Incidence and management of major urinary complications after pelvic exenteration for gynecological malignancies.

Urinary fistulae and obstruction following pelvic exenteration are frequent and life-threatening complications. They increase the mortality and morbidity rates of large exereses performed during pelvic exenteration for gynecological cancers. From a series of 97 patients who underwent pelvic exenteration for gynecological cancers we report the incidence, risk factors, and management of major urinary complications. Eighty patients had had previous surgery and/or pelvic radiation therapy at the time of pelvic exenteration. A urinary diversion was performed in 63 patients. Major early urinary comp

Pelvic Exenteration with En Bloc Excision of the Common or External Iliac Veins: To Reconstruct or Not?

BACKGROUND: En bloc resection of major pelvic sidewall neurovascular structures during pelvic exenteration is now deemed safe and oncologically feasible. This study aimed to evaluate the surgical, oncological, and quality-of-life (QoL) outcomes of patients who underwent en bloc common iliac or external iliac vein (CIV/EIV) resection during pelvic exenteration with and without venous reconstruction. METHODS: This was a retrospective cohort study of patients who underwent en bloc CIV/EIV resection during pelvic exenteration at the Royal Prince Alfred Hospital, Sydney (January 1994-July 2024). T

Pelvic exenteration for gynecological malignancies: an analysis of 15 cases operated on at a single institution.

We analyzed retrospectively the outcome of 15 patients treated with pelvic exenteration (nine total and six anterior) for gynecological cancer (six vaginal, four endometrial, three cervical, one vulvar, and one ovarian) from 1987 through 2008. Four operations were for primary tumors and 11 for recurrences. Eight patients had received prior radiotherapy. Median operation time was 310 (range 180-520) min, and median blood loss was 2,500 (range 600- 8,000) mL. Thirteen patients had altogether 59 postoperative complications, 25 (42%) early and 34 (58%) late. One patient each died of complications

Primary vaginal reconstruction at the time of pelvic exenteration for gynecologic cancer: morbidity revisited.

The aim of this study is to analyze our experience about the benefits and morbidity of primary vaginal reconstruction in pelvic exenteration. Over a 10-year period, 64 patients underwent a pelvic exenteration for gynecologic cancer, except for ovarian and fallopian cancer. Twenty-nine patients underwent pelvic exenteration with vaginal reconstruction [21 cases with transverse rectus-abdominis myocutaneous (TRAM) flap and eight cases with Singapore fascio-cutaneous flap]. Thirty-five patients did not undergo vaginal reconstruction. Postoperative morbidity was recorded and a comparative analysis

Total pelvic exenteration with or without sacral resection in patients with recurrent colorectal cancer.

Pelvic recurrence from colorectal cancer produces significant morbidity. Radiation can help palliate the pain produced by this recurrence. Frequently patients with recurrent colorectal cancer will progress to a constant unrelenting pain and obstructive uropathy with sacral and bladder involvement. These patients can be candidates for an aggressive surgical resection with the hope of significant palliation and prolonged survival. From October 1988 to December 1991, six patients had total pelvic exenteration at our institution. Of these six patients, two had en bloc sacral resection at levels S1

Reconstruction of sigmoid vagina and conduit in total pelvic exenteration for recurrent cervical carcinoma.

Although total pelvic exenteration is performed for central recurrent or persistent uterine cervical carcinoma, the patient is damaged both physiologically and psychologically by its massive surgical procedure and complications as well as the loss of the vagina. Vaginal reconstruction at the time of surgery brings much advantageous satisfaction to the patient's future life and a strong bond with her husband. Furthermore, a new reconstructed vagina occuping the pelvic cavity prevents intestinal herniation and is available for bimanual examination in the follow-up to surgery. A case of recurrent

Sacral resection with pelvic exenteration for advanced primary and recurrent pelvic cancer: a single-institution experience of 100 sacrectomies.

BACKGROUND: Recurrent and advanced primary pelvic cancers present a complex clinical issue requiring multidisciplinary care and radical extended surgery. Sacral resection is necessary for tumors that invade posteriorly but is associated with increased morbidity and mortality. OBJECTIVE: This study aimed to analyze the morbidity and survival associated with pelvic exenteration involving sacrectomy for advanced pelvic cancers at a single institution. DESIGN: This study used patient demographics, operative and pathologic reports, and prospective survival data to determine factors affecting pati

[Microsurgical reconstruction of the pelvic floor after pelvic exenteration. Reduced morbidity and improved quality of life by an interdisciplinary concept].

BACKGROUND: Pelvic exenteration for advanced or recurrent rectal cancer often results in complex defects associated with high complication rates and morbidity for the patients. The goal of therapy is therefore restoration of functional stability and adequate soft tissue coverage, thus enhancing the quality of life with limited life expectancy by an interdisciplinary approach. PATIENTS AND METHODS: We report on eight patients treated by combined interdisciplinary pelvic exenteration with resection of the sacrum and subsequent coverage of the pelvic floor defect with free latissimus dorsi muscl

A radiologist's guide to the galaxy of complications post total pelvic exenteration for rectal cancers.

Total pelvic exenteration (TPE) is a complicated morbid surgery with a patient having to cope with two permanent stomas lifelong. TPE is often the only option for potential cure that can be offered to patients with low/very low rectal cancers with multicompartment involvement. While the Clavien Dindo classification is used for clinically assessing the severity of complications, it does not guide making an imaging diagnosis (1). Radiologists are often unaware of the complications post-TPE surgery, what imaging modality to use, and how to diagnose these. The complications can be fatal if undiagn

A case of primary adenocarcinoma of the seminal vesicle treated by total laparoscopic pelvic exenteration.

An adenocarcinoma of the seminal vesicle is a rare malignancy with a poor prognosis. Its symptoms are non-specific, rendering diagnosis difficult. We present the case of a 74-year-old male with haematospermia. Magnetic resonance imaging revealed a bulky tumour of the seminal vesicle infiltrating the prostate gland, bladder and rectum. A transrectal ultrasound-guided biopsy revealed an adenocarcinoma. Computed tomography revealed multiple lung metastases. The serum CA-125 level was high. Androgen deprivation therapy and chemotherapy (docetaxel) were prescribed. After 12 courses of docetaxel, th

Guidelines of the Brazilian Society of Oncologic Surgery for pelvic exenteration in the treatment of cervical cancer.

BACKGROUND AND OBJECTIVES: The primary treatment for locally advanced cases of cervical cancer is chemoradiation followed by high-dose brachytherapy. When this treatment fails, pelvic exenteration (PE) is an option in some cases. This study aimed to develop recommendations for the best management of patients with cervical cancer undergoing salvage PE. METHODS: A questionnaire was administered to all members of the Brazilian Society of Surgical Oncology. Of them, 68 surgeons participated in the study and were divided into 10 working groups. A literature review of studies retrieved from the Nat

Total pelvic exenteration with distal sacrectomy for fixed recurrent rectal cancer in the pelvis.

PURPOSE: This study evaluates the effectiveness of total pelvic exenteration with distal sacrectomy for fixed recurrent tumor that developed from primary rectal cancer. METHODS: We investigated surgical indications, techniques to minimize blood loss and reduce complications, and oncological outcomes in 57 patients who underwent total pelvic exenteration with distal sacrectomy between 1983 and 2001. RESULTS: Forty-eight patients (84 percent) had negative margins. A comparison between two periods (1983-1992 and 1993-2001) showed that mean blood loss decreased from 4,229 to 2,500 ml (P = 0.002)

[Pelvic exenteration in the treatment of locally advanced tumors of surgical, gynaecological and urological origin].

Pelvic exenteration is a radical extirpative procedure designed to treat pelvic malignancy that has invaded more than one of the hollow organs of the gastrointestinal or genitourinary tracts. The operation comprises several radical procedures: resection of the rectum, hysterectomy (with adnexectomy) with partial or complete vaginectomy, pelvic lymphadenectomy and total (or partial) cystectomy with diversion of the urinary tract. The retrospective analysis of our results in 27 patients treated in the years 1995 to 2000 by means of pelvic exenteration showed a 7.4 % hospital mortality and 59.3 %

Eureka: objective assessment of the empty pelvis syndrome to measure volumetric changes in pelvic dead space following pelvic exenteration.

BACKGROUND: Large tissue defects following pelvic exenteration (PE) fill with fluid and small bowel, leading to the empty pelvis syndrome (EPS). EPS causes a constellation of complications including pelvic sepsis and reduced quality of life. EPS remains poorly defined and cannot be objectively measured. Pathophysiology of EPS is multifactorial, with increased pelvic dead space potentially important. This study aims to describe methodology to objectively measure volumetric changes relating to EPS. METHODS: The true pelvis is defined by the pelvic inlet and outlet. Within the true pelvis there

Successful management of locally advanced cervical cancer via total pelvic exenteration following neoadjuvant chemotherapy.

Stage IVA cervical cancer is a tumor that invades the mucosa of the bladder or rectum without distant metastasis and is difficult to treat, and concurrent chemoradiotherapy is recommended. Although radical surgery following neoadjuvant chemotherapy is a treatment option for stage IVA cervical cancer, the evidence is limited. A 51-year-old woman with bulky cervical cancer and rectal invasion was referred to our hospital. Paclitaxel and cisplatin were administered as neoadjuvant chemotherapies. After two cycles of chemotherapy, the tumor size decreased markedly. Total pelvic exenteration was per
