Functional reconstructions using only ulnar nerve and ulnar nerve-innervated muscles for traumatic musculocutaneous, median and radial nerve palsies.

A 20-year-old right-handed fisherman experienced a severe injury in a high-energy traffic accident. These injuries included a shaft fracture of the right humerus, open comminuted fracture of the right forearm and multiple peripheral nerve palsies involving the musculocutaneous, median and radial nerves. Six months after the initial treatment, the patient was referred to our hand surgeons for further treatment. Several operations for functional reconstruction using only the ulnar nerve and ulnar nerve-innervated muscles were performed: the main procedures included a partial ulnar nerve transfer

Nerve transfer to the median nerve using parts of the ulnar and radial nerves in the rabbit--effects on motor recovery of the median nerve and donor nerve morbidity.

In this study, motor re-innervation of the median nerve by transfer of one-third, one-half, and two-thirds of either the agonistic ulnar nerve or the antagonistic radial nerve was investigated in both extremities of 20 rabbits. Recipient median nerve: Muscle contraction force of the flexor digitorum sublimus muscle after a one-third and a one-half of the ulnar nerve transfer achieved an average of 75 and 97% muscle power respectively as compared to conventional end-to-end neurorrhaphy. Muscle contraction force after one-third or one-half of the radial nerve transfer was significantly lower (36

A new method of predicting radial nerve location: a cadaveric study.

This study aimed to present a new method to predict an area with no Radial Nerve (RN) distribution (safe zone of the RN) in the middle and lower of the upper limb. The RN(60 in total) were dissected from 30 adult cadavers (20 males and 10 females). Acromial Angle, Olecranon, and Lateral Epicondyle of Humerus were selected as reference points. The linear projection of the RN on Acromial Angle-Olecranon and Acromial Angle-Lateral Epicondyle of Humerus were marked as 'Intersection Point A' and 'Intersection Point B'. The lengths of Acromial Angle-Lateral Epicondyle of Humerus, Acromial Angle-Olec

Visualization of radial nerve activity at the upper arm using magnetoneurography.

OBJECTIVE: To evaluate the function of the radial nerve in the upper arm using Magnetoneurography (MNG). METHODS: Eight asymptomatic male volunteers (age 25-63 years) and one 67-year-old female patient with radial nerve palsy were included. The radial nerve was electrically stimulated in the right wrist, and the evoked magnetic field was recorded using a 132-channel bio-magnetometer system with a superconducting quantum interference device positioned below the upper arm. The measurements were divided into distal and proximal sessions because of the extensive range of the nerve. RESULTS: MNG

Advantages of simultaneous radial nerve and tendon reconstruction - a case report.

Transection of the radial nerve is frequently associated with humeral shaft fractures that are part of a very complex upper extremity injury. In the presented case, a 19-year-old man with a 10-cm radial nerve defect with a need for nerve grafting to recover complete sensory and motor deficit of the radial nerve. In our case, at the same time we provided the tendon transfer of musculus (m.) pronator teres to m. extensor carpi radialis brevis, m. flexor carpi ulnaris to m. extensor digitorum communis, m. palmaris longus to m. extensor pollicis longus, and long sural nerve graft because of an ext

[Pseudotumoral calcinosis of the wrist compressing the radial nerve in systemic sclerosis].

BACKGROUND: Pseudotumoral calcinosis, a rare complication of systemic scleroderma, is characterized by the presence in extra-articular tissue, but rarely intra-articular tissue, of large masses made up of hydroxyapatite crystals. PATIENTS AND METHODS: We report an original case of intra- and extra-articular pseudotumoral calcinosis of the wrist diagnosed in a patient followed for mild systemic scleroderma. The calcinosis was revealed in a highly unusual way via ductal syndrome secondary to compression of the radial nerve in the wrist. Surgical treatment resulted in marked clinical and functio

[Regeneration of a radial nerve after compression under an osteosynthesis plate].

In approximately 4-13 % of cases plating of the humerus for stabilizing a shaft fracture is complicated by a secondary lesion of the radial nerve. In the majority of cases this is due to intraoperative traction on the nerve. The compression of a radial nerve under an osteosynthesis plate is reported only rarely. This article presents the case of a spontaneous regeneration of a radial nerve despite ongoing compression by the surgical implant after compression plating of a humeral shaft fracture. Recommendations are given to avoid such an iatrogenic lesion of the radial nerve, which most commonl

Malignant schwannoma of the radial nerve with unusual presentation: a case report.

Malignant schwannoma (MPNST) is a rare tumor. Many of these cases occur in patients with neurofibromatosis type I (NF-1) and they usually present as a mass lesion which rapidly increase in size with or without neurological symptoms. Here we present an unusual case of MPNST along the radial nerve in which the patient has no underlying neurofibromatosis type I.

Peripheral projections of sensory fascicles in the human superficial radial nerve.

The sensory territories of different cutaneous fascicles of the superficial radial nerve were delineated by microneurography at the level of the distal forearm in humans. Three fascicular patterns were found at this level: one supplying the dorsum of the radial aspect of the dorsum of the hand over the first dorsal interosseous space; another supplying the lateral aspect of the first metacarpal extending to the lateral aspect of the thumb; and a third innervating the second interosseous space and the proximal phalanx of the index and middle fingers. The compound fascicular territory is compara

Anatomic characteristics and clinical importance of the superficial branch of the radial nerve.

The aim of this study was to demonstrate anatomic variations of the superficial branch of the radial nerve (SBRN) and to discuss the importance of this branch for surgical approaches. Upper extremities of 24 cadavers without any known history of disease or trauma were examined, the SBRN became subcutaneous at a mean of 9.20 cm and bifurcated at a mean of 4.90 cm proximal to the styloid process of the radius in 48 specimens. At the level of the extensor retinaculum, the mean distance of the closest branch to the first dorsal compartment was 0.54 cm, while its mean distance to the dorsal tubercl

[An anatomic study of the superficial radial nerve and its clinical implications].

In hand surgery, the k-wires are used frequently and effectively. Several texts mention complications following surgery of the wrist and hand. A safe zone has been described by Steinberg et al., where 40 K-wires were placed in the anatomic snuffbox of 10 cadaveric forearms. The placement of the k-wires were evaluated radiologically and by dissection to examine the underlying structures. A branch of the superficial radial nerve (NRS) was injured in 22% (2/9) of the specimens and the cephalic vein in 33% (3/9). The radial artery was never injured but had a K-wires at least 1 mm away in 22% of th

[The surgical treatment of injuries to the deep branch of the radial nerve in children].

On the basis of examination and treatment of II children the authors showed that the injury of the deep branch of children radial nerve sometimes is connected with operative interventions, performing for fractures and fracturodispositions of the radial bone in the elbow joint area, and in part of children-in forearm soft tissue wounds. Reconstruction of the deep branch with the help of autotransplants performed during the first year after injury results in the high recovery rate of reduction of the I-V finger extension.

A Rare Case of Elbow Synovial Cyst with Radial Nerve Compression.

Elbow synovial cysts are rare and can mimic more frequently encountered disorders such as lateral epicondylitis, presenting diagnostic challenges. This report describes a woman in her mid-40s with persistent pain and weakness in her right forearm due to a synovial cyst compressing the radial nerve at the Arcade of Frohse. Despite initial suspicions of lateral epicondylitis, deeper investigation using sonography confirmed the presence of a compressive synovial cyst. Ultrasound-guided aspiration of the cyst was performed, yielding clear synovial fluid and providing symptomatic relief. Post-proce
