Acute Phosphate Nephropathy Following Oral Sodium Phosphate Bowel Preparation.

Acute phosphate nephropathy (APN) is a rare but important cause of acute kidney injury (AKI), most commonly associated with the use of oral sodium phosphate bowel preparations administered prior to colonoscopy. In this report, we describe the case of a 60-year-old female patient who developed severe AKI necessitating temporary hemodialysis after bowel preparation with a phosphate-containing laxative. Quick identification, cessation of nephrotoxic agents, aggressive intravenous hydration, and supportive hemodialysis therapy facilitated gradual renal recovery. This case points out the importance

Screening bacterial phosphate solubilization with bulk-tricalcium phosphate and hydroxyapatite nanoparticles.

Phosphate-solubilizing bacteria can release phosphorus (P) from insoluble minerals and benefit either soil fitness or plant growth. Bulk sized P compounds have been suggested but little is known about solubilization of nanosized materials such as hydroxyapatite nanoparticles (HANP). A screening of the initial 43 strains from vanilla rhizospheres for phosphate solubilization with bulk Ca3(PO4)2 was carried out. Subsequently, 6 strains were selected on bulk rock phosphate (RP) and HANP. Two kinetics experiments were run out regarding evaluation at 5, 10 and 20 days after inoculation (dai). Bacte

Increasing Soluble Phosphate Species by Treatment of Phosphate Rocks with Acidic Waste.

The development of efficient fertilizers with a diminished environmental footprint will help meet the increasing demand for food and nutrients by a growing global population. Our objective was to evaluate whether an acidic mine waste (AMW) could be used beneficially by reacting it with sparingly soluble phosphate rocks (PRs) to produce more soluble P fertilizer materials. Three PRs from Brazil and Peru were reacted with different concentrations of AMW. Changes in mineralogy and P species were determined using a combination of X-ray diffraction and phosphorus K-edge XANES spectroscopy, in addit

Osteoconductive property of a mechanical mixture of octacalcium phosphate and amorphous calcium phosphate.

The present study was designed to investigate the extent of osteoconductive property of a mechanical mixture of octacalcium phosphate (OCP) and amorphous calcium phosphate (ACP). OCP was mixed with ACP in granules that had a diameter of 300 and 500 μm, respectively, and at 25, 50, or 75 wt %. The physicochemical characteristics and the osteoconductive properties of the mixtures were compared with OCP alone or ACP alone through implantation into rat critical-sized calvaria defects for up to 12 weeks and simulated body fluid (SBF) immersion for 2 weeks. The mixtures of OCP and ACP, in particular

Oral phosphate binders for the management of serum phosphate levels in dialysis patients.

Hyperphosphataemia is an inevitable consequence of end stage chronic kidney disease and is present in the majority of dialysis patients. Hyperphosphataemia is statistically associated with increased cardiovascular mortality among dialysis patients. Dietary restriction of phosphate and current dialysis modalities are not sufficiently effective to maintain serum phosphate levels within the recommended range so that the majority of dialysis patients require oral phosphate binders. However, benefits of achieving the recommended range have yet to be demonstrated prospectively. Unfortunately, conven

Radioactive rock phosphate: the feed stock of phosphate fertilizers used in Pakistan.

The existence of natural radioactivity in phosphate fertilizers originates from the rock phosphate that is used as a feed stock for the manufacturing of various types of phosphate fertilizers. Gamma ray activity mass concentrations have been determined in phosphate rock and fertilizer samples collected from different fertilizer manufacturing factories and suppliers in Pakistan. The technique of gamma ray spectrometry was applied using a HPGe (high purity germanium) gamma ray detector coupled with a personal computer (PC) based multichannel analyzer (MCA). The specific activity values of 40K, 2

Crystalline phase differentiation in urinary calcium phosphate and magnesium phosphate calculi.

UNLABELLED: Phosphates are encountered as the main components in about 15% of urinary calculi. Except for struvite, no specific correlations have been found between the crystalline phase of the phosphates and the cause of nephrolithiasis. OBJECTIVE: The relationship between aetiological factors and crystalline phases or carbonate rate in calcium phosphate stones were assessed. MATERIAL AND METHODS: From a series of 1148 phosphate calculi, we investigated the relationship between composition and aetiological factors. RESULTS: Carbapatite was the most frequent crystalline phase (74.0%). It wa

Candida albicans' inorganic phosphate transport and evolutionary adaptation to phosphate scarcity.

Phosphorus is essential in all cells' structural, metabolic and regulatory functions. For fungal cells that import inorganic phosphate (Pi) up a steep concentration gradient, surface Pi transporters are critical capacitators of growth. Fungi must deploy Pi transporters that enable optimal Pi uptake in pH and Pi concentration ranges prevalent in their environments. Single, triple and quadruple mutants were used to characterize the four Pi transporters we identified for the human fungal pathogen Candida albicans, which must adapt to alkaline conditions during invasion of the host bloodstream and

How do we sense phosphate to regulate serum phosphate level?

Abnormal phosphate levels result in several pathological conditions such as rickets/osteomalacia and ectopic calcification indicating that there must be a system that regulates phosphate level within a narrow range. FGF23 has been shown to be an essential hormone regulating serum phosphate level. FGF23 binds to Klotho-FGF receptor complex to reduce serum phosphate level. Several reports suggested that FGF receptor is involved in the regulation of FGF23 production. It has been also shown that high extracellular phosphate can activate several intracellular signaling pathways. However, it has bee

Inorganic phosphate homeostasis in sodium-dependent phosphate cotransporter Npt2b⁺/⁻ mice.

An inorganic phosphate (P(i))-restricted diet is important for patients with chronic kidney disease and patients on hemodialysis. Phosphate binders are essential for preventing hyperphosphatemia and ectopic calcification. The sodium-dependent P(i) (Na/P(i)) transport system is involved in intestinal P(i) absorption and is regulated by several factors. The type II sodium-dependent P(i) transporter Npt2b is expressed in the brush-border membrane in intestinal epithelial cells and transports P(i). In the present study, we analyzed the phenotype of Npt2b(-/-) and hetero(+/-) mice. Npt2b(-/-) mice

[Management of phosphate in chronic kidney disease--Chemistry and history of phosphate binder].

In 1960s and 1970s when hemodialysis therapy was started in Japan, calcium carbonate was used as a phosphate binder and as the purpose of calcium supplement. Aluminum preparations were widely used in Japan, Europe and United states since the end of 1970s, when these drugs were reported as a strong phosphate binder. After that, some adverse effects such as encephalopathy and bone lesions attributed to Aluminum administration have become serious problems in dialysis patients. As a result, Aluminum administration was prohibited in 1992 in Japan. Oral vitamin D pulse therapy was developed in 1990s

Regulation of rat intestinal Na-dependent phosphate transporters by dietary phosphate.

Hyperphosphatemia associated with chronic kidney disease is one of the factors that can promote vascular calcification, and intestinal P(i) absorption is one of the pharmacological targets that prevents it. The type II Na-P(i) cotransporter NaPi-2b is the major transporter that mediates P(i) reabsorption in the intestine. The potential role and regulation of other Na-P(i) transporters remain unknown. We have identified expression of the type III Na-P(i) cotransporter PiT-1 in the apical membrane of enterocytes. Na-P(i) transport activity and NaPi-2b and PiT-1 proteins are mostly expressed in t

Effects of insulin and glucose on renal phosphate reabsorption: interactions with dietary phosphate.

Both insulin deficiency and glycosuria are known to inhibit the tubular reabsorption of phosphate. This inhibition has previously been evaluated either in the fasted state or on a normal phosphate diet. The goal of this study was to evaluate how dietary phosphate depletion affected the relative effects of insulin deficiency and glycosuria on the tubular reabsorption of phosphate. Rats were maintained on either a low- (0.03%) or normal (0.8%) phosphate diet. After 5 days, one half of the animals in each group received streptozotocin to induce short-term insulin deficiency, whereas the other hal

Role of Phosphate in Biomineralization.

Inorganic phosphate is a vital constituent of cells and cell membranes, body fluids, and hard tissues. It is a major intracellular divalent anion, participates in many genetic, energy and intermediary metabolic pathways, and is important for bone health. Although we usually think of phosphate mostly in terms of its level in the serum, it is needed for many biological and structural functions of the body. Availability of adequate calcium and inorganic phosphate in the right proportions at the right place is essential for proper acquisition, biomineralization, and maintenance of mass and strengt

Mechanisms of phosphate transport.

Over the past 25 years, successive cloning of SLC34A1, SLC34A2 and SLC34A3, which encode the sodium-dependent inorganic phosphate (Pi) cotransport proteins 2a-2c, has facilitated the identification of molecular mechanisms that underlie the regulation of renal and intestinal Pi transport. Pi and various hormones, including parathyroid hormone and phosphatonins, such as fibroblast growth factor 23, regulate the activity of these Pi transporters through transcriptional, translational and post-translational mechanisms involving interactions with PDZ domain-containing proteins, lipid microdomains a

Phosphate-a poison for humans?

Maintenance of phosphate balance is essential for life, and mammals have developed a sophisticated system to regulate phosphate homeostasis over the course of evolution. However, due to the dependence of phosphate elimination on the kidney, humans with decreased kidney function are likely to be in a positive phosphate balance. Phosphate excess has been well recognized as a critical factor in the pathogenesis of mineral and bone disorders associated with chronic kidney disease, but recent investigations have also uncovered toxic effects of phosphate on the cardiovascular system and the aging pr

Renal phosphate transporters.

PURPOSE OF REVIEW: Phosphate homeostasis is tightly controlled by the coordinated activity of bone, kidney, intestine, and parathyroid gland. The renal phosphate transporters have emerged as key regulators of both total body phosphate homeostasis and serum phosphate concentration. This review focuses on the latest updates in phosphate transport and transporters with an emphasis on renal phosphate transporters. RECENT FINDINGS: Structure function analysis of type II sodium phosphate cotransporters has revealed motifs with significant similarity to those seen in other sodium-coupled solute tran

[Phosphate sensing and osteocyte].

Osteocyte expresses molecules responsible for hereditary hypophosphatemic rickets such as FGF23, DMP1 and PHEX. FGF23 exerts its effects in kidney by decreasing both renal phosphate (Pi) reabsorption and vitamin D activation. It is suggested that DMP1 and PHEX negatively regulate the expression of FGF23, although the underlying mechanisms remain to be elucidated. Circulating level of FGF23 appears to be regulated by systemic factors as well, including vitamin D and PTH. Although it is unknown how mammals sense the Pi availability, it has been recently reported that extracellular Pi itself trig
