Pathological baroreceptor sensitivity in patients suffering from somatization disorders: do they correlate with symptoms?

AIM: We conducted a study to investigate whether patients with somatization disorders (ICD-10, F45.0) show abnormal values in autonomic testing. METHOD: 35 patients with a diagnosis of somatization disorder (SP) were matched to 35 healthy volunteers (HV). International standardized autonomic testing based on heart rate variation and continuously measured blood pressure signals was used to assess autonomic activity and establish baroreceptor sensitivity (BRS). Three different statistical procedures were used to confirm the reliability of the findings. RESULTS: There were no statistical differ

Branchial innervation.

Inspection of the dorsal end of fish gills reveals an impressive set of nerve trunks, connecting the gills to the brain. These trunks are branches of cranial nerves VII (the facial) and especially IX (the glossopharyngeal) and X (the vagus). The nerve trunks carry a variety of nervous pathways to and from the gills. A substantial fraction of the nerves running in the branchial trunks carry afferent (sensory) information from receptors within the gills. There are also efferent (motor) pathways, which control muscles within the gills, blood flow patterns and possibly secretory functions. Underta

[A noninvasive assessment of the gain in baroceptor control of the heart rate in man].

In this study we addressed the problem of the noninvasive evaluation of the overall gain of baroreceptor control of heart period. We studied a population of healthy controls (n = 49, age 30 +/- 2 years, systolic arterial pressure [by Finapres] 114 +/- 1 mmHg) and a group of mild hypertensive subjects (n = 14, age 51 +/- 2 years, systolic arterial pressure 151 +/- 5 mmHg). Subjects were studied at rest both in absence and in presence of chronic beta-adrenergic receptor blockade (atenolol: controls 50 mg po oid x 4 days; hypertensives 100 mg po oid x 2 weeks). Spectral analysis of RR interval an

Therapeutic approach to hypertension in the elderly.

Hypertension is an important risk factor for cardiovascular disease in the elderly. This article addresses the factors that increasingly influence hypertension as age increases. In addition, the initial therapeutic approach to the elderly hypertensive patient should generally consist of a reduction in salt and caloric intake and increased exercise. Drug therapy should be initiated with lower doses of medication with special concern about orthostatic hypotension. The drug regimen should be made as simple as possible and be carefully explained to the patient. With these considerations in mind, h

Carotid sinus syndrome--clinical characteristics in elderly patients.

Carotid sinus massage was carried out on 130 consecutive patients referred for investigation of dizziness, syncope or unexplained falls. Carotid sinus syndrome was diagnosed in 33 for whom no other cause of symptoms was identified. Right-sided hypersensitivity was more frequent than left-sided. Thirty per cent identified a prodrome before syncope and 30% had retrograde amnesia for the event. In 52%, symptoms were precipitated by head movement and in 48% by vagal stimuli. Seven described 'drop attacks' but symptoms were reproduced with carotid sinus massage during head-up tilt. Injuries, includ

Application of a new technique--blood pressure clamping--for analysis of prostaglandin interference with sympathetic neurotransmission in man.

To circumvent baroreceptor reflexes following drug-induced interference sympathetic neurotransmission, a new technique - blood pressure clamping--has developed. This implies that the sympathetic activity in an awake human is 'clamped' at a supranormal level by infusion of a vasodilator. In 11 healthy volunteers nitroprusside or saline was randomly infused in two consecutive 2-h periods. Plasma and urinary catecholamine levels were analysed by liquid chromatography. The experiments were repeated after random administration of the prostaglandin synthesis inhibitor, ibuprofen, or placebo. In the

Effect of anesthetic induction agents on cardiovascular neuroregulation in dogs.

The goal of this study was to examine the hypothesis that intravenous anesthetic induction agents alter neuroregulation of the cardiovascular system. Additional goals were to investigate this in an animal model devoid of other drug effects. Healthy mongrel dogs had arterial catheters inserted and pneumatic occluders positioned around the thoracic aorta and inferior vena cava. After 10 to 14 days of recovery, neurocirculatory control mechanisms were assessed in the absence of anesthesia by recording changes in the RR interval of the electrocardiogram in response to alterations in systolic arter

Acute orthostatic hypotension.

Acute orthostatic hypotension is a common occurrence during a patient's first experiences out of bed after surgery or a period of immobility. It is imperative that acute care nurses understand the dynamics of this phenomenon to provide effective preventive and supportive care for these patients. In this article are outlined recommendations for this care. Preventing acute orthostatic hypotension entails careful assessment of the status of the patient's sympathetic reflexes and assisting these reflexes to gain maximal control over peripheral vasoconstriction before the patient leaves bed. Suppor

On the renal basis for essential hypertension: nephron heterogeneity with discordant renin secretion and sodium excretion causing a hypertensive vasoconstriction-volume relationship.

We propose herein that there are two functionally abnormal nephron populations in essential hypertension: (1) a group of ischemic nephrons with impaired sodium excretion which chronically hypersecrete renin. Numerically, these ischemic nephrons comprise a minor subgroup since most patients with essential hypertension exhibit no overt evidence of renal insufficiency. (2) In reaction to this, a more numerous group of normal nephrons appears in adaptive hypernatriuresis. They have an increased distal sodium supply and consequently, a chronically suppressed renin secretion. One difference between

Modulation of blood pressure by substance P: opposite effects of N- and C- terminal fragments on anesthetized rats.

Considerable evidence suggests that substance P (SP) may be a transmitter mediating the depressor effects of baroreceptor reflex activation within the brainstem, yet intracerebroventricular administration of SP is reported to result in a pronounced pressor effect. In this study, SP injected into the 4th cerebral ventricle produced a biphasic effect; a brief decrease in blood pressure followed by a lengthy increase. Similar injections of a carboxy-terminal fragment of SP produced only a pressor effect of long duration. Injection of an amino-terminal SP fragment produced only a brief, rapid depr

Alteration in baroreceptor function in rats produces typical pressure changes during sleep.

We have shown previously that both sinoaortic denervation (SAD) and high renin hypertension in the rat produce a pronounced alteration in the pattern of pressure change during sleep, namely from unchanged to a rise in pressure during synchronized sleep (SS) and from a slight rise to a marked fall during desynchronized sleep (DS). Since acute SAD also produces overactivity of the renin-angiotensin system (RAS), we investigated if this overactivity is essential for the development of the alterations. In rats studied 1 day after SAD (138 +/- 1.0 mm Hg) the MAP rose during SS (+14 +/- 0.7 vs. +1.0

Evidence that the arterial baroreceptors influence muscle blood flow and not subcutaneous flow in man.

Studies in man indicate that skeletal muscle blood flow is modulated via high pressure baroreceptors located in the carotid body. However, whether subcutaneous blood flow is influenced in this way remains controversial. Therefore, the aim of the present study was to determine whether subcutaneous as well as skeletal muscle blood flow was influenced by direct carotid body stimulation; this was done by means of neck suction. Six young healthy males were investigated. Subcutaneous and skeletal muscle blood flow in the forearm was measured by the local 133Xe washout technique. Neck suction of -20

Clonidine raises blood pressure in severe idiopathic orthostatic hypotension.

The hemodynamic effects of clonidine were studied in four patients with sever idiopathic orthostatic hypotension and one patient with baroreceptor dysfunction. No depressor response to clonidine was observed in any patient with idiopathic orthostatic hypotension at any dosage. Rather, two patients responded to 0.4 mg of oral clonidine with a 40 mm Hg increment in systolic blood pressure lasting several hours. Each has been receiving clonidine, 0.4 mg twice daily, for one year with greatly increased functional capacity. The other two patients with idiopathic orthostatic hypotension had an even

Carotid sinus baroreceptor control of arterial pressure in renovascular hypertensive subjects.

We used the neck chamber technique to study carotid baroreceptor control of blood pressure in 18 renovascular hypertensive subjects. Carotid baroreceptors were stimulated or deactivated for 2 minutes by applying graded reductions or increases in the neck tissue pressure (NTP) outside the carotid sinuses. The sensitivity of the baroreflex was separately calculated for these two conditions by the coefficients of the linear regressions relating the changes in NTP to the resulting changes in mean arterial pressure (MAP, catheter measurement). Baroreceptor deactivation increased MAP, and the sensit

Effects of central venous blood volume shifts on arterial baroreflex control of heart rate.

The purpose of this study was to investigate the effects of anesthesia, body position, and blood volume expansion on baroreflex control of heart rate. Five male rhesus monkeys (7.0-10.5 kg) were given bolus injection of 4.0 micrograms/kg phenylephrine during each of the following situations: awake sitting, anesthetized (AN) (10 mg/kg ketamine-HCl) sitting, AN recumbent, AN 90 degrees head down tilt, and AN 50% blood volume expansion with normal saline. beta-Receptor blockade was also performed on each treatment after anesthesia. Four additional animals were similarly treated after 20% blood vo

Opiate analogs, substance P, and baroreceptor reflexes in the rabbit.

The putative transmitters, enkephalins and substance P, and their binding sites have been identified in the nucleus of the solitary tract. Their role in the modulation of baroreceptor reflex activity is the subject of this study in the rabbit. A stable decarboxy analog of leu-enkephalin, RX 783016, which has mu receptor specificity, was used to attenuate the baroreflex sensitivity to intravenous phenylephrine. RX 783016, 50 micrograms/kg intracisternally, did not alter resting heart rate of blood pressure. Intravenous administration of the opiate receptor antagonist, naloxone, prevented the ef

Antidiuretic hormone excess.

Known physiologic mechanisms explain the elevated blood ADH levels observed in most patients with the syndrome of inappropriate ADH. Therefore the word "inappropriate" is a misnomer. It implies that the mechanisms that regulate ADH release are not functioning normally--which is not true. The term misleads the physician who, ideally, should determine why a patient has an excessive blood ADH level and initiate appropriate treatment. Patients with ectopic production of ADH and hyponatremia should be so labeled: "Hyponatremia due to ectopic ADH production." The term SIADH, if used at all, should b
