OBJECTIVE: Sacroiliac joint (SIJ) dysfunction, piriformis syndrome (PFS) and tarsal tunnel syndrome (TTS) produce symptoms similar to lumbar degenerative disease (LDD). Patients who have these diseases plus LDD sometimes experience residual symptoms after surgery for LDD. We therefore assessed the results of treatment of SIJ dysfunction, PFS and TTS associated with LDD.
PATIENTS AND METHODS: We assessed 25 patients who underwent surgery for LDD and were affected with SIJ dysfunction (12 patients), PFS (7 patients) or TTS (6 patients). SIJ dysfunction was treated with rest, drugs, pelvic band

Meralgia paresthetica (MP) is an entrapment neuropathy of the lateral femoral cutaneous nerve (LFCN) that is typically caused by mechanical compression. Piriformis syndrome (PS) is an uncommon neuromuscular disorder resulting from sciatic nerve compression or irritation by the piriformis muscle. The concurrent occurrence of MP and PS is rare and may pose significant diagnostic challenges because of overlapping sensory and radicular-like symptoms. We report the case of a 30-year-old previously healthy man who developed bilateral burning pain over the anterolateral thighs and right gluteal disco

BACKGROUND: Piriformis syndrome (PS) is an uncommon disease characterized by symptoms resulting from compression/irritation of the sciatic nerve by the piriformis muscle. Uncertainty and controversy remain regarding the proper diagnosis and most effective form of treatment for PS. This study analyzes the diagnostic methods and efficacy of conservative and surgical treatments for PS.
METHODS: From March 2006 to February 2013, we retrospectively reviewed 239 patients who were diagnosed with PS and screened them for eligibility according to our inclusion/exclusion criteria. All patients underwen

Piriformis syndrome (PS) is an uncommon entrapment neuropathy in which the sciatic nerve is compressed by the piriformis muscle, producing gluteal and radiating leg pain that closely mimics lumbar disc disease and other causes of sciatica, making accurate diagnosis a persistent clinical challenge. Musculoskeletal (MSK) ultrasound has emerged as a valuable imaging modality in this setting, offering real-time, dynamic assessment of muscle morphology, sciatic nerve architecture, and regional vascularity without the use of ionizing radiation. In this retrospective case series of three patients dia

The sciatic nerve (SN), the largest nerve in the human body, normally exits the pelvis inferior to the piriformis muscle (PM) before dividing into its two terminal branches, the tibial nerve (TN) and the common fibular nerve (CFN). Variations in the anatomy of the SN and the posterior femoral cutaneous nerve (PFCN) may occur during embryologic development and can have important clinical implications, including piriformis syndrome, sciatica, and increased susceptibility to iatrogenic injury. This case report describes a rare concurrent variation of both the SN and PFCN identified during routine

ContextPiriformis syndrome is a solely clinical diagnosis that often eludes the practitioner and goes underdiagnosed. PS is a pain syndrome and for those it affects, causes persistent pain and limits daily activity and work capacity. It is a form of deep gluteal syndrome that needs to be considered on the differential of low back pain as it comprises between 0.3% - 6% of all low back pain cases and is frequently underdiagnosed. Piriformis syndrome may be primary due anatomic anomalies or secondary, though the majority of cases are secondary to some insult. The objective of this manuscript is t

INTRODUCTION: Piriformis muscle syndrome (PMS) is a disorder that can lead to symptoms of buttock pain and limited hip-joint mobility, and may have an impact on quality of life.
METHODS: Thirty-two patients with PMS were randomized to the treatment group, which included three sessions of ultrasound-guided dry needling (DN) of the piriformis muscle (n = 16), or a waitlist control group (n = 16). The primary outcome was pain intensity measured on the visual analog scale recorded at baseline and then at 72 hours and 1 week after treatment.
RESULTS: At 1-week follow-up, pain intensity was signif

INTRODUCTION: We report on a patient with an unusual cause of non-discogenic sciatica.
MATERIAL AND METHODS: A 48-year-old woman presented with a 10-day history of gradually worsening left buttock pain radiating to the ipsilateral posterior thigh and calf. She had a similar episode of less intense pain 8 months before which lasted about 2 months. She denied any history of antecedent trauma or back pain. MRI scan revealed a well-defined, fat-containing lesion of the left piriformis muscle similar to a lipoma displacing the sciatic nerve but not invading it. The sciatica was relieved after exci

BACKGROUND AND OBJECTIVE: Conservative and surgical treatments, are recommended as the primary treatment in the management of patients diagnosed with deep gluteal syndrome; but evidence supporting superiority of one treatment over another is lacking. The aim of this review is to systematically review the effectiveness of treatments.
DATABASES AND DATA TREATMENT: MEDLINE, EMBASE, Web of Science, Scopus, AMED, Cochrane Library (Central Register of Controlled Trials), and PEDro were screened (to 24 July 2019). Risk of bias of trials and surgical case series were assessed using the Cochrane risk

Observation of a 60 year-old-man with superior gluteal nerve (SGN) entrapment neuropathy in the suprapiriformis foramen encouraged us to explore, through anatomical dissection, the possible morphological etiologies of this condition. Ten SGNs in five embalmed cadavers were dissected via gluteal and pelvic access. The origin, course and distribution of the nervous trunk and its relations were studied. In most cases, the nerve fibers of the SGN arose from ventral branches of L4, L5 and S1 to constitute the nervous trunk in the pelvis, then reached the gluteal area and divided into two branches,

Details of piriformis syndrome, including the proper diagnosis and most effective form of treatment, continue to be controversial. While the cause, diagnosis, and treatment of piriformis syndrome remain elusive, many studies have been conducted to investigate newly developed diagnostic techniques as well as various treatment options for piriformis-induced sciatica. Despite the quantity of literature, few studies have demonstrated statistically significant results that support one form of treatment over another. Thus, despite the evidence supporting the newer treatment methodologies for pirifor

Piriformis syndrome (PS) is an uncommon neuromuscular disorder caused by the piriformis muscle (PM) compressing the sciatic nerve (SN). The main symptom of PS is sciatica, which worsens with certain triggering conditions. Because the pathophysiology is poorly understood, there are no definite diagnostic and therapeutic choices for PS. This case report presents a young woman who mainly complained of bilateral leg weakness. Electromyography revealed bilateral sciatic neuropathy and magnetic resonance imaging confirmed structural lesions causing entrapment of the bilateral SNs. After a laborious

Piriformis syndrome (PS) is a sciatic nerve entrapment condition caused by compression by the piriformis muscle, often presenting with symptoms that mimic lumbar radiculopathy and gluteal and buttock pain. This narrative review explores its diagnostic challenges and highlights the role of advanced imaging techniques in improving diagnostic accuracy. Literature suggests that PS may account for 5% to 8% of low back pain cases, frequently manifesting as buttock pain exacerbated by sitting or hip motion. Traditional imaging often fails to detect PS due to nonspecific findings, but magnetic resonan

In a 50-year-old patient with unilateral pain in the buttocks and in the sciatica, a radicular compression syndrome was diagnosed. Because conservative therapy failed, surgical intervention for herniated disc was suggested. Clinical examination revealed a painfully contracted piriform muscle and irritation of the sciatic nerve. Through specific stretch exercises, quick recovery was effected. Based on this case report, typical clinical diagnoses and therapeutical possibilities for the piriformis syndrome are discussed.
