The efficacy of acoustic-based articulatory phenotyping for characterizing and classifying four divergent neurodegenerative diseases using sequential motion rates.

Despite the impacts of neurodegeneration on speech function, little is known about how to comprehensively characterize the resulting speech abnormalities using a set of objective measures. Quantitative phenotyping of speech motor impairments may have important implications for identifying clinical syndromes and their underlying etiologies, monitoring disease progression over time, and improving treatment efficacy. The goal of this research was to investigate the validity and classification accuracy of comprehensive acoustic-based articulatory phenotypes in speakers with distinct neurodegenerat

"Doing the Best I Can": Qualitative Outcomes and Participant Feedback From a Combined Communication and Counselling Treatment for Primary Progressive Aphasia.

INTRODUCTION: There is a growing evidence base supporting the utility of restitutive speech-language treatments for individuals with primary progressive aphasia (PPA). In contrast, there is limited research investigating treatment approaches that offer counselling to this population. Given that people with PPA are susceptible to negative impacts on their psychosocial well-being, research is needed to address this underrepresented area of inquiry. This paper provides qualitative research findings from a pilot and feasibility study in which individuals with PPA completed a novel, telehealth-base

Episodic memory in frontotemporal dementia: a critical review.

This review offers a critical appraisal of the literature on episodic memory performance in frontotemporal dementia. Historically, description of patients diagnosed with what was then known as Pick's disease included the presence of memory deficits and an underlying amnestic syndrome was noted in some of these patients. Over the last 20 years, however, the clinical view has been that episodic memory processing is relatively intact in the frontotemporal dementia syndrome. In particular, patients with the subtypes of behavioural variant frontotemporal dementia and progressive non-fluent aphasia

Speech Motor Profiles in Primary Progressive Aphasia.

PURPOSE: Previous research on motor speech disorders (MSDs) in primary progressive aphasia (PPA) has largely focused on patients with the nonfluent/agrammatic variant of PPA (nfvPPA), with few systematic descriptions of MSDs in variants other than nfvPPA. There has also been an emphasis on studying apraxia of speech, whereas less is known about dysarthria or other forms of MSDs. This study aimed to examine the qualitative and quantitative characteristics of MSDs in a prospective sample of individuals with PPA independent of subtype. METHOD: We included 38 participants with a root diagnosis of

miRNA Expression Is Increased in Serum from Patients with Semantic Variant Primary Progressive Aphasia.

Primary progressive aphasia (PPA) damages the parts of the brain that control speech and language. There are three clinical PPA variants: nonfluent/agrammatic (nfvPPA), logopenic (lvPPA) and semantic (svPPA). The pathophysiology underlying PPA variants is not fully understood, including the role of micro (mi)RNAs which were previously shown to play a role in several neurodegenerative diseases. Using a two-step analysis (array and validation through real-time PCR), we investigated the miRNA expression pattern in serum from 54 PPA patients and 18 controls. In the svPPA cohort, we observed a gene

A Combined Speech-Language and Counseling Intervention for Primary Progressive Aphasia: Acceptability, Feasibility, and Preliminary Outcomes.

PURPOSE: Individuals with primary progressive aphasia (PPA) face negative psychosocial repercussions, yet few studies have investigated counseling for this population. In a prior study, a novel intervention encompassing script training and aphasia-modified cognitive behavioral therapy was piloted with an individual with PPA, yielding promising results. The purpose of this study is to determine the acceptability and feasibility of this intervention for individuals with different PPA subtypes, with secondary aims of understanding preliminary treatment outcomes and evaluating the sensitivity of s

Effects of Modified Video-Implemented Script Training for Aphasia in the Three Variants of Primary Progressive Aphasia.

PURPOSE: Primary progressive aphasia (PPA) is a neurodegenerative disorder characterized by worsening of speech and/or language. Script training intervention promotes automatized speech production via repeated practice of scripted content. This study evaluated the acceptability, feasibility, and effects of a modified version of Video-Implemented Script Training for Aphasia (VISTA) in the three PPA variants and compared outcomes by intervention modality (teletherapy vs. in person). METHOD: Thirteen bilingual (Spanish-Catalan) participants were included (semantic variant, n = 5; logopenic varia

Clinical characteristics of Japanese patients with corticobasal degeneration.

INTRODUCTION: Corticobasal degeneration (CBD) is a clinically heterogeneous neurodegenerative disorder, for which pathological investigations are essential for a definitive diagnosis. This study explored the clinical characteristics of Japanese patients with pathologically confirmed CBD. METHODS: We reviewed the data of Japanese patients with pathologically confirmed CBD who were consecutively autopsied at our institute. Clinical data were obtained from medical records and clinicopathological conferences. RESULTS: Of the 34 patients initially reviewed, three were excluded because of a lack o

Reading and writing impairments in Spanish-speaking individuals with primary progressive aphasia: A single-case series study.

Reading and writing impairments are frequent in primary progressive aphasia (PPA), yet their characterization in Spanish-speaking populations remains limited due to language-specific orthographic properties. The present study examined reading and writing impairments in 17 Spanish-speaking participants diagnosed with PPA (9 logopenic, 4 semantic, and 4 nonfluent), with the aim of identifying patterns of acquired dyslexia and dysgraphia across PPA variants. A single-case series design was employed, including individuals diagnosed with PPA and matched healthy control subjects. Participants were a

Non-Invasive Brain Stimulation in Frontotemporal Dementia: A Systematic Review of Non-Pharmacological Treatment Approaches.

Frontotemporal dementia (FTD) is a heterogeneous disorder for which disease-modifying treatments are lacking. Non-invasive brain stimulation (NIBS) has emerged as a potential therapeutic approach to modulate dysfunctional neural networks and support residual plasticity. This systematic review aims to provide an updated overview of current NIBS applications across the main clinical syndromes associated with FTD, namely behavioral variant FTD (bvFTD), semantic variant of primary progressive aphasia (svPPA), and nonfluent variant of PPA (nfvPPA). According to PRISMA guidelines, twenty-seven studi

Progressive supranuclear palsy syndrome presenting as progressive nonfluent aphasia: a neuropsychological and neuroimaging analysis.

There is currently considerable interest in the clinical spectrum of progressive nonfluent aphasia (PNFA) and progressive supranuclear palsy (PSP) and the intersection of these two entities. Here, we undertook a detailed prospective clinical, neuropsychological, and neuroimaging analysis of 14 consecutive patients presenting with PNFA to identify cases meeting clinical criteria for PSP. These patients had further detailed assessment of extrapyramidal and oculomotor functions. All patients had high-resolution MR brain volumetry and a cortical thickness analysis was undertaken on the brain image

Subjective time perception in dementia: a behavioural and neuroanatomical analysis.

Subjective time perception-the modulation of elapsed clock time by sensory, homeostatic and psychological factors-is fundamental to how we experience the world. People with Alzheimer's disease and frontotemporal dementia often exhibit clinically relevant symptoms of altered temporal awareness but these are poorly understood. Here we addressed this issue in a cross-sectional, case-control study of 60 patients representing all major Alzheimer (n = 24) and frontotemporal (n = 36) syndromes [mean age 68.8 (range 48-77 years); 28% female] and 24 cognitively well age-matched controls [age 69.4 (6.5)

Clinic, neuropathology and molecular genetics of frontotemporal dementia: a mini-review.

Frontotemporal lobar degeneration (FTLD) represents a group of clinically, neuropathologically and genetically heterogeneous disorders with plenty of overlaps between the neurodegenerative mechanism and the clinical phenotype. FTLD is pathologically characterized by the frontal and temporal lobar atrophy. Frontotemporal dementia (FTD) clinically presents with abnormalities of behavior and personality and language impairments variants. The clinical spectrum of FTD encompasses distinct canonical syndromes: behavioural variant of FTD (bvFTD) and primary progressive aphasia. The later includes non

Progressive nonfluent aphasia: a rare clinical subtype of FTLD-TDP in Japan.

Progressive nonfluent aphasia (PNFA) is a clinical subtype of frontotemporal lobar degeneration (FTLD). FTLD with tau accumulation (FTLD-tau) and FTLD with TDP-43 accumulation (FTLD-TDP) both cause PNFA. We reviewed clinical records of 29 FTLD-TDP cases in the brain archive of our institute and found only one case of PNFA. The patient was an 81-year-old male at death. There was no family history of dementia or aphasia. He presented with slow, labored and nonfluent speech at age 75. Behavioral abnormality and movement disorders were absent. MRI at age 76 demonstrated atrophy of the perisylvian

Correlates of anomia in non-semantic variants of primary progressive aphasia converge over time.

To track neural correlates of naming performance with disease progression, we estimated key areas affected in nonfluent/agrammatic (nfvPPA) and logopenic (lvPPA) primary progressive aphasia variants over time and changes in naming correlates over time. Twenty-nine non-semantic PPA participants (17 nfvPPA and 12 lvPPA) were selected based upon current diagnostic criteria and PiB-PET status and conducted a confrontation-naming task and a structural MRI. Linear mixed-effect models implemented in FreeSurfer were used for tracking cortical thickness and epicenters of atrophy over time. Using averag

Increased telomere length in patients with frontotemporal dementia syndrome.

BACKGROUND: Telomeres are repetitive DNA sequences of TTAGGG at the ends of chromosomes. Many studies have shown that telomere shortening is associated with aging-related diseases, such as cardiovascular diseases, hypertension, diabetes, cancer, and various neurodegenerative diseases, including Alzheimer's disease, vascular dementia, Parkinson's disease, and dementia with Lewy bodies. However, changes in telomere length (TL) in patients with frontotemporal dementia (FTD) syndrome are unclear. Accordingly, in this study, we assessed TL in blood samples from patients with FTD syndrome. METHODS:

Automated Speech Analysis to Identify Clinical, Anatomical, and Pathological Variants of Primary Progressive Aphasia.

IMPORTANCE: Primary progressive aphasia (PPA) is defined by relatively isolated speech and language symptoms caused by neurodegeneration of language networks; classification of the different clinical, anatomical, and pathological variants relies on time-intensive, expert-dependent assessments that are not widely available. Scalable, interpretable speech-based tools could support diagnosis and monitoring in clinical care and trials. OBJECTIVE: To determine whether automated speech analysis of voice recording from a short picture description task can yield clinically interpretable speech and la
