The role of core stabilization in lumbosacral radiculopathy.

Lumbosacral radiculopathy is relatively common, and most cases resolve with appropriate conservative management. Along with pain control, the mainstay of treatment is a comprehensive rehabilitation program to correct for biomechanical factors that adversely affect spinal stability and predispose the patient to recurrent pain. Developing an appropriate treatment plan depends on thorough understanding of the structures providing core stability and the exercises to correct identifiable deficits. A comprehensive rehabilitation program includes postural training, muscle reactivation, correction of

Operative techniques for cervical radiculopathy and myelopathy.

The surgical treatment of cervical spondylosis and resulting cervical radiculopathy or myelopathy has evolved over the past century. Surgical options for dorsal decompression of the cervical spine includes the traditional laminectomy and laminoplasty, first described in Asia in the 1970's. More recently the dorsal approch has been explored in terms of minimally invasive options including foraminotomies for nerve root descompression. Ventral decompression and fusion techniques are also described in the article, including traditional anterior cervical discectomy and fusion, strut grafting and ce

Frequency of radiculopathies in motor vehicle accidents.

This retrospective study compared the frequency of electrodiagnostically confirmed cervical and lumbar radiculopathies in a motor vehicle accident (MVA) population to that of a non-MVA population in 24,651 consecutive initial electrodiagnostic reports. The frequency of cervical radiculopathy was slightly but significantly increased in 8% of the MVA compared to 6% of the non-MVA patients. The frequency of plexopathy was significantly increased in the MVA (3%) compared to the non-MVA patients (2%). The frequency of lumbar radiculopathy was not significantly increased (12% for both groups). Ninet

Radiculopathy as a manifestation of Langerhans' cell histiocytosis.

Langerhans' cell histiocytosis (LCH) is a rare condition of children and young adults in which Langerhans' cells proliferate. The clinical spectrum ranges from solitary or few focal lesions to multisystem involvement mimicking vasculitis or hematological malignancy. Focal bone lesions, known as eosinophilic granulomas, are the most common manifestations. Eosinophilic granuloma usually presents with a variable combination of pain, swelling, fracture, and fever. Facial bone involvement may manifest as an ear discharge, hearing loss, or exophthalmos. Nerve root pain is rarely reported, even in pa

[Use of needle electromyography for diagnosis of radiculopathies].

Radiculopathy is sometimes evaluated with needle electromyography as additional examination to radiographic imaging. The purpose of this study was to determine the optimal needle electromyography protocol and the use of needle electromyography in the diagnosis of radiculopathy. The authors analysed English language writings from years 1981-2000. The selected studies reported a wide range of needle EMG abnormalities--from about 30 to 100% of pathological EMGs. That is caused by wide variety of patient populations and various EMG protocols. EMG of leg muscles in radiculopathy can consist of the

Lumbosacral radiculopathies: conservative approaches to management.

The prevalence of sciatica in Western society and its economic and functional impact make it an important problem to understand for treatment. Such understanding will impact greatly both medical management and decisions regarding activity, which could affect the perceived disability and financial dependence of a patient. This knowledge could be used to direct primary prevention, patient education, and future research efforts in back pain. The article also discusses that such strategies could improve health, decrease disability, and rein in costs.

On the pathogenesis of the radiculopathy complicating multilevel corpectomy.

Postoperative fifth cervical radiculopathy has been described after cervical corpectomy. One explanation for this complication is thought to be the factor of traction on cervical roots caused by a shifting of the spinal cord consequent to decompression. This theory is supported by our experience with 176 patients undergoing corpectomies for whom a lesser width of decompression all but eliminated the complication.

Cervical radiculopathy: a case for ancillary therapies?

OBJECTIVE: To present two cases, one of a patient with a radicular syndrome and another of a patient with a pseudoradicular syndrome. CLINICAL FEATURES: A 45-yr-old man visited one chiropractic clinic complaining of a "pinched nerve" in his neck, with pain and paresthesia in his left hand. He reported that these symptoms began after a work accident 1 month before, when he lifted a heavy object. Radiographs revealed disk space thinning at C4-5, C5-6 and C6-7. CT scans revealed foraminal narrowing with a minor disk bulging at the level of C5-6 and a large disk protrusion at C7-T1. The second pa

Cervical radiculopathy for neurologists: the role of electrodiagnosis.

Cervical radiculopathy (CR) is a common condition encountered in the general population, usually related to a musculoskeletal degenerative condition. Conventional electroneuromyography (ENMG) consists of nerve conduction studies (NCS) and needle electromyography (EMG), and it is regarded as the most specific diagnostic evaluation in this scenario. Although CR is commonly encountered in clinical practice, ENMG as a diagnostic tool is not often discussed in neurology residency programs. Electromyography has demonstrated modest sensitivity (50-71%) but excellent specificity (approaching 100%) for
