Single-port transconjunctival vitrectomy for malignant glaucoma.

The management of malignant glaucoma involves either anterior vitrectomy with zonulectomy and iridectomy or 3-port core pars plana vitrectomy (PPV) by retinal surgeons. The proposed modification can be performed with reasonable success rates. In this technique, synechiolysis and anterior chamber irrigation were performed through a limbal incision, and a single-port 23- or 25-gauge vitrector was introduced through PPV superotemporally to perform anterior vitrectomy and central posterior capsulotomy in pseudophakic eyes. The same procedure can be performed after cataract surgery in phakic eyes.

Unilateral anterior persistent fetal vasculature in a child with blepharophimosis-ptosis-epicanthus inversus syndrome: A surgical challenge.

Blepharophimosis-ptosis-epicanthus inversus syndrome (BPES) is a rare autosomal dominant genetic disease. It is clinically characterized by four major features; blepharophimosis, ptosis, epicanthus inversus, and telecanthus. We report a case of a 1-year-old female with BPES with unilateral anterior persistent fetal vasculature (PFV). On examination, she was found to have all the clinical features of BPES, along with calcified and partially absorbed cataract with elongated ciliary processes in her left eye. B-scan of left eye showed attached retina with no evidence of posterior PFV. Systemic ex

Intraocular pressure after neodymium:YAG laser treatments in the anterior segment.

Intraocular pressure (IOP) increase soon after neodymium:YAG (Nd:YAG) treatments in the anterior segment has been reported. To evaluate this complication in a large sample of patients, we conducted a retrospective study of all Nd:YAG treatments during three years. Three hundred forty capsulotomies and 212 iridotomies were performed with the Q-switched Nd:YAG laser. An IOP elevation of 10 mm Hg or more within two hours of laser surgery was seen in 15 eyes (4.4%) that had a capsulotomy but in 45 eyes (21.2%) that had an iridotomy. In 13 of the 15 eyes in which the IOP increased after capsulotomy

Capsular block syndrome associated with femtosecond laser-assisted cataract surgery.

UNLABELLED: We report intraoperative capsular block syndrome occuring during the first 50 femtosecond laser-assisted cataract surgeries performed in our facility. Two patients had uneventful combined laser fragmentation, capsulotomy, and corneal incision procedures. In both cases, following transfer to the operating room and manual removal of the laser-cut capsulotomy, posterior capsule rupture was noted during hydrodissection, resulting in posterior dislocation of the lens. Pars plana vitrectomy, removal of the crystalline lens, and sulcus implantation of an intraocular lens were performed in

Vitreoretinal changes associated with rise in intraocular pressure after Nd:YAG capsulotomy.

In a chart review of 100 consecutive Nd:YAG laser capsulotomies, six presumed risk factors of the rise in intraocular pressure (IOP) after capsulotomy were identified. A group of 15 patients in which the pressure rose to at least 10 mmHg above baseline postoperatively showed a significant (P less than or equal to 0.01) association with myopia, vitreoretinal disease, and vitreous prolapse into the anterior chamber. The previously known association of high pressures with glaucoma, a large capsular opening, or absence of a posterior chamber lens were also confirmed. There was no correlation betwe

Sinskey Hook-Guided Mechanical Peeling of Primary Posterior Capsular Plaque During Cataract Surgery.

Primary posterior capsular plaque may be encountered during cataract surgery and can compromise the visual axis. Although Nd:YAG laser capsulotomy effectively treats postoperative posterior capsular opacification, it carries recognized risks, and intraoperative management may be preferable in selected cases. This report describes a simplified Sinskey hook-guided mechanical peeling technique performed before intraocular lens implantation. In a single adult case, plaque separation was achieved through controlled tangential glide-dissection beneath the plaque using a Sinskey hook, followed by for

Successful treatment of tractional corectopia using 2 mJ of energy with an Nd:YAG laser.

Although the Nd:YAG laser is most commonly used to perform posterior capsulotomies after cataract surgery, it has also been used to treat a variety of other anterior segment abnormalities including tractional corectopia, iridocorneal adhesions, persistent pupillary membranes, and posterior synechiae. Numerous reports on the use of the Nd:YAG laser to treat structures in the anterior segment have emphasized the need to use higher pulse energy for pupillary membranes, compared with the lower settings required for posterior capsulotomy. Steinert and Puliafito noted that single pulses of 4 to 12 m

Differentiated treatment of secondary cataract following extracapsular cataract operation.

161 eyes with Nd:YAG laser posterior capsulotomy after in the bag IOL implantation 1-3 years before, were actively followed over a period of 24 +/- 6 months. No CME nor retinal detachment nor induced glaucoma was found. Therefore a YAG capsulotomy in eyes with completely in the bag implanted IOL seems to assure the same level of security than capsular polishing in eyes with sulcus implanted lenses.

Lens regeneration in mammals: a review.

Lens regeneration occurs in New Zealand albino rabbits after endocapsular lens extraction, if the anterior and posterior capsules remain relatively intact. Research beginning in the 19th century showed that regeneration of the lens begins as early as 2 weeks postoperatively, depends on the size of the capsulotomy and how it has scarred, and is faster in younger animals. More recently, implantation of embryonic ectoderm at the time of lens removal has been shown to improve the growth and quality of the regenerated lenses. Lens fiber differentiation follows a process similar to embryological dev
