Familial Mahaim syndrome.

We describe the occurrence of Mahaim syndrome in a mother and her son. The occurrence of such a rare disorder in two members of a family is noteworthy, has not been reported before, and suggests the possibility of genetic transmission. A genetic transmission of supraventricular tachycardia has been described only in rare cases for the Wolff-Parkinson-White syndrome. No such data is available for the Mahaim syndrome.

A case of transient ventricular pre-excitation associated with takotsubo cardiomyopathy.

A 75-year-old woman, who had never exhibited a delta wave before, was diagnosed with sigmoid colon cancer and underwent surgical operation. Takotsubo cardiomyopathy (TC) occurred shortly after the operation. About 2 weeks following the onset of TC, a 12‑lead electrocardiogram revealed a delta wave that was present for 50 days, and the patient was diagnosed with transient ventricular pre-excitation. The delta wave disappeared prior to patient's discharge and was never observed thereafter. This transient appearance of a delta wave may be related to a pathological modulation of the autonomic nerv

Electroconvulsive Therapy for Treatment-Resistant Depression in a Patient With Wolff-Parkinson-White Syndrome: A Case Report and Literature Review.

Electroconvulsive therapy (ECT) has been recognized as an effective treatment option in severe major depression and treatment-resistant depression. However, its use has been associated with an initial increase in parasympathetic and subsequently sympathetic tone; this has relevance in the context of patients with arrhythmias or pre-excitation syndromes in which some adverse effects have been reported. We present the case of a young female with treatment-resistant depression and a history of Wolff Parkinson White Syndrome admitted for modified ECT. To date, there is no evidence of the anestheti

Refractory Angina Confounded by Preexcitation Syndrome.

BACKGROUND: Primary hypokalemic periodic paralysis (HypoPP), a rare skeletal muscle channelopathy resulting in episodic muscle weakness or paralysis under hypokalemic conditions, is caused by autosomal-dominant genetic mutations. HypoPP limits physical activity, and cardiac arrhythmias during paralytic attacks have been reported. We describe a rare familial HypoPP case complicated by sinus arrest and syncope requiring urgent temporary pacemaker implantation. CASE REPORT: A 27-year-old Vietnamese man with a family history of periodic paralysis presented with his third attack of muscle weakness

New approaches to the management of the Wolff-Parkinson-White syndrome.

Pre-excitation has been defined as "a condition in which all or some portion of the ventricular muscle is activated earlier in relation to atrial events, than would be expected had the impulse reached the ventricle by way of the normal atrioventricular conduction system". The susceptibility of patients with pre-excitation to paroxysmal tachycardias had been described many years previously. The Wolff-Parkinson-White syndrome was initially thought to have a relatively benign prognosis. Subsequently, it was recognised that some patients were prone to life-threatening arrhythmias and sudden death.

P-wave abnormalities in PRKAG2 cardiomyopathy: Expanding the electrocardiographic spectrum beyond preexcitation and hypertrophy.

PRKAG2 cardiomyopathy exhibits distinctive P-wave abnormalities-including notching, axis deviation, inter-atrial block, and enlarged P-wave terminal force in lead V1 (PTFV1)-that reflect underlying atrial involvement. These atrial electrocardiographic features, when integrated with comprehensive genetic testing, may enable earlier diagnosis, facilitating accurate genotype-phenotype correlation and refined risk stratification beyond traditional preexcitation or hypertrophy patterns, ultimately guiding timely intervention and family screening in this rare but potentially malignant inherited card

Ventricular pre-excitation in an elderly cat.

An 18-year-old female neutered Domestic Shorthair cat was referred for investigation of seizure-like episodes. Physical and neurological examination were unremarkable, as were systemic arterial blood pressure, cardiac troponin I, complete blood count and biochemistry profile. Diagnostic tests included transthoracic echocardiography which ruled out any structural cardiomyopathy, and electrocardiography which showed an underlying regular wide QRS-complex rhythm with pronounced shortening of the PR interval. Considering the findings, a diagnosis of suspected ventricular pre-excitation secondary t

Early-Excitation Segment Atrophy (EESA) Syndrome: Linking Electrical Dyssynchrony to Regional Myocardial Atrophy.

Identifying the underlying cause of cardiac dysfunction is essential for determining the appropriate treatment and prognosis. The current management paradigm for heart failure (HF) and cardiomyopathies predominantly emphasizes structural and ischemic etiologies, often overlooking the substantial role of electrical dyssynchrony in cardiac dysfunction and remodeling. This work introduces a novel conceptual framework that integrates existing evidence illustrating how electrical dyssynchrony induces mechanical dyssynchrony, culminating in regional cardiac impairment and structural remodeling. Echocardiographic characteristics of pediatric preexcitation cardiomyopathy.

Preexcitation cardiomyopathy is a rare cardiac anomaly but it can be life threatening. It may occur in type B preexcitation syndrome. A patients with this condition typically has an enlarged left ventricle and reduced systolic function without tachycardia-associated cardiomyopathy. Echocardiography plays an important role in diagnosis in identifying this condition by revealing the "rebounce" movement of the inter-ventricular septum (IVS). We report echocardiographic findings of five pediatric patients with preexcitation cardiomyopathy. All of them had a radiofrequency catheter ablation (RFCA)

AV nodal reentrant tachycardia with Mahaim fiber conduction.

Paroxysmal tachycardia with widened QRS complexes was found in a 46-year-old woman. In sinus rhythm, the patient had electrocardiographic evidence of type B preexcitation with a left bundle branch block pattern. The resting PR interval (160 msec) and A-H interval (100 msec) were within normal limits, but the H-V interval (10 msec) was abnormally short. Programmed atrial extrastimuli at progressively shorter coupling intervals resulted in sudden prolongation of the A-H interval from 120 msec to 250 msec, and the His bundle activities became incorporated just after initiation of the QRS complexe

Cryoablation of the accessory pathway in Wolff-Parkinson-White syndrome: initial results and long term follow up.

Cryoablation of the accessory pathway was used in the management of 20 patients with pre-excitation syndromes. All patients had presented with paroxysmal atrioventricular reentrant tachycardia; in addition, six had experienced atrial fibrillation. In 16 patients pre-excitation was overt and in four the accessory pathway was concealed. Intraoperative epicardial and endocardial mapping showed 10 left free wall pathways, seven septal pathways, and four right free wall pathways. One patient had two right free wall accessory pathways. There was one postoperative death (from a ruptured cerebral haem

Perioperative management of patients with genetic multisystem diseases associated with pre‑excitation.

Pre-excitation is associated with life-threatening arrhythmias. Apart from the well-known Wolff-Parkinson-White syndrome, a number of rare diseases are associated with pre-excitation due to the existence of accessory pathways. The present review aims to focus on anaesthesia and perioperative care of patients with rare genetic diseases associated with pre-excitation due to the existence of a bundle of Kent or other accessory pathways. The Danon disease, Fabry disease and Pompe disease, tuberous sclerosis, Leber hereditary optic neuropathy (LHON), and mitochondrial encephalopathy, lactic acidosi

A Fasciculoventricular Accessory Pathway Featuring Functional Decremental Conduction and QRS Variability.

Fasciculoventricular accessory pathways (FVAPs), once considered rare variants of pre-excitation syndrome, are now recognised as ubiquitous in both humans and murine. Nonetheless, most FVAPs are likely electrically silent. However, they can become evident, as reported for some glycogen storage diseases (such as Danon disease and PRKAG2) and during high-voltage septal pacing. Typically, FVAPs only exhibit antegrade and non-decremental conducting properties. A block at the FVAP results in a normal His-to-ventricle interval and a narrow QRS complex without signs of pre-excitation. Decremental con

Miniseries 2-Septal and paraseptal accessory pathways-Part II: Para-Hisian accessory pathways-so-called anteroseptal pathways revisited.

Surgeons, when dividing bypass tracts adjacent to the His bundle, considered them to be 'anteroseptal'. The area was subsequently recognized to be superior and paraseptal, although this description is not entirely accurate anatomically, and conveys little about the potential risk during catheter interventions. We now describe the area as being para-Hisian, and it harbours two types of accessory pathways. The first variant crosses the membranous septum to insert into the muscular ventricular septum without exiting the heart, and hence being truly septal. The second variant inserts distally in t

Distal type of nodo-ventricular pathway: Unique electrophysiological characteristics mimicking fasciculo-ventricular pathway.

Fasciculo-ventricular and nodo-ventricular pathways (FVP and NVP) are rare preexcitation variants. Normally, NVP is electrophysiologically different from FVP. We describe a unique type of NVP emerging from the distal part of the slow pathway, designated as "distal type" NVP. The distal type NVP resembled FVP but was proven by unexpected elimination of the NVP during the slow pathway ablation. Also, NVP was distinguishable from FVP by a careful comparison of the HV intervals during conduction over the fast and slow pathways. Demonstration of this novel type NVP provides insights into how the in
