New diagnostic techniques for the evaluation of orbital trauma.

Midfacial trauma can produce fractures and soft tissue injuries and can embed foreign bodies within the eye, orbit, and brain. A careful history and ocular examination are fundamental to the evaluation of any patient who has suffered orbital injuries. Damage to the paranasal sinuses and the nasolacrimal system should be considered as a possible consequence of trauma to the orbit. Fractures are best evaluated by conventional roentgenograms with linear or hypocycloidal tomography. Bony fragments can be well visualized by xeroradiography. Positive contrast orbitography may be hazardous and has be

Spontaneous Multivessel Coronary Spasm During Diagnostic Coronary Angiography.

Acute vasospastic angina, formerly known as Prinzmetal angina, is characterized by transient electrocardiographic changes that are not related to exertion. Its atypical presentation makes it difficult to establish the diagnosis, so it is probably underrecognized and therefore mismanaged. We treated a 49-year-old woman who presented with a 2-day history of chest pain associated with palpitations. Abnormal radionuclide stress test results prompted diagnostic coronary angiography, during which the patient reported chest pain and became hemodynamically unstable. Active coronary vasospasm at multip

Effects of the Mueller maneuver on global and regional left ventricular function in angina pectoris with or without previous myocardial infarction.

In patients with coronary artery disease, left ventricular (LV) regional wall akinesia can develop during the Mueller maneuver. The present study determines if the presence of myocardial ischemic disease with no infarction is a sufficient condition for this to occur, or if the presence of prior acute myocardial infarction (MI) is necessary. In men, first-pass radionuclide ventriculography was performed in the 30 degree left anterior oblique supine position to measure LV ejection fraction, end-diastolic and end-systolic volumes and heart rate, and to obtain an image of the LV cavitary perimeter

Inferoseptal myocardial infarction: another cause of precordial ST-segment depression in transmural inferior wall myocardial infarction?

Electrocardiographic ST-segment depression in the anterior precordial leads is a frequent observation during the initial hospital phase of acute transmural inferior myocardial infarction (MI), but is of uncertain significance. No available clinical studies have examined the prevalence of inferoseptal necrosis complicating inferior MI. Therefore, the clinical course, electrocardiographic features, radionuclide angiograms and cardiac enzyme changes in 57 patients with transmural inferior MI who did not have prior anterior or concomitant "true posterior" MI, associated anterior or posterolateral

[2 cases of transposition corrected in adulthood. The value of computed tomography and isotopic angiography and a review of the literature].

Corrected transposition of the great arteries is an unusual congenital heart defect, above all at ages over 40. Echocardiography and invasive angiography are still cornerstones for its diagnosis. However other non-invasive techniques, such radionuclide angiocardiography and computed tomography, not so used in this malformation, can help us in its detection. We present 2 cases of corrected transposition of the great arteries in adults in which these methods were used and review the literature.

Chylopericardium: a rare cause of pericardial effusion.

A 21 year old man presented with asymptomatic, isolated chylopericardium. Despite echocardiography, radionuclide-angiography, computer tomography, and chemical analysis of the chylous effusion, the etiology remained obscure. After patent blue dye infusion into peripheral soft tissues, the appearance of coloring material in the effusion at 4 hours suggested direct communication of the pericardium with an apparently large thoracic duct. Fifteen months later, cardiomegaly persists in site of medium-chain triglyceride dietary restriction.

Bleeding colonic varices controlled by propranolol.

A 64-year-old man with hepatic cirrhosis developed severe haemorrhage from oesophageal varices. He underwent a course of sclerotherapy injections which successfully obliterated the oesophageal varices and prevented further oesophageal bleeding. He later developed serious bleeding from a site in the region of the ascending colon; angiography and radionuclide imaging suggested that varices were present in that region. Therapy with oral propranolol was effective in preventing any recurrence of gastrointestinal bleeding.

Colonic angiodysplasia on CT colonography: case report and characteristic imaging findings.

Gastrointestinal angiodysplasia represents the cause of 6% of lower gastrointestinal tract bleeding, particularly in the elderly. Because of the common presentation and age range of affected patients, often patients with occult or massive gastrointestinal bleedings are investigated with colonoscopy, in the suspect of colonic cancer. Other methods are capsule enteroscopy, angiography, double-contrast barium enema, computed tomography angiography, and radionuclide scanning. In this contribution, we describe a case of colonic angiodysplasia first suspected during computed tomography colonography

Intraarterial Administration Boosts 177Lu-HA-DOTATATE Accumulation in Salvage Meningioma Patients.

Intravenous 177Lu-high-affinity (HA)-DOTATATE has shown promising results for the treatment of surgery- and radiotherapy-refractory meningiomas. We aimed to investigate the added value of intraarterial administration. Methods: Patients underwent at least 1 intravenous 177Lu-HA-DOTATATE treatment first and subsequent intraarterial cycles. Inpatient and intrapatient comparison was based on posttreatment 177Lu-HA-DOTATATE imaging 24 h after injection. The technical success rate and adverse events were recorded. Results: Four patients provided informed consent. The technical success rate was 100%,

[Uterine leiomyosarcoma which spread through the vena cava to the pulmonary artery].

We describe a case of uterine leiomyosarcoma with four elongations that, through the vena cava, reached the pulmonary artery, left atrium, and right renal artery respectively. Clinical features suggested congenital pulmonary artery stenosis with pulmonary emboli. Clinical diagnosis was not made in spite of angiography and radionuclide scans. We review the literature on the subject. We also discuss the amenability of surgical correction as well as the mechanisms of the intravenous growth of this type of tumor.

Three-phase radionuclide bone scanning in evaluation of local radiation injury. A case report.

The management of local radiation injuries is influenced by the degree of vascular compromise within the skin and underlying tissues. Other authors have used thermography and angiography in assessing the blood flow to radiation damaged tissues. This report describes the use of radionuclide imaging in the evaluation of a patient who developed necrosis of his distal digits following a radiation accident. In addition to determining the vascular status of the hands, imaging helped indicate an appropriate level for amputation.

[Congestive heart failure with intact systolic function].

A retrospective study was undertaken of the cases of patients admitted for congestive cardiac failure over a 4 year period, and investigated by radionuclide angiography to determine the prevalence of cardiac failure with normal left ventricular systolic function, to document the underlying mechanisms of this condition and to assess whether the clinical data could predict the presence or absence of left ventricular systolic dysfunction. After excluding patients with significant valvular disease, severe renal failure, or myocardial infarction in the previous 2 months, the study population compri

Long-term follow-up of neonatal renovascular hypertension.

Twelve neonates with hypertension have been followed for a mean of 5.75 years. At onset of hypertension, mean peak blood pressure was 159/99 mm Hg. Ten infants had umbilical artery catheters, 9 placed above the origin of the renal arteries. Radionuclide renal scan and/or angiography demonstrated renovascular disease, primarily renal artery thrombosis, in 11 infants. One-third of infants were asymptomatic, one-third had normal urinalyses and two-thirds had elevated peripheral plasma renin activities. Blood pressure normalized with medical therapy in all infants and remained normal when therapy

Quantitative radionuclide angiocardiography for left-to-right cardiac shunts in children.

Pulmonary to systemic flow ratios (Qp/Qs) were estimated by quantitative radionuclide angiocardiography (QRAC) in 135 children. The Qp/Qs ratios were derived from pulmonary time/activity curves using a gamma variate model. Eighty-five of these children also had Qp/Qs ratios estimated by oximetry at cardiac catheterization. Left-to-right shunts with a Qp/Qs range of 1.2 to 3.0 were detected and quantitated by radionuclide angiography with an accuracy similar to that for oximetry. The correlation between Qp/Qs, determined by QRAC and by oximetry was good (r = .93; SEE .31). This relatively nonin

Myocardial contusion in the stable patient: what level of care is appropriate?

To evaluate the significance of myocardial contusion, we evaluated 243 stable patients hospitalized for blunt chest trauma between 1982 and 1986. The groups were identified according to results of radionuclide angiography, mean injury severity score (ISS), and outcome. Group I (n = 71; mean ISS = 12.7) patients were those without myocardial contusion by radionuclide angiography. Two patients with cardiac complications were in this group. The patients with myocardial contusion were divided into two groups. Group II (n = 69; ISS = 19.5) patients had myocardial contusion as an isolated injury, an

Intestinal neurofibromatosis. Multiple complications in a single case.

The use of radionuclide gastrointestinal scanning with 99mTc sulfur colloid, supplementing angiography, is described in the diagnosis of the source of bleeding in a 60-year-old woman with neurofibromatosis. These two procedures succeeded in locating the origin of bleeding from neurofibroma in the jejunum, which was corrected by surgery. To our knowledge, this case report is the first description of a patient with neurofibromatosis, with all the reported gastrointestinal symptoms: intermittent abdominal pains, palpable abdominal mass, gastrointestinal bleeding and small bowel obstruction.

Adrenal imaging.

The problems inherent in visualizing the adrenal glands are illustrated by the variety of imaging modalities that have been employed. Although urography has been the basic screening examination, one can make a sound argument for CT as the procedure of choice in the evaluation of the patient with suspected adrenal pathology. Angiography, venous sampling, and radionuclide studies continue to have a role in selected problem cases. This article reviews the usefulness of available modalities in the context of the clinical evaluation of adrenal disease.

[Promising methods of studying perfusion in patients with atherosclerosis of peripheral arteries].

Prevalence of atherosclerotic peripheral artery disease (PAD) has steadily been increasing all over the world, affecting approximately 10% of the population. PAD dramatically decreases the patients' quality of life and is accompanied by high risks of limb amputation and death. Reconstructive and restorative interventions make it possible to achieve the highest success in treatment of PAD. Their results largely depend on the state of the patient's peripheral bed. Currently, the periphery is objectively assessed by means of ultrasonographic duplex examination, digital subtraction angiography, ro

Pediatric hypertension: an approach to imaging.

Since 1981, we have made diagnoses of secondary hypertension using state of art imaging equipment in 18 pediatric patients. The most valuable tests were ultrasound in identifying renal parenchymal disease, computed body tomography for adrenal tumors and intra-arterial renal angiography for renovascular disorders. Based on our experience, we have formulated an algorithm for the evaluation of the hypertensive pediatric patient. The initial step is careful clinical and laboratory screening to identify those patients that are likely to have essential hypertension and who should not undergo imaging
