Current Treatment Options and Emerging Technologies for Pelvic Bone Tumors: From Limb Salvage to Precision Oncology.

In our practice, the treatment of pelvic bone tumors should begin with a multidisciplinary sarcoma-board evaluation that integrates biopsy-proven histology, high-resolution CT/MRI/PET imaging, neurovascular and visceral involvement, Enneking zone, expected survival, and patient-specific functional goals. For potentially curable primary pelvic sarcomas, we consider negative-margin resection the non-negotiable oncologic priority. Limb-salvage internal hemipelvectomy is preferred when an R0 resection can be achieved while preserving limb viability and a meaningful postoperative function; external

Keratin Positive Giant Cell Rich Tumor of the Pelvic Bone with a HMGA2::NCOR2 Fusion: A Case Report.

Keratin positive giant cell rich tumor is a rare mesenchymal tumor first described in 2020. It can occur in both soft tissue and bone and predominantly affects young women. The tumor's biological behavior remains uncertain despite its low-grade classification. Characterized by keratin expression and a HMGA2::NCOR2 gene fusion, keratin positive giant cell rich tumors resemble conventional giant cell tumors but lacks H3-3A gene mutations. This article presents a 31-year-old woman with a keratin positive giant cell rich tumor in the pelvic bone, detailing her clinical presentation, diagnostic pro

The impact of halo-pelvic traction on deformity correction and bone mineral density in patients with severe spine deformity.

BACKGROUND: Severe spinal deformities require surgical intervention, with preoperative traction commonly used to improve spinal flexibility and surgical safety. However, the impact of traction on bone mineral density (BMD) and its relationship with correction outcomes remains poorly understood. METHODS: This retrospective study analyzed patients with severe spinal deformities who underwent halo-pelvic traction before surgical correction. Key parameters including Cobb angle, thoracic kyphosis (TK), height, and BMD were measured before and after traction. Paired t-tests and Pearson correlation

Three-dimensional Computed Tomography Scan of the Pelvic Bone in Isolated Epispadias: Analysis Before Surgical Correction.

OBJECTIVE: To determine intrapelvic angles and distances in pure epispadias patients and compare them to normal values. METHODS: Using three-dimensional computed tomography, 17 intrapelvic dimensions (8 angles and 9 distances) were measured in 26 patients with pure epispadias (21 boys and 5 girls). These values were compared to normal values obtained from 6 patients (5 boys and 1 girl) who underwent either pelvic or abdominopelvic computed tomography for purposes unrelated to their bony pelvis. RESULTS: Significant differences were observed in five angles (sacroiliac joint angle, S1 tilt ang

Is the Implant in Bone? The Accuracy of CT and Fluoroscopic Imaging for Detecting Malpositioned Pelvic Screw and SI Fusion Implants.

BACKGROUND: Spine fusions to the pelvis have been associated with increased strain to the sacroiliac joint (SI) and possibly continued postoperative low back pain. To minimize this, concomitant SI joint fusion at the time of lumbopelvic fixation has been advocated. This requires concomitant placement of sacral alar iliac screws (S2AI) for lumbopelvic fixation and triangular titanium rods (TTR) for the SI joint fusion. Traditionally, surgeons have mostly relied on fluoroscopic images to confirm final implant position and patient safety after pelvic instrumentation, although computer tomography

Grade 1 endometrioid endometrial carcinoma presenting with pelvic bone metastasis: a case report and review of the literature.

Most grade 1 endometrioid endometrial cancers are confined to the uterus at the time of diagnosis and confer a good prognosis. Rarely will a grade 1 endometrioid endometrial carcinoma present with distant metastasis, especially to the bone. We present the case of a 56-year-old woman with postmenopausal bleeding and right hip pain due to metastatic grade 1 endometrioid uterine cancer invading into the right ischium. We discuss treatment options as well as provide a review of prior published reports on bony metastasis in grade 1 endometrioid endometrial cancers. To date, this case is one of 10 o

Pelvic girdle reconstruction based on spinal fusion and ischial screw fixation in a case of aneurysmal bone cyst.

A case of lytic lesion of the pelvis in a 23-year-old woman is presented. A biopsy led to the diagnosis aneurysmal bone cyst (ABC). Due to the histologically very aggressive growth of the tumor, a low malignant osteosarcoma could not be excluded. In an initial operation the tumour, affecting the sacrum, the iliac crest and the lower lumbar spine was resected. Temporary restabilisation of the pelvic ring was achieved by a titanium plate. The histological examination of the entire tumour confirmed the diagnosis ABC. After 6 months, the MRI showed no recurrence. The observed tilt of the spine to

Serial Bone Density Changes in Women Undergoing Pelvic (Chemo) Radiation: Results From the PARCER Trial.

PURPOSE: Pelvic irradiation leads to substantial dose to the pelvic girdle. However, bone density loss as a function of radiation therapy dose and time has not been investigated. This study was undertaken to evaluate such a dose-response relationship. METHODS AND MATERIALS: Women undergoing pelvic radiation therapy for cervix cancer within a phase 3 trial were included. The study necessitated 2 computed tomography imaging sets acquired at least 12 months apart in patients with no evidence of relapse. All images were transferred to the treatment planning system to determine radiation dose and

Clinical study of 3D printed personalized prosthesis in the treatment of bone defect after pelvic tumor resection.

BACKGROUND: /Objective: In recent years, prostheses have been widely used for limb reconstruction after pelvic tumour resection. However, prostheses are associated with problems leading to tumour recurrence, poor implant matching, defects after tumour resection, and easy implant looseness or failure. To achieve a precise preoperative design, complete tumour resection, and better anatomical structure matching and prosthesis stability, this study used three-dimensionally (3D)-printed osteotomy guides and personalised prostheses for reconstruction after pelvic tumour resection. This study aimed t

Autogenous femoral head bone grafts for acetabular deficiency in total-hip arthroplasty for developmental dysplasia of the hip: long-term effect on pelvic bone stock.

Twenty-eight primary total-hip arthroplasties in 23 patients performed with autogenous femoral head bone graft augmentation for developmental hip dysplasia were retrospectively reviewed at 8 to 15 years. Five sockets were revised for different reasons. At revisions, 3 grafts were healed, the other 2 had substantial resorption. Immediately postoperatively, the mean medial to lateral graft thickness at the level of the superior border of the socket was 33 mm. Radiographs a mean of 4.8 years after operation demonstrated this thickness decreased by a mean of 1 mm (range, 0-10 mm). At 11 years (ran

Quantifying early pelvic radiation-induced bone injury with dual-energy CT: a rabbit model study of dynamic bone mineral and marrow adiposity changes.

PURPOSE: Dual-energy computed tomography (DECT) has been applied to tumor radiotherapy planning but has rarely been used in post-treatment toxicity assessment. This study assesses the feasibility of using DECT to detect early pelvic radiation-induced bone injury (pRIBI). METHODS: Thirty mature female New Zealand White rabbits were allocated at random into five distinct cohorts: one non-irradiated control group and four experimental groups subjected to a single 30-Gy radiation exposure, with euthanasia performed at 1, 2, 3, or 4 weeks following irradiation. All rabbits underwent DECT before an

What outcomes can be expected after surgical treatment of primary pelvic bone sarcomas in children at referral centers?

BACKGROUND: Primary pelvic bone sarcomas in children are rare and associated with poor outcomes, but large-volume, multicenter data are lacking. QUESTIONS/PURPOSES: We asked: What is the overall survival of pediatric patients with primary pelvic bone sarcomas following surgical treatment? What are the rates of local recurrence and postoperative complications? Which factors are associated with these outcomes? METHODS: We retrospectively reviewed 119 patients younger than 16 years treated surgically for primary pelvic sarcoma at five referral centers between 1990 and 2023. Survival was estimat

Biomechanical evaluation of an experimental internal ring fixator (RingFix) for stabilization of pelvic ring injuries on an osteoporotic bone model.

During the last decades, effective pain reduction and early mobilization were identified as the central priorities in therapy of insufficiency fractures of the pelvis. For operative treatment minimally-invasive stabilization techniques are favored. While there is consensus on the significance of sufficient dorsal stabilization the role of additional fixation of the anterior fracture component stays under discussion. Within the present study we developed an internal ring fixator system (RingFix) with the question whether an in-itself-closed construct can improve stability of the entire ring str

CT-Based Evaluation of Volumetric Posterior Pelvic Bone Density with Implications for the Percutaneous Screw Fixation of the Sacroiliac Joint.

Background: Operative treatment of fragility fractures of the pelvis has become a gold standard. Preoperative planning, including the assessment of the pathway for iliosacral screws, is crucial. The anchorage of the screw depends on the bone quality. Some recent studies have concentrated on assessing bone mineral density (BMD) with the use of Hounsfield unit (HU) values obtained from CT scans. The aim of the present study is to determine the best sacral levels of S1-S3 on the pathway of iliosacral screws for sacroiliac joint fixation. Methods: Patients admitted to the Independent Public Health
