BACKGROUND/PURPOSE: Rectal biopsies are performed as a definitive means of diagnosing Hirschsprung's Disease (HD) in children presenting with constipation. The authors hypothesized that key features in the history, physical examination, and radiographic evaluation would allow us to avoid unnecessary rectal biopsies.

Internal rectal prolapse was diagnosed in 21 patients with signs and symptoms consistent with this syndrome. They all underwent rubber-band ligation of the excess mucosa--2.2 procedures per patient on average. Eight patients became asymptomatic and 13 significantly improved clinically. A rectal cuff abscess following corrective surgery for Hirschsprung disease is an uncommon but serious complication requiring prompt diagnostic and therapeutic attention. Plain films and examinations using contrast medium are insensitive to detection of an abscess due to surgical disruption of the normal anatomy. The internal anal sphincter response was studied manometrically while applying electrical stimulation to the rectal mucosa. Seven patients with Hirschsprung's disease and ten healthy controls were investigated during general anaesthesia. In addition, six healthy controls were examined when awake. Internal sphincter relaxations elicited by electrical stimulation of the rectal myenteric nerve plexus were recorded in all anaesthetized controls, but in only two of the patients. Acetylcholinesterase activity was measured in rectal-biopsy specimens obtained from nineteen children. Seven children in whom the diagnosis of Hirschsprung's disease was established were found to have a significantly higher enzyme activity than the twelve in whom this diagnosis was excluded. The estimation may prove to be of value in the diagnosis of Hirschsprung's disease.

CA125 is a heavily glycosylated protein and was first identified as an ovarian cancer antigen in 1981 that is recognized by a monoclonal antibody generated by the hybridoma technique. Hirschsprung disease (HD) is reported in patients with Down syndrome with a frequency between 2% and 10%. The incidence of HD is 2% in our community-based registry that contains >700 patients with Down syndrome. We reviewed rectal biopsy findings in 32 of these patients who had suction rectal biopsy performed between 1980 and 2009 to investigate the cause of chronic constipation.
