[Intraoperative asystole in a patient with concealed sick sinus syndrome: a case report].

We report a patient with concealed sick sinus syndrome who developed intraoperative bradycardia and asystole. An 81-year-old man was scheduled to undergo total gastrectomy under general and epidural anesthesia. There was no history of syncope, and preoperative 12-lead ECG showed normal sinus rhythm. Anesthesia was induced with propofol and remifentanil, maintained with sevoflurane, remifentanil and thoracic epidural infusion of lidocaine, fentanyl and levobupivacaine. Bradycardia was detected on ECG 110 minutes after the start of surgery. Intravenous atropine (0.5 mg, repeated up to a total do

Perioperative Challenges in a Patient With Sturge-Weber and Obesity Hypoventilation Syndromes.

BACKGROUND: The rare concurrence of Sturge-Weber syndrome (SWS) and obesity hypoventilation syndrome (OHS) presents significant anesthetic challenges, with complexity in airway management and perioperative respiratory care. CASE PRESENTATION: A 55-year-old man with SWS and OHS underwent elective dental extraction. He was advised to undergo continuous positive airway pressure (CPAP) therapy (10 cmH2O) preoperatively. Premedication included intramuscular clonidine (2 μg/kg). Awake fiberoptic intubation was performed under high-flow nasal oxygen (HFNO) support. Anesthesia was maintained with pro

[General Anesthesia Using Remifentanil for Cesarean Section in a Parturient with Marfan Syndrome Associated with Heart Failure due to Severe Mitral Regurgitation].

A 24-year-old woman with Marfan syndrome was scheduled for cesarean section in order to avoid progression of heart failure due to severe mitral regurgitation and aortic dissection during labor. Cesarean section was performed under general anesthesia using remifentanil. Anesthesia was induced and maintained with remifentanil (0.1-0.3 μg x kg(-1) x min(-1)) and continuous administration of propofol (target-controlled infusion, 2-3 ng x ml(-1)). The trachea was intubated without a significant hemodynamic change. The patient's systolic blood pressure was maintained between 90 and 120 mmHg during s

Review of remimazolam and sedatives in the intensive care unit.

Remimazolam is a novel intravenous ultra-short acting benzodiazepine that has the potential of being a safe and effective new sedative for use in intensive care unit (ICU) settings. Because remimazolam metabolizes rapidly by being hydrolyzed to an inactive metabolite (CNS 7054) through non-specific tissue esterase activity, specific dosing adjustment for older adults and for patients with renal or hepatic impairment patients (except for those with severe hepatic impairment) is not required. In addition, research has shown that remimazolam may be reversed by administration of flumazenil, as its

General Anesthesia for a Child With Sjögren-Larsson Syndrome.

This case report describes the dental treatment performed under general anesthesia for a boy with Sjögren-Larsson syndrome (SLS), a rare autosomal recessive genetic disorder. The main symptoms of SLS are congenital ichthyosis, limb paraplegia or tetraplegia, and intellectual disability. The main concerns regarding the administration of general anesthesia to patients with SLS include difficulty with adhesion of the electrocardiogram electrodes and intravenous (IV) catheter dressings due to ichthyosis and heat intolerance secondary to hypohidrosis. Although it was first described approximately 7

Fibreoptic nasal intubation in children with anticipated and unanticipated difficult intubation.

The establishment of a tracheal airway with direct laryngoscopy can be either a very difficult or an impossible task in children with congenital or acquired facial malformations. Out of 46 patients categorized as difficult tracheal intubation, fibreoptic laryngoscopy was used successfully in 44 children anaesthetized by mask with sevoflurane and oxygen or by an intravenous infusion of propofol and mask oxygenation. There were two failures (4.3%). One was due to excessive bleeding and secretions produced by the multiple attempts to intubate with direct laryngoscopy and the other failure in a pa

A physiology-guided ketofol-based anaesthetic approach for emergency burr hole craniotomy in a child with Eisenmenger syndrome: a case report.

BACKGROUND: Eisenmenger syndrome presents substantial perioperative risk because small perturbations in systemic or pulmonary vascular resistance may precipitate cardiovascular collapse. Evidence guiding anaesthetic management during emergency neurosurgery in paediatric patients remains limited. CASE PRESENTATION: We report an 11-year-old boy with Eisenmenger syndrome and severe pulmonary hypertension who underwent emergency burr hole craniotomy for drainage of a cerebral abscess. Perioperative management was guided by physiology-driven strategy encompassing invasive haemodynamic monitoring,

[Malignant hyperthermia syndrome in the intensive care unit : Differential diagnosis and acute measures].

Malignant hyperthermia is a life-threatening disease caused by derangement of the autonomic nerve system and hypermetabolism of the peripheral musculature. Commonly body core temperatures of more than 40 °C will be found in this disease which is caused mostly by psychopharmacological drugs like antidepressants, neuroleptics but also antibiotics, pain killers, anti-Parkinson drugs, and volatile anesthetics. The inducers of malignant hyperthermia interact with postsynaptic receptors (serotonin, anticholinergics) or muscular intracellular structures responsible for calcium utilization (volatile a

Neurologic Assessment of the Neurocritical Care Patient.

Sedation is a ubiquitous practice in ICUs and NCCUs. It has the benefit of reducing cerebral energy demands, but also precludes an accurate neurologic assessment. Because of this, sedation is intermittently stopped for the purposes of a neurologic assessment, which is termed a neurologic wake-up test (NWT). NWTs are considered to be the gold-standard in continued assessment of brain-injured patients under sedation. NWTs also produce an acute stress response that is accompanied by elevations in blood pressure, respiratory rate, heart rate, and ICP. Utilization of cerebral microdialysis and brai

Leigh syndrome: anesthetic management in complicated endoscopic procedures.

BACKGROUND: Leigh's syndrome, a disorder of infancy and childhood, is characterized by gray matter degeneration and focal brainstem necrosis. It presents with special clinical features such as developmental delay, nervous system dysfunction, respiratory abnormalities, and hypertrophic cardiomyopathy that can be a real challenge to the anesthesiologist. Anesthesia or sedation has rarely been reported in patients with Leigh disease. We report our experience in sedating five children with Leigh syndrome in seven procedures undertaken in the endoscopy suite (outside the operating room). METHODS:

Remimazolam-based anesthesia with intraoperative motor evoked potential monitoring in a patient with Leigh syndrome undergoing scoliosis surgery: a case report.

BACKGROUND: We present a case of scoliosis surgery performed under general anesthesia with remimazolam in a 16-year-old patient with Leigh syndrome (LS), a subtype of mitochondrial encephalomyopathy. Anesthetic management in such patients is challenging because of the risks of malignant hyperthermia with inhalational agents and propofol infusion syndrome, and because many of these patients present with impaired consciousness and respiratory compromise, anesthetic management becomes extremely difficult. To date, very few reports have described remimazolam use in LS, and none have described case

First report of malignant hyperthermia syndrome in an American Guinea hog.

A 3-year-old intact female American Guinea hog that was presented for an elective ovariohysterectomy appeared systemically healthy, with no overt medical conditions. Premedication with clonazepam, tiletamine-zolazepam, and xylazine; anesthetic induction with propofol and maintenance with isoflurane with oxygen on spontaneous ventilation; a ketamine infusion; and morphine for analgesia were provided. Approximately 95 min after isoflurane was initiated, the pig acutely became hyperthermic and severely hypercapnic, with hyperkalemia, hyperlactatemia, and severe metabolic and respiratory acidosis.

Seizures in 3 juvenile dogs after intravenous anesthetic drug withdrawal during weaning from mechanical ventilation suspected to be a sign of iatrogenic withdrawal syndrome.

OBJECTIVE: To describe seizure activity in juvenile dogs successfully weaned from long-term mechanical ventilation. CASE SERIES SUMMARY: Three juvenile dogs (all approximately 3 months old) underwent long-term mechanical ventilation with IV anesthesia for suspected noncardiogenic pulmonary edema. Within 24 hours of extubation and within 10 hours of discontinuing midazolam continuous infusions, all dogs experienced seizures, which is 1 sign of iatrogenic withdrawal syndrome. Each dog was treated with an anticonvulsant protocol, and none experienced seizures after being discharged. NEW OR UNIQ

Anesthetic management of a pediatric patient with Leigh syndrome.

BACKGROUND AND OBJECTIVES: Leigh syndrome (LS) is a rare disease caused by abnormalities of mitochondrial energy generation. The central nervous system is most frequently affected, with psychomotor underdevelopment, seizures, nystagmus, ophthalmoparesis, optic atrophy, ataxia, dystonia, or respiratory failure. Surgical and anesthetic procedures stimulate the tracheal irritability, and could exacerbate risks of aspiration, wheezing, breathing difficulties, gasping, hypoventilation, and apnea. CASE REPORT: We present the anesthetic management for a six-year-old boy with severe form of LS, invol

Iatrogenic withdrawal syndrome frequently occurs in paediatric intensive care without algorithm for tapering of analgosedation.

BACKGROUND: Analgesics and sedatives are key elements to reduce physiological and psychological stress associated with treatment in paediatric intensive care. Prolonged drug use may induce tolerance and development of iatrogenic withdrawal syndrome (IWS) during the tapering phase. Our primary aim was to describe the prevalence of IWS among critically ill ventilated patients in two Norwegian paediatric intensive care units (PICUs), and secondary to investigate what motivated bedside nurses to administer additional drug doses. METHODS: Mechanically ventilated patients (n = 40) from newborn to e
