Prospective observational study of conjunctival scarring after phacoemulsification.

PURPOSE: To examine whether anterior segment-optical coherence tomography (AS-OCT) can detect conjunctival scarring after transscleral phacoemulsification and whether temporal transscleral phacoemulsification causes scarring in the superior conjunctiva. METHODS: Transscleral phacoemulsification was performed in the superior conjunctiva (superior incision group) or the temporal conjunctiva (temporal incision group). Anterior segment-optical coherence tomography (AS-OCT) images of the superior conjunctiva were obtained before and after surgery. We quantified the thickness of the conjunctiva and

Alternaria keratitis and hypopyon after clear-cornea phacoemulsification.

UNLABELLED: We report a case of Alternaria keratitis and hypopyon following clear-corneal cataract surgery. A 66-year-old woman presented with a painful red left eye several months after uneventful self-sealing clear-corneal phacoemulsification that was unresponsive to prolonged treatment with topical/oral quinolones and topical corticosteroids. A full-thickness stromal white dense infiltrate in the area of the intrastromal tunnel incision and a 2.0 mm hypopyon were observed. Culture from corneal scrapings revealed Alternaria species. Treatment included topical and subconjunctival injections o

Transient retinal artery occlusion during phacoemulsification cataract surgery.

PURPOSE: Transient retinal artery occlusion (TRAO) is a potentially underdiagnosed cause of immediate 'pad off' visual loss following phacoemulsification cataract surgery under sub-Tenon's anaesthesia. METHODS: We describe a series of three patients presenting with enigmatic 'pad off' visual loss following phacoemulsification surgery, each diagnosed with TRAO. We describe the variable clinical presentation, illustrate the value of optical coherence tomography (OCT) imaging in establishing the diagnosis, and present the final visual outcomes. RESULTS: Clinical findings alone may be subtle and

Migration of retained broken chopper tip after phacoemulsification.

We report removal of a retained broken chopper tip after phacoemulsification. Although the foreign body was thought to be stable in the ciliary sulcus 2 months postoperatively, it dislocated into the anterior chamber 8 months later. It was removed with a microforceps under gonioscopic visualization. Although previous reports suggest that retained metallic fragments after phacoemulsification can be left in place if they are difficult to access and appear stationary, our case highlights the potential for long-term instability. We therefore recommend consideration of surgical removal with the use

[Anterior capsule contraction syndrome after cataract phacoemulsification surgery].

Cataract facoemulsification with continuous anterior capsulorhexis and foldable intraocular lens implantation into the capsular bag has become basic, standard method in cataract surgery and curvilinear capsulorhexis s the most common technique for opening the anterior lens capsule. The opening, made during the operation, within several months of postoperative evolution gradually constricts and in extreme cases it may even close completely. That course of healing is not desirable because of evoking a noted impairment of vision. Decreasing in the anterior capsule opening is observed mostly in ey

Retained posterior segment indocyanine green dye after phacoemulsification.

We describe a patient who had indocyanine green (ICG) staining of the anterior capsule for phacoemulsification of a traumatic cataract. Intraoperatively, the ICG extended into the posterior segment. Postoperatively, the patient developed ICG toxicity to the retina and had a vitrectomy to remove the ICG. We believe ICG extended into the vitreous through an area of zonular dehiscence.

[Primary phacoemulsification following acute primary angle closure glaucoma].

Recent developments and clinical studies indicate that primary phacoemulsification and intraocular lens implantation are safe and effective for the surgical treatment of primary angle closure glaucoma (ACG) compared to conventional iridectomy or laser-iridotomy. When compared to control eyes treated using standard peripheral iridectomy, the outcome in terms of intraocular pressure control, adjunct anti-glaucoma medication, visual acuity, and the necessity for successive surgical interventions favored primary phacoemulsification and intraocular lens implantation. Earlier biometric data underlin

Early intraocular pressure after phacoemulsification combined with trabeculectomy.

BACKGROUND AND OBJECTIVE: To determine the efficacy of combined phacoemulsification--trabeculectomy in preventing early postoperative increases in intraocular pressure (IOP). PATIENTS AND METHODS: Eighty patients were enrolled in a prospective cohort study. Thirty-six patients with cataracts and uncontrolled glaucoma underwent combined phacoemulsification--trabeculectomy, and 44 patients with cataracts underwent phacoemulsification alone. The operations were performed in a standardized manner by one surgeon. IOP was measured at 4 hours, 1 day, and 7 days after surgery. The need for interventi

Visual acuity immediately following phacoemulsification with subconjunctival anesthesia.

The authors attempted to discover the quality of vision experienced immediately after cataract surgery by those patients receiving subconjunctival anesthesia and no postoperative patching. Eighty-five consecutive patients who underwent phacoemulsification were studied. Anesthesia was topical tetracaine followed by a subconjunctival injection of a lidocaine-bupivicaine hydrochloride-hyaluronidase mixture. Visual acuities were measured within a half hour after surgery and at 1 day postoperatively. Immediate postoperative visual acuities ranged from 20/25 to 20/200. Visual acuities were 20/40 or

Choroidal ischemia after extracapsular cataract extraction by phacoemulsification.

Three patients developed apparent choroidal ischemia after phacoemulsification. The outer half of the posterior retina appeared white after operation, with confluent lesions in the posterior pole and splotchy white areas in the midperiphery. The separate lesions appeared similar in pattern to the lobular division of the choriocapillaris, and the retinal vessels were not involved. The white lesions resolved in two to three weeks, leaving alterations in the pigment epithelium. Vision was transiently reduced in each eye but returned to a nearly normal level in two of three affected eyes, although

Corneal endothelial cells and acoustic cavitation in phacoemulsification.

Postoperative complications of phacoemulsification, such as corneal edema caused by human corneal endothelial cell (CEC) injury, are still a matter of concern. Although several factors are known to cause CEC damage, the influence of ultrasound on the formation of free radicals during surgery should be considered. Ultrasound in aqueous humor induces cavitation and promotes the formation of hydroxyl radicals or reactive oxygen species (ROS). ROS-induced apoptosis and autophagy in phacoemulsification have been suggested to significantly promote CEC injury. CEC cannot regenerate after injury, and

Combined endocyclophotocoagulation and phacoemulsification in the management of moderate glaucoma.

Cataract and glaucoma are the leading causes of blindness worldwide and commonly coexist in elderly patients. Endocyclophotocoagulation is a minimally invasive approach for glaucoma management that is amenable to combination with cataract surgery. We review the literature on endocyclophotocoagulation and evaluate intraocular pressure reduction efficacy when combined with phacoemulsification. Several studies demonstrate that phacoemulsification and endocyclophotocoagulation successfully reduces intraocular pressure and decreases medication burden. Phacoemulsification and endocyclophotocoagulati
