Reliance Management in Healthcare for Professional and Elite Athletes: Ethics, Patient-Centered Care, and Governance.

Professional and elite sport represents a high-pressure healthcare environment in which standard principles of patient-centered care, confidentiality, informed consent, shared decision-making, and clinical independence are tested by organizational and performance-related pressures. This narrative review examines reliance management in athlete-physician relationships. Reliance is conceptually distinguished from trust: whereas trust is an interpersonal attitude that can be betrayed, reliance denotes the broader dependence of the athlete-patient on the physician, the care process, and the surroun

Improving the patient care pathway and multidisciplinary management are major challenges in Hidradenitis suppurativa treatment.

Hidradenitis suppurativa (HS) is a chronic inflammatory disease resulting from the close interplay between genetic predisposition, dysregulation of the mechanisms involved in autoinflammatory pathways, dysfunctional response to bacterial biofilm changes, and environmental factors that potentially exacerbate the disease. Its cutaneous manifestations may represent just the tip of the iceberg, as numerous comorbidities may be associated with the disease. The risk of occurrence of inflammatory bowel disease is three times higher than that in the general population. At least 5% of patients also hav

Enhancing patient-centric care: the role of PROMs utilizing SRS-30 in pediatric scoliosis management.

BACKGROUND: Scoliosis is defined by a curvature of the spine greater than 10 degrees. The most common type of scoliosis is called Adolescent Idiopathic Scoliosis and is found in individuals between 11 to 18 years of age. It corresponds to 90% of the cases of scoliosis in the pediatric population, with an overall prevalence of 0.47-5.2%, affecting girls more than boys (3:1). There are different treatment options for scoliosis, and surgery is reserved for patients with curves greater than 45 degrees while still growing or greater than 50 degrees for skeletally mature patients. There is a growing

Predicting intubation for intensive care units patients: A deep learning approach to improve patient management.

OBJECTIVE: For patients in the Intensive Care Unit (ICU), the timing of intubation has a significant association with patients' outcomes. However, accurate prediction of the timing of intubation remains an unsolved challenge due to the noisy, sparse, heterogeneous, and unbalanced nature of ICU data. In this study, our objective is to develop a workflow for pre-processing ICU data and to develop a customized deep learning model to predict the need for intubation. METHODS: To improve the prediction accuracy, we transform the intubation prediction task into a time series classification task. We

Type 1 Diabetes Mellitus Management and Islet Cell Therapy: A New Chapter in Patient Care.

Diabetes mellitus (DM) is a complex endocrine disorder characterized by abnormally high levels of glucose, also called hyperglycemia. DM usually occurs when the body does not produce enough insulin or cannot respond to the insulin in the body. Type 1 diabetes mellitus (T1DM) or insulin-dependent diabetes is an autoimmune disease that affects around 8 million people in the world. Patients with T1DM experience an array of symptoms such as polyuria, polydipsia, and weight loss. These patients are prone to immediate life-threatening complications, including hypoglycemia and diabetic ketoacidosis.

Adult patient perspectives on phenylketonuria care: Highlighting the need for dedicated adult management and services.

Phenylketonuria (PKU) is an autosomal recessive metabolic disorder characterized by an inability to metabolize the amino acid phenylalanine (Phe). If left untreated, an accumulation of Phe results in neurodevelopmental, neurological and psychological impairments. Advancements in detection and treatment of PKU have improved outcomes and life expectancy for these patients, emphasizing the need for life-long, specialized care. Due to the paucity of adult-focused PKU clinics, patients who are well into adulthood are still being treated in pediatric centers. This retrospective study evaluates the p

Improving management and patient care in lentigo maligna by mapping with in vivo confocal microscopy.

IMPORTANCE: Lentigo maligna (LM) is a clinical, pathologic, and therapeutic challenge with a higher risk of local recurrence than other types of melanoma correctly treated and also carries the cosmetically sensitive localization of head and neck. OBJECTIVE: To determine whether in vivo reflectance confocal microscopy (RCM) mapping of difficult LM cases might alter patient care and management. DESIGN: Analysis of LM and LM melanoma (LMM) in a series of patients with large facial lesions requiring complex reconstructive surgery and/or recurrent or poorly delineated lesions at any body sites we

Management of arterial lines and blood sampling in intensive care: a threat to patient safety.

In 2008, the UK National Patient Safety Agency (NPSA) made recommendations for safe arterial line management. Following a patient safety incident in our intensive care unit (ICU), we surveyed current practice in arterial line management and determined whether these recommendations had been adopted. We contacted all 241 adult ICUs in the UK; 228 (94.6%) completed the survey. Some NPSA recommendations have been widely implemented - use of sodium chloride 0.9% as flush fluid, two-person checking of fluids before use - and their practice was consistent. Others have been incompletely implemented an

A randomized study comparing a patient-directed hypertension management strategy with usual office-based care.

This study aimed to compare the efficacy of a patient-directed management strategy with office-based management in maintaining blood pressure control in patients with chronic stable hypertension using a randomized trial of two months duration. The subjects had chronic stable essential hypertension without secondary causes or unstable cardiovascular disease and were selected through the offices of 11 family physicians and a tertiary care hypertension research unit. Patients were randomly assigned (2:1 ratio) to either a patient-directed management strategy using home blood pressure monitoring t

The effectiveness of patient-centered care vs. usual care in type 2 diabetes self-management: A systematic review and meta-analysis.

BACKGROUND: Patient-centered care in diabetes self-management might be a significant factor in improving health outcomes of adults with type 2 diabetes, yet the supporting evidence is inadequate. This review aimed at assessing the effectiveness of patient-centered self-management care interventions on glycemic control (HbA1c) and self-care behaviors compared with usual care. METHODS: CINAHL, PubMed, Cochrane Library, Google Scholar, and the HEC Digital Library were searched for studies in English language that assessed patient-centered self-management educational and/or behavioral interventio

Out-of-hours primary care management for patients with heart failure.

INTRODUCTION: Little is known about the management and disease trajectories of heart failure (HF) patients in the pre-hospital setting when experiencing exacerbating symptoms. Shortness of breath (SOB) is a main reason for contacting out-of-hours primary care (OHS-PC). We aim to describe the care trajectories of these patients with HF at the OHS-PC with exacerbating symptom SOB and to assess 6-month outcomes regarding mortality and hospital admissions. METHODS: We included patients who contacted Dutch OHS-PC for SOB between September 2020 and August 2021. We selected those in whom HF was cons

Point-of-care, goal-directed management of bleeding in trauma patients.

PURPOSE OF REVIEW: The purpose of this review is to consider the clinical value of point-of-care (POC) testing in coagulopathic trauma patients with traumatic brain injury (TBI) and trauma-induced coagulopathy (TIC). RECENT FINDINGS: Patients suffering from severe TBI or TIC are at risk of developing pronounced haemostatic disorders. Standard coagulation tests (SCTs) are insufficient to reflect the complexity of these coagulopathies. Recent evidence has shown that viscoelastic tests (VETs) identify haemostatic disorders more rapidly and in more detail than SCTs. Moreover, VET results can guid

[Critical care management of patients with spontaneous subarachnoid haemorrhage - a review].

Spontaneous subarachnoid haemorrhage is characterized by extravasation of blood into the subarachnoid space without a preceding trauma. The leading cause is a ruptured intracranial aneurysm. Serious neurologic complications can occur, such as rebleeding, cerebral vasospasm and delayed cerebral ischemia. Subarachnoid haemorrhage is a serious condition with a high mortality rate and those who survive often suffer long-term consequences. Prevention of rebleeding by aneurysm repair is essential and guidelines recommend this procedure should be done as soon as possible or within 72 hours. Managemen

Managing multimorbidity in primary care in patients with chronic respiratory conditions.

The term multimorbidity is usually defined as the coexistence of two or more chronic conditions within an individual, whereas the term comorbidity traditionally describes patients with an index condition and one or more additional conditions. Multimorbidity of chronic conditions markedly worsens outcomes in patients, increases treatment burden and increases health service costs. Although patients with chronic respiratory disease often have physical comorbidities, they also commonly experience psychological problems such as depression and anxiety. Multimorbidity is associated with increased hea

Management of acne scars: fulfilling our duty of care for patients.

Inflammatory lesions commonly associated with acne have a propensity to form scars which can substantially impact an individual's quality of life. As a result of variations in collagen levels during the wound-healing process, different types of scars can form requiring a tailored approach to treatment; treatment success depends on the modality of the treatment chosen. Here, I briefly discuss the different acne scar types, and their relationship with skin anatomy and histology, in relation to the different treatment options available. This paper was developed to accompany my state of the art pr
