Eye movement abnormalities in middle cerebellar peduncle strokes.

The middle cerebellar peduncle (MCP) is a major conduit for cortico-ponto-cerebellar fibers that convey information related to eye movements. This study aims to elucidate eye movement abnormalities that arise from lesions confined to the MCP. In 23 patients with acute strokes restricted to unilateral MCPs, we investigated the clinical features and ocular motor findings including spontaneous nystagmus, saccades, smooth pursuit, ocular tilt reaction, and head impulse tests. Bithermal caloric tests and audiometry were also performed. Patients with strokes restricted to the MCP usually developed a

Eye movements in Parkinson's disease: from neurophysiological mechanisms to diagnostic tools.

Movement disorders such as Parkinson's disease (PD) impact oculomotor function - the ability to move the eyes accurately and purposefully to serve a multitude of sensory, cognitive, and secondary motor tasks. Decades of neurophysiological research in monkeys and behavioral studies in humans have characterized the neural basis of healthy oculomotor control. This review links eye movement abnormalities in persons living with PD to the underlying neurophysiological mechanisms and pathways. Building on this foundation, we highlight recent progress in using eye movements to gauge symptom severity,

Neural implementation of Bayesian inference in a sensorimotor behavior.

Actions are guided by a Bayesian-like interaction between priors based on experience and current sensory evidence. Here we unveil a complete neural implementation of Bayesian-like behavior, including adaptation of a prior. We recorded the spiking of single neurons in the smooth eye-movement region of the frontal eye fields (FEFSEM), a region that is causally involved in smooth-pursuit eye movements. Monkeys tracked moving targets in contexts that set different priors for target speed. Before the onset of target motion, preparatory activity encodes and adapts in parallel with the behavioral ada

Involvement of the supplementary eye field in oculomotor predictive behavior.

The presentation of saccadic and smooth pursuit eye movements as two separate systems has recently been reconsidered: The two subsystems share a number of anatomical structures, and recent data suggest that this sharing also extends to physiological processes. The aim of our study was first to test whether these two subsystems share a common predictive process. We designed a new predictive smooth pursuit paradigm that requires the triggering of unpredictable saccades, performed either during low (ongoing pursuit) or high (pursuit direction reversal) predictive behavior. Saccade latency was use

Validity and limitation of manual rotational test to detect impaired visual-vestibular interaction due to cerebellar disorders.

The aim of this study was to investigate validity and limitation of the novel infrared system to record and analyze horizontal visual-vestibular interaction using whole-body rotation rapidly and conveniently in the routine vestibular clinic. We examined 11 patients with cerebellar dysequilibrium and 25 patients with peripheral dysequilibrium for vestibulo-ocular reflex in darkness (DVOR), visually-enhanced vestibulo-ocular reflex (VEVOR), and fixation suppression of vestibulo-ocular reflex (FSVOR), and compared the results with those of examination for head-fixed smooth pursuit and fixation su

Slow saccades in bulbar-onset motor neurone disease.

Historical studies of eye movements in motor neurone disease (MND) have been conflicting although current findings suggest that eye movement abnormalities relate to frontal lobe impairment. Numerous case reports, however, describe slow saccades and supranuclear gaze palsies in patients with MND often associated with bulbar-onset disease. We performed a study of saccades and smooth pursuit in a large group of patients with MND to examine for any differences between bulbar-onset and spinal-onset patients. Forty-four patients (14 bulbar-onset and 30 spinal-onset patients) and 45 controls were rec

Effects of diphenhydramine on human eye movements.

Peak saccadic eye movement velocity (SEV) and average smooth pursuit gain (SP) are reduced in a dose-dependent manner by diazepam and provide reliable, quantitative measures of benzodiazepine agonist effects. To evaluate the specificity of these eye movement effects for agents acting at the central GABA-benzodiazepine receptor complex and the role of sedation in benzodiazepine effects, we studied eye movement effects of diphenhydramine, a sedating drug which does not act at the GABA-benzodiazepine receptor complex. Ten healthy males, aged 19-28 years, with no history of axis I psychiatric diso

Ocular motility testing in the evaluation of visual hallucinations.

Distinguishing organic visual hallucinations from those caused by hysteria or malingering is important but difficult. We found that ocular motility testing is helpful in making the distinction in some patients who allege that their hallucinations move. A 9-year-old girl thought to have organic hallucinations, possibly caused by an occipital lobe neoplasm, was unable to generate smooth pursuit movements when she attempted to follow the movements of the hallucinations. Instead, she executed a series of small downward saccades. Her hallucinations were presumably stress-induced. Conversely, a 62-y

Milia en plaque: a novel manifestation of chronic cutaneous lupus erythematosus.

BACKGROUND: Prior studies of oculomotor function in Parkinson's disease (PD) have either focused on saccades without considering smooth pursuit, or tested smooth pursuit while excluding saccades. The present study investigated the control of saccadic eye movements during pursuit tasksand assessed the quality of binocular coordinationas potential sensitive markers of PD. METHODS: Observers fixated on a central cross while a target moved toward it. Once the target reached the fixation cross, observers began to pursue the moving target. To further investigate binocular coordination, the moving t

An eye movement disorder in amyotrophic lateral sclerosis.

Defective pursuit eye movements were recorded by electrooculography (EOG) in 11 of 18 patients (61%) with amyotrophic lateral sclerosis. Pursuit defects consisted of a breakdown of smooth tracking into saccadic motions that were grossly in excess (frequencies and amplitudes) of saccadic interruptions of pursuit in normal subjects. In nine patients, defective pursuits cogwheeling) were obvious by visual inspection as well as by EOG; in two, this abnormality was seen only by EOG. In eight patients, the pursuit defect was the only abnormality of oculomotor function; in three, there were also sacc

Oculographic analysis of acute esotropia secondary to a thalamic hemorrhage.

We used electrooculography to study the saccadic velocities, smooth pursuit, and vestibular ocular reflex in a patient with an acute thalamic hemorrhage. Our findings confirmed what others have shown in that there were hypometric saccades contralateral to the lesion, impaired smooth pursuit ipsilaterally, and a preserved vestibular ocular reflex. In addition, we demonstrated an asymmetry with the contralateral eye being more affected. It is also shown that the "convergence" movements seen on attempted upgaze are typical of saccades and not vergence movements. A discussion of possible pathophys

Supranuclear disorders of eye movement.

Eye movements are the result of activity in discrete systems. Saccades and smooth pursuit movements respond to target position and target velocity respectively. Compensatory responses to movements that alter the position of the eyes in space depend upon the vestibular and optokinetic systems. Vergence movements allow fusion and binocular vision. Eye movement abnormalities of supranuclear origin may be divided into nystagmus, other ocular oscillations, saccadic dysfunction, smooth pursuit dysfunction, gaze palsy, tonic deviation, and vergence disability. An algorithm for the analysis of supranu

The relationship between disordered pursuit and vestibulo-ocular reflex suppression.

The performance of the smooth pursuit reflex and the ability to suppress the vestibulo-ocular reflex were assessed in 10 normal subjects and in patients with a variety of diseases of the central nervous system. Pursuit was measured as the maximum velocity of slow phase eye movement in response to a laser target moving sinusoidally at various frequencies up to 1 Hz and with amplitudes stepped up to 35° peak. Suppression of the vestibulo-ocular reflex was assessed with subjects seated in a Barany chair rotating sinusoidally in yaw at matching frequencies. The breakpoint of vestibulo-ocular refle

Oculomotor Findings in Videonystagmography across the Lifespan.

BACKGROUND: When performing oculomotor testing during standard videonystagmography (VNG), the vestibular system is not actively stimulated. Therefore, responses are generated from the cerebellum, brainstem, and oculomotor tract. Many patients seen for vestibular testing fall outside of the standard age norms, making it difficult to determine whether an abnormal finding is due to age or oculomotor dysfunction. PURPOSE: The purpose of this study was to further evaluate the effect of age on a standard clinical VNG oculomotor test battery consisting of saccades, smooth pursuit, and optokinetic (O

Object recognition during foveating eye movements.

We studied how saccadic and smooth pursuit eye movements affect the recognition of briefly presented letters appearing within the eye movement target. First we compared the recognition performance during steady-state pursuit and during fixation. Single letters were presented for seven different durations ranging from 10 to 400 ms and four contrast levels ranging from 5% to 40%. For both types of eye movements the recognition rates increased with duration and contrast, but they were on average 11% lower during pursuit. In daily life humans use a combination of saccadic and smooth pursuit eye mo

Cerebral ocular motor signs.

Eye movement disturbances resulting from cerebral lesions are reviewed and the specific roles of the different ocular motor areas are summarized. Three cortical areas may trigger saccades: the frontal eye field (FEF), the supplementary eye field (SEF) and the parietal eye field (PEF). The FEF could be involved mainly in intentional visual exploration (intentional saccades), the PEF mainly in reflexive visual exploration (reflexive saccades) and the SEF in the preparation of motor programs (sequences of saccades). Only bilateral lesions affecting these areas result in visible saccade disturbanc

Specific motion processing pathway deficit during eye tracking in schizophrenia: a performance-matched functional magnetic resonance imaging study.

BACKGROUND: The neural mechanisms underlying smooth pursuit eye movement (SPEM) abnormalities in schizophrenia are not well understood. Previous evidence suggests that a deficit in the processing of internal representations of object motion (extraretinal motion) contributes to SPEM deficits in patients. Functional magnetic resonance imaging (fMRI) activation was compared between patients and control subjects to determine whether schizophrenia patients exhibit abnormal cerebral activation in regions associated with extraretinal motion processing during SPEM. METHODS: Patients and control subje

Progressive bradykinesia and hypokinesia of ocular pursuit in Parkinson's disease.

OBJECTIVES: Patients with Parkinson's disease characteristically have difficulty in sustaining repetitive motor actions. The purpose of this study was to establish if parkinsonian difficulty with sustaining repetitive limb movements also applies to smooth ocular pursuit and to identify any pursuit abnormalities characteristic of Parkinson's disease. METHODS: Ocular pursuit in seven patients with moderate to severe bradykinesia predominant Parkinson's disease was compared with seven age matched controls. Predictive and non-predictive pursuit of constant velocity target ramps were examined. Sub

Frontal-to-visual information flow explains predictive motion tracking.

Predictive tracking demonstrates our ability to maintain a line of vision on moving objects even when they temporarily disappear. Models of smooth pursuit eye movements posit that our brain achieves this ability by directly streamlining motor programming from continuously updated sensory motion information. To test this hypothesis, we obtained sensory motion representation from multivariate electroencephalogram activity while human participants covertly tracked a temporarily occluded moving stimulus with their eyes remaining stationary at the fixation point. The sensory motion representation o

Effects of age and sex on eye movement characteristics.

Abnormal eye movements are often associated with psychiatric disorders. Eye movements are sensorimotor functions of the brain, and aging and sex would affect their characteristics. A precise understanding of normal eye movements is required to distinguish disease-related abnormalities from natural differences associated with aging or sex. To date, there is no multicohort study examining age-related dependency and sex effects of eye movements in healthy, normal individuals using large samples to ensure the robustness and reproducibility of the results. In this study, we aimed to provide finding
