Intermittent neutrophil-monocyte bactericidal defects in a patient with sarcoidosis.

Reccurrent abnormalities of polymorphonuclear leukocyte and monocyte bactericidal activity were demonstrated in a patient with sarcoidosis. Defective function occurred during hypercalcemia complicating recovery from Listeria meningitis, and during separate, unrelated episodes of erythema nodosum, staphylococcal cellulitis, and pneumococcal pneumonia. Leukocyte morphology, oxidative metabolism, degranulation, and content of myeloperoxidase and lysozyme were normal, but low leukocyte alkaline phosphatase activity was demonstrable on one occasion. Despite defective bactericidal function of monocy

Severe Staphylococcus aureus infections caused by clonally related community-acquired methicillin-susceptible and methicillin-resistant isolates.

We investigated the genetic relatedness of 5 community-acquired (CA) Staphylococcus aureus isolates obtained from 4 consecutive pediatric patients presenting with sepsis syndrome and severe pneumonia during a 3-week period in 2000. Two patients were infected with methicillin-susceptible S. aureus (MSSA), and 2 were infected with methicillin-resistant S. aureus (MRSA). The pulsed-field gel electrophoresis patterns for the 2 CA-MRSA isolates were identical to each other, as were the patterns for the 3 CA-MSSA isolates. A 2-band difference reflecting the presence of a staphylococcal cassette chro

Influence of clavulanic acid on the activity of amoxicillin against an experimental Streptococcus pneumoniae-Staphylococcus aureus mixed respiratory infection.

An experimental respiratory infection caused by Streptococcus pneumoniae was established in weanling rats by intrabronchial instillation. Treatment of this infection with amoxicillin rapidly eliminated the pneumococci from the lung tissue. A beta-lactamase-producing strain of Staphylococcus aureus, when inoculated in a similar manner, did not persist adequately in the lungs long enough to permit a reasonable assessment of the therapy, but staphylococcal survival was extended in the lungs of rats infected 24 h previously with S. pneumoniae. Amoxicillin therapy was relatively ineffective against

The place of lung 99mTc DTPA aerosol transfer in the investigation of lung infections in HIV positive patients.

Pneumocystis carinii pneumonia (PCP) is the most common cause of pneumonia in HIV antibody positive patients, but other pneumonias remain important, i.e. streptococcal and mycobacterial infections. A definitive diagnosis relies on obtaining samples from the lung either noninvasively (induced sputum), or invasively (bronchoalveolar lavage, transbronchial or open lung biopsy). We have used the noninvasive technique of nebulized 99mTc DTPA transfer, to assess patients with PCP (n = 30) and other lung infections (n = 20) to see whether this test will distinguish between the various infections. The

The tetracyclines.

Tetracyclines continue to be used extensively on a world-wide basis because of their unusually broad antimicrobial spectrum and their relative safety. The first generation tetracyclines are used almost exclusively via the oral route; the second generation tetracyclines may be used orally or intravenously. Intramuscular administration is not recommended. Doxycycline is preferred in the treatment of upper respiratory tract infections, atypical pneumonias, intraabdominal/pelvic sepsis or trauma, venereal diseases, and in the treatment of prostatitis. Minocycline is preferred for meningococcal pro

Surgical management of pericarditis in Zaria, Nigeria.

Over eight years, 58 rural Nigerians with pericarditis were treated surgically in Zaria using basic surgical facilities. Eighteen patients had purulent pericarditis, associated with staphylococcal pneumonia in children, or pneumococcal pneumonia in adults. Treatment with antibiotics and prompt pericardiectomy appeared to be superior to drainage, since a quarter of those initially treated with surgical drainage developed early constriction and required pericardiectomy soon after. Thirteen patients had chronic pericardial effusions, of whom one had epicardial constriction and two had cardiomyopa

Bacteremia in HIV-positive and AIDS patients: incidence, species distribution, risk-factors, outcome, and influence of long-term prophylactic antibiotic treatment.

From a cohort of 837 adult, mainly homosexual HIV-infected patients, 76 bacteremic/fungemic episodes were identified in 63 patients over a 5-year period. Compared with an age-matched reference population with an incidence of 10.3 bacteremias/10,000 person-years, the incidence was 170 among pre-AIDS (p < 0.001) and 3,200 among AIDS patients (p < 0.001). Staphylococcal infections comprised 35% of all episodes, while the HIV-related pathogens Streptococcus pneumoniae, Salmonella spp. and C. neoformans together accounted for 34%. The overall mortality associated with clinical bacteremia was 12%, b

Pulmonary botryomycosis in a Scottish highland steer.

Pertinent necropsy findings in a 5 1/2-year-old Scottish Highland steer with chronic intractable pneumonia and cough were limited to the pulmonary system. The accessory lobe of the lung was collapsed, scarred, and multifocally adhered to parietal pleura. A polypoid mass almost completely obstructed the lobar bronchus and protruded into the trachea; mucopurulent exudate distended more distal bronchi. Botryomycosis was diagnosed when histologic examination revealed pyogranulomatous pneumonia with eosinophilic granules and "club" formation surrounding colonies of gram-positive cocci. Staphylococc

Efficacy of Azithromycin in a Mouse Pneumonia Model against Hospital-Acquired Methicillin-Resistant Staphylococcus aureus.

The use of macrolides against pneumonia has been reported to improve survival; however, little is known about their efficacy against methicillin-resistant Staphylococcus aureus (MRSA) pneumonia. In this study, we investigated the effect of azithromycin (AZM) and compared it with that of vancomycin (VCM) and daptomycin (DAP) in a murine model of MRSA pneumonia. Mice were infected with MRSA by intratracheal injection and then treated with AZM, VCM, or DAP. The therapeutic effect of AZM, in combination or not with the other drugs, was compared in vivo, whereas the effect of AZM on MRSA growth and

Population Analysis of Staphylococcus aureus Reveals a Cryptic, Highly Prevalent Superantigen SElW That Contributes to the Pathogenesis of Bacteremia.

Staphylococcal superantigens (SAgs) are a family of secreted toxins that stimulate T cell activation and are associated with an array of diseases in humans and livestock. Most SAgs produced by Staphylococcus aureus are encoded by mobile genetic elements, such as pathogenicity islands, bacteriophages, and plasmids, in a strain-dependent manner. Here, we carried out a population genomic analysis of >800 staphylococcal isolates representing the breadth of S. aureus diversity to investigate the distribution of all 26 identified SAg genes. Up to 14 SAg genes were identified per isolate with the mos

Staphylococcus aureus α-Toxin Response Distinguishes Respiratory Virus-Methicillin-Resistant S. aureus Coinfection in Children.

BACKGROUND: Development of methicillin-resistant Staphylococcus aureus (MRSA) pneumonia after a respiratory viral infection is frequently fatal in children. In mice, S. aureus α-toxin directly injures pneumocytes and increases mortality, whereas α-toxin blockade mitigates disease. The role of α-toxin in pediatric staphylococcal-viral coinfection is unclear. METHODS: We enrolled children across 34 North American pediatric intensive care units with acute respiratory failure and suspected influenza virus infection. Serial serum anti-α-toxin antibody titers and functional α-toxin neutralization c

Pyogenic infection and rheumatoid arthritis.

Ten episodes of severe pyogenic infection occurring in nine patients with rheumatoid arthritis are reported. There was a wide range of presenting features including pyoarthrosis in 7 episodes. Three cases presented with meningitis, bacterial endocarditis and probable multiple abscesses respectively. Infection was caused by Staphylococcus aureus in 7 episodes and by Staphylococcus epidermidis, Streptococcus pneumoniae and beta-haemolytic Streptococcus in each of one episode. Three infective episodes were fatal. Pyogenic, especially staphylococcal, infection should be considered in patients with

Silent acute gastric dilatation due to Salmonella infection in a diabetic elderly.

Acute gastric dilatation is a potentially life-threatening entity that has been reported in patients with some acute infections like pneumonia and staphylococcal bacteremia. We describe a case of acute gastric dilatation presenting atypically in a 65-year-old diabetic with Salmonella diarrhoea. By the fourth day of hospital admission the patient's abdomen was distended in the absence of pain, nausea or vomiting. An abdominal radiograph showed marked gastric dilatation with no evidence of obstruction or ileus. With nasogastric tube insertion and initiation of intravenous antibiotics, the stomac

[Clinical studies on panipenem/betamipron in pediatrics].

Panipenem/betamipron (PAPM/BP) was given by 30 minutes drip infusion to 15 children with acute bacterial infections including 11 with acute pneumonia, 2 each with staphylococcal scalded skin syndrome and urinary tract infections. Good to excellent clinical responses were obtained in all of the 15 patients and bacterial eradications were obtained for all 12 strains identified in these cases. Urticaria considered to be drug related was observed in 1 patient. Slight elevations of GOT and GPT and eosinophilia were observed in 1 case each. From the above clinical results, it appears that PAPM/BP is

Hyperimmunoglobulinemia E syndrome: radiographic observations.

Susceptibility to recurrent staphylococcal cutaneous and respiratory infections beginning in infancy associated with extreme hyperimmunoglobulinemia E is a recently described primary immunodeficiency syndrome. Other clinical features include depressed cellular immunity and deficient antibody formation. Recurrent pneumonia and cyst formation with variable persistence and expansion characterized the radiographic couse in 11 patients. Five cysts resolved with continuous antistaphylococcal therapy; 2 were resected without recurrence; and 4 persisted after surgery and/or antibiotics (2--8 years). T

Preoperative Pneumonia and Its Association With Periprosthetic Joint Infection and Postoperative Complications After Total Hip and Knee Arthroplasty.

BACKGROUND: Pneumonia (PNA) is a leading cause of hospitalization among older adults, yet its effect on complications following total joint arthroplasty (TJA) remains unknown. This study evaluated the association between PNA within two years prior to total hip arthroplasty (THA) or total knee arthroplasty (TKA) and periprosthetic joint infection (PJI) and other complications. METHODS: Patients undergoing primary THA or TKA were identified from a national administrative claims database. Patients who had prior PNA were matched to controls who did not have PNA based on demographics and baseline

Polymicrobial bacteremic pneumonia: report of three cases caused by Staphylococcus aureus and Streptococcus pneumoniae.

Three patients are reported with simultaneous bacteremia caused by Staphylococcus aureus and Streptococcus pneumoniae complicating community-acquired pneumonia. Polymicrobial bacteremia caused by pyogenic organisms has rarely been reported. Two of the patients had severe underlying hepatic disease, and the third had suffered a recent myocardial infarction. The infection ultimately proved fatal in each patient. The pathophysiologic aspects and clinical consequences of polymicrobial bacteremia arising from respiratory as well as from other sources is briefly reviewed.
