OBJECTIVE: The relationship of the maxilla, with its thin walls, to the nasal and oral cavities, the orbit, and the infratemporal and pterygopalatine fossae makes it a suitable route for accessing lesions involving both the central and lateral cranial base. In this study, we compared the surgical anatomy and exposure obtained by two unilateral transmaxillary approaches, one directed through an upper subtotal maxillectomy, and the other through a lower subtotal maxillectomy.
METHODS: Cadaveric specimens examined, with 3 to 40x magnification, provided the material for this study.
RESULTS: Both

The anatomical and radiological study of the greater palatine canal allows to show that this canal constitutes a significant communication between the oral cavity and the pterygopalatine fossa. It contains vessels and nerve directly related with the pterygopalatine segment of the maxillary artery and nerve. The radiological and CT study of this canal allows a better understanding and accomplishment of surgical or anaesthetic treatments concerned with the posterior maxillary region.

The purpose of this article is to explain the anatomy of the pterygopalatine ganglion (PPG), its location in the pterygopalatine fossa (PPF) in the skull, and the relationship it has to the Vidian nerve terminal branches and the fifth cranial nerve. An overview of the neuro-anatomical/clinical correlations, a spectrum of pathologies affecting the seventh cranial nerve and some therapies both medical and surgical are noted. The focus is the pterygopalatine region with discussion of the proximal courses of the seventh and fifth cranial nerves and their pathological processes. The ganglion is use

The surgical management of trigeminal schwannomas (TNs) entails the use of a variety of cranial base approaches for their effective surgical management. Although an extended middle fossa or posterior petrosal approach may be more appropriate for disease with primarily posterior fossa involvement, the expanded endoscopic approaches are suited for tumors with predominately middle fossa and/or extracranial involvement along the V2 and V3 divisions and limited posterior fossa extension. The endoscopic endonasal resection of TNs within the middle fossa, pterygopalatine fossa, and infratemporal foss

The internal maxillary artery (IMAX) is a promising arterial pedicle to function as a donor vessel for extracranial-to-intracranial (EC-IC) bypass procedures. The access to the IMAX through the anterior portion of the middle cranial fossa floor allows a much shorter interposition graft to be used to create a bypass to the ipsilateral middle cerebral artery and prevents a second incision in the neck. One of the challenges of this technique, however, is the difficulty to find the IMAX through an intracranial approach. The purpose of this cadaveric study is to establish a reliable method to local

OBJECTIVE: Classic surgical exposures of the infratemporal fossa region, including the adjacent intracranial space, temporal bone, and sinonasal region, require the extensive exposure associated with the transcranial, transfacial, and transmandibular approaches with their inherent neurological and cosmetic morbidities. In this study, we evaluated the feasibility and exposure afforded by combining 2 endoscopic transmaxillary approaches, endonasal and Caldwell-Luc supplement, to the infratemporal fossa.
METHODS: Endoscopic transmaxillary dissection was performed in 4 formalin-fixed cadaver head

The extracranial-intracranial (EC-IC) bypass using the maxillary artery (MA) has been successfully completed using a radial artery (RA) graft but the complicated anatomy and narrow exposure make it difficult. The purpose of this article is to define the microsurgical exposure of the MA through the middle fossa and describe the branches, diameter, and length of the MA available for the EC-IC bypass in the sphenopalatine fossa and anterior part of the infratemporal fossa. 5 cadaveric specimens were dissected bilaterally (10 MA dissections) to define the microsurgical anatomy of the MA through an

We present a case highlighting the critical role of early recognition of partial cavernous sinus syndrome (CSS), as diagnostic delay can lead to irreversible tumor progression. A 50-year-old woman presented with subtle binocular diplopia and facial tingling sensation for 1 month. Neurological examination revealed isolated right abducens nerve (VI) palsy and paresthesia at territories of right ophthalmic (V1) and maxillary (V2) nerves. Partial CSS was suspected. Brain magnetic resonance image (MRI) revealed a contrast-enhancing mass involving the pterygopalatine fossa, sphenoid, and maxillary s

A diabetic woman in her 40s presented with a subacute onset of a left temporal headache with no other symptoms, especially no visual or nasal symptoms. An initial diagnosis of giant cell arteritis was made due to a raised erythrocyte sedimentation rate (ESR) and the presence of jaw claudication; however, the temporal artery biopsy was negative. Corticosteroid therapy was commenced, but subsequent computed tomography (CT) imaging of the face revealed extensive left maxillary sinus disease with bony erosion of the greater palatine and vidian nerve canals. The ear, nose, and throat (ENT) examinat

The palatovaginal or pharyngeal artery is a small branch of the internal maxillary artery supplying the nasopharynx. Bleeding from this artery is exceptional and only one case of traumatic epistaxis from this artery has been reported previously. We report a case of a 66-year-old male presenting with right recurrent posterior epistaxis. Endoscopic dissection of the pterygopalatine fossa and direct visualization of the palatosphenoidal canal permitted to identify the origin of bleeding, and coagulation of the pharyngeal artery solved the epistaxis. Although rare, intractable posterior epistaxis

INTRODUCTION: The maxillary swing approach is a very efficient surgical procedure for treating advanced-stage juvenile nasopharyngeal angiofibroma (JNA). It allows wide access to the pterygopalatine fossa, infratemporal fossa, and skull base, allowing for the entire removal of malignancies with considerable local extension.
MATERIALS AND METHODS: The purpose of this study was to assess the efficacy and outcomes of the maxillary swing technique for the treatment of extensive nasopharyngeal angiofibroma. An observational study on the presenting features, surgical technique, outcomes, and compli

Meningoencephalocele is an uncommon condition in which brain tissue, meninges, or both protrude through a defect in the anterior cranial fossa and into the ethmoid sinus or nasal cavity. Much less often, brain tissue, meninges, or both protrude through a defect in the middle cranial fossa and into the sphenoid sinus. We report an unusual case of a middle fossa encephalocele that appeared as a lytic lesion of the skull base. The patient was treated successfully via a unique endoscopic transpterygoid approach--that is, an endoscopic approach through the maxillary sinus and pterygopalatine fossa

BACKGROUND: Juvenile nasopharyngeal angiofibroma often presents with lateral extensions. In countries with limited resources, selection of a cost-effective and least morbid surgical approach for complete excision is challenging.
METHODS: Sixty-three patients with juvenile nasopharyngeal angiofibroma, with lateral extensions, underwent transpalatal, transpalatal-circumaxillary (transpterygopalatine) or transpalatal-circumaxillary-sublabial approaches for resection. Clinico-radiological characteristics, tumour volume and intra-operative bleeding were recorded.
RESULTS: The transpalatal approac

PURPOSE: To develop and evaluate a transorbital endoscopic approach to the foramen rotundum to excise the maxillary nerve and infraorbital nerve branch.
METHODS: Cadaveric dissection study of 10 cadaver heads (20 orbits). This technique is predicated upon 1) an inferior orbital fissure release to facilitate access to the orbital apex and 2) the removal of the posterior maxillary wall to enter the pterygopalatine fossa (PPF). Angulations along the infraorbital nerve were quantified as follows: the first angulation was measured between the orbitomaxillary segment within the orbital floor and th

Inflammatory myofibroblastic tumour (IMT) is a rare distinctive neoplasm of intermediate biological potential with a predilection for the abdominopelvic region and lung of children and young adults. It is histologically composed of spindle cells (myofibroblasts) in a myxoid to collagenous stroma with a prominent inflammatory infiltrate composed primarily of plasma cells and lymphocytes. Its pathogenesis is controversial. Arising most commonly in the lungs, only two cases of histopathologically confirmed IMT originating from the infratemporal and pterygopalatine fossae have been documented in t

Endoscopic endonasal skull base surgery is increasingly prevalent, with its scope expanding from pathogens in the midline region to those in the paramedian region. Maximizing anterior sphenoidectomy is important for the median approach, and lateralizing the pterygopalatine fossa is crucial for the paramedian approach. Maximizing the surgical corridor in the nasal cavity and minimizing damage to neurovascular structures are vital for establishing a surgical field with minimal bleeding, ensuring safe, precise, and gentle procedures. However, the relationship between the maxillofacial and skull b
