A Case Report on Renal Abscess With Subsequent Perinephric Extension in a Patient With Poorly Controlled Type 2 Diabetes Mellitus.

Renal and perinephric abscesses are rare but potentially life-threatening complications of urinary tract infections (UTIs), especially in patients with diabetes mellitus. Diagnosis is often overlooked and delayed due to nonspecific clinical features and incomplete response to antimicrobial therapy. We report a case of a 62-year-old female with a history of poorly controlled type 2 diabetes mellitus (T2DM) who presented with flank pain, dysuria, vomiting, and fever. Imaging with contrast-enhanced computed tomography of the kidneys, ureters, and bladder (CT KUB) revealed right-sided acute pyelon

Acute pyelonephritis complicated by subcapsular and perinephric hematoma: Diagnostic role of CT scan and 6-month follow-up.

Acute pyelonephritis is a common renal infection that can become life-threatening when complicated by structural abnormalities, resistant pathogens, or systemic comorbidities such as diabetes mellitus. While common complications include renal abscess and sepsis, spontaneous subcapsular hematoma is a rare but serious manifestation that requires prompt recognition and intervention. This article reports the case of a 54-year-old female with a history of diabetes mellitus, hypertension, and hyperlipidemia, who presented with fever, flank pain, vomiting, and gross hematuria. She was diagnosed with

Association of Mayo Adhesive Probability Score With Perioperative Outcomes and Histological Characteristics of Adherent Perinephric Fat in Laparoscopic Adrenalectomy.

BACKGROUND/AIM: To evaluate the difficulty of laparoscopic adrenalectomy by investigating the usefulness of the Mayo Adhesive Probability (MAP) score for assessing adherent perinephric fat and its correlation with histological reality. PATIENTS AND METHODS: We retrospectively evaluated 103 patients who underwent laparoscopic adrenalectomies. Based on preoperative computed tomography images, the patients were categorized into two groups: high (3-5 points) and low MAP (0-2 points). Clinical characteristics and perioperative data were compared between the two groups. Additionally, we analyzed th

Renal and perinephric abscesses involving Lactobacillus jensenii and Prevotella bivia in a young woman following ureteral stent procedure.

is a gram-positive bacillus in the female genital tract believed to be a commensal organism that inhibits the growth of more virulent pathogens. Prevotella bivia is a gram-negative bacillus species also typically commensal in the female genital tract. Lactobacillus as the primary causative agent in perinephric abscesses and bacteremia has been documented, albeit very uncommon and opportunistic. Prevotella bivia is not classically associated with perinephric abscesses but has been implicated in rare cases of pelvic inflammatory disease and tubo-ovarian abscesses. In this report, we present a 26

Perinephric myxoid pseudotumor of fat: A very rare entity that can mimic a renal cyst and retroperitoneal liposarcoma on imaging.

We present a case of perinephric myxoid pseudotumor of fat, a rare benign entity that often occurs in patients with non-neoplastic renal disease. In our case, an 80 year old man with end-stage renal disease was imaged over the course of 5 years during evaluation for renal transplantation. Imaging identified a left perinephric mass whose appearance over time and on different imaging modalities variably suggested a simple cyst, cystic neoplasm, and liposarcoma. Contrast enhanced examination was necessary to discern the solid nature of this mass, and ultimately, tissue sampling with histopatholog

Risk Factors Influencing the Thickness and Stranding of Perinephric Fat of Mayo Adhesive Probability Score in Minimally Invasive Nephrectomy.

BACKGROUND Mayo adhesive probability (MAP) score, an accurate and reliable predictor of adherent perinephric fat (APF), consists of posterior perinephric fat thickness and perinephric fat stranding. The present study aimed to identify the potential clinical characteristics associated with these 2 variables to further our understanding of APF. MATERIAL AND METHODS Clinical data of 346 patients subjected to minimally invasive nephrectomy was collected within our prospectively maintained database, between January 2015 and December 2016. Radiological data was assessed by 2 readers in an independen

Clinical impact of perinephric fat stranding detected on computed tomography in patients with acute pyelonephritis: a retrospective observational study.

Perinephric fat stranding (PFS) is often detected on computed tomography (CT) in patients with acute pyelonephritis (APN). However, its clinical impact remains unclear. This study aimed to evaluate the clinical impact of PFS detected on CT in patients with APN. This retrospective observational study included patients with APN who underwent CT (median age, 79.5 years). Patients were classified into PFS (patients with PFS observed on CT) and non-PFS (patients without PFS observed on CT) groups, which were further classified into bacteraemia and non-bacteraemia groups. Clinical findings between t

Extensive perinephric hematoma following excessive irrigation pressure during flexible ureteroscopy: case report of a preventable complication.

BACKGROUND: Flexible ureteroscopy (fURS) is widely regarded as a safe and effective procedure for the management of urolithiasis. Nevertheless, inadequate management of intrarenal pressure during fURS represents an underrecognized patient safety hazard. The use of high-pressure irrigation to compensate for poor visualization—particularly when outflow is restricted—may expose patients to prolonged supraphysiologic intrarenal pressure and preventable harm. I report a severe pressure-related renal injury following fURS and analyze the event using a human factors and systems-based root cause analy

Diffuse Large B-cell Lymphoma in a Patient With Refractory Perinephric Fluid Collection and Pleural Effusion: A Case Report.

Perinephric fluid collections have a broad differential diagnosis and may be difficult to characterize by imaging or fluid analysis. Diffuse large B-cell lymphoma (DLBCL) typically presents as mass-like lesions in nodal or extranodal sites but may rarely present as isolated fluid collections. We report a 53-year-old woman with acute right flank pain after minor trauma. Imaging showed a large right perinephric fluid collection extending into the retroperitoneum and pleural cavity without an identifiable mass. Fluid studies and lymphangiography did not identify a clear underlying etiology. Despi

Perinephric fat volume and its negative correlation with the eGFR according to a population health screening: a cross-sectional study.

BACKGROUND: Chronic kidney disease (CKD) affects 10% of the global population. This study investigated the correlation between ultrasonographically assessed perinephric fat volume (PrFV) and the estimated glomerular filtration rate (eGFR) through a health screening of a Chinese population and explored the potential of the PrFV as a modifiable risk factor for CKD. METHODS: This cross-sectional study included 213 adults aged 18 years or older from the Second Hospital of Hebei Medical University. Demographic data, medical history, the results of anthropometric assessments, and laboratory data we

Robotic Resection of Retroperitoneal Perinephric Tumor. Application of Intraurethral Indocyanine Green Injection As an Adjunct to Avoid Ureteral Injury.

The majority of retroperitoneal mass excision is performed via conventional "open" laparotomy due to concerns of technical difficulty and adequate oncological margins in cases of a malignant sarcoma. A very few cases of minimally invasive resection by laparoscopy had been reported in the literature. Despite the rapid adoption of robotic technology in general surgery and surgical oncology, the robotic technique has not been applied for this pathology. We discussed a complete resection of a large perinephric tumor using a robotic platform. To our knowledge, this is the first study to report the

Verification of renal tissue regeneration using human perinephric fat-derived collagen sheet in a partially nephrectomized animal model.

Chronic kidney disease (CKD), a global health issue, affects approximately 10% of the population. However, limited treatment options, such as dialysis or transplantation, have significant drawbacks. Therefore, this study aims to investigate the potential of a collagen sheet derived from human perirenal adipose tissue for kidney regeneration. Collagen sheets were derived from discarded perirenal adipose tissues and implanted into partially nephrectomized mice. The right kidneys were completely removed, and 2 mm of the upper and lower poles of the left kidneys were resected. A collagen sheet mea

Successful treatment of giant perinephric abscess associated with decompensated cirrhosis caused by hepatitis B and alcohol consumption: A case report.

We describe the case of a 67-year-old male with decompensated liver cirrhosis caused by hepatitis B virus and alcohol consumption who presented with diarrhea and fever. Contrast CT of the abdomen revealed giant perirenal abscess. Klebsiella pneumoniae ssp pneumoniae was cultured from pus in perirenal abscess but not the blood. Haematogenous spread may have resulted in perirenal abscess in this case. The patient was successfully treated by percutaneous drainage, antimicrobial therapy and albumin infusion. With high mortality rates, early diagnosis and effectively treatment of perirenal abscess

Pantothenic acid refeeding diminishes the liver, perinephrical fats, and plasma fats accumulated by pantothenic acid deficiency and/or ethanol consumption.

OBJECTIVE: Pantothenic acid (PaA) is a vitamin that is an integral part of coenzyme A (CoA). CoA is an essential coenzyme in fat metabolism. The aim of this study was to determine whether PaA deficiency causes the accumulation of tissue fats and, if so, can refeeding of PaA decrease such accumulated fat. METHODS: Weaning rats were fed the PaA-free diet for 30 d. Rats were then divided into two groups. One group was continuously fed the PaA-free diet, and the other was fed the PaA-containing diet for an additional 13 d. At the end of the experiment, liver fat and perinephric fat were weighed,

Characteristics and factors influencing treatment outcome of renal and perinephric abscess--a 5-year experience at a tertiary teaching hospital in Taiwan.

BACKGROUND AND PURPOSE: Diagnosis of renal or perinephric abscess is still a challenge for physicians. This study investigated the effect of location and size of abscess and the time of diagnosis on treatment outcomes. METHODS: This retrospective analysis enrolled 159 adult patients with renal, perinephric or mixed-type (renal plus perinephric) abscess hospitalized between June 2001 and June 2006. The outcomes of these patients were classified into "poor" and "success" in order to elucidate associated risk factors. 106 patients had clear information of the timing of diagnosis and were categor

[Imaging of acute urinary infections in adults. Radiologic findings and strategy for imaging procedures].

Urinary infection is very common in women. Generally no imaging is needed. The role of imaging is mandatory in the following cases: renal infection, urinary obstructive syndrome, immuno-compromised patients. The results of imaging in pathology are described: pyelonephritis, renal and perinephric abscesses with or without phlegmon, perinephritis and emphysematous perinephritis. A strategy for imaging procedures is proposed.

Bilateral Renal Fungal Bezoars and Perinephric Abscess in an Infant With Arthrogryposis-Renal Dysfunction-Cholestasis Syndrome: A Clinico-pathologic Case Report.

The patients with Arthrogryposis-Renal dysfunction-Cholestasis (ARC) syndrome have genetic susceptibility to the opportunistic infections due to the involvement of VPS33B (vacuolar protein sorting 33 homolog B) in phagolysosome fusion in macrophages. Detailed pathologic studies in ARC patients are missing in literature due to the lack of autopsy. We described the first autopsy case of ARC syndrome in a 2-month-old male infant. His death was due to recurrent sepsis and multiorgan failure despite the appropriate poly-antibiotic therapy and supportive care. The autopsy showed invasive renal candi
