describe a 4-year-old girl who developed acute paronychia with persistent green nail-bed pigmentation following the application of nail polish to a nail with pre-existing cuticular disruption. Staphylococcus epidermidis was isolated from purulent discharge, while Pseudomonas aeruginosa was not detected. The clinical course supports a dual pathogenic mechanism involving bacterial invasion through a compromised cuticle and chemical irritation from cosmeti

Red light emitting diode as an adjuvant treatment for epidermal growth factor receptor inhibitors-induced paronychia.

We evaluated the efficacy and safety of red light LED as an adjuvant treatment for epidermal growth factor receptor inhibitor-induced paronychia. Eight patients were recruited in this randomized, single-blinded controlled trial. They were randomized to receive red-light on one hand or foot 2-3 times/week for 6 weeks while the contralateral side served as controls. The standard treatments were continued. Erythema and lesion elevation observed by Anthera® 3D, severity and pain scores were obtained at weeks 0, 2, 4, 6, and 8. The red light group showed significantly lower erythema, severity, and

Necrotizing soft tissue infection of the fingertips secondary to paronychia in a leukemic child: a case report and warning.

Acute lymphoblastic leukemia (ALL) is the most common childhood malignancy, for which chemotherapy remains the cornerstone of treatment. Skin toxicity is a frequent adverse reaction to chemotherapy; however, progression from paronychia to necrotizing soft tissue infection of the fingertips is uncommon. This report describes the case of a 7-year-old child with ALL who developed paronychia following combination chemotherapy with vincristine and daunorubicin. The infection progressed to involve multiple fingertips as necrotizing soft tissue infection, a course facilitated by the patient's underly

[Exudative onycholysis and acute bacterial paronychia related to BIBF-1120 and paclitaxel: response to topical therapy].

A case of a 50 years-old breast cancer patient treated with weekly paclitaxel and BIBF 1120 is reported herein. At the end of the twelfth cycle of chemotherapy, the patient developed distal onycholysis with intense hyponychium serous exudates, pain and malodor in all her fingernails. It was treated with topical fusidic acid and 1% methylprednisolone aceponate two times daily, with an excellent clinical response from the first three days of treatment. Bacterial paronychia with nail plate loss of the fifth left fingernail was observed a week after the topical therapy was started, with positive c

Acute Paronychia and Onychomadesis after a "Russian" Manicure: A New, Aggressive, and Destructive Form of Manicure for Dermatologists to Recognize.

INTRODUCTION: Cuticle reduction and removal techniques are commonly performed by nail technicians for nail cosmesis. However, manipulation of the nail cuticle can lead to localized infection and nail dystrophy. CASE PRESENTATION: In this case, a 20-year-old woman from the Philadelphia area in the USA presented with onychomadesis secondary to acute paronychia following a "Russian" manicure. In this technique, an electronic filer is used to completely remove the cuticle, leaving the proximal nail fold exposed and vulnerable. CONCLUSION: As this style of manicure is being inaccurately publicize
