Motor cortical excitability during voluntary inhibition of involuntary tic movements.

BACKGROUND: Tics can be voluntarily inhibited. However, the neurophysiology of voluntary tic inhibition remains underexplored. The objective of this study was to explore state-dependent effects of voluntary tic inhibition on M1 excitability. METHODS: Neurophysiological assessments (single motor-evoked potentials, corticospinal recruitment curves, short-interval intracortical inhibition, H-reflex) were performed in 14 adults with Tourette syndrome during voluntary tic inhibition and free ticcing. Regressions between behavioral performance and neurophysiological measures were also performed. R

Additive effect of tDCS and neuromotor recruitment on functional recovery in chronic paraplegia: A randomized controlled trial.

Functional recovery in chronic spinal cord injury (SCI) is traditionally viewed as limited, particularly after the spontaneous recovery window has closed. This single-blind randomized controlled trial investigated whether adding anodal Transcranial Direct Current Stimulation (tDCS) to the Neuromotor Recruitment Method (NEUROM)-a protocol combining motor imagery with intensive peripheral sensory stimulation-provides a additive benefit for sensorimotor recovery. Fifty participants with chronic paraplegia (mean time since injury: 15.4 ± 3.2 months; ASIA Impairment Scale A, B, or C) were recruited

Electromyographic laryngeal synkinesis alters prognosis in vocal fold paralysis.

OBJECTIVES/HYPOTHESIS: Synkinesis, or misdirected reinnervation, is likely a confounder when predicting return of function of an immobile vocal fold. Currently, no information exists on the incidence of synkinesis in unilateral vocal fold immobility (UVFI) or the effect synkinesis has on prognosis and treatment. Our objective was to examine a vocal fold adductor synkinesis screening protocol using diagnostic laryngeal electromyography (LEMG). We aim to determine the effect of synkinesis on prognosis of recovery of purposeful vocal fold motion. STUDY DESIGN: Retrospective review of LEMG data a

Relationship between facial asymmetry and masseter reflex activity.

PURPOSE: In this study, we evaluated the tonic vibration reflex (TVR) of the masseter muscles in patients with facial asymmetry. SUBJECTS AND METHODS: The study was conducted with 10 volunteers without facial asymmetry and 12 orthognathic patients with facial asymmetry. Subjects were seated in a chair and held a stimulator composed of an electric motor and an acrylic bite block between the upper and lower dentitions at facial midline, to elicit TVR. Electromyographic activity was recorded using a pair of silver electrodes affixed bilaterally with adhesive tape to the skin over the superficial

Recruitment of crab gastric mill neurons into the pyloric motor pattern by mechanosensory afferent stimulation.

1. The gastropyloric receptor (GPR) cells are stretch-sensitive muscle receptors in the crab stomatogastric nervous system that use both 5-hydroxytryptamine (serotonin) and acetylcholine as cotransmitters. Brief stimulation of these afferent neurons causes two gastric mill neurons to be recruited into the pyloric motor pattern. 2. The GPR cells evoke complex synaptic potentials in the lateral gastric (LG) and medial gastric (MG) motor neurons, two component neurons of the gastric mill central pattern generator. When the gastric mill is quiescent (as often happens in vivo), GPR stimulation tran

Local anaesthetic block protects against electrically-induced damage in peripheral nerve.

This study is one of a series addressing the mechanisms involved in the production of neural damage caused by continuous, prolonged electrical stimulation of peripheral nerve. It has been previously shown that sustained, high frequency electrical stimulation of the cat's peroneal nerve may cause irreversible neural damage in the form of axonal degeneration of the large myelinated fibres. In this study we demonstrate that blocking the action potentials on most of the nerve fibres with local anaesthetics (10% procaine or 2% lidocaine) almost completely prevents the axonal degeneration. The aboli

Patterns of reinnervation and motor unit recruitment in human hand muscles after complete ulnar and median nerve section and resuture.

Following complete ulnar or above-elbow median nerve sections, there was no significant correlation between motor unit size (twitch amplitude) and recruitment threshold, as assessed by spike triggered averaging. This absence of orderly recruitment was attributed to misdirection of motor axons during regeneration. Following median nerve section at wrist level, where the reinnervated muscles have more synergistic actions, orderly recruitment by size appeared to be re-established. Thus, the size principle of motor unit recruitment can be re-established after nerve section in humans, if motor axon

Variation of the lumbosacral myotomes with bony segmental anomalies.

Clinical localisation of a disc prolapse required dependable knowledge of the muscles supplied by the lumbosacral nerve roots. Localisation is most difficult in the 10 per cent of patients who have lumbosacral bony segmental anomalies. The lumbosacral plexus has been dissected in 11 cadavers with such anomalies and electrical stimulation studies carried out in 15 patients similarly afflicted. It is suggested that whatever the anomaly the "last fully mobile level" should be identified as the lowest level with a fully formed disc space, bilateral facet joints and two free transverse processes wh

Activity of muscle sympathetic neurons during normotensive pregnancy.

In pathological populations, elevated sympathetic activity is associated with increased activity of individual sympathetic neurons. We used custom action potential detection software to analyze multiunit sympathetic activity in 18 normotensive pregnant women (third trimester; 33 ± 5 wk) and 19 nonpregnant women at rest and a subset (10 and 13, respectively) during a cold pressor challenge. Although the number of action potentials per burst and number of active amplitude-based "clusters" were not different between groups, the total number of sympathetic action potentials per minute was higher i

Motor unit remodelling in multifocal motor neuropathy: The importance of axonal loss.

OBJECTIVE: To estimate the degree of axonal loss in patients diagnosed with multifocal motor neuropathy (MMN) using a novel assessment of motor unit numbers and size. METHODS: Automated motor unit number estimation using a compound muscle action potential (CMAP) scan was undertaken in median nerves with conduction block. Results were compared with 30 age-matched healthy controls. RESULTS: Compared with healthy controls, MMN patients had fewer motor units (MMN: 33±11vs HC: 93±36 [mean±SD]; p<0.0001) and larger 'size of the largest unit' (MMN: 1.2±0.5mVvs HC: 0.4±0.1mV; p<0.0001), despite havi

Enhancement of single motor unit inhibitory responses to transcranial magnetic stimulation in amyotrophic lateral sclerosis.

In healthy human subjects, transcranial magnetic stimulation (TMS) applied to the motor cortex induces concurrent inhibitory and excitatory effects on motoneurone activity. In amyotrophic lateral sclerosis (ALS), a neurodegenerative disease affecting both cortical and spinal motor neurons, paired-pulse studies based on electromyographic (EMG) recording have revealed a decrease in TMS-induced inhibition. This suggested that inhibition loss may promote excito-toxicity in this disease. Against this hypothesis, an abnormally high incidence of inhibitory responses to TMS has been observed in the pe

NTS catecholamine cell recruitment by hemorrhage and hypoxia.

Immunolabelling for Fos and tyrosine hydroxylase was used to determine the patterns of activation of nucleus tractus solitarius catecholamine cells in response to graded levels of hemorrhage (0, 4, 8, 12 and 16 ml/kg) and systemic hypoxia (21, 14, 12, 10 and 8% O2) in conscious rats. Both stimuli elicited graded catecholamine cell recruitment with thresholds of 8 ml/kg and 12% O2. The majority of responsive neurons were A2 noradrenergic rather than C2 adrenergic cells. After hemorrhage most Fos-positive catecholamine cells were found below obex whereas most hypoxia-responsive cells were rostra

Variation in firing order of human soleus motoneurons during voluntary and reflex activation.

The activation of motoneurons in a muscle pool is said to proceed as an ordered array in response to both dorsal root stimulation and voluntary activation, with small motoneurons being recruited before larger ones. We have examined 19 voluntarily recruited soleus motor units in 5 normal subjects and found that in 18 cases, the lowest threshold motor unit recruited by slowly increasing 'tonic' voluntary activity was different from the lowest threshold unit recruited by electrical stimulation of the tibial nerve in the popliteal fossa (phasic Hoffman reflex). The initially recruited voluntary un

Multiple point electrical stimulation of ulnar and median nerves.

A computer-assisted method of isolating single motor units (MUs) by multiple point stimulation (MPS) of peripheral nerves is described. MPS was used to isolate 10-30 single MUs from thenar and hypothenar muscles of normal subjects and patients with entrapment neuropathies, with the original purpose of obtaining a more representative mean motor unit potential for estimating the number of MUs in a muscle. The two important results that evolved from MPS however, were: (1) in the absence of 'alternation' MUs were recruited in an orderly pattern from small to large, and from longer to shorter laten

Distinctive recruitment of endogenous sleep-promoting neurons by volatile anesthetics and a nonimmobilizer.

BACKGROUND: Numerous studies demonstrate that anesthetic-induced unconsciousness is accompanied by activation of hypothalamic sleep-promoting neurons, which occurs through both pre- and postsynaptic mechanisms. However, the correlation between drug exposure, neuronal activation, and onset of hypnosis remains incompletely understood. Moreover, the degree to which anesthetics activate both endogenous populations of γ-aminobutyric acid (GABA)ergic sleep-promoting neurons within the ventrolateral preoptic (VLPO) and median preoptic nuclei remains unknown. METHODS: Mice were exposed to oxygen, hyp

Recruitment of periventricular parietal regions in processing cluttered point-light biological motion.

Recent findings point to the existence of a cortical-subcortical parietal network that drives attention-related integration of features and elements. Here we ask whether the functioning of this network might be modulated by early periventricular lesions. To this end, a cohort of adolescents who were born premature with different severity of bilateral periventricular leukomalacia (PVL) and two groups of matched peers (term-born adolescents and former preterms with normal MRI scan) were shown a set of impoverished point-light stimuli. Observers had to detect a point-light walker embedded in an a

Neurorespiratory pattern of gill and lung ventilation in the decerebrate spontaneously breathing tadpole.

A decerebrate, spontaneously breathing tadpole preparation (Taylor-Kollros stages 16-19) was used to test the general hypothesis that the efferent bursting activities of cranial nerves (CN) V, VII and spinal nerve (SN) II are respiratory in nature, and, in particular, to identify separate and specific neural correlates of gill and lung ventilation. Oropharyngeal pressure (POP), intrapulmonary pressure (PIP), electromyogram (EMG) of the buccal levator muscle (interhyoideus), and efferent neural activities of CN V, CN VII and SN II were recorded while the animal was exposed to hyperoxia (100% in
