Chronic nitrofurantoin reaction associated with T-lymphocyte alveolitis.

Nitrofurantoin use was associated with the development of severe interstitial lung disease which was characterized by bronchoalveolar lavage with a marked increase in lymphocytes to 60 percent (compared to normal of 7 +/- 1 percent). Lymphocytes were predominantly of the T-helper subset. Lavage may prove useful in the assessment of the immune response in drug-induced lung disease.

Fibrosis of two: Epithelial cell-fibroblast interactions in pulmonary fibrosis.

Idiopathic pulmonary fibrosis (IPF) is characterized by the progressive and ultimately fatal accumulation of fibroblasts and extracellular matrix in the lung that distorts its architecture and compromises its function. IPF is now thought to result from wound-healing processes that, although initiated to protect the host from injurious environmental stimuli, lead to pathological fibrosis due to these processes becoming aberrant or over-exuberant. Although the environmental stimuli that trigger IPF remain to be identified, recent evidence suggests that they initially injure the alveolar epitheli

Morphologic changes and methodological issues in the rabbit experimental model for diaphragmatic hernia.

Fetal lung development may be impaired by some congenital anomalies or in utero events. Animal models are used to understand the pathophysiology of these diseases and explore therapeutic strategies. Our group has an interest in the prenatal management of congenital diaphragmatic hernia (CDH). Isolated CDH remains associated with a 30% mortality because of lung hypoplasia and pulmonary hypertension. On day 23 of gestation (pseudoglandular stage) CDH was created in both ovarian-end fetuses (n= 28) in 14 time-mated pregnant white rabbits (hybrid of Dendermonde and New-Zealand White). At term (day

Persistent pulmonary hypertension of newborn due to congenital capillary alveolar dysplasia.

Congenital alveolar capillary dysplasia is a rare and fatal cause of pulmonary hypertension in neonates. We report on a term baby with severe pulmonary hypertension unresponsive to high-frequency ventilation and nitric oxide. A diagnosis of alveolar capillary dysplasia was established on autopsy. We review the literature regarding pathophysiology, clinical presentations, associated malformations, and treatment trials.

Alveolar surfactant and adult respiratory distress syndrome. Pathogenetic role and therapeutic prospects.

The adult respiratory distress syndrome (ARDS) is characterized by extended inflammatory processes in the lung microvascular, interstitial, and alveolar compartments, resulting in vasomotor disturbances, plasma leakage, cell injury, and complex gas exchange disturbances. Abnormalities in the alveolar surfactant system have long been implicated in the pathogenetic sequelae of this life-threatening syndrome. This hypothesis is supported by similarities in pulmonary failure between patients with ARDS and preterm babies with infant respiratory distress syndrome, known to be triggered primarily by

Pulmonary intravascular macrophages in rabbits experimentally infected with rabbit haemorrhagic disease.

Attention has already been drawn to the presence of pulmonary intravascular macrophages (PIMs) in species in which the lung plays an important part in the removal of particles from the bloodstream. The stimulation of PIMs under certain conditions has been related to pulmonary oedema and congestion. In order to ascertain whether these cells are found in rabbits, healthy animals were inoculated with a liver homogenate obtained from animals which had died of rabbit haemorrhagic disease, a condition characterized by vascular phenomena and pulmonary oedema. PIMs were detected both in experimental a

Presence of phospholipidic lamellar bodies on the mucosa of rabbit eustachian tube. Ultrastructural aspects.

Electron microscopy of the rabbit eustachian tube showed the presence of osmiophilic lamellar bodies in the cytoplasm of secretory cells and of osmiophilic free lamellar membranes in the lumen of tubal glands or above the epithelium. Similar structures have been reported in other parts of the respiratory system and in the pulmonary alveoli. The functional significance of these morphological observations is suggestive of a surface-tension-lowering effect on the mucosal lining layer of the eustachian tube and of a possible relationship to the phospholipid surfactant of the lung.

[Small pulmonary nodular opacities in inflammatory and granulomatous lung disease (author's transl)].

Small pulmonary nodular opacities are usually visible only due to the summation of several small foci. The size of a single focus and its number per area determine their radiographic appearance either that of small nodular lesions, or of a reticular pattern or of diffus opacification. The small nodular pattern is primarily observed in lung disease beginning in the interstitium rather than the alveoli. Pattern and course of small nodular lung lesions give important differential diagnostic clues.

The Role of GM-CSF Autoantibodies in Infection and Autoimmune Pulmonary Alveolar Proteinosis: A Concise Review.

Autoantibodies to multiple cytokines have been identified and some, including antibodies against granulocyte-macrophage colony-stimulating factor (GM-CSF), have been associated with increased susceptibility to infection. High levels of GM-CSF autoantibodies that neutralize signaling cause autoimmune pulmonary alveolar proteinosis (aPAP), an ultrarare autoimmune disease characterized by accumulation of excess surfactant in the alveoli, leading to pulmonary insufficiency. Defective GM-CSF signaling leads to functional deficits in multiple cell types, including macrophages and neutrophils, with i

Pulmonary hamartoma in an elk calf.

Hamartomas are benign tumor-like lesions composed of disorganized growth of mature mesenchymal or epithelial tissues indigenous to the organ involved. Sporadically observed in ruminants, vascular, fibrous, nasal, and pulmonary hamartomas have been reported in calves; pulmonary and cutaneous forms have been reported in sheep. A full-term elk calf found dead had a large intrathoracic mass replacing the left caudal lung lobe and compressing other thoracic organs. Histologically, cross- and tangential sections of bronchi were separated by collagenous mesenchyme and irregularly shaped canaliculi an

Eosinophilic pneumonia associated with azacitidine in a patient with myelodysplastic syndrome.

Eosinophilic pneumonia is characterized by cough, lung infiltrates on imaging, and by the presence of eosinophils in the alveoli and pulmonary interstitium. Azacitidine, a pyramidine nucleoside analog of cytidine, is FDA approved for the treatment of various myelodysplastic syndromes. We present a case of a 76-year-old man with recently diagnosed myelodysplastic syndrome, who developed eosinophilic pneumonia after initiating therapy with azacitidine. There was clinical and radiographic improvement with cessation of the drug and treatment with prednisone. Diagnosis of drug-induced eosinophilic

Cardiopulmonary consequences of congenital diaphragmatic hernia.

The cardiopulmonary consequences of CDH can be devastating, and despite intensive research the pathophysiologic makeup of this condition is not fully understood. It seems likely that its overall prognosis is related to the size of the pulmonary vascular bed and its capacity to accept the cardiac output at birth. The ability of the lungs to perform this task is impaired by a surfactant deficiency and by pulmonary vessels that respond excessively to normal stimuli of vasoconstriction. Understanding the mechanisms through which these pathophysiological abnormalities are occurring should provide u

Tissue remodelling in pulmonary fibrosis.

Many lung diseases result in fibrotic remodelling. Fibrotic lung disorders can be divided into diseases with known and unknown aetiology. Among those with unknown aetiology, idiopathic pulmonary fibrosis (IPF) is a common diagnosis. Because of its progressive character leading to a rapid decline in lung function, it is a fatal disease with poor prognosis and limited therapeutic options. Thus, IPF has motivated many studies in the last few decades in order to increase our mechanistic understanding of the pathogenesis of the disease. The current concept suggests an ongoing injury of the alveolar

Whole lung lavage: Treating pulmonary alveolar proteinosis at the time of COVID pandemic.

Pulmonary alveolar proteinosis (PAP) is a rare syndrome due to increased production or decreased clearance of surfactant in alveoli and terminal bronchi that cause hypoxemic respiratory insufficiency. Here we present a patient with PAP whose disease was exacerbated by superimposed COVID-19 pneumonia. He underwent whole lung lavage (WLL). Evaluation of the viral count of the first and the last lavage of the left lung showed viral load in the alveolar space dropped by approximately 10-folds, however the magnitude of the viral load was substantial in both lavage samples. Whole pulmonary lavage ma

Immersion pulmonary oedema: a cardiological perspective.

It is postulated that immersion pulmonary oedema (IPE) occurs because of combinations of factors that each increase the hydrostatic pressure gradient between the pulmonary capillaries and the alveoli. The factors, by definition, include the effects of immersion, particularly raised central blood volume and hence cardiac filling pressures. Breathing against a negative pressure is important but the magnitude of the effect depends on the relation of the diver's lung centroid to the source of the breathing gas and the breathing characteristics of diving equipment. Other factors are cold-induced va

The mouse as a model for pulmonary legionella infection.

Legionella pneumophila is an intracellular bacterium that was evolutionarily selected to survive in freshwater environments by infecting free-living unicellular protozoa. Once humans inhale contaminated water droplets, the bacteria reach the pulmonary alveoli where they are phagocytized by resident alveolar macrophages. Depending on host immunity and bacterial virulence genes, the infection may progress to an acute pneumonia called Legionnaires' disease, which can be fatal. Of note, an effective immune response is critical to the outcome of the human infection. These clinical observations high
