Pathology image of the month: death in a young adult with sickle cell disease.

A 27-year-old African-American female with known sickle cell disease was admitted for sickle cell crisis and presumed sepsis. The patient's past medical history was complicated by multiple sickle cell-related complications, including seizures and multiple prior blood transfusions. Her hospital course included Staphylococcus epidermidis bacteremia, for which broad spectrum antibiotics were prescribed. On hospital day nine, the patient was found unresponsive and declared dead after unsuccessful efforts at resuscitation. An unlimited autopsy examination was conducted under authorization of the co

Factors influencing somatic genetic testing for vascular anomalies at 2 academic centers.

Vascular anomalies (VAs) are rare disorders with abnormal development of blood and lymphatic vasculature and surrounding tissues. Understanding the genetic etiology of VAs guides treatment with targeted inhibitors. This study compares patterns and outcomes of genetic testing in VA populations at 2 centers. Of 421 patients (mean age, 10 years; 55.6% female), 61.7% underwent prior authorization (PA) for genetic testing, 84% had their testing approved, 93% completed the testing. About 70% of the completed tests had pathogenic or likely pathogenic results. A positive result was not associated with

Clinician perspectives on infusion site-of-care shifts in pediatric inflammatory bowel disease: An anonymous national web-based survey.

Commercial payers and pharmacy benefit managers use site-of-care and specialty drug distribution policies (including white bagging and brown bagging) that can redirect infusion-based biologic therapies from hospital outpatient departments to alternate settings. We conducted an anonymous web-based survey distributed via a national pediatric gastroenterology (GI) listserv (February 13-27, 2026) to characterize perceived access barriers and decision drivers for acceptable infusion sites among pediatric inflammatory bowel disease (IBD) clinicians. Among 100 respondents, prior authorization/payer f

Paxlovid-induced tacrolimus toxicity in a 16-year-old male with steroid-resistant nephrotic syndrome.

In December 2021, the Federal Drug Administration (FDA) approved emergency use authorization of nirmatrelvir-ritonavir (Paxlovid) to prevent serious SARS-CoV-2 infections in high-risk patient populations. We present the case of a 16-year-old male with steroid-resistant nephrotic syndrome who developed tacrolimus toxicity after initiation of Paxlovid therapy. The ritonavir component strongly inhibits CYP3A4 enzymes, thereby leading to the accumulation of tacrolimus in the blood. This patient's toxicity manifested in multiorgan dysfunction including elevated creatinine, gastrointestinal distress

EMA Review of Panobinostat (Farydak) for the Treatment of Adult Patients with Relapsed and/or Refractory Multiple Myeloma.

On August 28, 2015, a marketing authorization valid through the European Union was issued for panobinostat, in combination with bortezomib and dexamethasone, for the treatment of adult patients with relapsed and/or refractory multiple myeloma who have received at least two prior regimens including bortezomib and an immunomodulatory agent (IMiD).Panobinostat is an orally available histone deacetylase (HDAC) inhibitor that inhibits the enzymatic activity of HDAC proteins at nanomolar concentrations. HDAC proteins catalyze the removal of acetyl groups from the lysine residues of histones and some

Reforming Medicare payment for skin substitutes-ripping off the bandage.

In January 2026, the Centers for Medicare and Medicaid Services (CMS) implemented a major overhaul of Medicare reimbursement for skin substitutes in an effort to curb runaway spending. Between 2019 and 2024, annual Medicare Part B spending on these products increased more than 40-fold, from $250 million to more than $10 billion. Prior to the recent reform, CMS reimbursed clinicians for skin substitutes administered in nonfacility settings (eg, physician offices and patient homes) at a rate of an average sales price (ASP) of +6%. However, skin substitute manufacturers were omitted from federal

Expanding Patient Choice in Diabetes Care: Reducing Access Barriers to Inhaled Insulin.

Inhaled insulin provides a unique prandial insulin option, offering an alternative, with a more rapid onset of effect, to subcutaneous rapid-acting analog insulin. Clinical studies with inhaled insulin have demonstrated its efficacy and safety, with glycemic outcomes comparable to or in some cases better than those of contemporary insulin delivery approaches. However, the use of inhaled insulin for diabetes therapy in daily life remains limited due to payer restrictions, prior authorization requirements, formulary exclusions, clinician knowledge gaps, clinician time constraints, persistent mis

Small molecules as inhibitors of PCSK9: Current status and future challenges.

Proprotein convertase subtilisin/kexin type 9 (PCSK9) plays an important role in regulating lipoprotein metabolism by binding to low-density lipoprotein receptors (LDLRs), leading to their degradation. LDL cholesterol (LDL-C) lowering drugs that operate through the inhibition of PCSK9 are being pursued for the management of hypercholesterolemia and reducing its associated atherosclerotic cardiovascular disease (CVD) risk. Two PCSK9-blocking monoclonal antibodies (mAbs), alirocumab and evolocumab, were approved in 2015. However, the high costs of PCSK9 antibody drugs impede their prior authoriz

Primum Non Nocere and the 5 Rights.

Overutilization of hyperbaric oxygen therapy (HBOT) is commonplace and primarily associated with outpatient wound care. While the number of hospitals providing HBOT is at an all-time high, the number of those willing to treat patients in immediate need is at an all-time low. Huge areas of the country, including major population areas, are now completely devoid of 24/7 HBOT availability and inpatient access. Purchasers of healthcare, including the Centers for Medicare and Medicaid Services, have become increasingly concerned to the point that several strategies have been introduced to construct

Medicaid coverage of medications to treat alcohol and opioid dependence.

Substance use disorders affect 12% of Medicaid beneficiaries. The prescription drug epidemic and growing need for treatment of alcohol and opioid dependence have refocused states' attention on their provision of substance use disorder treatment services, including medications. This study characterized how Medicaid programs cover these treatment medications. Data were from 2013 Medicaid pharmacy documents, 2011 and 2012 Medicaid state drug utilization records, and a 2013 American Society of Addiction Medicine survey. Results showed that only 13 state Medicaid programs included all medications a

Prescribing patterns for topical retinoids: analyses of 15 years of data from the national ambulatory medical care survey.

OBJECTIVE: To examine the prescribing patterns of topical retinoids in the United States. METHODS: A retrospective, cross-sectional study was employed. Data from the National Ambulatory Medical Care Survey (1990-2004) were used. The impact of patient diagnosis of acne and other covariates on the probability of getting a retinoid prescription was examined using weighted multivariate logistic regression models. RESULTS: Among the national cohort of patients aged 10 years and older (number of patient visits = 11.7 billion), 41.5 million patients received a topical retinoid prescription. In the

Hepatitis C donor positive to recipient negative solid organ transplants: Early direct acting antiviral insurance approval rates with and without documented viremia.

BACKGROUND: Current guidelines support early initiation of direct-acting antivirals (DAA) in hepatitis C virus (HCV) donor positive and recipient negative (D+/R-) solid organ transplants (SOTs). According to experts, access to DAA therapy is a key barrier to early treatment. METHODS: This single-center, retrospective study assessed the rate of DAA prescription approval with or without confirmed HCV viremia, time to approval, and reasons for denial in HCV D+/R- SOTs. RESULTS: All 51 patients received insurance approval for DAA therapy following transplantation regardless of confirmed HCV vire

Pathology image of the month. Black esophagus detected at autopsy in a patient with abdominal pain and bloody diarrhea. DIAGNOSIS: Acute esophageal necrosis, ischemic and pseudomembranous colitis.

A 73-year-old African-American male was transported to the emergency department due to what emergency personnel described as "coffee ground emesis." He was pronounced dead shortly after arrival. An unlimited autopsy examination was conducted under authorization of the coroner's office. Medical record review revealed that the decedent had been discharged from the hospital just one day prior to his death following a three-day admission for abdominal pain, bloody diarrhea, and a 22-lb unintentional weight loss. Comparing timelines and evidence available to support new TB, HIV, and HCV drug approvals: The same, only different.

BACKGROUND: Tuberculosis (TB), human immunodeficiency virus (HIV), and hepatitis C virus (HCV) share a global presence and propensity to disproportionately affect marginalized populations. However, over recent decades, many fewer drugs have been brought to market for TB than for the others. Although three new anti-TB drugs have been approved in the US or Europe in the last 10 years, uptake of these drugs has been limited. Using case examples of drugs developed recently for TB, HIV, and HCV, we explore possible reasons. We examine the use and effect of regulatory pathways intended to address we

Who is responsible? A risk-stratified framework for pharmacist accountability in artificial intelligence-assisted managed care.

Artificial intelligence (AI)-enabled tools are increasingly integrated into managed care pharmacy workflows, particularly in utilization management and prior authorization processes. Although these systems aim to improve operational consistency and efficiency, federal regulatory guidance and professional standards make clear that coverage and medical necessity determinations must remain grounded in human clinical judgment. As AI tools become part of routine managed care, responsibility for final decisions may become unclear, creating an accountability gap. This article synthesizes federal regu
