Ultrasound-Guided Perineural Injection for Pronator Syndrome Caused by Median Nerve Entrapment.

Patients and physicians have increasingly sought minimally invasive procedures such as ultrasound-guided injection for the treatment of peripheral nerve entrapment syndromes. In this series, we assessed subjective outcome data in 14 patients who underwent ultrasound-guided perineural hydrodissection and steroid injection for pronator syndrome secondary to median nerve entrapment in the pronator tunnel. Excellent symptomatic relief (≥75% improvement) was achieved in 70% of nerves with 3-month follow-up data, with no significant change in symptoms between 3 and 6 months. These outcomes suggest t

Pronator sparing plate osteosynthesis in distal radius fractures: early functional outcome.

Thanks to modern anatomic stable angle plate and screw designs in distal radius fractures, early mobilization is possible and pronation torque of the wrist is more important in early recovery. Pronator sparing plate osteosynthesis of the distal radius is a relatively new approach. A short overview of the technique is given with some tips and tricks. The objective of the study is to investigate whether pronator sparing approach to distal radius fractures gives better pronation force or grip force in the postoperative rehabilitation. Two randomly assigned groups of 30 patients with dorsally disp

Concurrent carpal tunnel syndrome and pronator syndrome: A retrospective study of 21 cases.

PURPOSE: Concurrent carpal tunnel syndrome and pronator syndrome are rarely considered and the proximal compression sites are easily overlooked. We retrospectively studied 21 concurrent cases in our series from 2009 to 2015 and report the results. PATIENTS AND METHODS: The typical symptoms were pain, tingling, and numbness of the radial 3½ digits. If paresthesia involved the thenar eminence and proximal forearm pain was noted in cases of carpal tunnel syndrome, carpal tunnel syndrome combined with pronator syndrome was considered. Additionally, nocturnal paresthesia symptoms are absent in pro

A Brachioradialis Splitting Approach Sparing the Pronator Quadratus for Volar Plating of the Distal Radius.

Fractures of the distal radius account for up to 15% of all extremity fractures. Volar plating has become more popular, as it allows locking plate technology to be applied. Traditionally, access to the volar radius has been achieved through the approach of Henry using the interval between flexor carpi radialis and the radial artery, involving incising the radial border of the pronator quadratus (PQ). With this approach, PQ repair is difficult, and when attempted is often incomplete or tenuous, as it is a direct muscle-to-muscle repair. Theoretical advantages of repairing the PQ include the pro

Terrible triad elbow fracture-dislocation with triceps and flexor-pronator mass avulsion.

Terrible triad elbow injuries, consisting of fractures of the radial head and coronoid with ulnohumeral dislocation, are challenging to treat. They require a comprehensive understanding of the complex anatomy of the elbow to effectively treat all of the pathology and create a stable, congruent joint. The authors present a case of a terrible triad injury with avulsion of the triceps and flexor-pronator mass after a low-energy fall in a young patient. Although most terrible triad fracture-dislocations can be successfully treated with coronoid fixation, radial head fixation or replacement, and re

Pronated oblique view in assessing proximal scaphoid articular cannulated screw penetration.

PURPOSE: Volar and percutaneous techniques of screw fixation for scaphoid fractures usually do not involve direct visualization of the dorsoproximal scaphoid articular surface. Intra-articular screw penetration may be overlooked if the proper fluoroscopic views are not obtained. The hypothesis of this study is that the pronated oblique fluoroscopic view of the scaphoid is the most sensitive in detecting dorsal articular screw penetration of the scaphoid when compared with other views. METHODS: A 3.6-mm Steinmann pin was inserted along the central axis of the scaphoid using an open dorsal tech

[Pronator quadratus repair using the Henry approach with an outward-return running suture].

Repair of the quadratus pronator after osteosynthesis of the distal radius via the Henry approach is generally considered a difficult procedure. The quadratus pronator is a powerful muscle which stabilizes the distal radioulnar joint. The muscle is most often injured in association with distal fracture of the radius. Apposition osteosynthesis appears to be the most appropriate procedure in order to limit loss of reduction, i.e. distal radioular variance, observed in the first postoperative weeks. Tendon complications observed with dorsal approaches and the development of new volar plates with

Functional deficit after transfer of the pronator teres for acquired radial nerve palsy.

PURPOSE: A suitable muscle motor in reconstruction after acquired never injury should have adequate strength to perform the desired task, be aligned in the direction of pull, have synergistic action, and not result in unacceptable functional loss. In radial nerve palsy, the pronator teres is the most common motor donor used to restore wrist extension. Although the pronator teres remains aligned to provide pronation, the force deficit of the transfer is not known. METHODS: We used 6 cadavers and 6 patients to determine the loss of pronation strength both experimentally and clinically. RESULTS

Risk factors that may adversely modify the natural history of the pediatric pronated foot.

Flatfoot is one of the most common conditions seen in pediatric podiatry practice. There is no universally accepted definition for flatfoot. Flatfoot is a term used to describe a recognizable clinical deformity created by malalignment at several adjacent joints. Clinically, a flatfoot is one that has a low or absent longitudinal arch. Determining flexibility (physiologic) or rigidity (pathologic) is the first step in management. A flexible flat foot will have an arch that is present in open kinetic chain (off weight-bearing) and lost in closed kinetic chain (weight-bearing). A rigid flatfoot h

Pronator quadratus-sparing volar plating of distal radius fractures: A prospective study.

PURPOSE: Volar locked plates for the distal radius fractures (DRFs) are applied through the Henry approach, and its modification entails a routine step to disinsert the pronator quadratus from its radial and distal attachments. This muscle insertion is mostly fleshy with minimal tendinous tissue, and surgeons find it difficult to reattach at the end of the surgery, the hardware comes in direct contact with the flexor tendons. METHODS: The study was performed as a single-center prospective case series to investigate the possibility of preserving the pronator quadratus and, at the same time, ac

The Diagnostic Role of the Pronator Quadratus Sign in Paediatric Wrist Fractures: A Retrospective Study.

Background/Objectives: Wrist fractures are common paediatric injuries, yet some remain radiographically occult. The Pronator Quadratus Sign (PQS), defined by displacement or alteration of the fat stripe overlying the pronator quadratus muscle (PQm), remains a debated radiologic diagnostic marker. This study evaluated whether PQS can serve as a diagnostic adjunct for wrist fractures and whether specific PQS morphological types are associated with different fracture patterns. Methods: This retrospective study included paediatric patients presenting with wrist trauma. Radiographs were categorized

Utilizing the Pronated Forearm Technique for Measuring Glenohumeral External Rotation in Baseball Players.

BACKGROUND: Alterations in glenohumeral internal rotation (GIR), glenohumeral external rotation (GER), and total range of motion (TROM) have been linked with increased injury risk. GER capacity has been measured routinely with the forearm in neutral rotation (GERN), but a recent study reported GERN was greater than GER with the forearm in pronation (GERP) in Minor League pitchers. This work has not yet been replicated or extended to other groups. HYPOTHESIS: GERP would be significantly less than GERN in Independent League baseball pitchers, and there would be no difference in GERP or GERN mea

[Transfer of the pronator quadratus to restore opposition of the thumb: An anatomic study and a clinical report].

OBJECTIVE: The purpose of this study is to explore the feasibility of reconstructing the APB with a pedicled PQ flap, and to report results in a child presenting with bilateral radial deficiency. METHODS: Twenty-one injected cadaver upper extremities were dissected. The muscle was pedicled distally on the transverse carpal artery, and reinnervated with the flexor digiti minimi (FDM) motor branch. The transfer was evaluated on 3 parameters: surgical feasibility, length of the distal pedicle and distance from the coaptation site to the muscle entry of recipient nerve. A bilateral PQ pedicled tr

The Dynamicity of Steenbeek Foot Abduction Brace for Clubfoot in Dorsiflexion and Pronation: A Pilot Study.

PURPOSE: Steenbeek foot abduction brace (SFAB) has been widely used in various national clubfoot programs. The aim of the study was to define effectiveness and dynamicity of SFAB in terms of dorsiflexion and pronation for the corrected clubfoot. METHODS: Differences in foot dorsiflexion and pronation measurement with brace in knee flexed and extended position were recorded as dynamicity1 and dynamicity2, respectively. The residual soft tissue stretch lag despite brace use was calculated by determining the difference between maximum foot dorsiflexion (stretchlag1) and pronation (stretchlag2) a

Volar Stabilization of the Distal Radioulnar Joint for Chronic Instability Using the Pronator Quadratus.

In cases of chronic distal radioulnar joint (DRUJ) instability without DRUJ arthritis, reconstruction of the mechanical integrity of the radioulnar ligaments of the triangular fibrocartilage complex has been considered an ideal surgical treatment. However, reconstructive methods have several disadvantages. We evaluated volar stabilization of the DRUJ for chronic instability using the pronator quadratus (PQ) to determine whether it provided (1) proper stability, (2) restored wrist function, (3) was relatively convenient, and (4) was associated with a low complication rate. Altogether, 21 patient

A Pronator Teres-sparing Volar Approach for Osteosynthesis of Midshaft Diaphyseal Radius Fractures.

Henry's approach is widely considered the "workhorse" for exposing the volar radius and has not really changed since his original description in 1945. We describe a pronator teres-sparing volar approach to the forearm, for osteosynthesis of midshaft diaphyseal radius fractures. We believe this approach is safe, simple, and reproducible, and has several practical and theoretical advantages over Henry's original description.

A case of rotational restriction of the forearm due to abnormal configuration of pronator quadratus muscle.

We present a case of rotational restriction of the forearm due to abnormal configuration of the pronator quadratus muscle. A 20-year-old man developed right wrist joint pain on pitching of a baseball game and thereafter displayed rotational disorder of the forearm. Magnetic resonance imaging (MRI) revealed a space-occupying lesion from the volar side of the radius to the dorsal side of the ulna. The lesion was iso-hyperintense on T1-weighted (T1W) images and showed a mixed pattern of high intensities on T2-weighted (T2W) images. His symptoms were immediately reduced after removal of the mass.

The effect of pronation and inclination on the measurement of the hallucal distal metatarsal articular set angle.

OBJECTIVE: In this study, we tried to evaluate the effect of pronation and the inclination of the first metatarsal on the measurement of distal metatarsal articular angle (DMAA) in 10 cadaver first metatarsals. METHODS: Ten cadaver first metatarsals were fixed to a device. This device can change the inclination and pronation angles of the metatarsal. 15-30-45 degrees of inclination and 0-10-20 degrees of pronation were applied to the metatarsals. After applying radio-opaque putty to the medial and lateral articular edges and metatarsal dorsal diaphyseal ridge, the X-ray and digital images wer
