BACKGROUND: Pediatric intraoperative emergencies are rare but it is crucial for an anesthesia resident to be proficient in their management. Even the more common emergencies like anaphylaxis may not happen frequently for this proficiency to occur. Simulation increases exposure to these rare events in a safe learning environment to improve skills and build confidence while standardizing curriculum.
OBJECTIVE: Anesthesia residents participated in a simulated case of intraoperative pediatric anaphylaxis to evaluate knowledge and performance gaps. The study also sought to determine whether a diff

Optimal perioperative analgesia for infants and children after major abdominal surgery poses a challenge when central neuraxial techniques are contraindicated. As a regional anesthesia technique, the transversus abdominis plane (TAP) block has been shown to reduce opioid consumption and improve pain scores compared to traditional perioperative pain strategies. Accordingly, TAP blocks may be considered as an alternative to central neuraxial analgesia to optimize perioperative pain control. Advancements in ultrasound technology have further improved the reliability and safety profile of this tec

Peripheral regional anesthesia (PRA) techniques have gained increasing attention as adjuncts to multimodal analgesia and enhanced recovery pathways. This scoping review aims to summarize the current evidence on the use of PRA in pediatric patients undergoing cardiac surgery and seeks to inform clinical practice by answering 10 predetermined research questions. This scoping review adhered to the Preferred Reporting Items for Systematic Reviews and Meta-Analysis Extension for Scoping Reviews guidelines. We searched 6 databases and trial registries up to August 2025. Eligible studies were randomi

BACKGROUND: Computed tomography (CT)-guided cordotomy has been shown to be a safe and effective method for treating cancer pain in adults in a palliative care setting. Its effectiveness has been rarely reported in children. During cordotomy, patient feedback is considered essential. Unfortunately, in this report, the featured patient's age and demeanor precluded an awake cordotomy. The authors report the first case, to their knowledge, of percutaneous cordotomy with the patient under general anesthesia.

BACKGROUND: Pediatric spinal anesthesia is an old technique whose use is not widespread, in spite of reducing the risk of cardiorespiratory events (hypoxemia, bradycardia, and hypotension) associated with general anesthesia, especially in neonates and infants. This retrospective cohort study aimed to assess the safety and effectiveness of the pediatric spinal anesthesia program at our tertiary care hospital over 11 years.
METHODS: Two hundred children, between 8 days and 13 years of age, who underwent lower body surgery under spinal anesthesia from May 2010 to July 2021 were included. Demogra

Pediatric burns are a common and often devastating injury. Treatment, reconstruction, and rehabilitation are painful experiences. For some, the experience triggers post-traumatic stress disorder and/or a chronic pain syndrome. Given the role pain plays as a major secondary disease, it must be addressed to achieve optimal healing. Regional anesthesia has been used extensively to manage postoperative pain and reduce the need for opioids following other surgical procedures in children. However, regional anesthesia is not widely used in pediatric burn care. We present a descriptive, retrospective

Thousands of infants and children undergo complicated surgical procedures that require prolonged periods of anesthesia and/or sedation each year. A growing body of preclinical research suggests pediatric anesthetics are harmful to the developing brain; yet, the extent to which these effects generalize to the clinical setting remains unclear. As there will be a continuing need for surgical and other interventions requiring sedation and/or anesthesia during the neonatal period, it seems clear that research efforts should focus on determining the extent to which general anesthetics can affect the

STUDY OBJECTIVE: To perform a qualitative analysis of noncardiac patients who developed suspected intraoperative supraventricular tachycardia (SVT) during general anesthesia.
DESIGN: Retrospective database analysis and chart review.
SETTING: Operating room of a university-affiliated children's hospital.
MEASUREMENTS: The records of children without cardiac disease who received general anesthesia at The Children's Hospital of Philadelphia from July 1998 through June 2011 were reviewed. Patients with heart rate values above 180 beats per minute were identified, as were specific medications or

AIM: One-Stop Surgery (OSS) is a new day surgery combining preoperative evaluation and subsequent operation in one medical examination.
METHODS: This paper reports the authors' initial experience using this method. Referring physicians were informed of the methods and selected surgical and anesthetic procedures by fax. They were also faxed a form to fill in with case history details and instructions to the parents on the preoperative fast. The completed form was faxed back to the surgeon and pediatric anesthetist who determined patient suitability and scheduled hospitalization and surgery. On

The advantages of pediatric out-patient surgery are: 1) greater psychological ease; 2) lower rate of infection; 3) less impact on patient habits, and 4) lower cost. Surgery must not involve organs, must have a low rate of complications, and be short. The preanesthetic interview should include clinical history and complementary examinations, information on anesthetic technique, perioperative recommendations and psychological preparation of parents and child. Detailed information reassures parents and improves collaboration; their presence during induction may be useful. At this time complete fa

Induction of anesthesia is one of the most distressing moments for pediatric patients during their perioperative experience. They may consequently refuse induction at the last minute. Anesthesia providers are then placed in a challenging ethical dilemma: they must seek to gain the child's cooperation, force the child to undergo induction using restraint, or delay the case. In the moment, it can be difficult to parse the relevant clinical and ethical factors to determine the most appropriate course of action. The purpose of this perspective paper is to provide a clinical ethics framework to sup

Background Tonsillectomy is a surgical procedure primarily aimed at the removal of the tonsils, which are lymphoid tissues located at the back of the throat. This procedure is commonly indicated for conditions such as recurrent tonsillitis, chronic tonsillitis, or obstructive sleep-disordered breathing resulting from enlarged tonsils, which is often conducted under general anesthesia. The purpose of this research is to assess the safety and efficacy of dexmedetomidine-based anesthesia with minimal opioid supplementation in children undergoing adenotonsillectomy. Methods We studied pediatric pa

INTRODUCTION: Proton beam therapy is an oncological treatment, argued to be an appropriate tumor irradiation technique for childhood solid tumors. Due to its duration and the need for immobility, many children require anesthesia for proton therapy sessions. As not many centers in the world provide this therapy, there is little published research about pediatric anesthesia for these cases, and the available data suggest a preference for intravenous anesthesia or combined intravenous and inhalation anesthesia. We conducted this study with the aim of describing and analyzing the inhalation anesth

Acute severe asthma is a commonly encountered condition in the pediatric emergency room and the pediatric intensive care unit (PICU). Its treatment involves the use of bronchodilatory agents acting on different receptors, steroids to reduce ongoing inflammation, and non-invasive or invasive mechanical ventilation to offload the increased work of breathing from the respiratory muscles. Patients refractory to these therapies may require the use of inhaled anesthetic agents and extracorporeal gas exchange (ECMO) for life-threatening asthma exacerbations. Depending on institutional protocols, the

BACKGROUND AND OBJECTIVES: Guillain-Barré syndrome following measles vaccination is uncommon. Diagnosis is often delayed, leading to increased morbidity. This report describes an advanced Guillain-Barré case and the special approaches required during anesthesia.
CASE REPORT: Male patient, four years old, with Guillain-Barré syndrome diagnosed at 1 year of age, submitted to gastrostomy under uneventful general anesthesia with sevoflurane, without neuromuscular blockers.
CONCLUSIONS: The case highlights the low frequency with which this syndrome so important for anesthetic practice is diagnose

Williams syndrome is a rare congenital disorder affecting connective tissue and the cardiovascular and central nervous systems. Pediatric patients diagnosed with Williams syndrome face significant risk for cardiac collapse and death when undergoing anesthesia. We sought to evaluate our institution's historical practices, evaluate individual risk stratification, and create detailed standardized perianesthesia guidelines for management of this population, particularly during noncardiac procedures. The study included a retrospective chart review of pediatric patients with Williams syndrome who re

In this educational article, we summarize the changes in the new European Resuscitation Council guidelines for Pediatric Life Support, emphasizing the most important aspects for the anesthesiologist. Among these are: the use of two-thumb-encircling technique for thorax compressions in infants, 10 ml/kg as the standard volume fluid bolus and ventilation after intubation at an age-dependent rate. Using a fictitious case, we present a point-by-point summary of the changes and briefly mention some of the evidence behind them, referring the reader to the full guidelines for further evidence. We als
