Landscape of Allele-Specific Expression in Prostate Cancer Reveals Recurrent, Stage-Specific Events in AR Signaling and Resistance Pathways.

UNLABELLED: The effects of cis-regulatory alterations in prostate cancer are insufficiently characterized, presenting an opportunity to discover driver genes and therapeutic targets. To comprehensively study these effects, we identify genes undergoing allele-specific expression (ASE) in localized prostate cancer and metastatic castration-resistant prostate cancer (mCRPC) samples. By defining recurrent ASE events across prostate tissue and tumor-enriched ASE, we develop CASEDI, a computational framework for prioritizing cancer drivers by integrating ASE and clinical data. CASEDI reveals genes s

Quantum Dot Encoding for In-Solution Single-Molecule Biomarker Counting in Metastatic Prostate Cancer.

Digital assays are in wide development for biomarker quantification at the single-molecule level, but the common use of surface pull-down steps limits both analytical sensitivity and throughput. Here, we develop surface-free, wash-free, in-solution assays with an analytical sensitivity slope approaching unity for sequence-specific counting of microRNAs (miRs) relevant to metastatic castration-resistant prostate cancer (mCRPC). These assays are enabled by DNA nanoflowers (DNFs) densely encoded with ∼200 fluorescent quantum dots (QDs) that assemble in situ stoichiometrically to miRs. The QD-DNFs

Influence of concomitant distant lymph node metastases in metastatic hormone-sensitive prostate cancer patients with bone metastases.

INTRODUCTION: Most patients with metastatic hormone-sensitive prostate cancer (mHSPC) are burdened by bone metastases. However, the impact of concomitant distant lymph node metastases at diagnosis in these patients is unknown. MATERIALS AND METHODS: We relied on a single-center retrospective mHSPC patient cohort treated between 2014 and 2024 to compare time to metastatic castration-resistant prostate cancer (mCRPC) and overall survival (OS) between M1b vs. M1a + b mHSPC patients. Univariable and multivariable Cox regression models were applied. RESULTS: Among 432 patients, 64% harbored M1b vs.

Survival after spinal surgery for metastases in men with castration-sensitive vs castration-resistant prostate cancer: a nationwide register-based study.

This nationwide register-based cohort study examined the association between castration status and postoperative survival in men who had undergone surgery for spinal metastases from prostate cancer. Bone metastases are common in prostate cancer, with the spine being the most frequent site. Using data from the Swedish Spine Register (Swespine) and Prostate Cancer Database Sweden (PCBaSe), 306 men with prostate cancer who underwent spinal surgery were identified. In total, 81 were categorized as castration-sensitive and 225 as castration-resistant disease at the time of spinal surgery. Postopera

Bone health in advanced prostate cancer: pathophysiology and management strategies.

Bone loss and bone metastases can increase morbidity and impair quality of life for patients with prostate cancer. Use of hormonal agents can compromise bone integrity from treatment initiation, and the emergence of bone metastases further increases the risk of symptomatic skeletal-related events. Management of bone health should therefore be pivotal across the prostate cancer continuum. Current guidelines recommend that all men starting or receiving androgen-deprivation therapy for prostate cancer be assessed regularly for bone loss using predictive tools such as dual-energy X-ray absorptiome

Revisiting the role of metabolic reprogramming as a contributor to prostate cancer disease progression.

Prostate cancer (PCa) is one of the most common malignancies worldwide, and metabolic reprogramming plays a crucial role, particularly in tumor progression and therapeutic resistance. As PCa progresses into advanced stages, such as castration-resistant prostate cancer, significant alterations in tumor metabolic pathways, including glycolysis, amino acid utilization, and lipid acid metabolism, occur. These reprogrammed metabolic pathways support the survival and proliferation of tumor cells in altered tumor microenvironments. Glutamine metabolism is significant in advanced PCa because this path

Hormonal treatment and quality of life of prostate cancer patients: new evidence.

Androgen deprivation therapy (ADT) is the mainstay of treatment of patients with relapsed or metastatic hormone-sensitive prostatic carcinoma. The dramatic reduction of serum testosterone levels induced by ADT produces multiple side effects as vasomotor flushing, sexual dysfunction, fatigue, impairment of cognitive function, reduced quality of sleep, gynecomastia and anemia, that are able to decrease health-related quality of life (QoL). In addition, hormonal therapy can interfere with bone metabolism and induce metabolic and cardiovascular complications. Recently, new-generation hormonal ther

177 Lu-PSMA for prostate cancer: progress, challenges and future perspectives.

Prostate cancer is a malignant tumor posing significant threats to the health of men worldwide. In advanced stages, it frequently progresses to castration-resistant prostate cancer, which is characterized by complex mechanisms and poor prognosis. In such cases, conventional treatments have limited effectiveness. However, the emergence of radionuclide therapy has provided new hope. Lutetium-177-labeled prostate-specific membrane antigen ( 177 Lu-PSMA), currently the most promising radioligand in clinical research, received approval from the U.S. Food and Drug Administration (FDA) in 2022. HSD3B1, prostate cancer mortality and modifiable outcomes.

Androgen receptor stimulation by testosterone and dihydrotestosterone is crucial for prostate cancer progression. Despite the initial effectiveness of androgen deprivation therapy (ADT), castration-resistant prostate cancer eventually develops in most men. A common germline missense-encoding polymorphism in HSD3B1 increases extra-gonadal androgen biosynthesis from adrenal precursors owing to increased availability of the encoded enzyme 3β-hydroxysteroid dehydrogenase 1 (3βHSD1) - hence, it is called the adrenal-permissive enzyme. This mechanism explains the more rapid progression to castration

Convergent evolution of complex structural variants drives therapy resistance in metastatic prostate cancer.

BACKGROUND: Targeted therapy prolongs the lives of men with metastatic castration-resistant prostate cancer (mCRPC) but mCRPC is ultimately lethal. DNA copy gains that amplify the Androgen Receptor (AR) gene locus are a key driver of resistance to targeted therapy in mCRPC. Our group has recently shown that extra-chromosomal DNA (ecDNA) frequently drives this amplification. We hypothesized that ecDNA and other complex structural variants (cSVs) also affect other established drivers of therapy resistance in mCRPC and continue to evolve over time. To test this hypothesis, we reconstructed cSV pr

Prognostic value of baseline circulating tumor DNA levels in metastatic castration-resistant prostate cancer: a systematic review and meta-analysis.

BACKGROUND: Metastatic castration-resistant prostate cancer (mCRPC) remains a clinically aggressive and lethal disease. Circulating tumor DNA (ctDNA), as a minimally invasive biomarker, has shown prognostic utility in several solid tumors. However, its clinical relevance in mCRPC has not been comprehensively elucidated. METHODS: A systematic search of PubMed and EMBASE was conducted from inception to July 2025 to identify studies evaluating the prognostic impact of baseline ctDNA levels in patients with mCRPC. Eligible studies reported associations between ctDNA levels and survival outcomes.
