Xanthogranulomatous pyelonephritis with pancreatic infiltration: A rare case report.

Xanthogranulomatous pyelonephritis (XGP) is a rare chronic infection of the kidney, occasionally extending to adjacent organs. We report a 45-year-old woman with recurrent renal colic and left flank pain. Imaging showed a "bear paw" kidney with staghorn calculi and infiltration of the pancreatic tail. After antibiotic therapy, en bloc nephrectomy with splenopancreatectomy was performed. Histology confirmed XGP extending into the pancreas. Recovery was uneventful. Pancreatic involvement in XGP is extremely rare and may mimic malignancy; recognizing characteristic imaging features is crucial for

Xanthogranulomatous Pyelonephritis: A pooled quantitative analysis of published cases.

Xanthogranulomatous Pyelonephritis (XGP) is a serious and rare inflammatory disease of unknown etiology. This systematic review analyzes XGP cases. We performed a literature search for "Pyelonephritis, Xanthogranulomatous." The primary composite outcome was recovery with post-surgery complications, partial recovery, death, or chronic kidney disease. The secondary outcome was any presentation or treatment complication. Predictor variables consisted of demographics, history, symptoms, and diagnosis/management. Among the 251 patients, the mean age was 36.1 years, and 57.4% were female. The most c

Severe Urosepsis Secondary to Xanthogranulomatous Pyelonephritis: A Case Report.

Xanthogranulomatous pyelonephritis (XGP) is considered to be a rare variant of chronic pyelonephritis, which results in non-functioning kidneys in patients. The exact etiology of this disease is still unknown, and hence even its pathophysiology remains unclear. We present a case of a 27-year-old Saudi male patient who had been bed-bound with a known case of a congenital anomaly with severe kyphoscoliosis, bilateral lower limbs deformity with paraplegia, and a ventriculoperitoneal shunt since childhood. The patient was brought to the emergency department (ED) with right flank pain associated wi

Extensive renal sinus lipomatosis in xanthogranulomatous pyelonephritis simulating liposarcoma.

Renal replacement lipomatosis is a condition characterized by varying degrees of renal parenchymal atrophy and perirenal fibrofatty proliferation secondary to chronic inflammation such as xanthogranulomatous pyelonephritis. In severe cases, imaging findings can be misdiagnosed as retroperitoneal liposarcoma.

Xanthogranulomatous pyelonephritis and renal tubulopapillary adenomas: A rare coexistence.

Xanthogranulomatous pyelonephritis is an uncommon inflammatory condition accounting for 1% of chronic pyelonephritis cases. Clinically and radiologically it mimics other renal space occupying lesions. Hence, correct preoperative diagnosis is not possible in all cases and nephrectomy is done in most patients. Renal tubulopapillary adenomas are benign epithelial lesions of kidney found to be associated with papillary renal cell carcinoma, acquired renal cystic disease, long term hemodialysis, arteriosclerotic renal vascular disease, etc. Here, we report two cases of Xanthogranulomatous pyeloneph

Pyelonephritis xanthogranulomatous in childhood: case report and literature review.

BACKGROUND: Xanthogranulomatous pyelonephritis (XGP) is a specific form of chronic inflammatory kidney disease rarely seen in children. The Symptoms are often vague and non-specific AIM: the aim of this paper is to return the particularities of imaging features in xanthogranulomatous pyelonephritis, insisting on differential diagnosis with renal tumors, especially in case of no renal stone or tract obstruction evidence. CASE: We report a case of xanthogranulomatous pyelonephritis in a 2-year-old boy involving the lower renal pole which demonstrates the diagnostic difficulties encountered in

Renal parenchymal malacoplakia: a different stage of xanthogranulomatous pyelonephritis?

Malacoplakia is a rare inflammatory condition characterized by demonstrative Michaelis-Gutmann bodies, which are foamy histiocytes with distinctive basophilic inclusions. Malacoplakia is caused by the inadequate elimination of bacteria by macrophages or monocytes as a result of defective phagocytic activity. Xanthogranulomatous pyelonephritis is characterized by the destruction of renal parenchyma and its replacement by solid sheets of foamy lipid-laden macrophages. Prolonged infection of the kidney, which is frequently caused by an obstruction of the urinary tract, is the pathologic mechanism

Pediatric focal xanthogranulomatous pyelonephritis: dynamic contrast-enhanced MRI findings.

Xanthogranulomatous pyelonephritis (XPN) is the result of chronic renal infection. It is very rare in childhood and the focal form, which is said to be more common in children, is often misdiagnosed as a renal tumor. We report a case of a focal XPN in a 14-year-old girl. The true preoperative diagnosis may be very hard in children especially in the focal form but it seems to be possible by the help of dynamic contrast-enhanced MRI.

[Renal stone ileus in xanthogranulomatous pyelonephritis. A case report].

A peculiar case of intestinal occlusion caused by a renal stone in a patient with nephroduodenal fistula due to previous xanthogranulomatous pyelonephritis is reported. Only few cases of nephroduodenal fistula are described in the literature, generally as a single case report or in small series. A nephroduodenal fistula as a result of chronic renal inflammatory disease such as xanthogranulomatous pyelonephritis, is usually associated with renal stones, recurrent urinary tract infections or endocrine disorders. Finally, renal stone as a cause of ileus is an event rarely described in the literat

Anaerobic bacterial infection and xanthogranulomatous pyelonephritis: a case report.

A young man presented with a history of uncorrected ureteropelvic junction obstruction 18 months in duration and clinical acute pyelonephritis. Retrograde pyelography showed unilateral pyonephrosis and cultures of purulent drainage proximal to the ureteropelvic junction revealed predominantly anaerobic bacteria and no aerobic gram-negative bacilli. Subsequent nephrectomy established a diagnosis of xanthogranulomatous pyelonephritis. Nephrectomy in combination with intensive antimicrobial therapy resulted in recovery. The contribution of anaerobic bacteria in the pathogenesis of xanthogranuloma

Xanthogranulomatous pyelonephritis mimicking renal cell carcinoma: a case report.

UNLABELLED: Xanthogranulomatous pyelonephritis (XGPN) is a rare, serious, and chronic inflammatory disorder of the kidney characterized by a destructive process that invades the renal parenchyma, which is most commonly associated with urinary tract obstruction and infection. It affects women more often than men. CASE PRESENTATION: Herein, the authors report a case of a 48-year-old male presented to their hospital with complaints of malaise, fever, chills, left flank pain, and a history of a staghorn calculus in the renal pelvis, which was removed by surgery 7 years ago. Incidentally found focal xanthogranulomatous pyelonephritis with extensive venous thrombus.

A 71-year-old woman with history of asthma presented with 2 months history of shortness of breath; on imaging an incidental left renal mass was noted. Subsequent renal protocol CT was obtained that showed a 4.5 cm left upper pole exophytic mass with renal vein thrombus extending into the inferior vena cava to the level of the caudate lobe concerning for renal cell carcinoma. She underwent an open left radical nephrectomy and IVC thrombectomy with subsequent postoperative pathology demonstrating xanthogranulomatous pyelonephritis without renal cell carcinoma.

Xanthogranulomatous Pyelonephritis Causing Renocolic Fistula Presenting as Symptomatic Anemia.

Renocolic fistula is a rare clinical finding that is most commonly iatrogenic after surgical intervention. Herein, we present a case of renocolic fistula secondary to xanthogranulomatous pyelonephritis (XGP) with a subtle presentation as anemia. A 40-year-old female was found to have a hemoglobin of 6.5 g/dL after presenting for worsening fatigue. A urinalysis was remarkable for numerous white blood cell (WBC), positive bacteria, and nitrite. As part of her anemia workup, an esophagogastroduodenoscopy (EGD) was done which was normal while a colonoscopy showed a fistula opening with surrounding

Bear's Paw Sign: A Classic Presentation of Xanthogranulomatous Pyelonephritis.

A 41-year-old woman with recurrent urinary tract infections presented with right flank pain, fever, and anorexia. Physical examination revealed right-sided flank pain and a right-sided unilateral renal mass. Computed tomography of the abdomen and pelvis revealed xanthogranulomatous pyelonephritis with perinephric extension. The bear's paw sign was visualized. The classic radiology triad (unilateral large kidney, renal pelvis stone, and a nonfunctioning or poorly functioning kidney) was also found on computed tomographic imaging. She completed a course of antimicrobial therapy and underwent a n

Aspiration cytology of focal xanthogranulomatous pyelonephritis: a diagnostic challenge.

A case of focal xanthogranulomatous pyelonephritis presenting as a circumscribed mass in a functioning kidney of a young male is described. Clinical, radiological, and cytological features closely mimicked renal cell carcinoma leading to radical nephrectomy. Peroperative findings and gross pathological features also corroborated preoperative diagnosis. The final diagnosis was a histologic surprise. It is an uncommon entity with cortical location, no pelvic communication, and progressively destroying the kidney. It should be considered in the differential diagnosis of any mass lesion at any age

OBJECTIVE: To explore the imaging diagnosis of xanthogranulomatous pyelonephritis (XGP), especially the value of angiography. METHODS: Combined with literature review, a retrospective analysis was conducted of the imaging data of 6 pathologically confirmed XGP cases as an attempt to define the preoperative diagnosis of the disease. RESULTS: Four cases of XGP were diagnosed preoperatively, and 2 were misdiagnosed as renal abscess. Renal arteriography revealed more characteristic changes, and the diagnosis of XGP relied on the clinical presentations and multiple imaging examinations. CONCLUSI
