Detection and treatment of dysfunctional hemodialysis access grafts: effect of a surveillance program on graft patency and the incidence of thrombosis.

PURPOSE: To determine the value of a hemodialysis graft surveillance program in reducing the incidence of graft thrombosis and prolonging graft patency by means of early detection and percutaneous transluminal angioplasty (PTA) of graft-related stenoses. MATERIALS AND METHODS: For 4-1/2 years, routine graft examination and measurement of several dialysis parameters were used to identify 106 cases of suspected graft dysfunction in 57 patients (56 men, one woman; aged 27-76 years). Graft-related stenoses detected with angiography were treated with PTA. RESULTS: Abnormal physical examination fi

Perioperative Dextran-40 Infusion and Early Allograft Outcomes After Adult Liver Transplantation: An Overlap-Weighted Cohort Study.

INTRODUCTION: Dextran-40 has rheological and antithrombotic effects that could modulate early allograft injury, but contemporary evidence in adults is limited. We evaluated whether early postoperative dextran-40 use was associated with improved graft outcomes. METHODS: In this single-center retrospective cohort, we studied 900 adult liver transplant recipients (2009-2023) using a 48-h landmark to reduce immortal-time bias; 883 patients alive with their primary graft at 48 h formed the analytic cohort. Early exposure was defined as starting dextran-40 within 48 h after transplantation versus l

Acute lung allograft dysfunction predicts lung allograft failure: Construct validity of the 2026 International Society for Heart and Lung Transplantation spirometric definition.

INTRODUCTION: Acute lung allograft dysfunction (ALAD) has been proposed as a screening criterion to identify lung transplant recipients at increased risk for graft loss. ALAD was defined formally in a 2026 International Society for Heart and Lung Transplantation (ISHLT) Delphi consensus statement but has not been empirically evaluated. We sought to test the construct validity of the ISHLT ALAD definition by testing the hypothesis that ALAD is associated with increased risk of graft failure. METHODS: We included 86,885 spirometry measurements from 2,877 participants in the 8-center Lung Transp

Prognosis and Risks for Probable Chronic Lung Allograft Dysfunction: A Prospective Multicenter Study.

Rationale: Chronic lung allograft dysfunction (CLAD) hinders lung transplant success. A 2019 consensus refined CLAD diagnosis, introducing probable or definite CLAD based on persistence of lung function decline. Outcomes and risks for probable CLAD remain uncertain. Objectives: We sought to determine the prognosis and clinical risks for probable CLAD in a prospective multicenter cohort. Methods: Clinical Trials in Organ Transplantation-20 included 745 CLAD-eligible adult lung recipients at five centers and applied rigorous methods to prospectively adjudicate probable CLAD. The impact of probab

Molecular biopsy features associated with baseline lung allograft dysfunction in a multicenter international cohort.

BACKGROUND: Baseline lung allograft dysfunction (BLAD) is a state of abnormally low peak post-transplant lung function, but the mechanisms are poorly understood. In this study, we examined transbronchial biopsy (TBB) gene expression changes associated with BLAD in an international cohort of prospectively enrolled lung transplant recipients. METHODS: BLAD status was assessed at 1-year post-transplant and defined as failure to reach normal lung function, as defined by a forced expiratory volume in 1 s and a forced vital capacity > 80% predicted on two consecutive occasions > 3 weeks apart. Alcohol abuse in deceased liver donors: impact on post-transplant outcomes.

BACKGROUND: Many deceased liver donors with a history of alcohol abuse are excluded based upon medical history alone. This paper summarizes the transplant outcomes for a large number of deceased liver donors with a documented history of alcohol abuse. METHODS: The records for 1478 consecutive deceased liver donors were reviewed (2001-2012). As per the United Network for Organ Sharing criteria, heavy alcohol use by an organ donor is defined as chronic intake of two or more drinks per day. Donors with a documented history of alcohol abuse were divided into three groups according to duration of

Clinical and Histologic Risk Factors for the Development of Early Allograft Dysfunction in Donation After Circulatory Death Kidney Transplantation.

Transplantation of kidneys from donors after circulatory death (DCD) may help address the severe shortage of donor organs. However, they remain underutilized due to concerns for increased rates of delayed graft function (DGF) and primary nonfunction (PNF), despite similar long-term patient and allograft outcomes compared to kidneys from donors after brain death (DBD). Kidneys that recover from DGF have good long-term outcomes compared to those resulting in PNF. Considering immediate graft function (IGF), DGF, and PNF as a spectrum of renal allograft injury and recovery, we analyzed donor and r

Ventricular Assist Device in Children with Cardiac Graft Failure.

We sought to determine whether ventricular assist device (VAD) support is an effective therapy in children with cardiac graft dysfunction. We conducted a retrospective review of VAD usage in this scenario at our institution. Although short-term VAD support was highly successful (89% [eight out of nine] were bridged to recovery), only 29% (2 out of 7) with long-term VAD survived to retransplant. Of note, three out of five mortalities with long-term VAD were related to sepsis (two fungal and one Gram-negative bacterial). Infectious risk imposed by ongoing immunosuppressive therapy limits the rol

Donor liver apoptosis is associated with early allograft dysfunction and decreased short-term graft survival after liver transplantation.

BACKGROUND: The clinical significance of apoptosis in assessing the quality of donor liver grafts remains unknown. AIMS: This study aimed to determine whether apoptosis in a donor liver is predictive of early allograft dysfunction (EAD) and graft survival after liver transplantation (LT). METHODS: Donor liver specimens were analyzed for apoptosis using TUNEL assays. The prognostic factors for EAD were identified through logistic regression analyses, and a nomogram was developed. RESULTS: The apoptosis index of donor livers in EAD patients was significantly higher than that of donors livers

Protective effects of neutrophil serine protease inhibition against ischemia-reperfusion injury in lung or heart transplantation.

Transplanted organs are inevitably exposed to ischemia-reperfusion (IR) injury, which is known to cause graft dysfunction. Functional and structural changes that follow IR tissue injury are mediated by neutrophils through the production of oxygen-derived free radicals, as well as from degranulation which entails the release of proteases and other pro-inflammatory mediators. Neutrophil serine proteases (NSPs) are believed to be the principal triggers of post-ischemic reperfusion damage. Extended preservation times for the transplanted donor organ correlate with heightened occurrences of vascula

Small airway brush gene expression predicts chronic lung allograft dysfunction and mortality.

BACKGROUND: Chronic lung allograft dysfunction (CLAD) limits survival following lung transplant, but substantial lung damage occurs before diagnosis by traditional methods. We hypothesized that small airway gene expression patterns could identify CLAD risk before spirometric diagnosis and predict subsequent graft failure. METHODS: Candidate genes from 4 rejection-associated transcript sets were assessed for associations with CLAD or graft failure in a derivation cohort of 156 small airway brushes from 45 CLAD cases and 37 time-matched controls with >1-year stable lung function. iTRAQ- and MRM-Based Proteomics Identify Early Injury Biomarkers for Primary Dysfunction After Liver Transplantation.

BACKGROUND Primary nonfunction (PNF) is a severe complication following liver transplantation (LT), yet precise molecular biomarkers for early identification of patients at risk remain lacking, which can delay timely therapeutic intervention. MATERIAL AND METHODS Liver biopsies were collected from patients and classified into 4 groups: control, optimal graft (OG), early allograft dysfunction (EAD), and PNF. Samples were obtained at 3 time points: T0 (pre-cold perfusion), T1 (pre-reperfusion), and T2 (post-reperfusion). Isobaric tags for relative and absolute quantitation (iTRAQ) and multiple r

The impact of pre-existing hematologic disorders on morbidity and mortality following heart transplantation: Focus on early graft dysfunction.

BACKGROUND: Heart transplantation (HT) is the gold standard therapy for advanced heart failure, providing excellent long-term outcomes. However, postoperative outcomes are limited by bleeding, infections, and primary graft dysfunction (PGD) that contribute to early mortality after HT. HT candidates with pre-existing hematologic disorders, bleeding, and clotting, may represent a higher risk population. We assessed the short- and long-term outcomes of patients with pre-existing hematologic disorders undergoing HT. METHODS AND RESULTS: Medical records of all adult patients who received HT from J

Financial implications of using extracorporeal membrane oxygenation following heart transplantation.

OBJECTIVES: Primary graft dysfunction after heart transplant is associated with high morbidity and mortality. Extracorporeal membrane oxygenation (ECMO) can be used to wean patients from cardiopulmonary bypass. This study retrospectively reviews a single-centre experience of post-transplant ECMO in regard to outcomes and associated costs. METHODS: Between May 2006 and May 2019, a total of 267 adult heart transplants were performed. We compared donor and recipient variables, ECMO duration and the incidence of renal failure, bleeding, infection and cost analysis between ECMO and non-ECMO groups

Successful management of donor-acquired fat embolism after lung transplantation.

Fat embolism is a serious complication in patients with multiple traumatic injuries. It is often asymptomatic during the first hours of resuscitation, thus remains underdiagnosed in patients who progress to brain death. Lung transplantation issued from such grafts can lead to severe lung primary graft dysfunction, the management of which is deemed difficult. Herein, we report a successful management of donor-acquired fat embolism syndrome after lung transplant in a 22 years old woman for cystic fibrosis. Fat embolism was suspected because of the donor's traumatic injuries and confirmed by hist

Thrombotic microangiopathy caused by severe graft dysfunction after living donor liver transplantation: report of a case.

Thrombotic microangiopathy (TMA) is a life-threatening complication after transplantation including liver transplantation, and its typical clinical picture is characterized by hemolytic anemia, thrombocytopenia, renal dysfunction, neurological abnormalities, and fever. We report the case of a 56-year-old female with end-stage liver disease who underwent living donor liver transplantation (LDLT), and whose postoperative course was characterized by renal failure and progressive hyperbilirubinemia. Two weeks after LDLT, she started to show progressive thrombocytopenia, anemia, oliguria, and encep

Validation of recipient-specific kinetic prediction scores for allograft failure after liver transplantation: a single centre retrospective analysis.

Liver transplantation remains the definitive therapy for end-stage liver disease, yet Early Allograft Dysfunction (EAD) continues to challenge post-transplant outcomes. Several models have been developed to predict graft dysfunction and survival, including Olthoff's static definition, and three kinetic prediction scores: the Model for Early Allograft Function (MEAF), the Liver Graft Assessment Following Transplantation score (L-GrAFT), and the more recent Early Allograft Failure Simplified Estimation (EASE). While these scores have been validated in Western populations, data from Eastern Europ
