Perforator-plus flaps: a new concept in traditional flap design.

BACKGROUND: Conventional fasciocutaneous flaps in reconstructive surgery, especially in the lower extremities, have limited utility. Traditional flaps are essentially random pattern, often require delays, and are limited in mobility and reach. Islanded fasciocutaneous flaps pedicled on perforators can be raised anywhere on the body and have a reliable blood supply and greater freedom of movement. However, venous compromise is a common problem. METHODS: A new approach to raising conventional fasciocutaneous flaps while including and retaining perforators in their substance was used to offset t

Reconstruction of the natal cleft with a perforator-based flap.

Recurrent chronic conditions of the natal cleft, such as pilonidal sinuses, are difficult problems to treat. The deep natal cleft and the rolling effect of one buttock surface over the other contribute to the high recurrence rate. Wide excision of the affected area is the treatment of choice; to cover the defect many techniques have been described that flatten the natal cleft and shift the suture line away from the midline to try to reduce recurrence. These techniques include Z-plasties, rhomboid flaps, V-Y flaps, gluteus maximus myocutaneous flaps and others. Five patients with recurrent nata

Islanded perforator-based flap versus dorsal rectangular flap for syndactyly release: a prospective analytical study.

Syndactyly, a congenital hand deformity caused by failed phalangeal separation, often requires surgical correction to restore function and aesthetics. Traditional dorsal rectangular flap techniques involve skin grafting, increasing the risk of web creep and residual deformity. This study evaluates islanded perforator-based flaps, which offer better vascularization for improved outcomes. Preoperative Doppler ultrasound was used for vascular mapping to enhance surgical precision. This prospective observational study analyzed 31 webspaces in 26 patients undergoing syndactyly release. Doppler ultr

Clinical comparison of superior thyroid artery perforator flap and sternocleidomastoid myocutaneous flap for intraoral reconstruction.

The aim was to evaluate the techniques and outcomes of superior thyroid artery perforator flaps (STAPF) for intraoral reconstruction and to compare them with those of the sternocleidomastoid myocutaneous flap (SCMMF). The cases of 43 patients who underwent reconstruction with either a SCMMF or STAPF for the repair of a medium-sized intraoral defect, between January 2013 and December 2020, were reviewed retrospectively. Although both flaps are based on the superior thyroid artery, their specific harvesting techniques largely differ. All SCMMF (n = 23) were superiorly-based rotational flaps with

Coverage of Tendon Exposure after Radial Forearm Free Flap by the Dorsoulnar Artery Perforator Flap.

Objective This study was designed to assess the effectiveness of an alternative technique using a perforator flap to manage secondary tendon exposure after a radial forearm free flap in head and neck oncologic surgery. Study Design Prospective cohort study. Setting Plastic Reconstructive Surgery Unit, Nice University Hospital, Pasteur 2 Hospital, France. Subjects and Methods Despite its numerous advantages, the radial forearm free flap is associated with significant donor site morbidity and the risk of secondary tendon exposure. Conventional skin grafts for secondary tendon exposure can lead t

Dual-Perforator Flap With Wide Pedicle Versus Sural Neurocutaneous Flap With Peroneal Artery Perforator in the Treatment of Soft Tissue Defects of Foot and Ankle.

How to deal with large soft tissue defects around the foot and ankle is still controversial. The aim of this study was: (1) to display a new pedicled flap, also named the dual-perforator flap with wide pedicle (DPFWP), and (2) to compare it with the sural neurovascular flap with peroneal artery perforator (SNFPAP) in foot and ankle reconstruction. According to different surgical methods, 82 patients were divided into 2 groups: the DPFWP group (42 cases) and the SNFPAP group (40 cases). All cases underwent a flap surgery after radical debridement. The 2 groups were homogeneous in terms of age,

Perforator flap based on the third perforator of the profunda femoris artery (PFA)-assisted closure of the free vertical posteromedial thigh (vPMT) flap donor site.

INTRODUCTION: The primary closure of the vertical posteromedial thigh (vPMT) free flap donor site is very important to minimize donor site morbidity and maximize cosmetic appearance. However, sometimes due to the dimension of the defect, a vPMT flap is wider than the 8-10 cm requirement. The authors report their experience with the third perforator of the profunda femoris artery (PFA) during the vPMT free flap donor-site closure. PATIENTS AND METHODS: Between January 2016 and December 2017, 5 patients underwent reconstruction of lower extremity (2 pts.) and head and neck (3 pts.) area with th

Perforator flaps in breast reconstruction.

Patients are well informed and seek autogenous breast reconstruction. The motivating factors include a preference for autologous tissue reconstruction and the complementary improvement in body contour, safety concerns surrounding implants, and implant-related complications in the setting of previous radiation therapy. In this article a variety of perforator flaps from donor sites that include the trunk (thoracodorsal artery perforator and intercostal artery perforator), abdomen (deep inferior epigastric artery perforator and superficial inferior epigastric artery), and buttock (superior glutea

Free perforator flaps in children.

BACKGROUND: Raising perforator flaps is said to be a tedious procedure. The benefits, however, are great. In adults, perforator flaps have proved their usefulness and reliability in various clinical situations. In children, donor sites for free flaps are particularly scarce because of the need for a long and reliable vascular pedicle of sufficient size. There is also the need to minimize donor-site morbidity from aesthetic, functional, and psychological perspectives. METHODS: The authors present a series of 23 consecutive free perforator flaps performed by the first author in 20 children; age

[Conjunctival flaps in corneal perforations].

PURPOSE: To evaluate conjunctival flaps as an emergency surgical treatment of corneal perforations. METHODS: We revised a sequence of 15 patients who are yielded to total conjunctival flap and 3 patients yielded to partial conjunctival flap. They were made between 1980-1998. RESULTS: Diagnostic to surgery included chronic ulcerative keratitis (2), bullous keratopathy (2), central ulcers (6), chemical burned (2), failed graft (3) and others (3). Seven flaps finished off keratoplasty, tree flaps retractions occurred and one eye needed evisceration. The rest of the pathology were controlled wit

The Role of the Abdominal Perforator Exchange (APEX) Technique in the Perforator Selection Algorithm for Delayed Deep Inferior Epigastric Perforator (DIEP) Flap Breast Reconstruction.

Background: The deep inferior epigastric artery perforator (DIEP) flap is currently the most widely used method for autologous breast reconstruction. Its primary advantage over the transverse rectus abdominis muscle (TRAM) flap is the reduction in donor-site morbidity, as it preserves the integrity of the abdominal muscles and motor nerves. Importantly, each patient's unique vascular anatomy requires an individualized approach to perforator selection and the surgical technique. Objective: We aimed to minimize donor-site morbidity and refine the perforator selection strategy in delayed DIEP fla

Thin anterolateral thigh perforator flap using a modified perforator microdissection technique and its clinical application for foot resurfacing.

BACKGROUND: A thin skin flap is often required for optimal resurfacing of particular areas of the body. An anterolateral thigh perforator flap can be thinned to an extent to which it is vascularized by the subdermal plexus only. This study presents a novel flap thinning technique and its application for resurfacing the dorsum of the foot. METHODS: From July of 2002 to October of 2003, 18 patients underwent resurfacing of the dorsum of the foot with thin anterolateral thigh flaps. The main perforators were strategically located in the flap center to keep the peripheral area within the vascular

Reconstruction of wide soft tissue defects with extended anterolateral thigh perforator flap turbocharged technique with anteromedial thigh perforator.

BACKGROUND: The necrotic change may sometimes occur in peripheral parts of the flap when harvesting a wide flap in a patient with a relatively thin thigh in anterolateral thigh (ALT) flap. Here, we explore the prophylactic measure of turbocharging the ALT flap with anteromedial thigh (AMT) perforator based on the circumference of the patient's thigh to reduce the risk of partial flap necrosis for the reconstruction of wide defects. PATIENTS AND METHODS: From January 2015 to December 2017, extended ALT perforator turbocharged flaps were done in nine patients. The majority of patients had post

Dual perforator propeller internal mammary artery perforator (IMAP) flap for soft-tissue defect of the contralateral clavicular area.

The internal mammary artery perforator (IMAP) flap represents the evolution from axially pedicled flaps (deltopectoral flap) to perforator flaps. Both flaps are typically used for neck and tracheostoma reconstruction in male patients. We present the case of a 68-year-old obese female patient with a right upper thoracic radionecrosis secondary to breast irradiation. Soft-tissue defect measured 12×18 cm. She also complained of left breast hypertrophy. Following radical debridement, a left IMAP flap extending from midline to the anterior axillary fold was raised, based on the second and fourth IM

Comparison of the free thinned anterolateral thigh perforator flap with the free medial sural artery perforator flap for reconstruction of head and neck, and extremity defects.

BACKGROUNDThe free anterolateral thigh (ALTP) and free medial sural artery perforator (MSAP) flaps are time tested donor for head and neck, and extremities defect reconstruction. Proponents of either flap have concluded each as workhorse flap in their large cohort studies. However, we could not find any literature comparing the donor morbidities, or recipient site outcomes of these flaps, objectively.METHODSRetrospective data, such as demographic details, flap characteristics and post operative course, from patients who underwent free thinned ALTP (25 patients) and MSAP flap (20 patients) were

Comparison of outcomes of pressure sore reconstructions among perforator flaps, perforator-based rotation fasciocutaneous flaps, and musculocutaneous flaps.

BACKGROUND: Pressure sore reconstruction remains a significant challenge for plastic surgeons due to its high postoperative complication and recurrence rates. Free-style perforator flap, fasciocutaeous flap, and musculocutaneous flap are the most common options in pressure sore reconstructions. Our study compared the postoperative complications among these three flaps at Kaohsiung Chang Gung Memorial Hospital. METHODS: From 2003 to 2012, 99 patients (54 men and 45 women) with grade III or IV pressure sores received regional flap reconstruction, consisting of three cohorts: group A, 35 free-st

Pilot Study: Superficial Circumflex Iliac Perforator Flap for Coverage of Radial Forearm Free Flap Donor Sites.

BACKGROUND: The radial forearm free flap (RFFF) remains a workhorse for reconstruction, but donor-site morbidity is a major drawback. Traditional closure methods, such as skin grafting, can result in tendon exposure, contour deformity, and poor aesthetics. The superficial circumflex iliac perforator (SCIP) flap has emerged as a versatile option with low donor-site morbidity. This pilot study reports the use of an SCIP flap to provide coverage of RFFF donor sites. METHODS: We performed a retrospective review of patients who underwent RFFF reconstruction with SCIP flap coverage of the forearm d

Comparing Outcomes of Pedicled Submandibular Gland Flap and Submental Artery Perforator Flap in Oral Cavity Reconstruction.

OBJECTIVE: Submandibular gland flap (SMGF) has demonstrated reliability in repairing small- to medium-sized defects, yet it has not gained widespread international adoption. This study aimed to evaluate the efficacy and outcomes of SMGF for oral cancer reconstruction and compare its application with submental artery perforator flap (SMAPF). METHODS: All patients with primary oral cancer requiring reconstruction using either SMGF or SMAPF were included. Demographic and surgical outcome measures (such as harvesting time, use of lip splinting, and marginal mandibulectomy), short-term outcomes (i
