Laparoscopic lavage versus surgical resection for acute diverticulitis with generalised peritonitis: a systematic review and meta-analysis.

This systematic review and meta-analysis investigates current evidence on the therapeutic role of laparoscopic lavage in the management of diverticular peritonitis. A systematic review of the literature was performed on PubMed until June 2016, according to preferred reporting items for systematic reviews and meta-analyses guidelines. All randomised controlled trials comparing laparoscopic lavage with surgical resection, irrespective of anastomosis or stoma formation, were analysed. After assessment of titles and full text, 3 randomised trials fulfilled the inclusion criteria. Overall the quali

The use of computerized axial tomography versus peritoneal lavage in the evaluation of blunt abdominal trauma: a prospective study.

Computerized axial tomography (CAT) of the abdomen was prospectively compared with peritoneal lavage (PL) in 19 patients who sustained acute blunt abdominal trauma. All were selected carefully and were deemed stable, never having been in shock, and all required diagnostic PL. Seven patients underwent exploratory laparotomy on the basis of CAT and PL findings. There were no negative findings at laparotomy. Three significant splenic injuries, one hepatic laceration, and two hemoperitoneums were undetected by CAT. All seven cases explored were preceded by a grossly positive PL. Thus no major inju

Effectiveness of intraoperative peritoneal lavage with saline in patient with intra-abdominal infections: a systematic review and meta-analysis.

BACKGROUND: Intraoperative peritoneal lavage (IOPL) with saline has been widely used in surgical practice. However, the effectiveness of IOPL with saline in patients with intra-abdominal infections (IAIs) remains controversial. This study aims to systematically review randomized controlled trials (RCTs) evaluating the effectiveness of IOPL in patients with IAIs. METHODS: The databases of PubMed, Embase, Web of Science, Cochrane library, CNKI, WanFang, and CBM databases were searched from inception to December 31, 2022. Random-effects models were used to calculate the risk ratio (RR), mean dif

Standing laparoscopic abdominal lavage using a suction-irrigation device in 2 horses with primary suppurative peritonitis.

The use of a laparoscopic suction-irrigation device in 2 standing horses for lavage of the abdomen for the treatment of primary suppurative peritonitis is reported. Two horses were presented with a 1- to 2-week history of weight loss. Abdominocentesis revealed highly elevated total nucleated cell count. Peritoneal lavage systems were placed in both horses, but complications prevented adequate lavage. Both horses underwent standing laparoscopy; the dorsal abdomen was explored and the abdomen was profusely lavaged, using a suction-irrigation device. The procedure was efficient and allowed adequa

A decision analysis comparing three strategies for peritoneal lavage cytology testing in staging of gastric cancer in China.

BACKGROUND: Positive peritoneal cytology (PCY) indicates metastasis (M1) in gastric cancer (GC) patients; both the American and Chinese guidelines recommend laparoscopic peritoneal lavage (LPL) for cytology. However, relatively high costs impair the widespread use of LPL in some resource-limited regions in China, and the cost-effectiveness of PCY testing remains unclear. Therefore, we performed a decision analysis to evaluate the cost-effectiveness of PCY testing by comparing the guideline-recommended intraoperative LPL, a newly proposed preoperative percutaneous peritoneal lavage (PPL), and a

[Results of therapy combining peritoneal lavage and local application of antibiotics in 250 patients with acute perforating appendicitis].

Since 1967, 2480 appendectomies have been performed at the St. Lucas Hospital (Amsterdam) and the St. Annadal Hospital (Maastricht), of which 250 were for perforated appendicitis. A so-called 'combined method' of treatment was used in all these cases: during the operation saline was used to lavage the peritoneal cavity with the patients in the reverse-Trendelenburg position. Antibiotics were left in the peritoneal cavity. No intraperitoneal drainage was performed, but the wound was drained for 5 days. Postoperative treatment consisted of nasogastric suction and parenteral antibiotics for 5 day

[The place of lavage-irrigation of the peritonium in peritonitis from the appendix in children. A report of 10 cases (author's transl)].

Confronted with the difficult problems presented in acute appendicular peritonitis, whether generalized or multiple focused, the authors used the "lavage irrigation" method in ten cases. They analysed the local and general conditions that led them to employ this method, in light of the evolution of this technic which they had observed in their experiences whit it. They have attempted to elucidate, using information previously available in the literature, the indications, the steps, and the eventual complications of the "lavage irrigation" of the peritonemum in the child.

Efficacy and outcome of extensive intraoperative peritoneal lavage plus surgery vs surgery alone with advanced gastric cancer patients.

BACKGROUND: Gastric cancer (GC) is one of the most common malignant tumors. After resection, one of the major problems is its peritoneal dissemination and recurrence. Some free cancer cells may still exist after resection. In addition, the surgery itself may lead to the dissemination of tumor cells. Therefore, it is necessary to remove residual tumor cells. Recently, some researchers found that extensive intraoperative peritoneal lavage (EIPL) plus intraperitoneal chemotherapy can improve the prognosis of patients and eradicate peritoneal free cancer for GC patients. However, few studies explo

Lipidomic profiling of peritoneal lavage supernatant identifies PE/PA/PI-enriched recurrence biomarkers in cytology-negative gastric cancer.

BACKGROUND: Peritoneal dissemination accounts for more than 40% of recurrences after radical gastrectomy for gastric cancer. Although peritoneal lavage cytology (CY) is routinely used to detect intraperitoneal free cancer cells, peritoneal recurrence still occurs in a subset of CY-negative patients. Novel molecular biomarkers are urgently needed to identify high-risk individuals within this cohort. METHODS: Direct flow injection quadrupole time-of-flight mass spectrometry (DFI-Q-TOF-MS) in negative ion mode was applied to profile peritoneal lavage supernatants from gastric cancer patients str

Prospective Randomised Control Study to Evaluate the Effectiveness of Hypochlorous Acid as a Peritoneal Lavage Agent to Prevent Surgical Site Infection After Exploratory Laparotomy for Perforation Peritonitis.

Introduction Surgical site infections (SSIs) are a major concern following emergency gastrointestinal procedures, particularly exploratory laparotomy for perforation peritonitis, where infection risk is amplified by the inherent contamination of the operative field. Despite prophylactic antibiotics, the microbial burden frequently surpasses host defence thresholds. Intraoperative peritoneal lavage with antimicrobial agents such as hypochlorous acid (HOCl) has emerged as a promising strategy, combining physical removal of contaminants with direct microbicidal action. Methods This was a prospect

Laparoscopic peritoneal lavage versus surgical resection for the treatment of complicated perforated diverticulitis: A systematic review and meta-analysis.

Introduction: Colonic diverticular disease (diverticulosis) is a currently worldwide increasing gastrointestinal disorder with a particularly high prevalence in Western countries. The operative treatment of acute diverticulitis could be executed through open or laparoscopic techniques. A non-resectional procedure, namely laparoscopic peritoneal lavage (LPL) and drainage, has also been adopted as a less invasive treatment strategy to treat patients with diverticular perforation and purulent peritonitis.Aim: The present work was conducted to pool the current

Detection and isolation of free cancer cells from ascites and peritoneal lavages using optically induced electrokinetics (OEK).

Detection of free gastric cancer cells in peritoneal lavages and ascites plays a vital role in gastric cancer. However, due to the low content of cancer cells in patients' peritoneal lavages, traditional detection methods lack sensitivity and cannot satisfy clinical demand. In this study, we used an optically induced electrokinetics (OEK) microfluidic method for label-free separation and characterization of patient gastric cancer cells. This method showed high effectiveness and sensitivity. We successfully separated cancer cells from a simulated peritoneal lavage mixture of gastric cancer cell

CT scan and Diagnostic Peritoneal Lavage: towards a better diagnosis in the area of nonoperative management of blunt abdominal trauma.

BACKGROUND: The diagnosis of small bowel and mesenteric injuries (BBMI) after blunt abdominal trauma remains difficult, which results in delayed treatment and increased mortality and morbidity. Diagnostic peritoneal lavage (DPL) in patients with 1 or 2 abnormal CT findings that are suggestive of BBMI was proposed, but the rate of unnecessary surgical exploration remains high. PATIENTS AND METHODS: Blunt abdominal trauma patients with 1 or 2 CT findings predictive of BBMI from 2001 to 2014 underwent a DPL with calculation of a cell count ratio (CCR) dividing the ratio of white blood cells (WBC

Separation and single-cell analysis for free gastric cancer cells in ascites and peritoneal lavages based on microfluidic chips.

BACKGROUNDS: Detecting free cancer cells from ascites and peritoneal lavages is crucial for diagnosing gastric cancer (GC). However, traditional methods are limited for early-stage diagnosis due to their low sensitivity. METHODS: A label-free, rapid, and high-throughput technique was developed for separating cancer cells from ascites and peritoneal lavages using an integrated microfluidic device, taking advantage of dean flow fractionation and deterministic lateral displacement. Afterward, separated cells were analyzed using a microfluidic single-cell trapping array chip (SCTA-chip). In situ
