Auditory evoked potential and psychiatry.

An adolescent was implied in a car accident in which he suffered a head trauma without any substantial loss of consciousness. The posttraumatic syndrome was characterized by headaches and mental symptoms: nervousness, feelings of depersonalization, impaired memory, difficulty in concentration. Neurological examination and laboratory tests have always been normal but for a large perduring asymmetry at the Cortical Auditory Evoked Response. This finding is commented in the light of a review of the literature and of some neuropsychological findings.

Periodic EEG discharges in psychiatry.

Periodic EEG discharges are signs of severe CNS dysfunction. In psychiatry they usually indicate presenile dementia of the Jacob-Creutzfeldt type. Psychiatric patients are presented in whom the occurrence and EEG features of diffuse and lateralized periodic discharges are discussed. It appears that toxic conditions other than acute barbiturate intoxication, such as drug withdrawal states and psychotropic drug overdosage, represent important etiological factors for inducing periodic EEG discharges. Their relation to clinical signs such as epileptic symptoms, metabolic dysfunction, and disturban

Homicide and community-based psychiatry.

The authors studied the records of all persons convicted of homicide or found not guilty by reason of insanity during the uears 1963 through 1975, 6 years before and after the opening of the community-based psychiatric facility, in Albany County, New York. Using the Fisher Exact Probability Test, a statistically significant increase was found for those found not guilty by reason of insanity (p = .041) and those with a history of mental illness found guilty (p = .036). Other factors, including age of the perpetrator, use of alcohol, and relationship to the victim, were also examined. The author

Therapeutic drug monitoring in psychiatry.

In medicine, there are two main methods of improving the healthcare provided: perfecting (optimizing) the existing ones and seeking new treatment procedures. Despite of tremendous development in the central nervous system research, current treatment of severe mental illnesses, such as schizophrenia and depressive disorder, is suboptimal. Nowadays, optimization of treatment in psychiatry includes therapeutic drug monitoring (TDM) and pharmacogenomic testing, which examines genetic variation involved in medication metabolism and drug action. The TDM enables to determine drug concentrations in bl

Psychiatry, Disaster, Security: Mediterranean Assemblages.

This article traces the development of a transnational psychiatric collaboration between psychiatrists and psychologists in Turkey and Israel in the wake of a massive earthquake that struck Turkey in 1999. Based on extended ethnographic fieldwork in sites across the earthquake region, the project on which this article is based is concerned with how the Turkish mental health professionals who responded to the earthquake struggled to improvise a therapeutic response that could address the scale of psychological suffering precipitated by the earthquake. This article considers the development of a

Modern-Day Relics of Psychiatry.

Constantly shifting cultural views influence public perceptions of psychiatric diagnoses, sometimes accommodated by changes in diagnostic terminology. Evolving scientific knowledge of the era is at times used to justify and support mental illnesses. Too often, however, remasked nomenclatures fail to alter social stigma, in part because political arguments are used. Scientific validations of variant behaviors as symptoms with a pathologic status are unfortunately overshadowed. Examples of cultural bias effects on recurring diagnostic challenges illustrate a need for scientific validation. Renam

Social challenges of contemporary psychiatry.

Psychiatry and society are interrelated and the biopsychosocial model continues to dominate the clinical psychiatric practice. Some doubts have been expressed in recent years about the value and the wide acceptance of the biopsychosocial model. Ghaemi (2009)1 considers it to be anti-humanistic and advocates the use of less eclectic, less generic, and less vague alternatives. The fundamental changes that have been witnessed in our times across the spectrum of biology, psychology and sociology have made necessary that a conceptual clarity should prevail. The remarkable advances in neurosciences,

[Disaster psychiatry in late life].

Disaster preparedness in geriatric psychiatry was proposed on the basis of experience of the Great East Japan Earthquake. 1) Frail or demented elderly should be considered as a special population at risk for disaster victims and addressed in local disaster prevention programs. 2) To response to various psychiatric symptoms(delirium, BPSD, depression, anxiety, insomnia, and posttraumatic stress disorder) caused by medical conditions and rapid environmental changes due to disaster, linkage and coordination systems between psychiatric and medical sections should be established. 3) As a medium- an

[Near-infrared spectroscopy in psychiatry].

Near-infrared spectroscopy (NIRS) is a functional neuroimaging technique that has been increasingly employed in psychology and psychiatry. Because NIRS can detect only cerebral cortex reactivities with low spatial resolution and may suffer from contaminating signals from the skin and skull, its data should be interpreted as a global index of cerebral cortex reactivities. Within these limitations, the advantages of NIRS over fMRI such as complete non-invasiveness, small measurement apparatus, high time resolution, and natural examination setting, makes it the preferred method in studies of brai

[Family work in geriatric psychiatry].

Dementia and other late-life conditions in neuropsychiatry impose a heavy burden on society as a whole and in families in particular. A number of clinical presentations involving family aspects may be seen in clinical practice in a range of diagnoses (Alzheimer's disease, other dementias, depression or any other psychiatric disorder in the elderly). As a consequence, there is a growing trend towards working with families in geriatric psychiatry. Marital issues are not a frequent target for specific clinical intervention, but couple therapy may be useful regardless of age. The effectiveness of

[Central noradrenergic system in psychiatry].

The function and dysfunction of the central noradrenergic system were reviewed together with the implications for the pathophysiological understanding of functional psychosis. Like the peripheral noradrenergic system, the central noradrenergic system plays a pivotal role in fight/flight reactions and stress. Overactivity of the system induces not only a sympathotonic state but also hyperarousal accompanied by insomnia, anxiety, irritability, emotional instability, paranoia, and excitation. On the other hand, its underactivity lowers the sympathetic tone and arousal level, resulting in hypersom

Postural induced-tremor in psychiatry.

Postural tremor is the most common movement disorder in psychiatry, and often a difficult problem for clinicians. It can be classified as physiological, essential, drug-induced, and postural tremor in Parkinson's disease. Drugs used in psychiatry that can produce postural tremor, include lithium, valproic acid, lamotrigine, antidepressants, and neuroleptics. Clinical characteristics of postural tremor induced by each of these drugs are described. Pharmacological strategies for therapy in disabling drug-induced tremor include beta-blockers, primidone, gabapentin, topiramate, and benzodiazepines

Paroxetine: current status in psychiatry.

Paroxetine is a selective serotonin reuptake inhibitor (SSRI) with antidepressant and anxiolytic properties. It is commercially available in both an immediate-release (paroxetine) and a controlled-release formulation (paroxetine CR). The latter product was developed to improve gastrointestinal tolerability. Paroxetine is the most potent inhibitor of the reuptake of serotonin among the available SSRIs. It has approved indications for the treatment of major depression, obsessive-compulsive disorder, panic disorder, generalized anxiety disorder, post-traumatic stress disorder and social phobia in
