Clinical and pathologic characteristics of focal segmental glomerulosclerosis pathologic variants.

Histologic variants of idiopathic focal segmental glomerulosclerosis (FSGS) may have prognostic value. A recent working classification system has distinguished five FSGS variants. We evaluated a cohort of adult patients with biopsy-proven FSGS diagnosed between March 1982 and July 2001 to determine if subtypes were associated with renal outcome. Renal biopsies were reviewed by two pathologists. Demographic and clinical data were obtained from charts. Outcomes were partial and complete remission of the nephrotic syndrome, and renal failure. The frequency of FSGS variants was: 3% cellular (N=6),

Tau-Clinical Mismatch Identifies Individuals with Co-Pathology and Predicts Clinical Trajectory.

IMPORTANCE: The heterogeneous course of Alzheimer's disease makes it difficult to predict individuals' cognitive trajectories, which is particularly important in the era of disease modifying therapy. Identifying individuals more likely to have co-pathology and differing disease courses using clinically practical tools remains a critical gap. OBJECTIVE: To evaluate tau-clinical mismatch for identifying resilient and vulnerable individuals and compare levels of co-pathology and clinical trajectories between groups. DESIGN SETTING AND PARTICIPANTS: Participants were selected from the Alzheimer'

A conceptual framework for the diagnostic pathology of anxiety disorders: Clinical and pathological tetrads for identifying pathological anxiety.

Anxiety disorders (ADs) are among the most prevalent psychiatric conditions and contribute substantially to disability, healthcare burden, and impaired quality of life. However, current diagnostic systems rely primarily on descriptive psychopathology and symptom-based classifications, providing limited guidance for distinguishing adaptive from pathological anxiety during clinical assessment. The aim is to propose a Diagnostic Pathology Framework for ADs that operationalizes the distinction between adaptive and pathological anxiety using clinically identifiable markers. This conceptual paper pr

Pulmonary sarcoidosis: from clinical features to pathology- narrative review.

Sarcoidosis is a multisystem disease with unknown causes. The prevalence of sarcoidosis that occurs worldwide is highly variable. It is pathologically characterized by the formation of non-necrotizing epithelioid cell granuloma, mainly affecting the respiratory tract and involving extrapulmonary organs including the skin, heart, extrathoracic lymph nodes, central nerves and eyes. A correct diagnosis of sarcoidosis depends on the clinical symptoms, imaging observations, and characteristic histopathological findings. This study aims to review the recent contributions of pulmonary sarcoidosis. It

[Clinical case of combined genetic pathology in a patient].

Family hypercholesterolemia (HSX) is a form of genetically deterministic increase in blood lipid levels associated with a high risk of cardiovascular disease, usually at a young age. HSX is a common genetic disease found in the general population in most countries in 1:500 people. Clinically xantomas are found in achilles tendor and wrist flexors, lipoid arc of the cornea, concentration of total cholesterol and low-density lipoproteins is 4.911.6 mmol/l. Gilberts syndrome is a hereditary benign hyperbilirubinium, associated with a decrease in the functional activity of the liver enzyme uridinf

Carcinoid Heart Disease: Pathophysiology, Pathology, Clinical Manifestations, and Management.

Carcinoid heart disease (CHD) is a rare and potentially lethal manifestation of an advanced carcinoid (neuroendocrine) tumor. The pathophysiology of CHD is related to vasoactive substances secreted by the tumor, of which serotonin is most prominent in the pathophysiology of CHD. Serotonin stimulates fibroblast growth and fibrogenesis, which can lead to cardiac valvular fibrosis. CHD primarily affects right heart valves, causing tricuspid and pulmonic regurgitation and less frequently stenosis of these valves. Left heart valves are usually spared because vasoactive substances such as serotonin

Eosinophilic Granulomatosis with Polyangiitis: Clinical Pathology Conference and Review.

Eosinophilic granulomatosis with polyangiitis (EGPA) (formerly Churg-Strauss syndrome) is a small vessel vasculitis associated with asthma and eosinophilia. Despite its rarity, continuous gains are being made in understanding the disease with knowledge advancements regarding its epidemiology, heterogeneous clinical manifestations, management, and outcomes. Large knowledge gaps remain, however, particularly surrounding pathophysiologic and diagnostic uncertainties. There is still an incomplete understanding of the interplay between the eosinophilic and vasculitic processes that are features of

Immune myopathies with perimysial pathology: Clinical and laboratory features.

OBJECTIVE: Immune myopathies with perimysial pathology (IMPP) have a combination of damage to perimysial connective tissue and muscle fiber necrosis, more prominent near the perimysium. We studied the clinical and laboratory correlates of patients with pathologically defined IMPP. METHODS: This is a retrospective chart and pathology review of 57 consecutive patients with IMPP myopathology and, for comparison, 20 patients with dermatomyositis with vascular pathology (DM-VP). RESULTS: Compared with DM-VP, IMPP patients more commonly had interstitial lung disease (ILD) (p < 0.01), Raynaud pheno

Anaplastic large cell lymphoma: pathology, genetics, and clinical aspects.

Anaplastic large cell lymphoma (ALCL) was first described in 1985 as a large-cell neoplasm with anaplastic morphology immunostained by the Ki-1 antibody, which recognizes CD30. In 1994, the nucleophosmin (NPM)-anaplastic lymphoma kinase (ALK) fusion receptor tyrosine kinase was identified in a subset of patients, leading to subdivision of this disease into ALK-positive and -negative ALCL in the present World Health Organization classification. Due to variations in morphology and immunophenotype, which may sometimes be atypical for lymphoma, many differential diagnoses should be considered, inc

Molecular Pathology and Novel Clinical Therapy for Uterine Leiomyosarcoma.

Patients with uterine leiomyosarcoma (LMS) typically present with vaginal bleeding, pain, and a pelvic mass, with atypical presentations of hypercalcemia and eosinophilia also being reported. Radiographic evaluation with combined positron-emission tomography/computed tomography may assist in diagnosis and surveillance in women with uterine LMS; these are commonly used with stage and tumour grade as prognostic indicators and a recently developed risk-assessment index to predict disease-specific survival. Recent studies have shown that the addition of adjuvant therapy after surgical management d

Chinese Erdheim-Chester disease: clinical-pathology-PET/CT updates.

UNLABELLED: Erdheim-Chester disease (ECD), one type of systemic non-Langerhans cell histiocytosis, has been rarely seen and is characterized by the accumulation of foamy CD68+CD1a- histiocytes. We reported a case of ECD and reviewed the clinical features of 13 cases of ECD reported so far in China. A 53-year-old male was diagnosed with central diabetes insipidus in March 2014, followed by fever, splenomegaly and anemia in July 2014. His initial pituitary magnetic resonance imaging (MRI) revealed the absence of high signal at T1-weighted image in posterior pituitary without any lesion. A furthe

Trueperella pyogenes infections in swine: clinical course and pathology.

Trueperella pyogenes is an opportunistic pathogen causing purulent infections in pigs and other animal species. T. pyogenes infections in pigs are local and/or generalized depending on the immune status of the animals, their individual susceptibility and environmental factors. The occurrence of these infections on pig farms causes substantial economic losses in breeding and rearing. In sows from the breeding herd, the disease leads to infertility, embryonic death, abortion, and disorders of the menstrual cycle and lactation. Mastitis is the major cause of losses in piglets. Disorders of the mu
