Necrotizing pneumonia is a rare but serious complication of community-acquired pneumonia in children, often associated with parenchymal destruction, cavitation, and empyema. Despite the availability of pneumococcal conjugate vaccines, necrotizing pneumonia may still occur in immunized and otherwise healthy children due to non-vaccine serotypes or viral-bacterial co-infections. We report the case of a three-year-old developmentally normal female child who had received all age-appropriate immunizations, including three doses of the pneumococcal polysaccharide conjugate vaccine (13-valent, adsorb

A 59-year-old Japanese diabetic woman was admitted to a small private hospital with general malaise, fever, and a 1-month history of low back pain. A computed tomography scan of the abdomen revealed left abdominal necrotizing fasciitis with suspected left psoas muscle abscess. She was transferred to Gunma University Hospital, received antibiotic therapy, and underwent debridement of the infected subcutaneous tissue, fascia, and necrotic left psoas muscle. She was transferred to the intensive care unit to receive mechanical ventilation and inotropic support. Blood culture showed growth of Klebs

Bacterial co-infection in patients with coronavirus disease 2019 (COVID-19) is uncommon but represents a clinically significant complication. Such infections present both diagnostic and therapeutic challenges and are often associated with poor outcomes. We report a rare case of refractory necrotizing pneumonia caused by co-infection with Legionella pneumophila and Streptococcus pneumoniae in a patient with COVID-19 who was treated with dexamethasone and baricitinib. This case underscores the importance of maintaining a high index of suspicion for concomitant bacterial infections in immunosuppr

Empyema is a severe complication of pneumonia with high morbidity and mortality rates. Rapid diagnosis and tailoring of antibiotic therapy are crucial to treatment success for these severe bacterial lung infections. A Streptococcus pneumoniae (S. pneumonia) antigen test drawn from the pleural fluid rather than a urine sample has been found to have equivalent diagnostic utility to the urinary antigen test. Discordance between these tests is rare. We report a case of a 69-year-old female with CT imaging findings consistent with empyema and a bronchopulmonary fistula. A rapid S. pneumonia antigen

We report the use of point-of-care ultrasound to identify a hydropneumothorax in a 6-year-old previously healthy girl arriving at the emergency department in severe respiratory distress. The use of point-of-care ultrasound in this instance allowed for the expedient management and stabilization of the hydropneumothorax with emergent thoracentesis. The patient was ultimately found to have Streptococcus pyogenes bacteremia and necrotizing pneumonia.

Brucellosis is a zoonotic infectious disease widely seen worldwide, especially in developing countries and endemic regions where livestock farming is common. Brucella primarily affects the reticuloendothelial system and joints but can also localize in the central nervous, cardiovascular, and genitourinary systems. Pulmonary and urinary involvements are very rarely reported in the literature. This article presents a 24-year-old male patient who was admitted with syncope. Initially admitted with a preliminary diagnosis of Crimean-Congo hemorrhagic fever, the first thoracic computed tomography fi

BACKGROUND: Pseudomembranous necrotizing laryngotracheobronchitis refers to an acute diffuse necrotizing inflammation in the mucosa of the larynx, trachea, and bronchus. It often occurs in infants and children having viral infections secondary to bacterial infections. Mycoplasma pneumoniae (M. pneumoniae) is a common pathogen that causes pneumonia in children. In recent years, serious complications due to M. pneumoniae infection, including necrotizing pneumonia, pulmonary embolism, and pleural effusion, have been increasingly reported.

Necrotizing pneumonia can lead to respiratory insufficiency in previously healthy children. Extracorporeal membrane oxygenation can be used for hemodynamic salvage and subsequent lung rest awaiting recovery. We present a case of a child initially placed on veno-arterial extracorporeal membrane oxygenation and converted to veno-venous extracorporeal membrane oxygenation. This was done under deep hypothermia in the operating theater.

BACKGROUND: Vibrio vulnificus related necrotizing fasciitis is a fatal, rapidly progressive soft-tissue infection. Necrotizing fasciitis caused by Klebsiella pneumoniae is rare, which is indistinguishable from V. vulnificus infection in the emergency room. The purpose of this study was to compare the clinical characteristics and outcome between these two pathogens in diabetic patients.
METHODS: Thirty diabetic patients were retrospectively reviewed over an 8-year period. Necrotizing fasciitis caused by V. vulnificus was found in 19 patients and by K. pneumoniae in 11 patients. The demographic

Necrotizing fasciitis caused by Klebsiella pneumoniae has been described in Southeast Asia, but has only recently begun to emerge in North America. The hypermucoviscous strain of K. pneumoniae is a particularly virulent strain known to cause devastatingly invasive infections, including necrotizing fasciitis. Here we present the first known case of necrotizing fasciitis caused by hypermucoviscous K. pneumoniae in North America.

Neurofibromatosis type 1 (NF1), also known as von Recklinghausen disease, is a relatively common inherited neurogenetic disorder that predisposes individuals to various benign and malignant tumors, primarily in the nervous system. Diagnosis depends on clinical manifestations; however, the clinical presentation of NF1 is highly variable, and complications of NF1 may show great heterogeneity. In this manuscript, we report the case of a 45-year-old male patient who presented with frequent episodes of fever (up to 39°C) and productive cough initially non-responsive to standard-of-care medical trea

BACKGROUND Necrotizing pneumonia (NP) is a severe, life-threatening pediatric pulmonary infection associated with substantial diagnostic and therapeutic challenges, often leading to rapid lung tissue destruction, cavitation, and pleural complications that demand urgent, multidisciplinary clinical management. Flexible bronchoscopy (FB) with bronchoalveolar lavage (BAL) is a well-established tool for pathogen identification, yet the role of medical thoracoscopy in managing refractory pleural effusion in pediatric NP caused by Streptococcus pyogenes, an uncommon but highly virulent pathogen, has

Three Florida pumas ( Puma concolor coryi) that had spent time in captivity prior to being released in the wild were found exhibiting respiratory signs and reluctance to move. All 3 pumas died shortly after immobilization, despite supportive veterinary care. Significant autopsy findings included necrotizing interstitial pneumonia, with pulmonary edema and hyaline membranes, and suppurative myocarditis. Organisms morphologically consistent with Bartonella henselae were identified in intravascular histiocytes in the lung of one of the pumas on histopathology and confirmed via transmission electr

We present the case of a patient with a necrotizing multilobar pneumonia caused by community-acquired methicillin-resistant Staphylococcus aureus (MRSA). The patient presented with shortness of breath and a productive cough of 3 days duration. On arrival to the emergency department she was intubated for increased work of breathing and given vasopressors for hypotension refractory to fluid resuscitation. Blood cultures taken at admission, sputum cultures from the patient's endotracheal tube, and bronchoalveolar lavage cultures all grew S. aureus resistant to penicillinase-resistant penicillins.

Necrotizing pneumonia (NP) is a severe complication of bacterial pneumonia, characterized by progressive lung necrosis and cavitation. Although advances in imaging and supportive care have improved outcomes, NP remains difficult to manage due to poor antibiotic penetration, frequent polymicrobial infection, and the lack of standardized treatment protocols. Bacterial superantigens (SAg) stimulate T-cell hyper-activation resulting in immune modulation and severe systemic illnesses such as Staphylococcus aureus toxic shock syndrome. However, all known S. aureus SAgs are encoded by mobile genetic elements and are made by only a proportion of strains. Here, we report the discovery of a novel SAg staphylococcal enterotoxin-like toxin X (SElX) encoded in the core genome of 95% of phylogenetically diverse S. aureus strains from human and animal infections, including the epidemic community-associated methicillin-resistant S. aureus (CA-MRSA) USA300 clone

BACKGROUND: Panton-Valentine leukocidin (PVL-)producing Staphylococcus aureus is emerging as a serious problem worldwide. There has been an increase in the incidence of necrotizing lung infections in otherwise healthy young people with a very high mortality associated with these strains. Sporadic severe infectious complications after incision of Bartholin's abcesses have been described but involvement of S. aureus is rare.
CASE REPORT: We present a 23-year-old apparently healthy female patient without any typical predisposing findings who developed severe sepsis with necrotizing pneumonia and
