CORRELATION BETWEEN AVASCULAR NECROSIS AND EARLY STABILIZATION OF PROXIMAL FEMORAL FRACTURES IN CHILDHOOD.

OBJECTIVE: We developed this study with the aim of evaluating the results from treating patients with proximal femoral fractures, in a series of cases. We sought to observe the influence of the most prevalent complications on the final results after a minimum follow-up of two years. We especially considered the relationship between establishment of avascular necrosis and the time between the accident and the therapeutic intervention. METHOD: We retrospectively studied proximal extremity fractures of the femur in 29 patients under 14 years of age between 1988 and 2007. We analyzed the followin

Antibiotic prophylaxis for surgery for proximal femoral and other closed long bone fractures.

BACKGROUND: Surgical site infection and other hospital-acquired infections cause significant morbidity after internal fixation of fractures. The administration of antibiotics may reduce the frequency of infections. OBJECTIVES: To determine whether the prophylactic administration of antibiotics in people undergoing surgical management of hip or other closed long bone fractures reduces the incidence of surgical site and other hospital-acquired infections. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (December 2009), the Cochrane Central Reg

Functional and Radiological Outcomes of Ipsilateral Femoral Neck and Shaft Fractures Treated with Proximal Femoral Nail Antirotation-2.

BACKGROUND: Ipsilateral femoral neck and shaft fractures present a unique challenge due to their complex biomechanics and the need for optimal implant selection. Traditional dual-implant fixation has been widely used, but a single implant approach, such as the Proximal Femoral Nail Antirotation-2 (PFNA2), offers potential advantages, including reduced surgical time, minimized blood loss, and lower implant-related complications. However, the stability provided by PFNA2 and its impact on early functional recovery remain areas of ongoing investigation. MATERIALS AND METHODS: This prospective stu

Individual thigh muscle and proximal femoral features predict displacement in femoral neck Fractures: An AI-driven CT analysis.

INTRODUCTION: Hip fractures, particularly among the elderly, impose a significant public health burden due to increased morbidity and mortality. Femoral neck fractures, commonly resulting from low-energy falls, can lead to severe complications such as avascular necrosis, and often necessitate total hip arthroplasty. This study harnesses AI to enhance musculoskeletal assessments by performing automatic muscle segmentation on whole thigh CT scans and detailed cortical measurements using the StradView program. The primary aim is to improve the prediction and prevention of severe femoral neck frac

Positional difference in deep femoral artery during intramedullary nailing for proximal femoral fractures: a within-subject comparative study.

BACKGROUND: We aimed to evaluate the positional changes in the deep femoral artery (DFA) during intramedullary nail surgery. We hypothesized that the femur and DFA may be closer due to the medial compression of the thigh on a traction table during the surgery. METHODS: This within-subject comparative study included 20 patients with femoral trochanteric fracture (five males, 15 females; mean age: 83 ± 10.2) who underwent intramedullary nailing. Procedures were performed by five orthopedic surgeons. We performed computed tomography (CT) scans on the healthy thigh in the neutral and surgical lim

Mid-term outcomes after intramedullary fixation of peritrochanteric femoral fractures using the new proximal femoral nail antirotation (PFNA).

BACKGROUND: Controversy persists concerning the preferred treatment of peritrochanteric femoral fractures. The purpose of the present study was to evaluate the mid-term outcomes of the newly developed implants - proximal femoral nail antirotation (PFNA) for the stabilisation of this type of fracture. METHODS: Between April 2006 and March 2008, 169 patients with peritrochanteric femoral fractures were treated with PFNA. As many as 26 patients were excluded from the study. According to the Orthopaedic Trauma Association (OTA) classification system, the remaining 143 fractures were classified as

Numerical comparison of the intramedullary nail for the fixation of different proximal femoral fractures.

BACKGROUND: This study aimed to compare the mechanical responses, including the stability and implant stress, of different proximal femoral fractures stabilized with an intramedullary system by finite element simulation. Furthermore, the effect of number of the lag screws, including one and two screws, was also investigated. METHODS: A numerical hip model was created first, and then four different types of proximal femoral fractures-namely femoral neck, intertrochanteric, reverse intertrochanteric, and subtrochanteric fractures-were employed in this study. An intramedullary nail system was us

Giant Cell Tumor of the Proximal Femur with Pathological Fracture of Femoral neck.

INTRODUCTION: Giant cell tumor (GCT) of bone is a locally aggressive, benign neoplasm, accounting for approximately 20% of all bone tumors. While the distal femur and proximal tibia are the most common locations for GCTs, with the majority arising in the epiphyseal regions, their occurrence in the proximal femur is relatively rare, representing only 5.5% of cases. These tumors pose significant management challenges due to their tendency to cause pathological fractures, aggressive local behavior, and involvement of critical weight-bearing bones. Effective treatment requires careful consideratio

Remarks on the management of proximal femoral fractures in times of COVID-19 pandemic.

Proximal femoral fractures (PFFs) are among the main causes of hospitalization of elderly patients. They are often challenging to manage at the perioperative stage, which is why appropriate monitoring is recommended in order to prevent and - whenever possible - avoid tforeseeable complications (infections, bedsores, bronchopneumonia, psychicological deterioration and worsening general conditions), reduce the mortality rate, facilitate functional recovery and restore the patient's ability to walk and return to everyday life activities and a normal family context. PFFs constitute a major adverse

Proximal Femoral Fractures in the Elderly: A Few Things to Know, and Some to Forget.

Hip fractures are a leading cause of hospitalisation in elderly patients, representing an increasing socioeconomic problem arising from demographic changes, considering the increased number of elderly people in our countries. Adequate peri-operative treatment is essential to decrease mortality rates and avoid complications. Modern management should involve a coordinated multidisciplinary approach, early surgery, pain treatment, balanced fluid therapy, and prevention of delirium, to improve patients' functional and clinical outcomes. The operative treatment for intertrochanteric and subtrochant

Is There an Influence of Surgeon's Experience on the Clinical Course in Patients With a Proximal Femoral Fracture?

OBJECTIVES: Teaching of surgical procedures is of paramount importance. However, it can affect patients outcome. The aim of this study was to evaluate if teaching of hip fracture surgery is an independent predictor for negative in-hospital outcome. DESIGN AND SETTING: Retrospectively, we analyzed all hip fracture patients between 2008 and 2013 recorded in a national quality measurement database (AQC). Inclusion criteria were proximal femoral fracture (ICD-10 diagnostic codes S72.00-S72.11), surgical care of those fracture and a documented teaching status of the intervention. Variables were so

Catastrophic cement reaction following cementation for megaprosthesis for proximal femoral fracture.

Bone cement implantation syndrome (BCIS) is a well-described and potentially fatal complication of orthopaedic surgery involving pressurised bone cement. Although also described for certain spinal procedures, it is most commonly associated with cemented hip and knee arthroplasty and with cemented hemiarthroplasty following neck of femur fracture in particular.Donaldson et alproposed the definition of BCIS as a syndrome "characterized by hypoxia, hypotension or both and/or unexpected loss of consciousness occurring around the time of cementation, prosthesis insertion, reduction of the joint or,

Femoral fracture at the proximal end of an intramedullary supracondylar nail: a case report.

Retrograde intramedullary supracondylar nails have been added to the orthopedic armamentarium for treatment of distal femoral fractures. Major complications of this new technique have not been noted. We report a case of a femoral fracture at the proximal end of a retrograde intramedullary supracondylar nail. We hypothesize that this complication may be due to the proximal end of the nail acting as a stress riser, with the cortical holes drilled for the interlocking screws compounding this effect.

The association of age, race, and sex with the location of proximal femoral fractures in the elderly.

A retrospective study of the data on 27,370 hospital discharges of patients who had been admitted to non-federal Maryland hospitals from 1979 through 1988 for a fracture of the proximal part of the femur and who had been at least sixty-five years old at the time of the fracture showed that the ratio of trochanteric fractures to fractures of the femoral neck increased linearly with age in white and black women. For men, this ratio was stable across age-intervals, being slightly more than one in white men and less than one in black men. Black patients who had a fracture of the hip were more like

Salvage Total Hip Replacement for Failed Proximal Femoral Fracture Fixation.

INTRODUCTION: Salvage arthroplasty for failed proximal femoral fracture fixation is a complex procedure. This involves the removal of the primary failed or broken implant followed by a hip joint replacement procedure. The complications and technical difficulties associated with these surgeries are often difficult to anticipate. METHODS: Initially, to further understand the position in the literature with regard to salvage arthroplasty, we completed an informative scoping review. Search terms were selected, and databases Embase and PubMed were utilised to form a literature search. Relevant art
