Is conservative treatment an effective option for pilonidal sinus disease in children?

Pilonidal sinus disease (PSD) is associated with a complex disease process in children, and its management remains controversial. There are a few published studies on PSD in the paediatric literature; therefore, we present our experience of conservatively treating PSD in children. This study involved a retrospective review with telephone follow up. All children diagnosed with PSD in 2012 to 2017 were identified at the outpatient clinic of the Department of Paediatric Surgery. All patients initially underwent conservative treatment (meticulous hair removal, improved perianal hygiene, warm sitz

Effectiveness of Limberg and Karydakis flap in recurrent pilonidal sinus disease.

OBJECTIVE: Sacrococcygeal pilonidal sinus is common in young men and may recur over time after surgery. We investigated whether a factor exists that can aid in the determination of the preferred technique between the early Limberg flap and Karydakis flap techniques for treating recurrent pilonidal sinus. MATERIALS AND METHODS: This prospective and randomized study enrolled 71 patients with recurrent pilonidal sinus in whom the Limberg flap or Karydakis flap techniques were applied for reconstruction after excision. Patients were divided into two groups as follows: 37 patients were treated wit

Safety and/or effectiveness of methylene blue-guided pilonidal sinus surgery.

UNLABELLED: The effectiveness of methylene blue-guided sacrococcygeal pilonidal sinus disease (SPSD) surgery for the prevention of incomplete excision was evaluated. METHOD: It was a prospective randomized clinical trial that included 33 patients diagnosed with SPSD. Before the excision, methylene blue was injected into the opening of each sinus. At the first excision, all areas that were stained with methylene blue were resected along with the opening of the sinus (Group 1). At the discretion of the surgeon, all areas with granulation tissue were macroscopically re-excised as a single specim

Comparison of the three surgical flap techniques in pilonidal sinus surgery.

We aimed to study the efficacy of three surgical flap techniques in pilonidal surgery. Pilonidal disease is characterized by chronic inflammation and infection in the sacrococcygeal region. Complications and recurrence are common after treatment and optimal treatment for the disease has not been established yet. We enrolled a total of 401 patients in this study. Patients have been treated with Karydakis (n = 113), modified Limberg (n = 179), or Limberg (n = 109) flap techniques. Mean off-work period, time to walk without pain, time to sit on the toilet, time to take the drainage catheter off,

A fatal case of carcinoma arising from a pilonidal sinus tract.

We report a male patient with carcinoma arising on the basis of neglected sacrococcygeal pilonidal sinus disease. Following initial operation, performed without suspicion of malignancy, histology demonstrated cellular atypia and an increased mitotic rate. A second, wider tissue excision was recommended but the patient declined further surgery. Two years later, he presented with fungating carcinoma involving the rectum but again declined surgery. This rare case demonstrates that the presence of carcinoma should be suspected in long-standing, although innocent-looking, pilonidal sinus disease. I

Bascom's operation in the day-surgical management of symptomatic pilonidal sinus.

BACKGROUND: Many treatments for symptomatic pilonidal sinus disease have been described. Ambulatory treatment with minimal morbidity and a rapid return to normal activity is desirable. Bascom's operation fulfils these requirements. This paper describes the operative technique and results of treatment. METHODS: Some 218 patients of mean age 27 years were treated as day cases. The mean duration of symptoms was 2.4 years. Sixty-eight per cent of patients had complex disease with more than two midline pits and/or a lateral discharging sinus. One hundred and eighty-three patients (84 per cent) wer

Pilonidal sinus: a 5-year appraisal of the Millar-Lord treatment.

One hundred and two patients with a pilonidal sinus treated by minor excision had a life-table of 5-year freedom from recurrence of 89% provided that they attended a dressing clinic until the wound healed. With failure to attend the clinic, the recurrence freedom fell to 57% (P less than 0.004). Age, sex, acute abscess, previous excision, presence of hairs and the number of midline pits and lateral abscesses did not significantly influence the result. The method supersedes more extensive procedures, but should be tested against even less traumatic techniques.

A case report of sacrococcygeal foreign body presenting as pilonidal sinus.

Pilonidal disease occurs most commonly in those males who have to sit long hours at their work place e.g. online office workers or drivers. It is caused by piercing of broken hairs into the sacrococcygeal region which causes localised inflammation. Inflammation in this area due to any other foreign body is very rare. Among many treatment options for pilonidal sinus, instillation of crystalloid phenol showed promising results in terms of low recurrence rates, low post-operative complications and less healing time. Here, we present the case of a 13-year-old female student who had pilonidal sinus

Rethinking the causes of pilonidal sinus disease: a matched cohort study.

Our understanding of pilonidal sinus disease (PSD) is based on a paper published 29 years ago by Karydakis. Since then, surgeons have been taught that hair more easily penetrates wet skin, leading to the assumption that sweating promotes PSD. This postulate, however, has never been proven. Thus we used pilocarpine iontophoresis to assess sweating in the glabella sacralis. 100 patients treated for PSD and 100 controls were matched for sex, age and body mass index (BMI). Pilocarpine iontophoresis was performed for 5 min, followed by 15 min of sweat collection. PSD patients sweated less than thei

Pilonidal sinus disease of the penis: A case report and review of the literature.

Pilonidal sinus disease (PSD) is an acquired pathological condition more commonly seen in the natal cleft of the sacrococcygeal area, although its presentation is not limited to the natal cleft and it can present in other regions of the body such as the breast, umbilicus, scalp and penis. We present the case of a 28-year-old gentleman who presented to his local Urology outpatient clinic with an unusual penile lesion that was later identified as a pilonidal sinus. This was treated with radical circumcision and penile reconstruction with a good functional outcome. For surgeons unaccustomed to PS

Outcomes of conservative treatment of 134 cases of umbilical pilonidal sinus.

BACKGROUND: There are different opinions about the treatment of umbilical pilonidal sinus. The aim of the present study was to evaluate the outcome of conservative treatment of this disease. METHODS: All patients with umbilical pilonidal sinus who attended our private surgical clinic in Erbil, Kurdistan, from January 1997 to August 2011, were treated by conservative treatment. Umbilectomy was performed for those who did not respond to the conservative treatment after three sessions. RESULTS: A total of 134 patients were seen; 121 (90.3 %) were men. Only 105 (78.35 %) patients came back for t

Pilonidal sinus cyst of the penis: a rare manifestation of a common disease.

Pilonidal sinus is a chronic inflammatory condition owing to the subcutaneous trapping of hair. Most commonly it is found in the sacrococcygeal region (natal cleft) but rarely it is found on the penis with very few cases reported in the literature worldwide. We are reporting a case of a pilonidal sinus growing on the distal penile shaft with the sinus opening to the mucosal layer of the foreskin, in a young and fit patient. The cyst was removed with a circumcision and found to contain hair. This was confirmed by histology as a pilonidal sinus cyst.

Evaluation of the risk factors of pilonidal sinus: a single center experience.

BACKGROUND/AIMS: We aimed to evaluate which factors, if prevented, could facilitate a decrease in the rate of pilonidal sinus. MATERIALS AND METHODS: From November 2008 to 2010, all patients referred to the surgery clinic were examined by the general surgery attending physician. Patients with a diagnosis of pilonidal sinus were considered as the trial group. The control group included healthy persons who accompanied the patients to the clinic. Both groups completed a questionnaire form, which included age, sex, occupation, weight, height, number of baths taken per week, mean duration of sitti

Modified Limberg transposition flap in the treatment of pilonidal sinus disease.

BACKGROUND: Pilonidal sinus disease (PSD) is a common problem in surgical practice. Different non-surgical and surgical methods have been used for treating PSD. Flap techniques including the Limberg flap have become more popular in recent years. A modified Limberg flap was used to reduce the problems of skin maceration and recurrence associated with the conventional Limberg flap technique. The aim of this retrospective study was to assess the effectiveness of the modified Limberg flap technique for PSD. METHODS: Medical records of 94 patients with PSD who had been treated with a modified Limb
