Single-Fraction Radiotherapy (SFRT) For Bone Metastases: Patient Selection And Perspectives.

Bone metastases are a frequent and important source of morbidity in cancer patients. Stereotactic body radiation therapy (SBRT) is an established treatment option for local control and pain relief of bone metastases, and it is increasingly used as upfront treatment, postoperative consolidation or salvage treatment after prior RT. However, heterogeneity of dose schedules described in literature represents a severe limitation in the definition of the role of SBRT as a standard of care. No consensus is available on the use of single versus multiple fraction SBRT for bone metastases. Advantages of

Natural orifice specimen extraction in colorectal surgery: patient selection and perspectives.

Over the past 30 years, colorectal surgery has evolved to include minimally invasive surgical techniques. Minimally invasive surgery is associated with reduced postoperative pain, reduced wound complications, earlier return of bowel function, and possibly shorter length of hospital stay. These benefits have been attributed to a reduction in operative trauma compared to open surgery. The need to extract the specimen in colorectal operations through a "mini-laparotomy" can negate many of the advantages of minimally invasive surgery. Natural orifice specimen extraction (NOSE) is the opening of a

Patient Selection for Urethroplasty Technique: Excision and Primary Reanastomosis Versus Graft.

There are many management options for patients with urethral stricture disease. Regarding strictures of the bulbar urethra, ideal management strategies remain controversial. This article reviews important anatomic principles for surgeons to consider when repairing bulbar urethral strictures. Specific presenting features of a bulbar urethral stricture that can serve as a guide for the optimal surgical approach are described. Patient characteristics that are important and may have an impact on the outcome of repair are discussed. The optimal repair of a bulbar urethral stricture should be guided

[Catheter based mitral valve repair for severe mitral regurgitation: patient selection].

The catheter-based mitral valve repair is a novel technology for the treatment of severe mitral regurgitation (MR). This technique is suitable for elderly patients with pronounced co-morbidities who are deemed to be high risk for conventional heart surgery. A meaningful reduction of mitral regurgitation leads to improvement of symptoms and quality of life. Studies also demonstrate reverse remodeling of the left ventricle. In heart failure patients with severe MR percutaneous repair reduces re-hospitalization rates>50% in comparison to optimal medical treatment. For degenerative MR conventional

Percutaneous repair of paravalvular prosthetic regurgitation: patient selection, techniques and outcomes.

Paravalvular prosthetic regurgitation is common, affecting 5-10% of surgical prostheses and 40-70% of transcatheter valves. While many patients may suffer no significant morbidity, paravalvular prosthetic regurgitation can lead to heart failure and haemolytic anaemia, and, in some studies, has been associated with impaired survival. Over the past several years, percutaneous repair of paravalvular prosthetic regurgitation has been demonstrated to be a highly efficacious therapy. When performed in experienced centres, procedural success with percutaneous repair occurs in 90% of patients. Due to

Results of Endoscopic Endonasal Dacryocystorhinostomy; Necessity of Teamwork and Patient Selection.

Our aim was to evaluate the clinical results of endoscopic endonasal surgical dacryocystorhinostomy (EES-DCR) as team work by an ophthalmologist and an ear-nose-throat (ENT) surgeon and the appropriate selection of the patients. All candidates for DCR underwent computed tomography (CT) scan of the paranasal sinuses (PNS). Patients who did not want a scar on the medial canthus skin or who had intranasal problems received EES-DCR, which was performed as team work by an ophthalmologist and an ENT surgeon. Surgical success was the resolution of epiphora (i.e., functional success) and free passage

Percutaneous spinal cord stimulation for chronic pain: indications and patient selection.

Percutaneous spinal cord stimulation has a clinical benefit in selected patients with chronic neuropathic pain related to failed back surgery syndrome and complex regional pain syndrome. Patients with other peripheral neuropathic pain syndromes may also respond favorably. The cause of the chronic pain must be established to rule out remediable causes. Drug abuse or aberrant drug-related behaviors should be assessed before proceeding. A psychological evaluation identifies patients who may not benefit. Risk factors for infection should be corrected where possible. This safe, reversible treatment

Immediate placement of implants in breast reconstruction: patient selection and outcomes.

BACKGROUND: With the advent of skin-sparing mastectomy techniques, it became clear that immediate placement of an implant could be utilized for breast reconstruction in select patients. The authors assessed the safety, patient selection factors, and aesthetic results with this technique. METHODS: Thirty-five consecutive patients (eight unilateral and 27 bilateral) who underwent immediate implant-based breast reconstruction were analyzed. Patient data and complication rates were obtained from a retrospective chart review. Postoperative photographs were evaluated by a blinded panel and scored o

Influence of patient age on procedural selection in mitral valve surgery.

BACKGROUND: Previous studies suggest that mitral valve replacement is comparable to repair in the elderly, and a national trend exists toward tissue valves. However, few direct comparison data are available, and this study evaluated the effects of patient age on risk-adjusted survival after mitral procedures. METHODS: From 1986 to 2006, 2,064 patients underwent isolated primary mitral operations (±CABG). Maximal follow-up was 20 years with a median of 5 years. Valve disease etiology was the following: degenerative, 864; ischemic, 450; rheumatic, 416; endocarditis, 98; and "other," 236. Overal

Patient selection for neonatal extracorporeal membrane oxygenation: beyond severity of illness.

OBJECTIVE: To explore how neonates with respiratory failure are selected for extracorporeal membrane oxygenation (ECMO) once severity of illness criteria are met, and to determine how conflicts between ECMO providers and parents over the initiation of ECMO are addressed. STUDY DESIGN: A cross-sectional study was conducted using a data collection survey, which was sent to the directors of neonatal respiratory ECMO centers. RESULT: The lowest birth weight and gestational age at which respondents would consider placing a neonate on ECMO were frequently below recommended thresholds. There was wi

Patient selection and surgical technique for surface arthroplasty of the hip.

Factors such as patient selection, surgical technique, and durable fixation of the components are critical in surface arthroplasty of the hip. The Surface Arthroplasty Risk Index helps determine the risk of failure, with an index score </=3 having a 97% survivorship at 4 years. Because surface arthroplasty of the hip is conservative in nature and has the goal to closely reproduce the normal anatomy of the proximal femur, surgeons performing this procedure need to take into consideration the underlying pathology that led to the degenerative changes. Consequently, choice of surgical approach and

Ambulatory surgery adult patient selection criteria - a survey of Canadian anesthesiologists.

PURPOSE: An increasing number of patients with complex medical problems are now considered suitable for ambulatory surgery. The purpose of this study was to identify the current clinical practice of ambulatory surgical patient selection. METHODS: A standardized questionnaire specifying 30 clinical conditions was sent to all practicing anesthesiologists who are members of the Canadian Anesthesiologists' Society. Recipients were asked to indicate if they would provide ambulatory anesthesia (yes/no answers) for an adult patient with each of those isolated conditions. A 75% agreement was consider

Patient selection reduces thrombotic complications of emergent stenting for failed PTCA.

A comparison of consecutive initial and later patients receiving emergent coronary artery stents to salvage failed PTCA procedures was made to determine whether experience with this procedure can improve patient outcome. Twenty-six consecutive patients underwent emergent, unplanned implantation of one or more Palmaz-Schatz coronary stents at our institution over a 3-1/2 year period. Indications for stent insertion included impending vessel closure (decrease in TIMI flow grade and clinical evidence of ischemia) or acute occlusion (TIMI flow grade 0 or 1 and > or = 99% stenosis) after PTCA. [Percutaneous automated lumbar nucleotomy. Patient selection, practical performance and clinical results].

Percutaneous nucleotomy is designed to treat small and medium-sized contained disk hernias. It is offered to patients who have not responded to conservative treatment for three months or more. The nucleotomy procedure is well tolerated by a majority of patients and the rate of complications is low. The patients can be treated as out-patients. Among a total material of 130 patients, a primary good response to the treatment was observed in 81 (62%). However, owing to recurrencies the success rate had fallen to 52% at 12-39 months (mean 22 months) after nucleotomy. In selected patients with sympt

Controversies in percutaneous balloon pulmonary valvuloplasty: timing, patient selection and technique.

Balloon valvuloplasty is currently the preferred therapeutic alternative to surgical valvotomy in selected patients with isolated congenital pulmonary stenosis. Not only does pulmonary valvuloplasty consistently afford safe and dramatic reduction of the valvular obstruction, it also produces less trauma and an equivalent sustained gradient relief when compared to surgical intervention. However, despite its established role, controversies persist in certain aspects of balloon pulmonary valvuloplasty (BPV). They include the timing of intervention, the suitability of certain valve morphologies an

Patient selection and results of cardiac transplantation in patients with cardiomyopathy.

Cardiac transplantation for the treatment of end-stage congestive heart failure has been shown to be of benefit regardless of the etiology. With few exceptions, the evaluation of patients with end-stage heart failure is the same, regardless of the etiology. In those with cardiomyopathy not as a result of CAD, special attention must be given to exclude secondary causes of cardiomyopathy such as amyloidosis, hemochromatosis, and sarcoidosis, as well as generalized systemic illnesses that may also involve the heart, either secondary or hereditary, because special consideration must be given to th
