Does temporary bridge plate fixation preserve joint motion after an unstable Lisfranc injury?

BACKGROUND: Recently, temporary bridge plate fixation has gained popularity in the treatment of unstable Lisfranc injuries. The technique aims to reduce the risk of posttraumatic osteoarthritis, and after plate removal, the goal is to regain joint mobility. Here we explore marker-based radiostereometric analysis (RSA) to measure motion in the 1st tarsometatarsal (TMT) joint and asses the radiological outcome in patients treated with this surgical technique. METHOD: Ten patients with an unstable Lisfranc injury were included. All were treated with a dorsal bridge plate over the 1st TMT joint a

Stability and migration across femoral varus derotation osteotomies in children with neuromuscular disorders.

Background and purpose - Studies have indicated that one-third of children with cerebral palsy (CP) develop dislocation of the hip that needs surgical intervention. When hip dislocation occurs during childhood surgical treatment consists of tenotomies, femoral varus derotation osteotomy (VDRO), and acetabuloplasty. Relapse is observed in one-fifth of cases during adolescence. In this prospective cohort study, we performed a descriptive evaluation of translation and rotation across VDROs in children with neuromuscular disorders and syndromes by radiostereometric analysis (RSA). We assessed "RSA

Continued growth after fixation of slipped capital femoral epiphysis.

PURPOSE: When treating slipped capital femoral epiphysis (SCFE), a smooth pin with a hook or a short threaded screw can be used to allow further growth, which could be important to prevent the development of impingement and early arthritis. The purpose of this investigation was to measure growth in three dimensions after fixation of SCFE. METHODS: Sixteen participants with unilateral SCFE, nine girls and seven boys with a median age of 12.0 years (range 8.4-15.7 years), were included. The slipped hip was fixed with a smooth pin with a hook, and the non-slipped hip was prophylactically pinned.

Clinical accuracy of imaging techniques for talar neck malunion.

OBJECTIVES: To compare the ability of plain radiographs, computed tomography (CT), and radiostereometric analysis (RSA) to detect changes in talus fracture fragment position and alignment using an in vitro model. METHODS: Eight cadaveric tali were osteotomized at the talar neck. RSA beads were inserted into each talar fragment. The talus was anatomically reduced and stabilized with a pair of 3.5-mm cortical screws. Plain radiographs and RSA films were obtained. The fragments were then displaced and rotated to create a varus and supination deformity, and screw fixation was repeated in nonanato

Rigid fusion after cloward operation for cervical disc disease using autograft, allograft, or xenograft: a randomized study with radiostereometric and clinical follow-up assessment.

STUDY DESIGN: In this study, 43 patients scheduled for a single-level cervical Cloward procedure for disc disease were randomized prospectively to fusion with autograft, allograft, or xenograft. OBJECTIVE: To outline any differences in fusion over time in terms of final mobility and clinical outcome between the three bone grafts. SUMMARY OF BACKGROUND DATA: Fusion is used to relieve pain from a spinal segment. The bovine xenograft gives a fibrous fusion in contrast to the solid bone fusion obtained with autograft from the iliac crest, but no definite differences in clinical outcome have been

Contributing factors to the initial femoral stem migration in cementless total hip arthroplasty of postmenopausal women.

In cementless total hip arthroplasty (THA), femoral stems rely on the initial press-fit fixation against cortical bone to achieve osseointegration. Decreased bone mineral density (BMD) in postmenopausal women poses natural difficulties in achieving axial and rotational femoral stem stability. The present study examined contributing demographic, surgery-related and postoperative factors in determining the magnitude of early stem migration prior to osseointegration. A prospective cohort of 65 postmenopausal women with hip osteoarthritis (Dorr type A or B femur anatomy) underwent THA with implant

Elbow Biomechanics, Radiocapitellar Joint Pressure, and Interosseous Membrane Strain Before and After Radial Head Arthroplasty.

Complex radial head fracture and elbow instability can be treated with radial head arthroplasty. Good clinical results have been described after this surgical treatment. However, the revision and complication rate reported in the literature is concerning. This might be due to altered kinematics after radial head arthroplasty. Eight human native elbows were examined with dynamic radiostereometric analysis and compared with a radial head arthroplasty. Translations of the radial head in the x-, y-, and z-directions relative to the humerus and the ulna were measured. The radiocapitellar joint pres

Use of National Joint Registries to Evaluate a New Knee Arthroplasty Design.

BACKGROUND: The introduction of new technology in joint replacement surgery requires close monitoring to identify early successes and failures. This monitoring can be effectively performed through the analysis of registry data and radiostereometric analysis studies. This study examined the revision rates of a contemporary knee system for total knee arthroplasty (TKA) using National Joint Replacement Registries. METHODS: A review of the literature was performed to identify comparative studies and registry databases reporting the revision rates of a specific contemporary knee design between 201

Influence of design variations on early migration of a cemented stem in THA.

UNLABELLED: The clinical results of using different cemented stems in total hip arthroplasty may vary because of shape, surface finish, and choice of metal alloy. Less is known about the influence of size and offset using one and the same design. Data from 140 patients (140 hips) of a cohort constituting 197 patients (231 hips) implanted with cemented Spectron Primary stems were extracted from patients studied in four randomized studies primarily designed for other purposes. Stem size (1, 2, 3, or larger), normal/extra offset, neck length and true offset (horizontal length between the stem and

Effect of Medial Soft Tissue Releases During Posterior-Stabilized Total Knee Arthroplasty on Contact Kinematics and Patient-Reported Outcomes.

BACKGROUND: Minimal to extensive medial soft tissue releases are part of the exposure and achieving adequate varus knee balance in total knee arthroplasty (TKA). However, the effect of these releases on knee kinematics and patient-reported outcomes is unclear. Our objective was to compare the postoperative in vivo tibiofemoral contact kinematics of a posterior-stabilized TKA between patients who received minimal medial soft tissue releases intraoperatively to those who received extensive releases. We also compared these groups using patient-reported outcomes. METHODS: A prospective imaging st

Peri-apatite coating decreases uncemented tibial component migration: long-term RSA results of a randomized controlled trial and limitations of short-term results.

Background and purpose - Biological fixation of uncemented knee prostheses can be improved by applying hydroxyapatite coating around the porous surface via a solution deposition technique called Peri-Apatite (PA). The 2-year results of a randomized controlled trial, evaluating the effect of PA, revealed several components with continuous migration in the second postoperative year, particularly in the uncoated group. To evaluate whether absence of early stabilization is diagnostic of loosening, we now present long-term follow-up results. Patients and methods - 60 patients were randomized to PA-

Less polyethylene wear in monobloc compared to modular ultra-high-molecular-weight-polyethylene inlays in hybrid total knee arthroplasty: A 5-year randomized radiostereometry study.

BACKGROUND: A modular polyethylene (PE) inlay in total knee arthroplasty (TKA) may wear on both sides. PE particles may induce osteolysis, which can lead to implant loosening. The aim of this study was to determine if PE wear in monobloc TKA differs from that ofmodular TKA at 60-month follow-up. PATIENTS AND METHODS: In a prospective, patient-blinded trial, 50 patients were randomized to hybrid TKA surgery with either acementless high-porosity trabecular-metal tibial component with a monobloc UHMWPE inlay (MONO-TM) or a cementless low-porosity screw-augmented titanium fiber-mesh tibial compon

Denosumab reduces early migration in total knee replacement.

Background and purpose - Aseptic loosening is a main cause of late revision in total knee replacement (TKR). Migration of implants as measured by radiostereometric analysis (RSA) can predict future loosening. This migration is associated with bone resorption. Denosumab is a human monoclonal antibody that binds to receptors on osteoclast precursors and osteoclasts. This prevents osteoclast formation, resulting in less bone resorption in cortical and trabecular bone. We investigated whether denosumab can reduce migration of TKR, as measured with RSA. Patients and methods - In this 2-center, rand

Rotator cuff repair failure in vivo: a radiostereometric measurement study.

BACKGROUND: The prevalence of failure among repairs of the rotator cuff is well known, but very few objective data exist regarding either the scale or timing of this complication. The aim of this study was to use a previously validated modified technique of roentgen stereophotogrammetric analysis to monitor the behavior of the rotator cuff after repair to establish at what point failure may occur. MATERIALS AND METHODS: A series of 10 patients had metal beads and wire sutures embedded into the humeral greater tuberosity and supraspinatus tendon, respectively, during open cuff repair procedure

Highly Porous Tantalum Acetabular Components Without Ancillary Screws Have Similar Migration to Porous Titanium Acetabular Components With Screws at 2 Years: A Randomized Controlled Trial.

BACKGROUND: It is proposed that highly porous coatings on acetabular components, such as a porous tantalum coating, provide adequate fixation without ancillary screw fixation in primary total hip arthroplasty (THA). However, tantalum acetabular components have been associated with higher rates of revision than other uncemented components in national registries. The aim of this randomized controlled trial is to determine whether the early migration of a solid-backed tantalum acetabular component was no greater than that of a titanium acetabular component with ancillary screw fixation that has p

Early fixation of the humeral component in stemless total shoulder arthroplasty : a radiostereometric and clinical study with 24-month follow-up.

AIMS: Stemless humeral implants have been developed to overcome stem-related complications in total shoulder arthroplasty (TSA). However, stemless implant designs may hypothetically result in less stable initial fixation, potentially affecting long-term survival. The aim of this study is to investigate early fixation and migration patterns of the stemless humeral component of the Simpliciti Shoulder System and to evaluate clinical outcomes. METHODS: In this prospective cohort study, radiostereometric analysis (RSA) radiographs were obtained in 24 patients at one day, six weeks, six months, on

Long-term migration of a cementless stem with different bioactive coatings. Data from a "prime" RSA study: lessons learned.

Background and purpose - Little is known about the long-term migration pattern of cementless stems in total hip arthroplasty (THA). Furthermore, the role of bioactive coatings in fixation, and thus migration, remains uncertain. Hydroxyapatite (HA) is the most commonly used bioactive coating. However, delamination of the coating might induce loosening. Alternatively, fluorapatite (FA) has proved to be more thermostable than HA, thereby potentially increasing longevity. We assessed the long-term migration of cementless stems with different coatings using radiostereometric analysis (RSA), thereby

Long-term outcomes after Instrumented Bone Preserving total elbow arthroplasty: a radiostereometric study with a minimum follow-up of 10 years.

BACKGROUND: Aseptic loosening is a main concern in elbow arthroplasty. Evaluation of implant migration using radiostereometric analysis (RSA) might increase understanding of implant loosening. Previously, 2-year RSA results of 16 Instrumented Bone Preserving (IBP) elbow prostheses showed migration of the humeral component in the first weeks but most components stabilized within 6 months postoperatively. In follow-up, the present study evaluated long-term survival, the relation between early migration and survival, and the long-term migration and clinical outcomes. METHODS: Sixteen patients wh

Comparison of Contact Kinematics in Posterior-Stabilized and Cruciate-Retaining Total Knee Arthroplasty at Long-Term Follow-Up.

BACKGROUND: There is controversy regarding the superiority of posterior-stabilizing (PS) total knee arthroplasty (TKA) and cruciate-retaining (CR) TKA. Substantial work has made comparisons between PS and CR TKA at follow-ups of less than 5 years. It was the goal of the present study to compare the kinematics at greater than 5 years postoperatively between CR and PS TKA, with a secondary goal of comparing patient function. METHODS: A total of 42 knees were investigated, with equal representation in the PS and CR TKA groups. Patients underwent radiostereometric analysis imaging at 0°, 20°, 40°
