BACKGROUND: Rapid sequence intubation (RSI) have been shown to be effective in preventing reflux aspiration in patients with a full stomach during anaesthesia induction and endotracheal intubation. However, there is currently no standardized operation protocol or anaesthesia induction drug standard for RSI. Furthermore, there is a lack of evidence regarding the use of RSI in patients older than 65. In this study, we aimed to investigate the cardiovascular effects of different doses of alfentanil combined with propofol and etomidate during RSI in elderly patients aged 65-80 years.

BACKGROUND: Airway management is an important part of the management of the critically ill and injured patients in the Emergency Department (ED). Numerous studies from developed countries have demonstrated the competency of emergency doctors in intubation. To date there have been no published data on intubations performed in EDs in Malaysia.
METHODS: Data on intubations from 7 August 2007 till 28 August 2008 were prospectively collected.
RESULTS: There were 228 intubations included in the study period. Cardiopulmonary arrest was the main indication for intubation (35.5%). The other indicatio

Patients with sepsis often require emergency intubation. In emergency departments (EDs), rapid-sequence intubation with a single-dose induction agent is standard practice, but the best choice of induction agent in sepsis remains controversial. We conducted a randomized, controlled, single-blind trial in the ED. We included septic patients who were aged at least 18 years and required sedation for emergency intubation. Patients were randomly assigned by a blocked randomization to receive 0.2-0.3 mg/kg of etomidate or 1-2 mg/kg of ketamine for intubation. The objectives were to compare the surviv

BACKGROUND AND OBJECTIVES: Hypertrophic pyloric stenosis is a relatively common affection of gastrointestinal tract in childhood that results in symptoms, such as projectile vomiting and metabolic disorders that imply a high risk of aspiration during anesthetic induction. In this way, the carrying out of a technique with general anesthesia and intravenous rapid sequence induction, preoxygenation and cricoid pressure are recommended. After the correction of systemic metabolic alkalosis and pH normalization, cerebrospinal fluid can keep a state of metabolic alkalosis. This circumstance, in addit

A male patient was scheduled for urgent amputation of his right forearm. His right forearm was stuck inside the insertion slot of a meat grinder, resulting in severe pain to his injured arm. His upper body could not move to sit in a semi-upright position. An endotracheal tube was successfully placed after rapid sequence intubation using a video laryngoscope from behind the patient on the first attempt. This case report is the first documentation of successful anesthetic induction with subsequent endotracheal intubation using a video laryngoscope from behind an injured patient whose upper body

OBJECTIVES: To evaluate high-fidelity medical simulation as an assessment tool for pediatric residents' ability to manage an acute airway.
METHODS: We performed a prospective, observational study in which 16 pediatric residents were consented and then brought to the medical simulation center. They were placed in 2 different computer-driven scenarios and asked to manage the cases. The first scenario was a 3-month-old infant with bronchiolitis and severe respiratory distress and was programmed to develop respiratory failure. The second case was a 16-year-old adolescent with alcohol intoxication

PURPOSE: The optimal hypnotic agent for rapid sequence intubation (RSI) in critically ill patients remains uncertain. We evaluated whether Ketamine is a better alternative than propofol.
METHODS: In this investigator-initiated, randomized, controlled, open-label trial conducted in two mixed ICU, adults (≥ 18 years) requiring RSI were assigned 1:1 to Ketamine or propofol. The primary outcome was the lowest mean arterial pressure (MAP) within the first 10 min after induction, analyzed with a linear regression model adjusted for age, baseline MAP, and total vasopressor dose within 10 min.

INTRODUCTION: Infants with hypertrophic pyloric stenosis have gastric outlet obstruction, indicating a pyloromyotomy. To prevent aspiration, a rapid sequence induction (RSI) of anesthesia used to be preferred. However, due to concerns about the side-effects of this technique in infants, a modified RSI with gentle mask ventilation is nowadays mostly used. This research investigates if induction with succinylcholine (classic RSI), cisatracurium (modified RSI), or no neuromuscular blocking agent (NMBA) influences time until extubation and incidence of complications in infants undergoing laparosco

BACKGROUND: In some categories of patients, a rapid sequence induction using a fibre-optic method may be indicated. The aim of the present study was to examine the effect of cricoid pressure (CP) on the duration of fibre-optic intubation. The hypothesis was that CP would prolong the intubation time.
METHODS: The study was a randomised, double-blind, cross-over study. The patients were intubated twice, in a randomised way, using a flexible fibrescope once with and once without CP. The intubation time and the visualisation of the glottis were registered. If the intubation was not completed with

PURPOSE: Cricoid pressure is commonly used during rapid sequence induction and intubation to minimize the risk of aspiration. The objective of the study was to evaluate the ease of fibreoptic (WuScope System(TM)) intubation in anesthetized adults receiving cricoid pressure.
METHODS: The intubation difficulty scale (IDS) was used to measure tracheal intubation difficulties in 33 patients undergoing elective surgery with general anesthesia and complete neuromuscular blockade. Each patient had their trachea intubated under two conditions: with and without cricoid pressure. The order of condition

Tracheal intubation in coronavirus disease 2019 (COVID-19) patients creates a risk to physiologically compromised patients and to attending healthcare providers. Clinical information on airway management and expert recommendations in these patients are urgently needed. By analysing a two-centre retrospective observational case series from Wuhan, China, a panel of international airway management experts discussed the results and formulated consensus recommendations for the management of tracheal intubation in COVID-19 patients. Of 202 COVID-19 patients undergoing emergency tracheal intubation,

BACKGROUND: Videolaryngoscopy has been proposed to improve glottic visualization and intubation success during rapid sequence intubation (RSI). Evidence on the effectiveness of the McGrath videolaryngoscope in this setting remains limited.
METHODS: In this multicenter, patient-blinded, randomized trial, 400 adults undergoing elective non-cardiac surgery requiring RSI were assigned to McGrath videolaryngoscopy (VL, n = 193) or direct laryngoscopy (DL, n = 201). The primary outcome was glottic visualization using the modified Cormack-Lehane (CL) classification. Secondary outcomes included first

STUDY OBJECTIVE: Emergency department (ED) clinicians often avoid succinylcholine due to concerns about potassium elevation leading to adverse events, though clinical evidence beyond case reports is limited. Rocuronium is a non-depolarizing paralytic that does not affect potassium levels. Therefore, it is perceived as safer despite causing longer paralysis than succinylcholine. This study aimed to compare adverse clinical outcomes in patients with pre-existing hyperkalemia who received either succinylcholine or rocuronium during rapid sequence intubation (RSI) in the ED.
METHODS: We conducted

Penetrating neck injury presents significant challenges for anesthesiologists due to distorted anatomy, bleeding, and the risk of sudden deterioration. Airway management requires careful consideration of the technique selection to balance urgency with patient safety. We report a 30-year-old male with a self-inflicted knife injury to the anterior neck. Imaging revealed the blade traversing the sternohyoid muscle, grazing the thyroid, and extending toward the apical pleura, without vascular or airway breach. The patient was hemodynamically stable on arrival. After multidisciplinary planning, ane

BACKGROUND: Infants with pyloric stenosis are considered to be at high risk of aspiration on induction of anesthesia. Traditionally, texts have recommended classic rapid sequence induction (RSI) or awake intubation (AI). AI has generally fallen out of favor, while the components of RSI have become increasingly controversial. Infants are at high risk of hypoxemia if ventilation is not maintained while waiting for neuromuscular blockade to establish. The efficacy of cricoid pressure (CP) to prevent aspiration has not been proven. It can impair visualization of the glottis and make intubation dif

BACKGROUND AND AIMS: Airway management in cervical spine injury is challenging, and combining videolaryngoscopy with a flexible tracheoscope could enhance glottic visualisation, addressing the limitations when used alone. This prospective, open-label, randomised trial compared videolaryngoscopy-assisted flexible tracheoscopy (VLF) with videolaryngoscopy using a rigid stylet (VLS) for orotracheal intubation under simulated manual in-line stabilisation (MILS).

Twenty patients were studied in a double-blind manner to investigate whether magnesium sulphate, when given during a rapid-sequence induction of anaesthesia, lessens the side effects caused by suxamethonium. Patients were randomly allocated to two groups; equal volumes of either magnesium sulphate (40 mg.kg-1) or saline were given during rapid-sequence induction of anaesthesia, after thiopentone but before the administration of suxamethonium (1.5 mg.kg-1). The changes in the serum potassium concentration, the degree of muscle fasciculations and the presence of postoperative myalgia were record

INTRODUCTION: Rapid Sequence Induction (RSI) is widely used in pediatric anesthesia for children at risk of pulmonary aspiration. Modified RSI has been proposed to reduce the risk of hypoxemia during induction while maintaining airway protection. This proof-of-concept study aimed to describe peri-intubation outcomes associated with the use of intravenous bolus remifentanil during modified RSI in a selected pediatric population at risk of pulmonary aspiration.
METHODS: This proof-of-concept study analyzed data from pediatric patients, with at least one risk factor for pulmonary aspiration, und

BACKGROUND: Controversy and speculation exist regarding intracranial pressure (ICP) changes produced by various combinations of rapid sequence intubation (RSI) agents. In this pilot study, we sought to develop a swine model to investigate these changes in classic RSI.
METHODS: Eight adult swine were instrumented with arterial and intracranial pressure monitors. Four different versions of rapid sequence intubation were then performed sequentially in each animal in a crossover trial design: regimen 1, thiopental; regimen 2, thiopental and succinylcholine; regimen 3, lidocaine, thiopental, and s
