[Micro-modified Fulkerson osteotomy for the treatment of habituation patellar dislocation].

OBJECTIVE: To evaluate clinical effect of micro-modified Fulkerson osteotomy under arthroscope in treating habituation patellar dislocation. METHODS: Twenty patients of habituation patellar dislocation (25 knees) were treated by the medial retinaculum plication, lateral retinaculum releasing and modified Fulkerson osteotomy (tibial tubercle anteromedial transfer) under arthroscope. There were 5 males and 15 females with the age from 16- to 28-years-old (average of 21 years). The subjective symptoms and the joint function were evaluated according to Lysholm and Tegner scoring system. RESULTS:

Acute patellar dislocation in children and adolescents: a randomized clinical trial.

BACKGROUND: The treatment of acute patellar dislocation in children is controversial. Some investigators have advocated early repair of the medial structures, whereas others have treated this injury nonoperatively. The present report describes the long-term subjective and functional results of a randomized controlled trial of nonoperative and operative treatment of primary acute patellar dislocation in children less than sixteen years of age. METHODS: The data were gathered prospectively on a cohort of seventy-four acute patellar dislocations in seventy-one patients (fifty-one girls and twent

Arthroscopic release of the vastus lateralis tendon for recurrent patellar dislocation.

BACKGROUND: Arthroscopic release of the vastus lateralis tendon for treatment of recurrent patellar dislocation has been criticized on the grounds that it may weaken the quadriceps. HYPOTHESIS: Quadriceps strength and outcome measures improve after arthroscopic release of the vastus lateralis tendon in patients with documented patellar dislocation. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Twenty patients who had recurrent patellar dislocation underwent arthroscopic lateral retinacular release that included a complete release of the vastus lateralis tendon from the superior

Isokinetic thigh muscle performance after long-term recovery from patellar dislocation.

Eighty-two patients (50 women, 32 men) underwent isokinetic muscle testing on average 13 years after a conservatively treated unilateral primary patellar dislocation. Three study groups were formed according to the natural history of recovery: group A (n = 32), patients with only primary conservative treatment; group B (n = 34) patients with conservative (group B1; n = 24) or surgical (group B2; n = 10) treatment of redislocations; group C (n = 16) patients with other residual complaints (anterior knee, pain subluxations) requiring surgery. The Cybex 6000 dynamometer system was used as the tes

Femoral Nerve Palsy With Concomitant Patellar Dislocation in a Ballet Dancer.

Femoral nerve palsy with concomitant patellar dislocation is a rare clinical entity that has not previously been well documented. The authors present the case of a 16-year-old female ballet dancer who sustained a patellar dislocation with concomitant femoral nerve palsy. She experienced muscle weakness after the initial injury and developed neuropathic symptoms through the anterior left thigh. The patient exhibited muscle atrophy in her left lower extremity verified by circumferential thigh measurements as well as magnetic resonance imaging showing clear atrophy of the anterior compartment. Factors Affecting Tibial Tuberosity-Trochlear Groove Distance in Recurrent Patellar Dislocation.

BACKGROUND: The tibial tuberosity-trochlear groove (TT-TG) distance is used to determine the necessity of tibial tubercle osteotomy. We conducted this study to determine the extent to which each of the tibial tuberosity lateralization, trochlear groove medialization, and knee rotation angle affects the TT-TG distance in both normal and patella dislocated patients and thereby scrutinize the rationale for tuberosity transfer based on the TT-TG distance. METHODS: Retrospective analysis of rotational profile computed tomography was done for patella dislocated and control group patients. Femoral a

Patellofemoral Arthritis After Lateral Patellar Dislocation: A Matched Population-Based Analysis.

BACKGROUND: The rate of patellofemoral arthritis after lateral patellar dislocation is unknown. Purpose/Hypothesis: The purpose of this study was to compare the risk of patellofemoral arthritis and knee arthroplasty between patients who experienced a lateral patellar dislocation and matched individuals without a patellar dislocation. Additionally, factors predictive of arthritis after patellar dislocation were examined. The hypothesis was that the rate of arthritis is likely higher among patients who experience a patellar dislocation compared with those who do not. STUDY DESIGN: Cohort study;

Superior patellar dislocation: the value of clinical examination and radiological investigation.

A 57-year-old obese woman with a history of osteoarthritis presented with an acutely painful left knee and an inability to bear weight following a twisting injury. The initial diagnosis in A&E was a ruptured patellar tendon. A more thorough clinical examination revealed a high-riding patellar with the knee hyper-extended and a patellar dimple, consistent with superior patellar dislocation. The diagnosis was confirmed with imaging and the patient successfully underwent closed reduction. She re-presented 2 months later with ipsilateral knee pain after climbing a step. The initial diagnosi

Familial association of femoral trochlear dysplasia with recurrent bilateral patellar dislocation.

Femoral trochlear dysplasia is an anatomic deformity that predisposes patients to patellar instability, including patellar subluxation and dislocation, and can lead to severe patellofemoral joint degeneration if left untreated. Femoral trochlear dysplasia leading to recurrent bilateral patellar dislocation has rarely been reported as having a familial association. Orthopedic surgeons who encounter patients presenting with chronic patellar instability with no underlying disease or syndrome should be aware of the presence of femoral trochlear dysplasia leading to recurrent bilateral patellar dis

Results of medial retinacular imbrication in patients with unilateral patellar dislocation.

Many surgical methods are used for treating malaligned patella after patellar dislocation, but one surgical method may not be appropriate for all patients. The purpose of this study was to determine the radiographic and subjective outcomes of patients who underwent imbrication of redundant medial structures. A total of 42 patients met the inclusion criteria of suffering a traumatic, unilateral patellar dislocation and had no J-sign, no patella alta, asymmetry on Merchant view radiograph with normal patella alignment on the noninvolved side, and MRI showing injury to the medial retinaculum. Operative versus non-operative management of patellar dislocation. A meta-analysis.

PURPOSE: Conservative management of patellar dislocation can result in recurrent instability in up to 60% of patients. Surgery is therefore advocated in an attempt to reduce the incidence of recurrent dislocation and instability. The purpose of this study was to compare the clinical outcomes of operative to non-operative treatment strategies for patients following patellar dislocation. METHODS: A systematic review of published and unpublished literature was undertaken. Following data extraction, a meta-analysis was conducted to compare the two treatment strategies. The methodological quality

The mechanism of primary patellar dislocation: trauma history of 126 patients.

BACKGROUND AND PURPOSE: Several mechanisms are responsible for patellar dislocation. We investigated how the primary pathomechanism relates to patient characteristics and the outcome. METHODS: 126 patients (81 females) with primary patellar dislocation reported the knee position before the episode, the movement during it, and whether the patella was locked in dislocation. The median age was 20 (9-47) years. The subjective outcome and Kujala, Hughston VAS, and Tegner scores were evaluated after an average of 7 years. RESULTS: 102 patients moved to flexion during the dislocation, 98 from a str

Sonographic evaluation of the injuries after traumatic patellar dislocation in adolescents.

BACKGROUND: Traumatic patellar dislocation (TPD) may be associated with other injuries requiring additional treatment. The superficial localization of the knee extensor apparatus and knee articular cartilage make them accessible to sonographic evaluation (ultrasonography [USG]). PURPOSE OF THE STUDY: Prospective analysis of the value of USG in TPD. METHODS: In 21 patients, 22 knees were examined by USG after clinical and routine radiographic evaluations. Based on USG findings, operative or nonoperative treatment was done (15 and 7 cases, respectively). RESULTS: In 17 knees, lesions of the m

Treatment of chronic patellar dislocation with a modified Elmslie-Trillat procedure.

BACKGROUND: This paper describes a modification of the Elmslie-Trillat procedure that is usually performed in severe cases of habitual or recurrent patellar instability. METHODS: Eighteen knees (7 men and 8 women) treated for recurrent or habitual patellar dislocation were evaluated clinically and radiographically at a mean follow-up of 5 years (range 24 months to 9 years). The mean age at follow-up was 26.3 years (range 17-44 years). The IKDC and Kujala and Tegner scores were used for the clinical evaluation. Anteroposterior, lateral and Merchant views were done for radiographic monitoring.

Articular hypermobility and chondral injury in patients with acute patellar dislocation.

Thirty adolescents with acute, primary patellar dislocation were assessed for a relationship between articular hypermobility and chondral injury. The 15 patients without articular hypermobility had a 2.5 times increased frequency of articular lesions when compared with 15 patients with hypermobility. Assessment of articular hypermobility should be part of the examination in patients with patellar dislocation.

Surgical management for recurrent patellar dislocations in skeletally immature patients.

BACKGROUND: Due to his multifactorial aetiology, treating patellofemoral instability can be a challenge for the orthopaedic surgeon. The incidence of patellofemoral instability shows a peak during adolescence, especially from 11 to 14 years old. AIM: Several clinical studies focusing on recurrent patellar dislocations in skeletally immature patients have been published, reporting inconsistent or controversial results. Currently, there is a lack of consensus regarding the surgical management of these patients. The purpose of this study is to update current evidence and systematically review in

Risk Factors and Time to Recurrent Ipsilateral and Contralateral Patellar Dislocations.

BACKGROUND: Previous studies have reported variable rates of recurrent lateral patellar instability mainly because of limited cohort sizes. In addition, there is currently a lack of information on contralateral patellar instability. PURPOSE: To evaluate the rate of recurrent ipsilateral patellar dislocations and contralateral patellar dislocations after a first-time lateral patellar dislocation. Additionally, risk factors associated with recurrent dislocations (ipsilateral or contralateral) and time to recurrence were investigated. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS:
