Alcohol and Opioid Use Disorder in Older Adults: Neglected and Treatable Illnesses.

The purpose of this article is to provide an overview of alcohol use disorder (AUD) and opioid use disorder (OUD) in older adults for general psychiatrists. The rapid growth of the geriatric population in the USA has wide-ranging implications as the baby boomer generation ages. Various types of substance use disorders (SUDs) are common in older adults, and they often take a greater toll on affected older adults than on younger adults. Due to multiple reasons, SUDs in older adults are often under-reported, under-detected, and under-treated. Older adults often use substances, which leads to vari

Isoniazid-Associated Serotonin Toxicity in the Critical Care Setting: A Case Report.

Serotonin syndrome presents with the triad of neuromuscular excitability, autonomic disturbance, and altered mental status, resulting from excess serotonergic tone. Isoniazid (INH), a core agent for the management of tuberculosis (TB), is a weak, non-selective monoamine oxidase inhibitor (MAOI), and there have been minimal reports of its potential to contribute to serotonin toxicity. We present a complex case of INH-associated serotonin toxicity in a patient with autism spectrum disorder and co-occurring severe TB in the critical care setting. The patient was on rifampin, INH, pyrazinamide, an

Addressing Sexual Violence in Psychiatric Facilities.

Preventing sexual violence in psychiatric facilities has been a longstanding challenge that has received only limited attention from researchers and regulators. Numerous factors at the patient-, staff-, facility- and health care system-level contribute to sexual assaults within these facilities and difficulties in obtaining justice for victims. In the wake of the #MeToo movement, significant strides have been made in addressing sexual violence within society. Extending these efforts to the isolated worlds of psychiatric facilities is a logical next step; the time has come for psychiatry to bet

DARK Classics in Chemical Neuroscience: Opium, a Friend or Foe.

Opium has found great use medicinally for its analgesic properties and has been witnessed as one of the most popular medications used in psychiatry. Opium derivatives have been shown as efficacious for relieving pain and the treatment of epileptic seizures, but progressive research toward their use in the treatment of neurodegenerative diseases remain elusive. To gain more insight into the other properties of opium such as anti-inflammatory properties, herein we discuss basic information regarding opium, opium content and mechanism of action, pharmacology of opium derivatives, the role of opiu

Self-mutilation of tongue and lip in a patient with simple schizophrenia.

Self-inflicted lesions that cause mutilation are observed in schizophrenic patients. This case report describes the diagnosis and treatment of simple schizophrenia in a 31-year-old male patient who bit his own tongue and lower lip. The dental treatment proposed included the construction of a splint to prevent new lesions and to allow healing of existing ones. The treatment afforded to avoid tooth extraction before the patient responded to psychiatric drug treatment. The importance of a transdisciplinary approach to self-mutilation due to psychiatric disorders is stressed. The interaction betwe

Clinical implementation of preemptive pharmacogenomics in psychiatry: Τhe "PREPARE" study.

PREemptive Pharmacogenomic testing for Preventing Adverse drug REactions (PREPARE) is the first prospective, pre-emptive pharmacogenomic study conducted in Europe, within the frame of the Horizon 2020 program. It aims to determine whether implementing pre-emptive pharmacogenomics (PGx) testing of clinically relevant biomarkers, so as the dose and drug selection to be guided, will result in an overall reduction of both the occurrence and the severity of drug-genotype-associated adverse drug reactions (ADRs). To achieve that, two groups of patients will be recruited; one that will receive treatm

Traumatic shock and electroshock: the difficult relationship between anatomic pathology and psychiatry in the early 20th century.

In the conviction that a look at the past can contribute to a better understanding of the present in the field of science too, we discuss here two aspects of the relationship between early 20th century anatomic pathology and psychiatry that have received very little attention, in Italy at least. There was much debate between these two disciplines throughout the 19th century, which began to lose momentum in the early years of the 20th, with the arrival on the scene of schizophrenia (a disease histologically sine materia) in all its epidemiological relevance. The First World War also contributed

The effectiveness of sexual offender treatment for juveniles as measured by recidivism: a meta-analysis.

Published and unpublished data from nine studies on juvenile sexual offender treatment effectiveness were summarized by meta-analysis (N=2986, 2604 known male). Recidivism rates for sexual, non-sexual violent, non-sexual non-violent crimes, and unspecified non-sexual were as follows: 12.53%, 24.73%, 28.51%, and 20.40%, respectively, based on an average 59-month follow-up period. Four included studies contained a control group (n=2288) and five studies included a comparison treatment group (n=698). An average weighted effect size of 0.43 (CI=0.33-0.55) was obtained, indicating a statistically s

Therapist awareness and responsibility in working with sexual offenders.

The article aims to address the ethical implications of therapist reactions to sexual offender treatment and, in doing so, increase awareness and understanding of the interaction between the clinician and the challenges of working with sexual offenders. The effects of providing psychological services in general, and to sexual offenders specifically, are reviewed, including the impact on services provided. Practice implications are also discussed as the authors consider the negative effects (e.g., burnout) that therapists experience due to treatment quality and the ethical issues and responsibi

Hostility and recidivism in sexual offenders.

In this study, we examined the association of hostility, as measured by the Buss-Durkee Hostility Inventory (BDHI), with offence characteristics and recidivism in 656 adult male sexual offenders. Hostility was significantly associated with having prior violent charges, the use of violence in the index sexual offence, sexual recidivism, and violent recidivism. After controlling for risk level, as measured by a modified version of the Rapid Risk Assessment for Sexual Offence Recidivism (RRASOR-mod), the significant association between hostility and sexual and violent recidivism remained. When ex

[Roles of brain science in psychiatry].

At the opportunity of celebrating the 100th Anniversary of the Japanese Society of Psychiatry and Neurology, I am filled with deep emotion when I recall the time only a few decades ago, when mental disorders were believed to be an illness of the mind without any disorder in the brain. It has now become apparent that a mental disease emerges from the malfunction of the brain's unique biological mechanisms, so that any mental disease can be cured or prevented if the cause of the malfunction is clarified. Analyses of intercellular as well as intracellular signal transduction and the mechanisms un

Duplicate Prescription Patterns in Patients With Substance Use Disorders.

This study sought to investigate the frequency and characteristics of duplicate prescriptions (DPs) in psychiatric inpatients with substance use disorders using a DP classification that differentiates between appropriate DPs (ADPs) and potentially inappropriate DPs (PIDPs). The study was conducted as a monocentric retrospective cross-sectional study on the addiction-specific ward of the Department of Psychiatry, Social Psychiatry and Psychotherapy of Hannover Medical School, a large university hospital in northern Germany. The outcome measures were the nature and frequency of PIDPs compared wi

"A world crazier than us": Vanishing social contexts and the consequences for psychiatric practice in contemporary Romania.

Since the end of Communism, mental health care in Romania has increasingly sought to align its practices with idealized models of Western psychiatric practice. Much of this realignment has been made possible by accessing and integrating new pharmaceuticals into psychiatric hospital settings. Less straightforward have been the painful attempts to create a system modeled on international standards for training and certifying psychotherapists. Unfortunately, the political, economic, infrastructural, and epistemological environment of the Romanian mental health care system has prevented many other

Mental health as an advocacy priority in the lesbian, gay, bisexual, and transgender communities.

This column reviews the evolution of lesbian, gay, bisexual, and transgender (LGBT) mental health advocacy in relation to modern mental health advocacy efforts. In addition to developments in organized psychiatry (e.g., American Psychiatric Association's LGBT caucus), grassroots LGBT community initiatives are playing an important role (e.g., Trevor Project providing crisis intervention/suicide prevention services to LGBT youth, face-to-face mental health services in LGBT community centers). Studies have found that LGBT individuals are at increased risk for mental health problems (e.g., depress

[Anxiety and Generalized Anxiety Disorder in Elite Athletes].

Physical activity is recommended as a strategy for promoting mental health, and it can even be integrated into treatment plans for psychiatric disorders affecting the general population. However, elite athletes are a subpopulation with protective and risk factors inherent to high-level competition and are not shielded from psychiatric symptoms and conditions. The mental health of elite athletes has been the subject of increasing attention, driven in part by the initiatives of the International Olympic Committee and the growing importance of sports psychiatry. This article is a narrative review

Understanding the Interplay between Air Pollution, Biological Variables, and Major Depressive Disorder: Rationale and Study Protocol of the DeprAir Study.

Major depressive disorder (MDD) is a serious and disabling condition, whose etiological mechanisms are not fully understood. The aim of the DeprAir study is to verify the hypothesis that air pollution exposure may exacerbate neuroinflammation with consequent alterations in DNA methylation of genes involved in circadian rhythms and hormonal dysregulation, resulting in the worsening of depressive symptoms. The study population consists of 420 depressed patients accessing the psychiatry unit of the Policlinico Hospital (Milan, Italy), from September 2020 to December 2022. Data collection is still
