Mandibular positioning using patient-specific guides and osteosynthesis implants in bimaxillary orthognathic surgery with maxilla-first approach. A 3D-analysis.

The aim of this study was to evaluate the accuracy of a fully 3D planned digital workflow for bimaxillary osteotomies, utilizing patient-specific osteotomy and drill guides (PSDOG) and osteosynthesis implants (PSOI), within a maxilla-first approach. Emphasis was placed on the accuracy of guided mandibular positioning. This retrospective study included 30 patients undergoing bimaxillary orthognathic surgery [bimaxillary PSDOG/PSOI (Group 1; n = 11) versus maxillary PSDOG/PSOI (Group 2; n = 19)]. 3D CT datasets were used for accuracy assessment. Primary outcome parameter was the accuracy of the

Management of a Patient With Ankylosing Spondylitis and Multiple Comorbidities Who Underwent a Percutaneous Nephrolithotomy Using an Innovative Positioning Strategy.

Ankylosing spondylitis (AS) is a chronic inflammatory disorder leading to bony ankylosis, ossification, and fibrosis of the spine. Airway management is difficult in these patients due to restricted head and neck movement as well as a stiff body posture. This also poses challenges in lying down supine as well as surgical positioning. We report a case of a patient with AS and multiple co-morbidities who underwent a percutaneous nephrolithotomy for renal calculi, and the customisations made in anaesthetic and surgical techniques to safely perform the procedure.

Dual Application of Patient-Specific Occlusion-Based Positioning Guide and Fibular Cutting Guide for Accurate Reconstruction of Segmental Mandibular Defect.

In this study, the authors introduced a dual application of patient-specific occlusion-based positioning guide and fibular cutting guide to obtain ideal occlusal relationship and mandibular contour in patients undergoing mandibular reconstruction. A retrospective review was performed in 21 patients who underwent mandibular reconstruction with a fibular osteocutaneous free flap. Using computed tomography and intraoral scanning data, fibular cutting guide and occlusion-based positioning guide were simulated in a modeling software and 3-dimensionally printed. Both guides were applied in 9 patient

Operative Positioning Technique for an Intertrochanteric Fracture in a Patient with an Ipsilateral Below-the-Knee Amputation: A Case Report.

CASE: A 56-year-old man with a remote history of a left below-the-knee amputation (BKA) presented to the emergency department with an ipsilateral intertrochanteric fracture (IT). This case produced an operative challenge because commonly used intraoperative traction methods could not be applied to a patient with a BKA. CONCLUSION: We describe a rarely utilized surgical technique of applying traction to an amputated extremity through an inverted boot on a fracture table during open reduction and internal fixation of an IT.

Advanced Monte Carlo Modeling of Automatic Exposure Control Vulnerabilities: Patient Positioning and Metallic Artifact Effects on Emergency CAP CT Dose Distribution.

This study investigates the influence of arm positioning and metallic objects on radiation dose distribution during emergency CT scans, where time-critical workflows demand both speed and precision. Sub-optimal arm positioning and metallic artifacts can significantly alter dose patterns and compromise patient safety. Five clinical cases undergoing chest, abdomen, and pelvis (CAP) CT scans with automatic exposure control (AEC) at 120 kVp were analyzed using GATE/Geant4 Monte Carlo simulations. Patient-specific voxel models were generated from clinical CT datasets, and the scanner was modeled wi

Effect of positioning and expiratory rib-cage compression on atelectasis in a patient who required prolonged mechanical ventilation: a case report.

BACKGROUND: Pulmonary complications can be caused by intraoperative mechanical ventilation. In particular, prolonged mechanical ventilation is associated with a high mortality rate, a risk of pulmonary complications, prolonged hospitalization, and an unfavorable discharge destination. Pre- and postoperative rehabilitation are important for the resolution of pulmonary complications in acute cases. However, there has been a lack of studies on interventions for pulmonary rehabilitation of patients with chronic pulmonary complications caused by prolonged mechanical ventilation. Accordingly, we des

Beach-Chair Versus Lateral Decubitus Positioning for Arthroscopic Posterior Shoulder Labral Repair: A Retrospective Comparison of Clinical and Patient-Reported Outcomes.

BACKGROUND: Both beach-chair and lateral decubitus patient positioning are often utilized for shoulder arthroscopy, with each offering its unique advantages and disadvantages. The surgical position is often selected according to each surgeon's preference, with no clear superiority of one position over the other. PURPOSE/HYPOTHESIS: The purpose was to compare clinical and patient-reported outcomes between patients who underwent arthroscopic posterior labral repair in the beach-chair versus the lateral decubitus position. We hypothesized that patient positioning would not affect clinical and pa

A method for positioning the premaxilla during impression making for a patient with bilateral cleft lip and palate: a clinical report.

In patients with bilateral cleft lip and palate (BCLP), orthodontic and facial orthopedic treatments alone occasionally fail to resolve problems because of the absence of alveolar and palatal hard and soft tissue or contraction resulting from previous surgical repair. Prosthetic reconstruction of the anterior maxilla is important for these patients. Patients with BCLP have premaxillary displacement in up to 3 dimensions. It may be difficult to make impressions without deformation. This clinical report describes a method for positioning the mobile premaxilla during impression making for a patie

Prone Positioning for Management of Fat Embolism Syndrome in a Patient with Spine Fracture; An Unusual Scenario and Review of Literature.

Fat embolism syndrome is a rare but fatal complication seen commonly in patients with polytrauma. Its earliest manifestation is hypoxemia due to deposition of fat globules in pulmonary circulation which can progress to severe acute respiratory distress syndrome, the treatment of which is mainly supportive. We describe the case of a 17-year-old male who was admitted in our intensive care unit (ICU) for severe hypoxemia due to fat embolism. He had burst fracture of 5th lumbar vertebra with canal compromise along with other fractures. Failing conventional ventilation, the patient was placed in pr

A Simple Technique for the Positioning of a Patient with an above Knee Amputation for an Ipsilateral Extracapsular Hip Fracture Fixation.

The positioning of the patient on the fracture table is critical to the successful reduction and operative fixation of hip fractures which are fixed using the dynamic hip screw system (DHS). There is a standard setup which is commonly used with relative ease. Yet the positioning of patients with amputations either above or below knee of the affected side can pose a significant challenge. We describe a novel positioning technique used on a 51-year old patient with a right above knee amputation who sustained an intertrochanteric extracapsular hip fracture.

Determination of the best point of entry of percutaneous insertion of sacroiliac screws depending on patient positioning for surgery: A cadaveric study.

BACKGROUND: The standard starting point for percutaneous sacroiliac screw insertion was initially determined at the intersection of the line posterior to the anterior superior iliac spine and the line continuing the anatomical axis of the femur. The technique was pioneered in patients lying prone in surgery, although it has been used with patients in the supine position. The optimal starting point for patients in both prone and supine positions remains uncertain. OBJECTIVES: This cadaveric study aimed to determine the best entry point for the percutaneous insertion of sacroiliac screws depend

Comparison of free-hand technique and patient-specific guiding template for pedicle screws positioning in adolescent idiopathic scoliosis surgery using neurophysiological monitoring.

INTRODUCTION: Screws mispositioning is an identifiable cause of intraoperative neurophysiological monitoring changes. To prevent screw mispositioning, a patient-specific guiding template for pedicle screw positioning (PSGT) was introduced to avoid implant mispositioning. The study aims to compare the changes in neurophysiological monitoring and screw mispositioning between free-hand (FH) and PSGT in patients surgically treated for adolescent idiopathic scoliosis (AIS). MATERIALS AND METHODS: All patients treated between January 2016 and December 2021 for AIS surgery, aged 14 to 20, were inclu

Ultrasound-guided stellate ganglion blockade - patient positioning is everything: a case report demonstrating the efficacy of a modified out-of-plane approach.

BACKGROUND: Insomnia has become increasingly prevalent in modern society and is notoriously difficult to treat. Many patients exhibit a poor response to pharmacological interventions. Stellate ganglion block (SGB) has emerged as an effective method for managing insomnia; however, its efficacy may be compromised in some patients, primarily due to a variant vertebral artery anatomy. CASE PRESENTATION: This case report describes a patient with severe insomnia accompanied by anxiety. Through cervical ultrasound scanning, we identified richly branched cervical arteries at the C6-C7 segment of the
