Understanding intestinal vulnerability to perforation in the extremely low birth weight infant.

Spontaneous intestinal perforation (SIP) occurs commonly in extremely low birth weight (ELBW) infants. Our understanding of its etiologies has improved dramatically over the last decade. Included in this comprehension is an ongoing reconciliation of the iatrogenic risk factors, the microbiology, and the histopathology. The latter shows focal perforations with necrosis of the muscularis externa and no sign of ischemic damage (typically characterized by mucosal necrosis in the preterm bowel). Associations include extreme prematurity, early postnatal steroids (EPS), early use of indomethacin (EUI

A safe, simple technique for transseptal catheterization.

Transseptal catheterization is required for atrial fibrillation ablation and many ablations for atrial tachycardias, left atrial flutters, and accessory pathways. Using a Brockenbrough needle or other specialized device adds time, expense, and risk of potential complications such as atrial or aortic perforation, pericardial effusion, and tamponade to these procedures. We present a simple, low-risk technique for transseptal catheterization.

CT detection of intraabdominal disease in patients with lower extremity signs and symptoms.

The initial clinical presentation of intraabdominal disease can be in an extraabdominal location. This phenomenon most commonly occurs in the setting of bowel perforation secondary to diverticulitis, appendicitis, or carcinoma, with resultant spread of infection caudal to the abdomen. Hematomas and pancreatic fluid collections may also dissect out of the abdomen. The spread of these disease processes is likely to occur in a predictable fashion along anatomic tissue planes. Computed tomography (CT) is well suited to demonstrate the extraabdominal site of disease, the pathway of spread from the

Hippocampal grey matter changes across scales in Alzheimer's Disease.

Alzheimer's disease (AD) is a progressive and debilitating neurodegenerative disease of the central nervous system, characterized by deterioration in cognitive function including extensive memory impairment. The hippocampus, a medial temporal lobe region, is a key orchestrator in the encoding and retrieval of memory and is believed to be one of the first regions to deteriorate in AD. In this work we examined hippocampal macrostructure (specifically gyrification and thickness) and microstructure in Alzheimer's disease (AD) and mild cognitive impairment (MCI) relative to healthy aged controls in

Fingolimod Administration Following Hypoxia Induced Neonatal Seizure Can Restore Impaired Long-term Potentiation and Memory Performance in Adult Rats.

Neonatal seizures commonly caused by hypoxia can lead to long-term neurological outcomes. Early inflammation plays an important role in the pathology of these outcomes. Therefore, in the current study, we explored the long-term effects of Fingolimod (FTY720), an analog of sphingosine and potent sphingosine 1-phosphate (S1P) receptors modulator, as an anti-inflammatory and neuroprotective agent in attenuating anxiety, memory impairment, and possible alterations in gene expression of hippocampal inhibitory and excitatory receptors following hypoxia-induced neonatal seizure (HINS). Seizure was in

Age-dependent evaluation of long-term depression responses in hyperthyroid rats: Possible roles of oxidative intracellular redox status.

BACKGROUND: According to the free radical theory, a gradual accumulation of the free radicals normally produced in the body underlies the changes associated with aging. Thyroid hormones (THs) are related to oxidative stress not only due to their stimulation of metabolism but also due to their effects on antioxidant mechanisms. Thyroid dysfunction increases with age; thus, changes in TH levels in elderly individuals could be a factor affecting the development of neurodegenerative diseases. However, the relationship is not always clear, based on current evidence regarding synaptic plasticity. M

Radiofrequency-assisted transseptal perforation for electrophysiology procedures in children and adults with repaired congenital heart disease.

BACKGROUND: Patients with congenital heart disease carry a high burden of arrhythmias and may pose special challenges when these arrhythmias are addressed invasively. We sought to describe our early experience with radiofrequency (RF) needle transseptal perforation to facilitate ablation procedures in this population. METHODS: Retrospective chart review to identify all cases of attempted transseptal access with a commercial RF needle at Children's Hospital Boston between February 2007 and January 2010. RESULTS: A total of 10 patients had attempted RF transseptal perforation. Median age was 2

Can Tissue Engineering Bring Hope to the Development of Human Tympanic Membrane?

The tympanic membrane (TM), more commonly known as the eardrum, consists of a thin layer of tissue in the human ear that receives sound vibrations from outside of the body and transmits them to the auditory ossicles. The TM perforations (TMPs) are a common ontological condition, which in some cases can result in permanent hearing loss. Despite the spontaneous healing capacity of the TM to regenerate in the majority of cases of acute perforation, chronic perforations require surgical interventions. However, the disadvantages of the surgical procedure include infection, anesthetic risks, and hig

Mitigating risk following upper gastrointestinal Endoscopic Submucosal Dissection: preventing bleeding, perforation and stricture formation.

Endoscopic submucosal dissection (ESD) is an effective organ-sparing technique for superficial neoplasia in the upper gastrointestinal tract, although it remains associated with a non-negligible risk of complications. This narrative review provides an integrated overview of the entire ESD pathway, from pre-procedural risk stratification to intraprocedural management and post-procedural care. Adverse events should be interpreted as the result of the interaction between patient-related factors, lesion characteristics, and procedural complexity. During the procedure, maintaining stable control of

Perforated-patch recording does not enhance effect of 3-isobutyl-1-methylxanthine on cardiac calcium current.

Conventional whole cell voltage-clamp recording results in washout of the cardiac Ca2+ current (ICa) response to the beta-adrenergic agonist isoproterenol (Iso), for reasons which are not clear. When dose-response curves for the phosphodiesterase (PDE) inhibitor 3-isobutyl-1-methylxanthine (IBMX) were compared using perforated-patch vs. conventional whole cell recording in guinea pig ventricular myocytes, the conventional whole cell IBMX responses were unexpectedly larger than the perforated-patch responses. Furthermore, during conventional whole cell recording the response to repeated applica

Low frequency stimulation for seizure suppression: Identification of optimal targets in the entorhinal-hippocampal circuit.

BACKGROUND: Mesial temporal lobe epilepsy (MTLE) with hippocampal sclerosis (HS) is a common form of drug-resistant focal epilepsy in adults. Treatment for pharmacoresistant patients remains a challenge, with deep brain stimulation (DBS) showing promise for alleviating intractable seizures. This study explores the efficacy of low frequency stimulation (LFS) on specific neuronal targets within the entorhinal-hippocampal circuit in a mouse model of MTLE. OBJECTIVE: Our previous research demonstrated that LFS of the medial perforant path (MPP) fibers in the sclerotic hippocampus reduced seizures

The effect of 1,3-dinitrobenzene on the functioning of the auditory pathway in the rat.

1,3-Dinitrobenzene (DNB) has previously been shown to be neuropathic, causing gliovascular lesioning in the rat brainstem, with the nuclei of the auditory pathway being particularly affected. Lesion severity was shown to be dependent on functional activity, which could be markedly decreased within one pathway by monaurally reducing sensory input. The aim of this study was to characterise the changes in electrophysiological and vascular function associated with this asymmetric lesioning. Depth electrodes located in the inferior colliculi were used to measure wave II and IV of the auditory evoke

'Impact of a Regional Damage Control Surgery Pathway on Time to Surgery for Neonates With Severe Necrotising Enterocolitis. A Prospective Cohort Study'.

BACKGROUND: In the UK, published time from diagnosis of perforation to surgery for babies with Necrotising Enterocolitis (NEC) was 23·5 h. We aimed to determine if a standardised regional referral pathway, combined with a damage control surgical approach (DCS) could reduce timings associated with referral to first surgical intervention. METHOD: Our pathway standardised referrals, transfer, blood-crossmatching and DCS. Criteria to trigger pathway was a neonate with NEC and either perforation or requiring inotropes. Pathway implementation used quality improvement and appreciative inquiry princi
