Reasons for Not Participating in Scleroderma Patient Support Groups: A Cross-Sectional Study.

OBJECTIVE: Peer-led support groups are an important resource for many people with scleroderma (systemic sclerosis; SSc). Little is known, however, about barriers to participation. The objective of this study was to identify reasons why some people with SSc do not participate in SSc support groups. METHODS: A 21-item survey was used to assess reasons for nonattendance among SSc patients in Canada and the US. Exploratory factor analysis (EFA) was conducted, using the software MPlus 7, to group reasons for nonattendance into themes. RESULTS: A total of 242 people (202 women) with SSc completed

Participation of the thalamic CM-Pf complex in movement performance in patients with dystonia.

INTRODUCTION: The centrum medianum- parafascicular complex of the human thalamus has a critical influence on cortical activity and significantly influences somatosensory function, arousal, and attention. In addition to its cortical connections, this region of the intralaminar thalamic nuclei is also connected to motor areas of the basal ganglia and the brain stem. OBJECTIVE: The goal of this study was to identify movement-related neurons in the centrum medianum-parafascicular complex and analyze the changes in their activity during voluntary movements in patients with cervical dystonia. Rehabilitation in Patients With Coronary Heart Disease: Participation and Its Effect on Prognosis.

BACKGROUND: In Germany, rehabilitation is considered to be indicated after an acute hospital stay for the treatment of a severe cardiac condition. In comparative studies, at least 51% of German hospital patients with coronary heart disease (CHD) who were entitled to rehabilitative measures actually took part n rehabilitation. METHODS: We examined data on 1910 patients with CHD who took part in two prospective cohort studies at the University Hospital of Halle (Saale) in the years 2007-2011. We contacted these patients again with a questionnaire to determine which ones had undergone rehabilita

Patients' rationale for declining participation in a cancer-associated weight loss study.

BACKGROUND: Fewer than 5 % of cancer patients participate in clinical research. Although this paltry rate has led to extensive research on this topic, previous studies have not sought verbatim comments in a real-time, comprehensive manner to understand why patients decline. METHODS: This study used a low-risk, non-interventional parent study that focused on cancer-associated weight loss to understand patients' reasons for declining research participation. A research assistant wrote down the name and verbatim reason of all patients who declined to participate. These comments with accompanying

Attitudes and expectations of patients with neuromuscular diseases about their participation in a clinical trial.

AIM: This study aimed to gain a better understanding of the psychological impact of participating in a clinical trial for patients with Pompe disease (Acid Maltase Deficiency). Attitudes and expectations of adult patients with neuromuscular diseases regarding medical trials are as yet unreported. In order to learn about the psychological consequences of participating in a clinical trial, we conducted a prospective assessment of patients with late-onset Pompe Disease, a rare genetic condition, for which no treatment had been available before. This psychological study was carried out as an ancil

Participation and social functioning in patients with fibromyalgia: development and testing of a new questionnaire.

BACKGROUND: While there are numerous instruments for capturing the symptoms of fibromyalgia syndrome (FMS) patients, there is a lack of questionnaires capable of measuring in detail FMS patients' participation and social functioning. It was our aim to develop and methodologically test a new patient questionnaire specific to FMS measuring these concepts (the "Fibromyalgia Participation Questionnaire" FPQ). METHODS: We first conducted a qualitative prestudy (focus groups, N = 38) to identify which impairments FMS patients experience in daily life because of their illness. To analyze the data we

How do clinical trial participants compare to other patients with schizophrenia?

OBJECTIVE: Patients with schizophrenia enrolled in a trial of long-acting injectable risperidone at multiple sites of the Veterans Health Administration (VHA). We considered if the trial participants were representative of the targeted group of high-utilization patients with poor adherence to anti-psychotics. METHODS: Participants' characteristics, health services utilization, and cost in the year prior to randomization were compared to a randomly selected time-matched cohort of 10,000 other patients with schizophrenia who were not in the trial. RESULTS: There were few differences in the cha

Prevalence and improvement in psychopathology in opioid dependent patients participating in methadone maintenance.

Questions continue in the literature concerning potential cause and effect relationships between opiate dependency and several organically-based psychiatric disorders. For example, does opiate dependency produce secondary anxiety and dysthymic syndromes in otherwise healthy persons? or is narcotics misuse by a patient an attempt to self-medicate pre-existing psychopathology? Does the severity of psychopathologic symptoms decrease with time in treatment? To resolve such questions, we routinely conduct psychiatric evaluations on all opioid dependent patients enrolled into methadone maintenance.

Planned time burden for patients participating in phase 2 cancer trials.

BACKGROUND: Participation in cancer trials requires a commitment of time and research burden from patients. Prior work has highlighted the time burden of clinic visits during phase 1 trial participation. We extended these findings to phase 2 cancer trials. MATERIALS AND METHODS: We reviewed a random sample of 73 phase 2 cancer trial arms with results posted on ClinicalTrials.gov 2023-2024. We extracted scheduled visit frequency and procedures from each protocol. Primary outcomes included the absolute number of planned research days (PRDs) in the first 30 days and subsequent months of particip

Psychosocial predictors for cardiac rehabilitation participation for patients with coronary artery disease.

AIMS: Cardiac rehabilitation (CR) is a class Ia recommendation by international guidelines. However, the participation remains disappointingly low. Multiple barriers were already identified. Nevertheless, the relationship with psychosocial factors remains unsettled. The aim of this study is to analyse the role of psychosocial factors in CR participation. METHODS AND RESULTS: A prospective study on 165 patients with a percutaneous coronary intervention. The main outcome was participation in a centre-based CR programme. Exploring patient-reported barriers to participating in proton therapy clinical trials.

INTRODUCTION: Clinical trials lead the progress in healthcare. To ensure reliable research conclusions, it is essential to enroll diverse patient groups. Identifying and understanding patient-reported barriers to clinical trials may help enhance recruitment among diverse patient groups. The clinical potential of proton therapy (PT) to reduce late effects is being investigated in clinical trials worldwide. Thus, for some patients, PT is only accessible by participating in clinical trials.Individuals with smoking-related head and neck cancer (HNC) are sometimes socioeconomically deprived, leading

Exercise Participation for Patients with Valvular Heart Disease: a Review of the Current Guidelines.

PURPOSE OF REVIEW: Valvular heart disease affects many individuals who aspire to partake in competitive or recreational sports. This manuscript reviews the most recent European and American guidelines related to exercise and sport participation in individuals with valvular heart disease (VHD) and identifies areas not addressed by these guidelines. RECENT FINDINGS: Exercise recommendations for individuals with VHD have been presented since at least 1984. There is limited data on the impact of intensive physical activity on the progression and outcomes of VHD. Therefore, current recommendations

'Sick and tired': Patients reported reasons for not participating in clinical psychiatric research.

BACKGROUND: Meaningful and generalizable research depends on patients' willingness to participate. Studies often fail to reach satisfactory representativeness. OBJECTIVE: This paper aims to investigate reasons for not participating in research among young adult patients with psychiatric illness. METHOD: A quantitative cross-sectional study was performed based on questionnaires reported on by 51 psychiatric patients (14 males, 35 females and two unspecified) who had previously declined participation in an ongoing research project. Thereafter, a qualitative interview with subsequent content an

Glutathione system participation in thoracic aneurysms from patients with Marfan syndrome.

BACKGROUND: Aortic dilatation in Marfan syndrome (MFS) is progressive. It is associated with oxidative stress and endothelial dysfunction that contribute to the early acute dissection of the vessel and can result in rupture of the aorta and sudden death. We evaluated the participation of the glutathione (GSH) system, which could be involved in the mechanisms that promote the formation and progression of the aortic aneurysms in MFS patients. PATIENTS AND METHODS: Aortic aneurysm tissue was obtained during chest surgery from eight control subjects and 14 MFS patients. Spectrophotometrical deter

Participation of oleic acid in the formation of the aortic aneurysm in Marfan syndrome patients.

Marfan syndrome (MFS) is associated with progressive aortic dilatation and endothelial dysfunction that lead to early acute dissection and rupture of the aorta and sudden death. Alteration in fatty acid (FA) metabolism can stimulate nitric oxide (NO) overproduction which increases the activity of the inducible form of NO synthase (iNOS) that is involved in endothelial dysfunction. We evaluated the participation of FA in the formation of thoracic aneurysms in MFS and its relation to the iNOS. Oleic acid (OA), iNOS, citrulline, nitrates and nitrites, TGF-β1, TNF-α, monounsaturated FA and NO synt

Motivations of patients with pulmonary arterial hypertension to participate in randomized clinical trials.

BACKGROUND: There is substantial need to rigorously evaluate existing and new therapies for pulmonary arterial hypertension (PAH) and other severe and relatively rare conditions affecting younger patients. However, the ability to conduct meaningful randomized clinical trials (RCTs) in such contexts often is limited by difficulties obtaining adequate patient enrollment. PURPOSE: To understand the motivations of patients with PAH for participating in RCTs so as to facilitate enrollment in future trials among patients with similar diseases. METHODS: We conducted semistructured interviews of a d
