Prevalence of pressure ulcers in Canadian healthcare settings.

Although statistics regarding the number of pressure ulcers in the US and other countries are available, little information is known about the number of individuals in Canada who have pressure ulcers. Such information is important to assess the scope and healthcare costs of pressure ulcers and develop public policies. To obtain estimated pressure ulcer prevalence rates in Canada, existing data (gathered between 1990 and 2003) from different healthcare settings across the country were obtained from peer-reviewed published studies and from unpublished studies provided by individuals and pressure

Pressure ulcers in the chronically critically ill patient.

All chronically critically ill patients are at high risk for development and progression of pressure ulcers. Constant surveillance including daily examination of the skin must be part of the care protocol. All pressure ulcers are chronic wounds that have an inherent, physiologic impairment to healing. As soon as a pressure ulcer develops, intervention should begin immediately, and a treatment plan should be determined. We believe that early intervention and appropriate treatment, guided by the paradigm we have described, can retard progression and promote healing [49]. Treatment decisions shou

Necrotizing fasciitis: an uncommon consequence of pressure ulceration.

Pressure ulcers may occur in patients with chronic illnesses, especially in those who are bed-bound or chair-bound. Local measures usually suffice to allow primary ulcer healing and support skin grafting or tissue transfer reconstruction. On rare occasions, however, pressure ulcers may progress to invasive infection and necrosis of adjacent soft tissues, possibly leading to necrotizing fasciitis. Early recognition and aggressive medical and surgical therapy are required to halt disease progression and prevent patient mortality. Two cases are presented to describe the severity of this soft-tiss

Seeking quality care for patients with pressure ulcers.

Improving quality care for patients with pressure ulcers resulted in a two-year intensive effort to educate staff, keep morale high, standardize care plans by stage of ulcer, and participate in a comparison study of two support surfaces (Thera-Pulse, Kinetic Concepts and Geo-Matt, SpanAmerica). When all pressure ulcers were considered, the analysis of covariance revealed no statistically significant difference in the healing of pressure ulcers with respect to type of support surface used (F[1, 78] = 0.35, p > .05). For patients with stage III and IV pressure ulcers, the proportion of patients

Pressure and prolapse--the cause of solitary rectal ulceration.

The cause of solitary rectal ulceration has been investigated using a method that radiologically visualises rectal voiding whilst simultaneously measuring intrarectal pressure and external anal sphincter electromyographic activity. Control subjects and patients with the solitary rectal ulcer syndrome, both with and without mucosal ulceration, have been studied. A high incidence of rectal prolapse (94%) was present in the patients who voided. Overactivity of the anal sphincter during evacuation contributed to the fact that patients with mucosal ulceration required higher intrarectal pressures t

Local blood pressure in chronic leg ulcers.

The local arterial blood supply was evaluated according to clinical criteria in 30 consecutive patients with chronic leg ulcers. Separately, the blood pressure was measured at the ankle level with both strain gauge plethysmography and a Doppler ultrasonic method. In patients with clinical signs of arterial obliterative disease the systolic ankle pressure and the ankle/arm systolic pressure ratio were lower than in patients without such signs. In none of the patients was the local pressure decreased to such a degree that ischemia was the main cause of the ulcer, nor was gangrene imminent. Hence

Treating stage IV pressure ulcers with negative pressure therapy: a case report.

Decubitus ulcers remain a significant healthcare concern today, especially in the elderly and immobile population. Following the observation of three Stage IV decubitus ulcers refractory to standard medical and surgical therapy for 10 months, a new vacuum-assisted closure device (V.A.C.) was initiated to speed wound healing. The V.A.C. was initiated in August 1996. The three Stage IV ulcers were located on the patient's right ischium, left ischium, and sacrum. On initiation, they measured 7 1/2 cm x 2 1/2 cm x 2 1/2 cm, 8 cm x 3 1/2 cm x 2 1/2 cm, and 3 1/2 cm x 2 cm x 2 cm respectively. The t

Development of Cyclic Pressure Offloading Insole for Diabetic Foot Ulcer Prevention.

Introduction. The likelihood of developing a diabetic foot ulcer (DFU) during one's lifetime for individuals with diabetes mellitus is around 19% to 34%. Continuous and repetitive loading on soft tissues are the major causative factors for DFU. This paper introduces an air cell array insole designed for cyclically offloading pressure from plantar regions to reduce repetitive stress and loading on foot. Materials and Methods. The insole comprises an air cell array insole and a pneumatic control unit. The interface pressure was evaluated in static and dynamic conditions at 3 different air cell i

Tracheal ulcer development from high volume, low-pressure cuffed endotracheal tubes.

INTRODUCTION: Iatrogenic injury to the larynx, particularly the vocal cords from prolonged intubation, has been well-studied; however, tracheal injuries are rarely reported. This study investigates the effectiveness of cuffed, high-volume, low-pressure endotracheal tubes in preventing the development of tracheal ulcers in intubated subjects. METHODS: A retrospective, IRB-approved review was performed on 1355 subjects who underwent percutaneous tracheostomy from 2002 to 2018. The presence and severity of tracheal ulcers were collected using documentation and photos during percutaneous tracheos
