Hydroxyapatite pseudopodagra. A syndrome of young women.

Six women with acute calcific periarthritis of the first metatarsophalangeal joint (hydroxyapatite pseudopodagra) are described, and 10 previously reported cases (8 female patients and 2 male patients) are reviewed. The onset was characterized by acute pain, swelling, erythema, tenderness, and limitation of movement of the first metatarsophalangeal joint, symptoms indistinguishable from those of gouty podagra, which is associated with transient amorphous calcific deposits in the vicinity of the joint. Based on these results it appears that hydroxyapatite pseudopodagra is a disorder that predom

Oxaprozin: kinetic and dynamic profile in the treatment of pain.

Oxaprozin (4,5-diphenyl-2-oxazolepropionic acid) is a non-steroidal anti-inflammatory drug (NSAID) which is effective in models of inflammation, pain and pyrexia. It is effective and well tolerated in the clinical management of adult rheumatoid arthritis (RA), osteoarthritis (OA), ankylosing spondylitis, soft tissue disorders and post operative dental pain. Oxaprozin has a high oral bioavailability (95%), with peak plasma concentrations at 3 to 5 hours after dosing. It is metabolised in the liver by oxidative and conjugative pathways and readily eliminated by the renal and faecal routes. Oxapr

[Xerography of diseases caused by microcrystalline deposits].

In this work, the authors have pointed out the role of xeroradiography in the diagnostic assessment of joint disorders associated to dystrophic microcalcifications (gout, pseudogout, acute calcific periarthritis). Within this class of pathology, the physical and technical characteristics of xeroradiography imaging (side-effect; showing of details; wide range of x-ray exposition; small density differences) are capable to depict all the joints and related structures involved. In addition particular attention was paid to drew out the joint disorders associated to dystrophic microcalcifications fr

[Apatite deposition diseases].

Deposition of apatite crystals can be observed as calcific periarthritis and articular calcifications. Deposition of these crystals in the tendons and bursae of the rotator cuff of the shoulder is found in about 3% of adults; it is most of the time asymptomatic. Calcifications often totally disappear after an acute flare up. Intraarticular apatite crystals can be identified in synovial fluids of patients with severe destructive osteoarthrosis, mainly of the shoulder joints. Management with NSAID's and local corticosteroid injections is often very helpful. In rare cases the aspiration with a ne

Calcium hydroxyapatite deposition disease.

Calcium hydroxyapatite crystal deposition disease is characterized by the presence of basic calcium phosphate crystals--predominantly hydroxyapatite--in the periarticular soft tissues, especially the tendons. The entity is best recognized as "calcific tendinitis" at its most frequent site about the shoulder, but the disease involves numerous other sites and may be more appropriately termed calcific periarthritis. This article illustrates typical and atypical radiographic features of hydroxyapatite deposition, as well as some unusual manifestations of the disease. A brief review of various theo

Acute calcific quadriceps tendinitis.

Acute calcific quadriceps tendinitis can be the cause of acute periarthritis of the knee. Physical examination can detect cellulitis and a tender tendon sheath. Lateral roentgenograms of soft tissues show thickening of the tendon and sometimes small flecks of amorphous calcification within the body of the tendon near the attachment. Although hyperostosis of the patella near the tendon attachment is often present, it is well defined ossification and not associated with the acute symptoms. The amorphous calcium deposits quickly disappear during the acute tendinitis and are not detected on follow

[Diagnosis and clinical aspects of the cervicobrachial syndrome as seen by the rheumatologist (author's transl)].

Among the various diseases classified as cervicobrachial syndrome, from the rheumatological point of view rheumatic conditions of the soft parts such as periarthritis humeroscapularis, tendomyoses and insertion tendinoses are particularly important. Their exact clarification and differentiation from polymyalgia rheumatica and the rare polymyositis is also of great therapeutic significance. Moreover the rheumatologist is also frequently confronted with inflammatory and non-inflammatory conditions of the cervical vertebral column and the shoulder joint which may set up various disease symptoms.

[Apatite rheumatism].

Acute periarthritis is a typical example of apatite arthropathy related to intratendinous calcifications. The attack may be followed by virtually complete radiological disappearance of the calcification. Within the joint one may exceptionally demonstrate the presence of hydroxyapatite crystals in the synovial fluid during a fluxional flare-up of arthrosis or in the fluid withdrawn from the humeral joint following rupture of calcification in the glenohumeral cavity. More frequently, the crystals are detected in the synovial fluid from joints damaged by advanced destructive arthrosis. These crys

Unusual cause of the thumb basal joint pain: osteoid osteoma of the trapezium.

The trapezium is rare site of osteoid osteoma development. The diagnostic challenge lies in its rare occurrence, and requires differentiation from various disease entities causing thumb basal joint pain. We report the case of a 29-year-old male who presented with severe thumb basal joint pain. He was initially treated for calcific periarthritis because of concomitant calcifications around the thumb basal joint, but had undiscovered osteoid osteoma. A high index of suspicion to a patient with wrist pain unresponsive to prior treatment is necessary for diagnosis of osteoid osteoma.

The rheumatologic manifestations of diabetes mellitus.

The rheumatologic disorders associated with diabetes mellitus have been reviewed. From the evidence presented, it can be concluded that neuroarthropathy and osteolysis are definitely assoicated with diabetes. Ankylosing hyperostosis and periarthritis probably represent valid associations, and possible, but still unproven associations exist for gout, pseudogout, the carpal tunnel syndrome, osteoarthritis, Dupuytren's contracture and joint contractures. Despite the lack of a proven pathophysiologic basis these interrelationships may be clinically relevant. The discovery of one of these disorders

Osteoid osteoma of the scaphoid bone associated with flexor carpi radialis calcific tendinitis and treated with CT-guided RF ablation.

Osteoid osteomas of the wrist are relatively rare and the diagnosis is challenging due to atypical clinical features. We describe a case of an osteoid osteoma of the scaphoid bone associated with calcific tendinitis of the adjacent flexor carpi radialis tendon and periarticular soft tissue calcifications in a 21-year-old man presenting with radial-sided wrist pain. The lesion was successfully treated with CT-guided RF ablation. To our knowledge, this is the first description of an osteoid osteoma of the wrist associated with calcific tendinitis and periarthritis. In addition, we discuss the te

The structural consequences of calcium crystal deposition.

Calcium pyrophosphate dihydrate and basic calcium phosphate (BCP) crystals are the most common calcium-containing crystals associated with rheumatic disease. Clinical manifestations of calcium crystal deposition include acute or chronic inflammatory and degenerative arthritides and certain forms of periarthritis. The intra-articular presence of BCP crystals correlates with the degree of radiographic degeneration. Calcium crystal deposition contributes directly to joint degeneration. Vascular calcification is caused by the deposition of calcium hydroxyapatite crystals in the arterial intima. Th

Lancefield group B and C streptococci in East African camels (Camelus dromedarius).

Seventeen Lancefield group C streptococci (13 Streptococcus equi zooepidemicus and four Streptococcus dysgalactiae equisimilis) and 185 Lancefield group B streptococci (Streptococcus agalactiae) were isolated from camels (Camelus dromedarius) in Kenya and Somalia; 59 of the isolates were from healthy nasopharynx, vaginal and rectal mucosa and from non-abscessed lymph nodes, and the other 143 isolates were from clinical infections of the respiratory tract, tick bite lesions, abscessed lymph nodes, abscesses and other purulent skin lesions, periarthritis and arthritis, puerperal infection and gi

Heterotopic ossification in paraplegia: a possible disturbance of the paravertebral venous plexus.

The distribution of the heterotopic ossifications that complicate paraplegia and other neurological disorders is remarkably similar to that of the paravertebral venous plexus of Batson. Stasis within this system consequent to immobilization and spinal trauma, combined with demineralization of the skeleton, are postulated to be the main factors promoting precipitation of calcium salts in the soft tissues in these conditions.

[Subacromial impingement. Evaluating the concept--diagnosis--therapeutic concepts].

Subacromial impingement is a condition that belongs to the group of diseases known collectively as periarthritis humeroscapularis and has a typical clinical presentation. The underlying cause is a late of sufficient space of various origin in the subacromial region that finally leads to rotator cuff damage. Depending on cause, three forms can be distinguished, primarily extrinsic, primarily intrinsic and secondary impingement. The diagnosis is primarily clinical (pain on raising the arms above the head, painful arc syndrome). The value of imaging techniques, and the diagnostic approach are des

Primary chest wall tumor appearing as frozen shoulder. Review and case presentations.

The term frozen shoulder may apply to a primary, common, recognizable entity with a predictable course to a painful stiff condition with periarthritis secondary to trauma, rotator cuff or arthritic source. We studied 140 cases of frozen shoulder, referred to a surgical clinic for manipulation when conservative care was not effective. Investigations showed only 40 cases had "primary" frozen shoulder and among these were 3 patients with a local primary invasive neoplasm mimicking the exact features of the common condition. The attending physician and surgeon should be suspicious of tumor in youn
